Show Notes   What is virtual care and how can it be used in treating patients with eating disorders? What are the limitations or advantages of using virtual care in treating eating disorder patients? In this podcast episode, Kate Fisch speaks with Dr. Erin Knopf about virtual care for Eating Disorders. She discusses the benefits and challenges around virtual care and the role it plays in assisting our clients and developing more effective treatments.   MEET DR. ERIN KNOPF Dr. Knopf is dedicated to treating patients with eating disorders holistically. As a triple-boarded physician (pediatrician, adult psychiatrist, child/adolescent psychiatrist) and Certified Eating Disorder Specialist, she uses her combined training with intentionality to assess disease severity and devise comprehensive plans to address the eating disorder behaviors as well as comorbid symptoms of anxiety, depression, OCD, trauma and more. She describes herself as the “mechanic of the body” and prioritizes psychoeducation and biological education for patients and families to foster insight and achieve treatment alignment. Dr. Knopf previously worked in an IP/RES/PHP program for almost 5 years and at the onset of the COVID-19 Pandemic, she served as unit psychiatrist for ten months at the ACUTE Center for Eating Disorders. The challenges faced with treating eating disorders. Childhood obesity, diets and restrictive eating. Lab results and eating disorders.   Do you feel like delivering care virtually gets in the way of connecting with patients? “I will admit, as an extrovert, I don't think there will ever be a way to replace the beauty of in person connection. There are nuances that are missed. But ultimately, would I turn away from virtual care? Absolutely not!” Dr. Erin Knopf   When we think about virtual care, the cons might be pretty clear, but the pros just far outweigh, especially when we're talking about someone getting care versus not. . .   “It's an opportunity to bring care to people who would never have it, it's flexible, convenient. So I think overall, the virtual care frontier is really exciting and we can only do it better with our technological advances.” Dr. Erin Knopf   What is a big challenge you are currently faced with? “Right now we are, unfortunately, private pay, which I know limits access, but that is not our end game. We are currently working on credentialing with three insurance companies.We offer discounted bundles for services. We do super bills, and we even have a single case agreement.” Dr. Erin Knopf   Eating disorders don't discriminate based on socioeconomic status. They also don't discriminate based on geographic location. It is important for all patients to have access to some level of professional care and help.   What are your thoughts on the American Academy of Pediatrics'recently issued a statement on childhood obesity? “The first part is it's still relying on BMI as the measure of obesity. It actually doesn't include in the definition any other medical considerations that people get worried about - type two diabetes, hypertension, hyperlipidemia, things like that.” Dr. Erin Knopf   The steps that follow such a statement are extremely drastic, especially when we consider its referral to children. . .   “ It just says if a child's body is 90th percentile for age match, BMI percentage, then they meet criteria for obesity and therefore should be considered for, drum roll, either weight loss surgery, like bariatric surgery, or weight loss medications.” Dr. Erin Knopf   What are your thoughts on why diets and restrictive eating are so common? “Human beings like diet culture because it helps them feel in control and powerful and capable of incredible discipline. Think of how often that's overly valued and praised.” Dr Erin Knopf   When treating somebody. What is your number one concern, medically? “Medical stability and medical fragility are the terms that I consider the most. Everyone is pretty accustomed to picturing an emaciated person who meets criteria for eating disorders and has low heart rate, low blood sugar, low body temperature. But really, only 6% of patients with eating disorders are actually underweight. All it takes is 15 pounds of weight loss in three months and your body is malnourished to some degree..” Dr Erin Knopf   How do we handle clients who have good lab results but may still have an underlying issue? “Your body is miraculous, that it is going to work for you and protect you as best it can by improving the movement of electrolytes, of improving the mobilization of energy molecules from stored tissue in order to meet your metabolic need. But eventually that will give out. And treatment should not occur when you are in the middle of crisis and your body is comping out. It needs to happen sooner than that.  So if the behaviors are there and the weight loss is there or the weight interruption for whatever way it looks is there, yeah. you meet criteria in my book, and I think it is important that we have more early recognition and prevention.” Dr Erin Knopf     The one thing that virtual care provides is access to those patients who may not have thought they were able to get treatment and thus there's motivation to get better. Especially now that virtual care has been supported nationally. Hopefully it continues to get the same support even with laws changing. Now that the Pandemic emergency actions are expiring, this does give us an opportunity to bring that expert care to corners that would never have known expert care mattered, would never have known that they actually do meet criteria for an eating disorder and deserve that expert care. Let's talk body stuff.  So much of what happens to someone's body when they engage in eating disorder behavior is individualized. It's based on genetics, as well as other factors that we don't quite understand. The complexities of these effects are multifaceted, and to some extent, the specific causes may not matter.  