One of the best things about the Dental Hacks podcast was our Brain Trust segment! Several Very Dental people sitting around talking shop. And now...it's back! Alan is joined by OG Brain Trust members Dr. Dawn Grimes, Dr. Frank Clayton and Dr. Melissa Zettler to discuss the evolution of their careers in dentistry. They delve into their early experiences in dental school, transitioning to practice ownership, and the challenges of hiring and mentoring new dentists. The conversation touches on the impact of social media on the dental profession, the complexities of associate expectations, and the unique relationships built with long-term patients. Being a "seasoned" practitioner give you some insights that these younger dentists don't really understand. As much as you might not want to hear it...you've got to pay some dues! Alan takes a trip down memory lane, revisiting "The Scallion," a satirical dental news project he and colleagues created (inspired by The Onion) back in the Dental Hacks era (circa 2017-2018). In this episode, Alan reads several archived "Scallion" articles, finding they still hold up with humor poking fun at: Aggressive sales reps at dental meetings. Dentists avoiding shop talk at parties. Exaggerated links between periodontal disease and other health issues. Hilariously inaccurate dental stock photography. The shocking revelations that can come with using dental loupes for the first time. Over-the-top (and potentially condescending) marketing advice. The classic "I hate dentists" patient comment. Stubborn dentists arguing online. We don't laugh at the profession enough. This should help! Episode Summary In this episode of Practice Growth HQ, I speak with Dr. Alan Mead about the hidden mental health challenges faced by healthcare professionals. What I call the "underbelly of healthcare". Alan is a Michigan-based dentist and host of The Very Dental Podcast, formerly Dental Hacks, with over 4 million downloads. He's also a recovering addict with a 20+ year journey through alcohol and opiate addiction, and recovery. In this raw, powerful conversation, Alan opens up about what led to his addiction, how he coped with stress, and the moment that changed everything. If you're feeling the weight of stress in your practice or know someone who is, this is a must-listen. What you'll learn: (15:50) The slippery slope from alcohol to opiates in a dental practice (17:45) The turning point. A surgical sedation gone wrong and a friend's intervention (20:05) What recovery really looked like and why relapse wasn't part of Alan's story (22:10) The 30-day test to check if you might have a problem (23:30) How to support someone you love without pushing them away (25:10) Why therapy isn't just for people in crisis And much more! Alan welcomes back Dr. Mike Melkers for a wide ranging conversation. Mike and Al go WAY back (DentalTown in the "oughts" anyone?) and we catch up a little bit. Today is a throwback episode featuring Kevin, Zach and Al talking about how they've evolved on caries removal. It's a classic and I hope you enjoy it! Original show notes: Dr. Alan Mead of the DentalHacks podcast was gracious enough to join us again this week. And we start off with old school internet videos: Bonus: He got a web redemption! Then we got into some hot caries removal talk. What burs do you use? To dye, or not to dye? Do you "leave" any caries? Why or why not? Caries removal is a divisive topic. Did we get it right? Wrong? This throwback episode features a replay of a Dental Hacks episode featuring Brian Colao of Dykema! Brian Colao knows DSOs. He's an attorney that helps people create and administer DSOs. He's been a frequent guest on the Dental Hacks and he gave Jason and Alan some of his time at the recent Voices of Dentistry meeting. We went deep on the topic of DSOs and covered a lot of information: What is a DSO? (spoiler: "A DSO is an entity that provides nonclinical administrative support services to a dental practice that is owned by a licensed dentist.") From one practice to thousands When did we start to really see DSOs take hold The episode where we met Brian, "DSOs and Digital Smile Design (DHP186)" Was there a DSO bubble? Denovo vs. Acquisitions What to do if a DSO approaches you? How do you research a DSO that might want to acquire your office? Brian thinks that there is probably a DSO that could fit most any office...go to a conference What are DSO deals looking like for selling dentists? The facts from Brian's mouth to your ears: DSOs are at 23% market penetration in the U.S. dental market right now. He predicts that there will be 80% market penetration in 7-10 years. Today we're featuring a throwback to the Spear Summit all the way back to 2019! Jason and Alan were still podcasting as the Dental Hacks and we had the pleasure of sitting down with Dr. Jim McKee. Jim is one