Importance of Training & Support: Scott emphasizes the critical role of knowledgeable support and hands-on training (like that provided by 3D Tech/3D Dentist) when integrating technology, contrasting it with less specialized support from large suppliers. Support is ongoing for products purchased through them. Medit Overview: A South Korean company celebrating 25 years (mostly lab scanners, ~7 years for intraoral). Medit's Value Proposition: No annual fees, free software, and free software upgrades are key differentiators. Medit Link Software Suite (The "Apps"): A powerful, free suite accessible even to non-Medit scanner users (importing STLs). Includes free apps like Model Builder, Crown Design, Night Guard/Bite Splint, Ortho Simulation. Newer, more powerful CAD features (AI Final Crown, Provisional, Inlays/Onlays, Bridge Design) are being introduced with a per-export fee, but the core remains free. AI Crown Design: Discussion on how AI differs from standard proposals (better morphology matching, less tweaking, potential AI margination) vs. algorithms. Practical Scanning Tips & Troubleshooting: Marking margins: Alan does, Zach doesn't – lab preferences vary. Sluggish scanning fixes: Ensure the laptop is plugged in, perform all updates (Windows, graphics drivers, Medit Link), check scanner tip condition (rated for ~100 autoclave cycles, look for degradation/spots). Technique: Dry teeth well, use the white light setting for scanning through blood, proper path/tissue capture (especially around canines). Hardware: i700 vs. i900: The i900 is smaller, lighter, has a larger scan area, multiple tip sizes, and is generally smoother, especially for full arches and scanning through blood. The i900 "Classic" brings back the physical button many users missed. Recommended Medit Apps to Explore: Model Builder & Bite Splint: Great, easy entry points for CAD, good for team delegation (requires printer for output). Ortho Simulation: Automated simulation of tooth movement. Smile Design: Powerful tool using even basic phone photos, creates a shareable before/after slider for patient communication. App Access: Apps run on the scanning computer but can be accessed remotely by logging into your Medit Link account on another computer (requires downloading case files). Integrations: Medit Link integrates with third-party software and platforms (SprintRay Cloud, Exocad, etc.). I use Live Online Learning Activities to ensure people get a chance to practice their understanding of the new concepts. "What better way is there to give meaning to what we say than to draw to make our thoughts explicit? When we add visual information to what we're saying, suddenly our intention becomes much more precise, and we can begin to work on shared understanding. This is why I love bringing people in contact with the power of Visual Thinking, of getting back to our roots and using pen and paper to bring people closer together." Low-dose aspirin (LDA), typically prescribed at a dosage of 81 mg daily, is primarily used during pregnancy to prevent or delay the onset of preeclampsia. However, the question of whether to continue LDA postpartum is less clear and requires careful consideration of the benefits and risks associated with its use in the postpartum period. After all, the American College of Cardiology and American Heart Association recommend that low-dose aspirin use (75 to 100 mg/d) might be considered for the primary prevention of atherosclerotic CVD among select adults ages 40 to 70 years at higher CVD risk but not at increased risk of bleeding. Meanwhile, the 2022 US Preventive Services Task Force (USPSTF) recommendation notes that the decision to initiate daily aspirin therapy for primary prevention of cardiovascular disease (CVD) should be made on a case-by-case basis for adults ages 40 to 59 with a 10% or greater 10-year CVD risk. The recommendation applies to those without signs or symptoms of clinically evident CVD who are not at an increased risk of bleeding. So, does continuation of LDA after delivery reduce the persistence of, or development of, chronic hypertension. Is that evidence-based? The answer is both YES and NO. Listen in for details.
The saying says, “The Devil is in the details”. This idiomatic phrase that means even the grandest project depends on the success of the smallest components. So is the case with low dose ASA for preeclampsia prevention. Currently, the ACOG states, “Low-dose aspirin (LDA) (81 mg/day) prophylaxis is recommended in women at high risk of preeclampsia and should be initiated between 12 weeks and 28 weeks of gestation (optimally before 16 weeks) and continued daily until delivery” (ACOG CO 743; 2018). But the DOSE of LDA is controversial with mounting data that the minimal dose should be 100mg, with leading commentaries pushing for 162 mgs here in the US. We have covered this MANY times on our show. But there is a separate issue here that is often missed. Does it matter WHEN in the day this is taken? Is taking LDA first time in the morning best? What about after lunch? At bedtime? There is data here to guide us. In this episode, we will review the CHRONOBIOLOGIC response of LDA in pregnancy. Yes, that is a real term! This is also reflected in the March of Dimes data. Plus, there is Level I data supporting the best time to take aspirin- and it is fascinating. Listen in for details.