In this episode, I'm joined by Jennifer Gaudiani. Jennifer L. Gaudiani, MD, CEDS-S, FAED, is the Founder and Medical Director of the Gaudiani Clinic. Board Certified in Internal Medicine, she completed her undergraduate degree at Harvard, medical school at Boston University School of Medicine, and her internal medicine residency and chief residency at Yale. You might be wondering what do we do in terminal cases of Anorexia. Terminal Anorexia is different than Severe and Enduring AN- where sufferers do often make a recovery after 20 years (research shows). Terminal AN is diagnosed when the sufferer will die from malnutrition within 6 months. This is an extremely small portion of AN sufferers that often falls through the cracks of dignified, whole person care. Dr. Philip Mehler founded the ACUTE Center for Eating Disorders & Severe Malnutrition at Denver Health and continues to serve as its Chief Executive Officer. He began his career at Denver Health more than 30 years ago and was formerly its Chief of Internal Medicine. He was Denver Health's Chief Medical Officer (CMO) for 10 years until he was promoted to its Medical Director, a position he held until his retirement in 2014. He is also the Glassman Professor of Medicine at the University of Colorado School of Medicine and has conducted research into the optimal medical treatment of the most severe cases of Anorexia Nervosa and Bulimia.  Dr. Mehler has authored more than 500 scientific publications, including three textbooks published by Johns Hopkins University Press and with a fourth edition slated for release later this year. Dr. Mehler was the recipient of the Academy of Eating Disorders 2012 Outstanding Clinician Award, has been recognized among the “Best Doctors in America” for the past 22 years in a row, and was voted the “Top Internal Medicine physician in Denver” multiple times by 5280 Magazine.  Dr. Mehler is a member and fellow of the Eating Disorders Research Society and the Academy of Eating Disorders, as well as a member of the editorial board of the International Journal of Eating Disorders and serves as the Senior Editor of The Journal of Eating Disorders. Our society is sick. Diet culture, the beauty industry, and the media have sold us a narrowly defined ideal and it has warped our views on food, our bodies, and beauty. Dr. Gauidiani completed her undergraduate degree at Harvard, medical school at Boston University School of Medicine, and her internal medicine residency and chief residency at Yale. In 2008, she was one of the founding team members of the ACUTE Center for Eating Disorders at Denver Health and, after seven great years there, she left as its Medical Director to pursue her vision of outpatient care, founding the Gaudiani Clinic. During her years at ACUTE, Dr. Gaudiani became a nationally recognized internist for her work on the medical complications of eating disorders. She also wrote a book on this same topic, released in 2018, Sick Enough: A Guide to the Medical Complications of Eating Disorders.Dr. Gaudiani has lectured nationally and internationally and is widely published in the scientific literature as well as on blogs. She is one of the only outpatient internists in the US who carries the Certified Eating Disorder Specialist and Supervisor designation and has received the honorable designation of Fellow from the international organization Academy for Eating Disorders. In our time together, Dr. Gaudiani covers so many important topics, including the shocking percentage of people whom she believes have a disordered relationship with food, how diet culture has impacted the medical field, and how most doctors don't understand eating disorders. She also talks about thin privilege, weight stigma, weight-inclusive medical care, and what a balanced relationship with food and our bodies looks like. Dr. Gaudiani's wisdom and compassion are palpable in her work and in this episode. My hope is that her experience and insights might help us identify and shed the harmful messages we have absorbed, so we might move into a more peaceful relationship with food, our bodies, and ourselves.

Dr Phillip Mehler, MD, CEDS Don't be afraid of IBS these are very real concerns  Trabecular bone score and DEXA together inform treatment  Eating disorders as Metabo-psychiatric illness (along with Dr. Bulik)  If you're in ER Medicine, you're seeing a good number of folx with EDs.  Patterns of the microbiome that predict weight gain.  Why patients can't hear and feel like they are talking in a tunnel, and what's going on in the olfactory area  “Atypical” AN = bradycardia very marked, refeeding hypophosphatemia is very real, a bone disease with rapid weight loss even though BMI is in a normal range.  Amenorrhea is common.  We can no longer applaud drastic forms of weight loss.  The focus at ACUTE includes IBS and cancer cachexia.  Angel Med Flight is a covered service for ACUTE Cross-reference Dr. Voss's episode on differentials. She has completed training to become a pelvic health physical therapist and completed her certification as a pre and postnatal coach through Girls Gone Strong.  Throughout her career, Michelle has advocated for physical therapy to be a recognized profession in the field of eating disorders and, in February 2020, became the first physical therapist to become a Certified Eating Disorders Specialist (CEDS) through iaedp™… and hopeful to be among many other physical therapists to hold this designation soon! An enthusiast for movement, Michelle Laging became a fitness instructor and worked part-time for both 24-Hour Fitness and Curves in 2003. After shadowing a few Physical Therapists, it became obvious that this was the path she needed to pursue to fulfill her professional needs. Michelle graduated from the University of Colorado Anschutz Medical Campus in 2008 as a Doctor of Physical Therapy and began her career at Denver Health Medical Center in the late fall of 2008.   By day I am a doctor, by night I am getting my hands-on any outdoor adventure. Saw a gap in the care of the health of the female athlete. Found she was able to blend her experience with medical complication secondary to disorder and issues seen within athletic population specifically female athlete. Trying to change the culture of amenorrhea being a natural consequence. The loss of a menstrual cycle is a RED flag! For every year without a menstrual cycle, you can lose 2% bone density Debunks why birth control is not beneficial at protecting or rebuilding bone. Is this an athlete with a good strong heart or an athlete under fueling causing a “starved” heart? Feels there has been a great effort to get the word out on RED-S Under fueling in sport doesn’t just happen in females. Only 37% of physicians in a survey could identify the components of the female athlete triad. In any medical system many times you have to advocate for yourself.     Where you can find Dr. Rosen: So you know she is legit:   Dr. Rosen is a board certified in internal medicine and eating disorders expert physician at the Gaudiani Clinic.