of a special breed of dentists that has created a practice that focuses on pain patients and patients with TMJ issues. How did Jim become interested in occlusion and TMJ? How has imaging changed the game for Jim's patient treatment? "The imaging isn't for you. The imaging is for the patients!" How does pain relate to malocclusion and why is pain often the last thing to show up? How does Jim "front load" the patient education process? What are Jim's favorite educational aids for TMJ/pain? How did oral surgeon Dr. Mark Piper influence Jim's philosophy? Which comes first: airway or joints? What's the best way to learn TMJ? How is he using CBCT in his office? How does he use the MRI? How did Jim build his restorative practice using the what he'd learned about TMJ and occlusion? Can you create new patient flow by focusing on TMJ? Why shouldn't TMJ be a specialty? Are TMJ specialists a "dumping ground?" Direct vs. indirect treatment options for joint/muscle pain and the 4 R's What is the best way to screen for TMJ issues? What questions should you ask? How can you find a TMD specialist in your area? What questions should you ask? Who should be on the interdisciplinary TMD team? Today we throw back to one of the greatest Dental Hacks episodes of all time! Jason and Alan had the honor of talking with Dr. Frank Spear of Spear Education! This was their first conversation with him since the COVID outbreak and the conversation focused a lot on changes in the landscape of dentistry and dental education! How has COVID affected dentistry and dental practices? Some dental practices have done BETTER in 2020! What are the things that these practices have in common? Messaging (stay in touch, let them know you're available!) Proactive messaging about safety "We're concerned about your dental health." Efficiency ("if you're coming in, let's get it done!") "The Big Dip" (hygiene scheduling) The history of dental infection control...why is dentistry ahead of this curve? How is "on campus" going at Spear Education? "Live Virtual" seminars...better than in person? Faculty club round tables...the best value at Spear! The new occlusion curriculum on Spear Online! New research in occlusion...now incorporated into the Spear curriculum! Cause and effect is often not as simple as we'd like What cases does Frank wish he could have a time machine to re-treat (spoiler: it's about airway and orofacial pain) What does Frank think about Orofacial Pain becoming a specialty? This week we have a very special episode for you! You may already know that I've interviewed other house-call dentists here on the show. I'm excited to share that I recently had the honor of interviewing one of the very first dentist podcasters! If you're listening to this show, it's likely that you listen to other dental podcasts too, so you may very well know this week's guest, Dr. Alan Mead! Alan is a pioneer in the space of dental podcasting. Communication and education in dentistry has always been extremely formal. There are dental schools and dental conferences, but thanks to Alan there is the Very Dental Podcast, formerly known as the Dental Hacks podcast. You likely know, especially based on last week's episode, that I feel strongly about connecting with other dentists. Even though he isn't a house-call dentist, Alan values a lot of things that you and I value, like time freedom and having other interests outside of dentistry. In this conversation, we address his early steps into dental podcasting, business ethics, mindset, and so much more. From discussing the challenges faced by dentists looking for alternatives to private practice to exploring the importance of work-life balance and developing a supportive community, this episode offers valuable insights and inspiration for dental professionals and individuals considering a career transition. In addition to my interview with Dr. Mead, I'm excited to share that the ResiDENTAL Movement Official Membership is finally available! If you're interested in connecting with me and other dentists on their house call journey, please visit our membership page to enroll. In this private community, we'll share exclusive content and challenges, but we'll also meet monthly for Q&A. We'll meet another time each month, too, for Book Club. Finally, Official Membership includes discounts on any other ResiDENTAL Movement offers. I hope to see you inside of this official membership. Thank you so so so much for tuning into this week's episode. I am so grateful for your loyal listening. There was once a time where we started a podcast. A podcast by dentists, for dentists. And in the very first episode we were OBSESSED with Botox. That's right, I'm throwing back to the very first episode of the Dental Hacks podcast. The podcast features Dr. Jason Lipscomb and myself interviewing the one and only Dr. Lance