We thought it lent itself nicely to the podcast, as well. In this special discussion, Lori Niles-Hofmann and Stella Lee school Matt on the basic ins and outs of AI. We talk about what AI is. We talk about the different types of AI. Matt asks whether the world of Terminator is soon coming to pass! Stella and Lori correct that notion. We dive into the ethical issues that using AI impact both short-, and long-term. We delve into how information gets curated on different AI platforms. We explore the implications in general, as well as for L&D, when using AI. Specifically we look at the practical implications. Can we use AI to help us solve some of those wicked problems we face? Lori and Stella share advice on how to engage with vendors pushing their AI offerings. Or, at least some questions one might ask when others are authorized to buy, but you have to use it. Speaking of practical… and speaking of ethics… when is it ok to use AI when constructing designs, writing, or researching? What are hallucinations? When does the AI misunderstand us? How do we misunderstand what we get from it? While many are way more educated and knowledgeable about AI than Matt. Truth in Learning: in Search of Something! Anything!! Anybody?
We are rejoined by Will Thalheimer to talk about one of his favorite topics, awards in the L&D industry. Should we have them? Should we give them out? How do we do it better… if at all? We explore a pretty exhaustive list of the general issues with using and distributing awards among industry professionals by industry professional groups. Heck, we even find a few good points to support the tradition, as well. Are these industry awards fair? Are they judged effectively and fairly? What are, and should, be the criteria for winning an award? What rubric is used? Who designs the rubric? Are we even rewarding the “right” stuff (whatever that means)? What about money? Application fees? And paying for the awards? Bias? Vendor relationships? Heck, are these awards even manageable… doable in order to ensure claimed standards and fairness? Is an evolving approach, perhaps to think about certification? Who judges the judges? Will even hands out an award called the Neon Elephant Award, as he attempts to bypass many of the issues we discuss. Nancy McCaughey is a returning guest.Nancy identifies herself by her most treasured life roles; wife, mother, daughter, sister, newly-retired midlife transition coach and late discovery adoptee (LDA). She is in the process of completing her memoir, Reunion. It is about her own midlife ‘identity quake' that took place at her 20th high school reunion when her best friend from childhood revealed a 37 year old family secret: Nancy was adopted. Nancy returned home from the reunion without telling her mom what she had learned in order to process the crushing grief of her loss alone. Her husband begged her to not tell her mother what she learned. Nancy had no idea how many people knew but her. She researched why her birth certificate's information was false and learned about Illinois' sealed records laws and the amended birth certificate that replaced her original one. The injustice that adoptees born in Illinois were not allowed to know their identity only made Nancy more determined to find out the truth and the woman responsible for giving her life. The real breakthrough occurred several weeks later, when she stopped asking “Why now, God? Why me,?” and instead, opened her heart and mind to the possibility that this may be happening for a reason. It was then that Nancy could see the mysterious synchronicities that were happening and unexpected mentors that were appearing, much like Joseph Campbell's description of the Hero's Journey. One by one, she found more clues that led her to find and reunite with her birth mother, Donna and four half siblings. Twenty-eight years later, as Nancy prepared to start writing her memoir, she took an Ancestry.com test, hoping it would give her the missing pieces of her story. It did, including who her father was, that he was the son of Croatian immigrants and she is the oldest of eight siblings.What Nancy could not see in 1991 was that her fateful decision to attend her reunion with her friend was the beginning of a 28-year soulful and transformative journey to find her true identity, purpose and belonging. After searching and reuniting with her birth mother, Nancy transitioned from a successful 30-year sales and marketing career in the travel and pharmaceutical industries and completed a year-long certification program with Hudson Institute of Coaching. It was at Hudson, when she discovered that her true purpose and passion was to coach people who were at a midlife crossroads, asking themselves life's deepest questions: “Who am I?” and “Why am I here?” Over the past two decades, she has had the privilege of guiding and encouraging hundreds of global professionals seeking the clarity and confidence to craft their new life narrative and successfully transition into their next chapter. Nancy lives with her husband of 22 years in Bainbridge Island, Washington and is lucky enough to have her son and daughter in law who live close by in Bremerton. Why You Should Listen: In this episode, you will learn about heavy metals, metal allergy, and Type IV delayed hypersensitivity responses. About My Guest: My guest for this episode is Shari Guess. Shari Guess almost died from severe allergies due to metal hypersensitivity after a litany of orthopedic surgeries, all performed within a few short years, which implanted a large amount of metal and dental hardware in her body. At the time she got sick, she was working as a full-time realtor and attending college, studying pre-med, hoping to eventually become a Physician's Assistant. After being bedridden for nearly 3 years and feverishly researching everything she could to help find out the cause of her severe systemic symptoms, she had a lucky break in the case and was able to connect the dots with all of her symptoms. Suddenly everything made sense! Shari has undergone numerous surgeries to date to remove and revise hardware. These surgeries have restored her to near-full health as the symptoms related to her metal allergies began to subside by leaps and bounds immediately following the procedures. While in the thick of her illness, due to a compromised immune system, she also acquired Lyme Disease, Alpha Gal Syndrome, and several other autoimmune diseases. Shari is blessed to have been able to put almost all of that behind her, experiencing near complete relief from the symptoms of Lyme and co-infections after LymeStop treatment in Coeur d'Alene, Idaho, along with the restoration of her immune system through the removal of the metals and implementing limbic system retraining.
