Pain is complex and multifactorial, making diagnosis challenging. The traditional impingement model is being challenged in favor of a broader understanding of shoulder pain. Imaging often does not influence management decisions for rotator cuff issues. Corticosteroid injections provide only short-term relief and can have negative effects on tendon quality. Exercise-based management is crucial for effective rehabilitation of shoulder pain. The rotator cuff tendons are positional and strain less than energy storage tendons. Rotator cuff tears are common and can exist without pain or dysfunction. Age is the biggest risk factor for developing rotator cuff pathology. Understanding the tipping point to pain is essential for effective treatment. Rotator cuff tears are often associated with poor vascular supply and degeneration over time. Metabolic factors like diabetes and smoking can increase the risk of rotator cuff tears. Exercise can be as effective as surgery for massive rotator cuff tears. Expectations of recovery significantly influence rehabilitation outcomes. Education about the commonality of tendon tears can help reduce patient anxiety. Pain during exercise can be tolerated up to a certain level without adverse effects. Sleeping positions can impact shoulder pain and should be modified accordingly. Adjunct treatments like shockwave therapy and corticosteroids have limited long-term benefits. Tendon stiffness is important for efficient force transfer, but its role in pain management is still being studied. Understanding the psychological aspects of pain can enhance recovery from tendon injuries. Send us a textParsonage-Turner Syndrome can mimic several other shoulder conditions, leading to misdiagnosis and ineffective treatment if not properly identified and differentiated.• PTS typically presents with sudden onset of severe shoulder pain lasting 2-3 weeks, often triggered by viruses or vaccinations• Common symptoms include limited active and passive range of motion, weakness, atrophy (especially in deltoids), and sometimes altered sensation• Rotator cuff tears differ by having better passive than active motion and usually having a clear mechanism of injury• Cervical nerve root compression can be distinguished by performing Spurling's test and gentle cervical traction• Adhesive capsulitis has a slower onset than PTS and typically doesn't cause the significant atrophy seen in PTS cases• Diagnostic imaging should be used after thorough clinical examination to confirm suspected diagnosis• EMG/nerve conduction studies are most helpful for confirming PTS after 3-4 weeks of symptoms• Always check for skin changes like pustules or rashes that might indicate shingles, which can cause brachial neuritisJoin us for our upcoming live course on May 31st, 2025 where we'll cover more differential diagnoses like these. His journey into physical therapy began after a change from pursuing medicine. The understanding of tendinopathy has evolved significantly over the years. Loading strategies are crucial in the rehabilitation of tendons. Patient education is essential for effective treatment and management. Miller emphasizes the importance of simplifying rehabilitation exercises. He shares personal experiences with various tendon injuries. The role of lifestyle factors in tendon health is significant. Miller advocates for a tailored approach to each patient's needs. Understanding the difference between tolerance and capacity in tendon treatment is key. Recovery requires minimizing compressive load during rehabilitation. Step count and stride length can impact gluteal muscle health. Loading is crucial for effective shoulder rehabilitation. Single exercises can be more effective than multiple in rehab. Rotator cuff injuries often relate to tendinopathy. Education coupled with exercise enhances rehabilitation outcomes. Shoulder pain can stem from both compressive and tensile loading. Strengthening the shoulder complex is vital for overall function. Psychological factors play a significant role in shoulder pain management. Maintaining activity levels is important to prevent underloading. In this episode of The Shoulder Physio podcast, I am joined by Dr John Kuhn, who is an orthopaedic surgeon and researcher from the United States. Doctor Kuhn and his colleagues, through the MOON shoulder group, have published several high impact studies investigating the management of individuals with rotator cuff tears and the results of these studies have caused some controversy (good controversy, that is). Rotator cuff tears are ubiquitous in musculoskeletal health care. They