Welcome to the annual Patreon Summer Sampler where you get an idea of the content we offer weekly, exclusively on Patreon.September 20, 1994. Los Angeles, California. Alfredo Newball returns to his apartment and discovers that his 32-year old pregnant wife, Cecilia Newball, and his six-year old stepson, Rene Perez, have vanished without a trace and Cecilia's Jeep and all of their personal possessions have been left behind. Even though Cecilia is only two weeks away from giving birth, Alfredo is soon mailed a typewritten note in which Cecilia reveals that she and Rene have run off with a doctor from Honduras. Prior to her disappearance, Cecilia had been contacted by a woman who wanted to arrange a meeting to show her some baby furniture on the day she went missing, but she cannot be identified and Cecilia and Rene are never found.February 21, 1991. Las Cruces, New Mexico. 35-year old Edyth Warner, an art student at New Mexico State University, leaves her class and does not return, and 11-year old Nicholas Smith, Edyth's son from a previous marriage, also seems to go missing at the same time. According to Edyth's current husband, Henry Warner, Edyth took Nicholas and went to stay at a friend's house and never came back, but she inexplicably left behind the couple's three-year son, Andrew. Investigators eventually come to believe that Edyth and Nicholas were the victims of foul play, but no suspects are named and the case is never solved. On this week's episode of “The Path Went Chilly”, we explore two unsolved missing persons cases in which a mother and son disappeared together.patreon.com/julesandashleypatreon.com/thetrailwentcoldAdditional Reading:https://unsolved.com/gallery/cecilia-newball-rene-perez/https://unsolvedmysteries.fandom.com/wiki/Cecilia_Newball_and_Rene_Perez_Jr.https://charleyproject.org/case/cecilia-elizabeth-newballhttps://charleyproject.org/case/rene-perez-jrhttps://www.latimes.com/archives/la-xpm-1994-09-29-me-44410-story.htmlhttps://web.archive.org/web/20130718060712/http://www.lacountymurders.com/wanted/099-00062-3199-011.pdfhttps://charleyproject.org/case/edyth-ann-warnerhttps://charleyproject.org/case/nicholas-vincent-smithhttps://www.latimes.com/archives/la-xpm-1991-10-30-vw-653-story.htmlhttps://www.newspapers.com/image/431536244/https://www.newspapers.com/image/997619150/

CoROM cast. Wilderness, Austere, Remote and Resource-limited Medicine.

This week, Aebhric O'Kelly is joined by Bill Vasios and Rhod Jordan for an interactive discussion on one of the most important and often overlooked areas of remote medicine: Austere Primary Care.Whilst trauma often dominates training, the reality is that most remote clinicians spend far more time managing diarrhoeal illness, skin infections, musculoskeletal injuries, respiratory complaints, fever, environmental illnesses, and preventative healthcare. The panel discusses the clinical mindset required to work in resource-limited environments, introduces CoROM's CHART patient assessment system, and explores practical approaches to diagnosis when advanced investigations are unavailable.Whether you work in expedition medicine, offshore healthcare, humanitarian operations, military medicine, or remote primary care, this episode provides practical insights into becoming a more capable austere clinician.Chapters 00:05 – Welcome to CoROM Conversations00:17 – Why primary care matters more than trauma in remote medicine01:10 – Defining austere and resource-limited environments02:00 – Developing the austere clinical mindset02:40 – Becoming comfortable with uncertainty and limited resources04:00 – Building confidence through deliberate practice05:00 – Wilderness medicine and learning outside the ambulance07:15 – Resilience and supporting expedition teams08:00 – Common primary care presentations in austere environments08:15 – Gastrointestinal illness, dehydration and traveller's diarrhoea08:45 – Skin disease, wound care and blister management09:30 – Environmental illness: hypothermia, frostbite and heat injury10:10 – Musculoskeletal injuries in expedition medicine11:55 – Learning orthopaedic assessment through repetition13:00 – ENT, eye and dental emergencies14:20 – Introducing the CoROM CHART assessment framework16:40 – Chief Complaint, Condition and the CPRO assessment17:20 – Recognising the critically ill patient20:15 – History taking using SAMPLER with additional risk assessment22:00 – Secondary assessment using the BEAST observations26:10 – Review of systems using CRANES27:30 – Treatment, disposition and clinical trending28:10 – Diagnostic tools for austere medicine30:30 – Essential point-of-care investigations31:00 – Why every diagnostic tool needs a backup plan33:00 – Altitude, pulse oximetry and interpreting observations34:00 – Fever assessment in austere environments34:45 – Measuring temperature correctly in older adults and children35:40 – Managing fever of unknown origin and malaria36:10 – Nursing care and prolonged patient management36:45 – Public health and preventative medicine37:30 – Recognising clinical red flags38:00 – Key learning points and deployment preparation39:00 – The importance of clinical examination over technology40:00 – Trusting your senses and treating the patient—not the monitor41:00 – Closing remarks and CoROM CPD opportunitiesKey Topics DiscussedAustere primary careClinical reasoning in resource-limited environmentsThe austere clinical mindsetPreventative medicineTravel medicineWilderness medicineExpedition healthcareRemote diagnosticsFever of unknown originMalaria diagnosisEnvironmental medicineMusculoskeletal injuriesSkin diseasePoint-of-care ultrasoundNursing care in prolonged field careThe CoROM CHART assessment systemCPRO and BEAST observationsDifferential diagnosisKey TakeawaysMost remote clinicians spend considerably more time managing primary care conditions than major trauma.Clinical confidence develops through experience, deliberate practice and mentorship.Every piece of diagnostic equipment should have a backup plan.Careful history taking remains one of the most valuable diagnostic tools available.Trend observations over time rather than relying on single measurements. In malaria-endemic regions, malaria should remain high on the differential diagnosis for every patient with fever.

