Cracks put teeth at risk of infection and fracture if left untreated, but treating cracks in teeth can create many challenges for practitioners. Removing cracks without good magnification, visibility and an understanding of crack propagation puts the nerve and critical tooth structure at risk. Once the crack is removed, the non-standard defect requires specific techniques to bond to deep areas of the tooth while maintaining a strong bond.Dr. David Alleman, DDS, first began researching treatment options for cracks because he recognized that full coverage crowns weren't yielding predictable results. He directs the Northwest Center to Reduce Oral Health Disparities. He holds academic appointments at Case Western University, University of Rochester, and University of California, San Francisco. He maintains a dental practice limited to the care of fearful patients and served as Director of the UW Dental Fears Research Clinic. Dr. Milgrom's work includes research on xylitol, the effectiveness of fluoride varnish and iodine in preschoolers, clinical efficacy and safety of diammine silver fluoride, motivational strategies to increase perinatal and well child dental visits in rural communities, and studies of cognitive interventions in pediatric and adult dental fear. The NIH, Maternal and Child Health Bureau, HRSA, and the Robert Wood Johnson Foundation support his work. Dr. Milgrom is author of 5 books and over 300 scientific articles. His latest book, Treating Fearful Dental Patients, was published in 2009. Dr. Milgrom was Distinguished Dental Behavioral Scientist of the International Association for Dental Research for 1999. In 1999, and again in 2000, his work was recognized by the Giddon Award for research in the behavioral sciences in Dentistry. He received the Barrows Milk Award from IADR in 2000, recognizing his work for public health including the development of the Access to Baby and Child Dentistry (ABCD) program in Washington State. In 2003, Dr. Milgrom received a Special Commendation Award from the National Legal Aid and Defenders Association and the University of Washington Medical Center Martin Luther King, Jr. Community Service Award. In 2010, he received the Aubrey Sheiham Research Award for his work on xylitol. He serves on scientific review committees for the NIDCR, NIMHHD, NINDS, Center for Scientific Review at NIH and as a consultant to the FDA. In 2005, Dr. Milgrom was appointed the SAAD Visiting Professor of Pain and Anxiety Control at the King's College Dental Institute, University of London, UK for a six-year term. In 2008 he was awarded the degree of Doctor Honoris Causa from the University of Bergen, Norway in recognition of his work in social and behavioral dentistry. In 2012, he received the University of California, San Francisco Dental Alumni Gold Medal for his contributions to Dentistry. In 2012 he was also awarded the Norton Ross Award for Excellence in Clinical Research by the American Dental Association. In 2013, he was appointed to the Council of Scientific Affairs of the American Dental Association. In 2014, he received the Irwin M. Mandel Distinguished Mentor Award from the IADR. In 2015, he served as HMDP Expert in Dental Public Health for the Singapore Ministry of Health. Dr. Milgrom received his DDS from the University of California, San Francisco in 1972 and had a previous position at the National Academy of Sciences, Engineering, and Medicine. In the last few years, Dr. Milgrom has spoken to dental associations in Argentina, Colombia, Peru, Philippines, and USA and at major universities in USA and abroad.

She is dedicated to bridging the gap between medical knowledge and patient understanding and reducing health disparities. She has led projects funded by the NIH and HRSA. Dr Lee is involved in teaching, clinical practice, and research. In addition to her academic pursuits, Dr. Lee is actively involved in leadership, community outreach and education initiatives. Dentists have been attempting to treat cracked teeth for decades, with the goal of alleviating patient pain and conserving teeth through restorative treatment. But as Dr. David Alleman, DDS, describes in this episode, “it's a coin toss of success.” Methods such as retention to hold the crack together or bonding over the crack with adhesion are unpredictable. Sometimes the treatment works, and sometimes it doesn't, leaving patients and practitioners frustrated. The solution: treat cracked teeth like engineers have been treating cracks for over a century. Special Guest: Dr. Alessandro Villa For more information, show notes and transcripts visit https://www.ada.org/podcast Show Notes In this episode, we dive into the new ADA Living Guideline Program, the first in oral health, and learn all about this new initiative and its first set of recommendations on oral cancer. Our special guest is Dr. Alessandro Villa, DDS, PhD, MPH, Chief of oral medicine, oral oncology and dentistry at Miami Cancer Institute, where he also serves as program director of the Oral Oncology Fellowship. He is a Professor at the Herbert Wertheim College of Medicine at Florida International University. Dr. Villa specializes in oral mucosal and salivary gland diseases, oral precancers and oral complications related to cancer treatment. Before joining Miami Cancer Institute in 2022, he was Chief of Oral Medicine and Oral Oncology at the University of California, San Francisco (UCSF); he also directed the oral medicine residency programs at UCSF and Harvard University. His research focuses on oral leukoplakia, cancer therapy-related complications, and the prevention of HPV-associated oropharyngeal cancers. Dr. Villa is actively involved in several investigator-initiated and sponsored clinical trials and has authored more than 180 peer-reviewed publications. Dr. Villa provides an in-depth overview of the ADA's Living Guideline Program, explaining its purpose, how it works, and the key ways it differs from traditional guidelines. Living Guidelines are developed with the same level of rigor as traditional guidelines, but they use a distinct infrastructure design for continuous updates to emerging evidence, Dr. Villa shares. The development of these Living Guidelines is driven by a multidisciplinary panel that typically includes general dentists, dental hygienists, oral medicine and oral pathology specialists, oral surgeons, and other relevant experts. These panels also incorporate patients and additional stakeholders. Dr. Villa shares that they use the great evidence-to-decision framework to help dentists efficiently move from data to recommendations in a timely manner. All of this is built inside a digital platform. When evidence is conflicting or limited, Dr. Villa explains, the transparent process for Living Guidelines ensures clarity and integrity. The first set of recommendations from the ADA's Living Guideline program is on the evaluation of potentially malignant disorders in the oral cavity. Dr. Villa gives us more details about these recommendations, and the findings to date.