are both common and often burdensome, so it is important that shoulder surgeons and physiotherapists can agree on best practice management, for the betterment of the individual with the rotator cuff tear. Achilles. Rotator cuff. Patella. Hamstring. They're all structures involving tendons, and you probably either know someone who's injured them, or have experienced the dreaded tendon injury yourself. Aaron Rodgers experienced an acute Achilles tendon rupture on Monday Night Football in September 2023, just 4 plays into his first season as QB for the New York Jets. Fans watched as he dramatically pulled up lame after a tackle, having snapped his Achilles, and subsequently sitting out the 2023 season. Just this week he stated in an interview "my summer ends Sunday", confirming that he is back to play this upcoming 2024 football season for the Jets. So what the heck is actually going on? Why do tendons hurt sometimes? Why do they snap? Is it all just degenerative, "getting old"-type stuff? How can we prevent an injury like Rodgers' from happening to US?? In this episode, we reveal it all, friends! We have a world's expert in tendon pathology, treatment and injury prevention to tell us why our tendons get injured, what we can do to treat them and most importantly, the things we can do prevent them from breaking down. Jill Cook, PhD is a professor in musculoskeletal health in the La Trobe Sport and Exercise Medicine Research Centre at La Trobe University in Melbourne Australia. Jill's research areas include sports medicine and tendon injury. She is the "tendon guru" and has dedicated her career to learning about and improving our care of tendons. Jeremy recently had the privilege of attending Jill's educational lecture at the NBA Team Physician's national meeting, and she so graciously agreed to be our guest for this episode! Your Doctor Friends bring the professional-level expert education straight to you, friends! Topics covered in today's episode include: What actually ARE tendons? What do they do? Types of tendon injuries- including acute tears, "overuse" injuries, and degenerative "wear and tear". Risk factors associated with tendon injuries. What is "tendinopathy"? Is it there forever? What is the role of imaging/MRI in the diagnosis and treatment of tendon injury? What about shots? Do cortisone or platelet-rich plasma (PRP) injections help in tendon injuries? How do you actually treat tendinopathy? Can it be "cured"? Thanks for tuning in, friends! Please sign up for our “PULSE CHECK” monthly newsletter! It's time for another listener Q&A, Eric covers three questions from our audience in this week's podcast:What are your thoughts on high school students playing a winter sport? My son has played basketball and baseball since he was 4. He wants to play college baseball and decided as a sophomore he wants to quit all sports but baseball and focus on that. We think he is too young and should play basketball this winter and still attend his indoor baseball session (pitching). Thoughts?Rotator cuff drills are a key part of rehabilitation and prevention programs, but I'm struggling to see where all the different kinds fit into my program. Do you have any ideas? You previously talked about lessons learned in terms of Tommy John timelines. I know a lot of surgeons and rehab specialists have adjusted their approaches to surgeries and the associated timelines well. What are some of your key takeaways now that there have been some changes to the way elbow surgeries were approached in recent years?Support Our Sponsor: Hawkin Dynamics

PainExam Podcast Show Notes: Exploring Traumeel as an Alternative for Back Pain Relief  Claim CME The CE experience for this Podcast is powered by CMEfy - click here to reflect and earn credits: https://earnc.me/HoEWFd   Board Prep and Continuing Education    Introduction: Today, Long Island Based Pain Physician David Rosenblum, MD  delves into the latest advancements and alternative treatments for pain management. In today's episode, we shine a spotlight on Traumeel®, a homeopathic alternative gaining traction for its anti-inflammatory properties, with fewer reported side effects compared to corticosteroids. Understanding Traumeel: Traumeel, a fixed combination of diluted plant and mineral extracts, has been available over-the-counter in Europe for over 60 years. Contrary to corticosteroids, Traumeel's popularity has surged due to its limited side effect profile, with reported contraindications primarily linked to allergies [9]. View Full Calendar Scientific Insights: A study by Lussignoli et al. demonstrated Traumeel's