Timmerman followed by a Brain Trust featuring Dr. Tammy Bailey, Dr. Jason Luchtefeld, Dr. Dawn Kulongowski and Dr. Andy Hayes! To be honest, we weren't great. At least the technology wasn't great. But there was magic from the very first episode. I hope you enjoy the very first podcast we produced! Today's throwback episode was from all the way back in 2016! This throwback episode reaches all the way back to 2019 where the Dental Hacks talked with Dr. Steve Ratcliff at Spear Education! Steve was an early team member of Spear Education (all the way back in 2009 when it was the "Scottsdale Center") and helped create the way study clubs worked and the online offerings at Spear Education. Steve gave us a pretty intimate look into his past and his evolution as a clinician and educator. We think you'll walk away with a lot from our talk with Dr. Ratcliff. Bill Handel stresses caution when approaching trending online 'DIY hacks', especially when they pertain to your mouth; dental 'hacks' online are frightening your dentist and could end up being dangerous and painful. And time seems to fly by quicker the older you get - why is that?
Years before COVID and the whole "Very Dental" thing, the Dental Hacks would show up at the Chicago Midwinter and record at a booth! This was a sound check from some years ago that I think is one of the funnier things we ever did! I hope you enjoy! The original show notes: I promised Jason that I wouldn't put this one out. Promise broken. Jason and Alan decided to do an impromptu sound check podcast at the Premier Booth at the Chicago Midwinter and the result was hilarious. If we have any Russian listeners please let me apologize in advance for our horrible accent. We discuss the stuff we were excited to check out at the Midwinter, the McCormick Center security team, Jason's discomfort with women and endodontics and Alan's inability to fit into black lab coats. A little over two years ago our profession was faced with something we'd never experienced. The pandemic closed most of us down in March of 2020. We had a LOT of time on our hands and not many answers. Jason and I turned to Dr. Gary DeWood to help us make sense of things. If you remember correctly, there were a LOT of things being stated as fact (negative pressure operatories, anyone?) that seem kind of silly in retrospect. So I thought I would present this podcast a couple years down the road to see how we fared. Let me know what you think! --Original show notes from the March 27, 2020 release: Jason and Alan had the privilege of speaking with Dr. Gary DeWood from Spear Education about what we can expect during the Coronavirus crisis. Gary's message is one that we all could use right now and we're grateful for him giving us some time. Some of the things we discussed: What do you do when you find yourself at the edge of the cliff and you don't know what to do? This is a "government mandated" economic downturn which makes it interesting and scary What should we be doing with our teams? What about the potential bill being passed? Tax credits vs. tax "cut" Do a 4 week "break even" Own the fear/angst "It's hard to make a plan when you don't know what the rules are" Furlough vs. layoff What should we expect with regard to infection control? Are "universal precautions" still universal? testing and how many people actually have the virus? hopeful information about this virus what's a the story on the n95 mask How is coronavirus like a horrible game of telephone Join the Very Dental Facebook group using the password "Timmerman," Hornbrook" or "McWethy." Who better to talk about then yourself? For episode 200, Elvis and Barb have turned the microphones on themselves. When asked “who should be on episode 200", multiple people said it was the hosts turn to sit in the hot seat. They asked a very special friend and the creator of the “Fan of Voices From the Bench (https://www.facebook.com/groups/4141108352651184)” Facebook page, Joe Young from Young Dental Laboratory (https://youngdentallab.com/) in Philadelphia to take over host duties. Temporarily. Joe asks the hard-hitting questions like how they got into the industry, how the podcast came to be, and what have Elvis and Barb learned over the last 199 weeks. Don't worry, they didn't learn much..... It's oscillating, multi-speed stirrer produces a tornado-like vortex every 30-seconds, and guarantees efficient, effective and powerful cleaning whether the units are individual or still attached to the build plate. The two alcohol baths make an effective step-wash system. It cleans more efficiently and there is less alcohol needed, since it is re-used for both a fresh bath and a dirty bath. The VeriWash's intelligent design offers features such as Mode, Time, and Start/Stop button display which gives the operator full and automatic control of the cleaning process. A