At the top of my list of summer goals this year is to eat way too much ice cream. There's nothing like a delicious frozen treat on a hot summer day. Put me in an ice cream shop, and I start to get a little impatient with the people in front of me, sinfully so if they're a Sampler. You know the type, maybe you are the type. "Oh my, so many choices, flavors I've never had before, how many miniature spoonfuls can I get you to scoop for me to try..." Come on now, did you really need a taste of 'Triple Chocolate Fudge' to know what you were getting? Step aside, some people came prepared. Sorry, I better stop before I have to repent. It's the desire behind these misdirected people that's actually the encouragement of our Psalm this Sunday. David is calling us to bring your senses to attention, take a taste of the goodness of God, and savor it until it results in praise. Join us this Sunday as David calls us out of senselessness and to a new experience of the glory of God.

The Word of God is different from any other writings, II Timothy 3:16 All scripture is given by inspiration of God, and is profitable for doctrine, for reproof, for correction, for instruction in righteousness: John 17:17 Sanctify them through thy truth: thy word is truth. John 1:14 And the Word was made flesh, and dwelt among us, (and we beheld his glory, the glory as of the only begotten of the Father,) full of grace and truth. That is why I put so much scripture into these programs, God's Word will do what no other words can do. It is our spiritual food we live by.

Last year, I compiled my first-ever “Best Of the Year” show. It was such fun to make, and received such a great response from listeners, that I decided to make it an annual tradition. While I could only include a handful of authors from the past year, this episode provides a fun Whitman's Sampler of the kinds of conversations available in our archives. Listen as Adam Johnson, Wally Lamb, and Chris Whitaker tell us where their stories come from. Laila Lalami teaches us to read like a writer. Amy Bloom and Bruce Holsinger offer their thoughts on revision. Patrick Ryan (via Ann Patchett) shares his insights about how to write war scenes with authority, and Richard Russo talks about the moral dilemma of using the people in our lives for material. Authors in this episode include Jess Walter (So Far Gone), Wally Lamb (The River is Waiting), Richard Russo (Life and Art), Amy Bloom (I'll Be Right Here), Laila Lalami (The Dream Hotel), Joan Silber (Mercy), Chris Whitaker (All the Colors of the Dark), Adam Johnson (The Wayfinder), Patrick Ryan (Buckeye), Bruce Holsinger (Culpability), Eric Puchner (Dream State) and Colum McCann (Twist). The Jane Smiley essay that Laila Lalami refers to can be found here. And Joan Silber's book about how to use time in fiction can be found here. For nearly 28 years, Writers on Writing has delivered MFA-level advice from some of our world's most accomplished living authors – all without the hefty price-tag. We've grown only by word of mouth and rely only on listener support. So, if you like what you hear, help us spread the word! Health care did not have a quiet year. AI, experience, platforms, and “vibe” all took turns in the spotlight. In this first half of our Holiday Sampler, hosts Chris Boyer and Reed Smith revisit four of the most downloaded Touch Point episodes of 2025. These clips capture the themes that kept listeners coming back. This episode features standout moments from: Episode 421: The Evolution of Patient Experience We look at how patient experience grew up. From the days of satisfaction surveys and HCAHPS to a broader, human centered view that includes families, communities, and digital expectations. This clip returns to the point where COVID, consumer behavior, and online access collided and forced health systems to rethink what experience really means. Episode 428: Breaking the Healthcare Content Mold AI quietly moved from pilot projects to everyday content work. In this segment, we revisit how health systems are using AI to draft education materials, website copy, social content, and SEO at scale, while staying compliant and avoiding PHI. The clip focuses on the human plus AI dynamic and why better prompts and guardrails lead to better outputs. Episode 431: AI, Are You Killing Our Vibe Vibe marketing became one of the buzz phrases of the year. Here we unpack what people actually mean when they talk about “vibe,” how it influences discovery and trust, and where AI fits in as a tool to shape and scale emotional connection. This highlight walks through concrete examples of how teams brought vibe into campaigns without losing substance. Episode 435: The Website Is Now a Platform In this conversation, we challenged the idea of the hospital website as a static brochure. The clip you will hear looks at what happens when you treat the site as a true platform for access, engagement, and growth. We talk about scheduling flows, self service tools, data capture, and the privacy and trust questions that come with a more intelligent experience. It is a snapshot of how digital front doors are becoming digital operating systems. Taken together, these four episodes show how quickly the ground is shifting under healthcare marketers and digital leaders. Experience is broader. Content is more automated. Brand is more emotional. The website is more operational. The leaders who follow these threads are already working differently heading into 2026. Our annual listener survey is open through December 19. It is your chance to vote for the best guest and best interview of the year and to tell us what you want more of in the year ahead. We will share the results and celebrate the winners in our final episode of 2025.