efficacy in decreasing systemic interleukin-6 production and reducing edema, countering an unregulated inflammatory response [10]. In vitro studies revealed Traumeel's inhibition of pro-inflammatory mediators (IL-1β, TNFα, IL-8) in immune cells, suggesting its potential in stabilizing the immune system [8]. Notably, Traumeel's effectiveness seems to surpass the sum of its individual components, indicating a synergistic interaction [9-10]. Clinical Applications: Traumeel has shown efficacy comparable to nonsteroidal anti-inflammatory drugs (NSAIDs) in treating various inflammatory conditions. It is available in oral, topical, and injectable preparations, making it a versatile option for pain relief [8-9]. Research Gaps and Potential: Despite its established use, current research lacks information on Traumeel's efficacy in epidural injections for short-term back pain relief. No studies have compared Traumeel to corticosteroid injections, although the TRARO study protocol proposes a potential avenue for comparison in rotator cuff syndrome patients [11]. Pain Management Board Prep Clinical Cases: Five patients seeking back pain relief opted for Traumeel injections due to either a contraindication to or a preference against steroids. This real-world scenario lays the groundwork for further exploration into Traumeel's efficacy in epidural injections, providing additional pain-relieving options for patients unable to tolerate corticosteroid injections. Conclusion: Traumeel presents a promising alternative for pain management, particularly in cases where corticosteroids may be unsuitable, however the FDA has yet to approve it and therefore it has failed to gain traction in the US. [14] As we wrap up, stay tuned for future developments in the research landscape surrounding Traumeel and its potential role in enhancing pain relief options. It's a complicated system, a mechanical marvel, that has been designed for an impressive range of motion. The shoulder is made up of three bones - the clavicle (collarbone), the scapula (shoulder blade), and the humerus (upper arm bone). These bones work together through four distinct joints, namely the glenohumeral, acromioclavicular, sternoclavicular, and the scapulothoracic joints.Ackland, D. C., Pak, P., Richardson, M., & Pandy, M. G. (2008). Moment arms of the muscles crossing the anatomical shoulder. Journal of Anatomy, 213(4), 383-390. Surrounding these bones and joints is a network of muscles, tendons, and ligaments. This includes the rotator cuff, which comprises four key muscles providing stability to the shoulder joint, and the deltoid muscle, which powers the lifting of the arm.Lewis, J. S. (2016). Rotator cuff tendinopathy: a model for the continuum of pathology and related management. British Journal of Sports Medicine, 50(19), 1187-1191. This complex structure enables your shoulder to perform a myriad of tasks every day. From reaching for a can on the top shelf, to throwing a ball, or even brushing your hair, few body parts offer such diversity of movement. However, this benefit comes at a cost. This inherent mobility of the shoulder makes it susceptible to a range of injuries and conditions. Overuse, trauma, or strain can cause a variety of shoulder conditions, including rotator cuff tears, impingement, and frozen shoulder. The intensive use of the shoulder in sports, especially those requiring repetitive overhead movements, further raises the risk of injuries.Yang, J. L., Chang, C. W., Chen, S. Y., Wang, S. F., & Lin, J. J. (2017). Mobilization techniques in subjects with frozen shoulder syndrome: randomized multiple-treatment trial. Physical Therapy, 97(10), 1003-1013. While our shoulder's dynamic capability is a boon for performing diverse movements, it also increases the risk of injury and pain. In the following sections, we will delve into how everyday habits, such as posture, can further affect your shoulder health. The Common Causes of Shoulder Pain Understanding the causes of shoulder pain isn't always straightforward. While injuries and conditions related to the shoulder anatomy we've discussed are prevalent, there are other potential causes to consider. Age-related wear and tear is a common cause. With time, the cartilage in our shoulders naturally deteriorates, leading to conditions like osteoarthritis. A research article in the American Journal of Sports Medicine confirms that age-related changes can predispose individuals to a host of shoulder disorders, including rotator cuff tears and glenohumeral osteoarthritis.Minagawa, H., Yamamoto, N., Abe, H., Fukuda, M., Seki, N., Kikuchi,