mesh basket used in the wash container makes it easy to keep track of small printed parts when cleaning. The affordable unit's one-year warranty ensures peace-of-mind for the owner. They help us create content that is, for lack of a better word...Very Dental! Why should you care about single patient use burs? I can think of a lot of reasons, but the one your patients are thinking about is this one. What's that sound? That's the sound of a rotary instrument that's never been used on another patient. That's the sound of no cross contamination. That's the sound of your patient knowing that you are looking out for them. Sure you get a super sharp, never been used bur each and every time you open a Microcopy bur, but mostly your patients know that you take cross contamination seriously. Go check out the wide variety of single patient use products at verydentalpodcast.com/microcopy. A new (and sterile) bur…every time. Microcopy Dental. Ideal preps. That's what we aim for. One excellent way to get there is to use a core material to build the tooth back up to ideal contour. Cosmecore from Cosmedent is my choice for building up a broken down tooth. It's dual cure, comes in a variety of colors and cuts just like a tooth. You'll get a super smooth finish with no gouging. Right now I've got the 50 gram dispenser that fits in a small impression gun, but when I need to refill I think I'll go with the 8 gram syringe. Both work great, but those syringes are handy and don't take much room on a set up. Check it out at verydentalpodcast.com/cosmecore! It's funny, but you never really know how your branding effects people until you see it in your own life. So Jacob, my youngest 11 started Invisalign with a local orthodontist. He's following all the instructions, including taking them out to eat. So I'm CONSTANTLY seeing a case for his aligners with our orthodontist's brand. Like every single day. I'm realizing that cases for aligners, splits and removable offer a brilliant way for people to remember your office and your brand! And then I thought of our friends at Zirc who offer aligner sized and denture sized boxes in tons of colors that you can customize with YOUR logo! They're offering a buy one, get one deal on retainer or denture boxes with your logo and 4 lines of custom print! Back when this podcast was called "The Dental Hacks" we used to include a segment in most shows that we called "Go Hack Yourself." It was a chance for us to talk about something we'd been using in our practice, listening to, reading, eating watching on Netflix or whatever caught our fancy at the time. I'm bring in this tradition back starting with today's episode! I'm going to drop some spicy recommendations for the Very Dental Nation to try. Or not try. Either way is cool with me. Recommendations in today's episode: Berman Instruments #3 and #4 Flexible-Thin Composite instruments "Land of the Giants: The Apple Revolution" from Recode and Vox Quad Lock phone cases/mounts Waterpik Sonic 2.0 flossing toothbrush Some links from the show: The Voices of Dentistry (use code VeryDental) The Very Dental Facebook group If you would you like support the Very Dental Podcast Network you should support our sponsors! They help us create content that is, for lack of a better word...Very Dental! The Very Dental podcast is brought to you by our friends at Microcopy Dental. Neodiamonds from Microcopy Dental are very fine. I mean, all of them are awesome, but some of them are actually “very fine.” Specifically I mean the yellow striped ones they call “finishing” diamonds. I use Microcopy finishing diamonds on just about any patient that I might be spinning a bur on. Very fine finishers do the heavy lifting on composites so polishing is simple and easy. I use them to finish crown preps. I've been known to adjust Emax and Zirconia with them. Once you try them you're going to find a ton of places to use them! And just like all of Microcopy's burs, they're sterile out of the package and single use means they're sharp and efficient and can be disposed of at the end of the appointment! Go check them out at verydentalpodcast.com/veryfine (all one word)! Cosmecore by Cosmedent is one of the most versatile materials in dentistry. I've come up with a lot more ways to use it than just core buildups. What new ways have you found to use it? Check it out at verydentalpodcast.com/cosmecore! My 11 year old son is in Invisalign with one of the orthodontists that I refer to. He's doing REALLY well for an 11 year old boy. I see the case for his aligners all the time. With the orthodontist's logo gloriously blazed across the lid. And then I thought to myself...aligners, splits and removable offers a brilliant way for people to remember your office and your brand! And then I thought of our friends at Zirc who offer aligner sized and denture sized boxes in tons of colors that you can customize with YOUR logo! In this Encore Edition of the Dental Hacks Dr. John Khademi and Dr. Eric Herbranson join Alan to talk about the state of endodontics, conserving dentin and how we know what we know. I got to know Dr. John Khademi in the mid-2000's from Dental Town. I was introduced to Dr. Eric Herbranson in 2011 at the Academy of Microscope Enhanced Dentistry where he talked about the 3D Tooth Atlas he had created. Both are influential endodontists who have controversial ideas about how we've been treating teeth over the years. Today's episode features a journey from patient based outcomes to case selection, access openings to how we know what we know. There's a fair amount of hero worship here I've been a huge fan of these guys for quite some time. And a bonus...the one and only Dr. David Clark of Bioclear sits down at the end and talks endo! If you like endo (or even if you don't), this interview will change the way you look at saving teeth! Dr. Mike Skramstad and Dr. Meena Barsoum are part of the team at CEREC Doctors and they sat down to talk with Jason and Al! We talk about the history of CEREC and education in the chariside CAD/CAM world. This history is very interesting and the future is even more so! Some of the topics we discussed: Is German a harder language to learn than Spanish? Does Rich Rosenblatt thing Meena is the worst student he's ever had? How many CEREC mills do they actually have at CEREC Doctors (spoiler alert: it's 36!) What's the sweet spot for length of video? When did the hands on courses switch to Primescan? Is now the very best time to be a CEREC dentist? What's the future of CEREC software? Will it ever be separate from the hardware? Will milling outside of the CEREC ecosystem always be for experts? Implant workflows in the CEREC system (milling custom healing abutments anyone?) How has seeing CEREC failures helped with preparation styles? What is a "ghost contact?" How much are user concerns/suggestions taken into account for software/hardware upgrades? Edentulous arches with CEREC Primescan and digital dentures Is CEREC Doctors an international phenomenon? Dr. Michael Gunson is an oral surgeon who does just one thing. Orthognathic surgery. He joins Jason and Alan to talk about his understanding or the TM joint, occlusion and how that's changed over the years. Some of our topics include: Treating the TMJ is complicated. No two joints are the same. No two patients are the same. What is temporomandibular dysfunction? Does the literature measure what we think it's measuring? How is treating TMJ troubles like understanding schizophrenia? How does communication, listening and empathy relate to treating pain and dysfunction patients? Nose breathing and pattern recognition Pathology isn't in the thing, the pathology is in the relationships of the things Adaptation and asking the wrong question Hormone imbalances and classic presentations of TM dysfunction/facial development The checklist gets longer and longer (like a CVS receipt) What is interdisciplinary treatment? Where do dentists fit in facial esthetics? Memorie Valentine joins Alan to consider the idea that patients don't really understand their condition. Even when we do a decent job of explaining what we're seeing, how well do patients really understand it. Sometimes it's in the patient's interest to NOT understand? Lots of wisdom and humor coming from the Dental Hacks favorite hygienist! You should have a system for polishing. Our friends at Microcopy Dental have just the thing! Different types of composites have different characteristics depending on particle size, etc. Ohhh...that's right. It seems like all you ever hear about composites is "universal." But let's be honest, one size does NOT fit all! If you do any anterior composite work, you really need a microfill! Microfilled composites like Cosmedent's Renamel microfill handle really nicely, have gorgeous translucency and polish like a dream. If it's been awhile since you've used microfill, you owe it to yourself to give it a try again! Our friend and mentor Buddy Mopper was right! Don't dumb down composites to just one type! Get yourself a microfill and see what a difference it makes. It comes in syringes AND compules and I guarantee you'll find a bunch of great uses for it! Check it out at dentalhacks.com/microfill! The Dental Hacks have taken a LOT of CE over the years. What no one ever wants to tell you is that some really great CE wasn't a great fit. Some highlights: Should you borrow money to take CE? Choose CE that's realistic for your practice, your budget and your personality it's OK if you don't like certain parts of dentistry. In fact, it's NORMAL! Peer pressure/social media is NOT a good way to choose CE You should have a system for polishing. Our friends at Microcopy Dental have just the thing!
