Dr. Sharon Kolasinski is a professor of clinical medicine at the University of Pennsylvania and chief of the Division of Rheumatology at Penn Presbyterian Medical Center. Part 1 Her objectives in the presentation are to help listeners by learning about evidence-based treatment for patients with osteoarthritis (OA), to understand the process by which guidelines are developed that might help us figure out our evidence-based approach, to review the recommended treatments for OA, and to review some treatments that are not recommended for OA. Her basic evidence-based reference is a University of Pennsylvania guideline that was published in 2020. Numerous other guidelines are available, which she described. She discussed her work with a case involving a 55-year-old male patient. He arrived for routine follow-up care for immunosuppressive medication monitoring. He described pains that he was experiencing and was diagnosed with OA. She indicated the impacts OA has on patients. Clinicians find it a daunting challenge to provide satisfactory treatment. For example, for some clinicians, the guidelines do not appear to be clear and provide a roadmap. Also, they do not always believe in the recommendations contained in the guidelines. In this presentation, she wants to see if she can change some minds about guidelines. She provided an example based on investigations conducted at her institution. The outcome was a series of recommendations that she described. Part 2 Dr. Kolasinski began Part 2 of her presentation by continuing to focus on the importance of having patients with OA engage in physical activity. She stated that “they are worn out and the implication is that a doctor is needed to fix them.” Perhaps a perspective should be taken of a more participatory discourse where we encourage physical therapy and emphasize that physical exercise is safe when you have arthritis and focus on what the patient can do, empowering them to exercise. A starting point is to give patients a physical therapy prescription. She discussed the extent to which physical therapy is useful, along with indicating how much and how frequently exercise is beneficial (e.g., for 20 minutes, three times a week). Losing weight is an effective way of reducing symptoms. Food choices can affect OA symptoms. Diet and exercise used together can produce effective results. References were made to several studies that involve the status of steroid injections on improving patient health status. Acupuncture also was mentioned. She indicated conditional recommendations on the use of pharmacological interventions. She concluded by describing how to treat the 55-year-old patient she mentioned in Part 1 of her presentation.

Welcome to episode 60 of the Eat for Endurance podcast! My guest today is John Kelly - an elite ultrarunner, triathlete, father of four, and an engineer. John is perhaps best known as one of the few finishers of the infamous Barkley Marathons. In 2017, he was the fifteenth and, until today (March 16th), the most recent finisher in the race's history. It just so happens that the 2023 Barkley Marathons occurred this week, culminating in three finishers tonight! John completed all five loops yet again, making him the third runner in history to finish the race more than once. And in case you're wondering, we recorded this interview on February 23rd, about three weeks before this year's race, and I swear that the timing of this episode being published immediately after is pure coincidence!  But enough about the Barkley Marathons, as John's  athletic accomplishments extend far beyond this race, of course. He has completed many Ironman events, including two finishes in the Kona Ironman World Championships, and he is a 2x Ironman Age Group Champion. John has run numerous road marathons, including a 2:26 last year at CIM. He's a Tennessee native but lived in the UK for awhile and during his time there, he won the Spine Race in 2020 - a brutal winter ultra marathon that follows the 268-mile Pennine Way. He also broke a 31 year old record on the Pennine Way, winning an award for the men's 2021 FKT of the year. He was the first to complete “The Grand Round” - a combination of the UK's 3 big fell running rounds, while biking between them - there's an awesome short film on this that I'll link to in the show notes. This past summer, he placed tenth at Hardrock. The list goes on. Oh and did I mention that he works and has four kids too?! Episode 74 is an interview with John Kelly.  John made trail running history on April 3, 2017 by becoming the 15th person to ever complete the Barkley Marathons. The Barkley Marathons in Frozen Head State Park, Tennessee is known to the toughest endurance challenge ever with over 160 kilometres of tough terrain, sheer drops and thick forest.  He grew up exploring the dense mountains and forests around his family’s rural home in Morgan County, TN, constantly trying to keep up with his older brother and cousins. John ran during high school where he describes himself as a good, not great runner. During college and grad school he took a full decade off from the sport. After grad school he rediscovered his love of the mountains hiking the John Muir Trail with his wife Jessi. john ended up running the 2013 Marine Corps Marathon, calling it an absolute disaster. He decided he could do better, so he got himself a Boston qualifier. As it was some time before he could actually run Boston, he decided to give ultras a go, and then also tried triathlons because he loved cycling too. It was this background in a variety of disciplines that led to John creating the Grand round, which involved running the three most difficult rounds in the UK (The Bob Graham, Paddy Buckley and Charlie Ramsay) and cycling between them. Captain Alexander Jolly and Captain Dave Fielding are commercial airline pilots. I met them through Project Wingman, a pandemic-induced collaboration between the UK aviation industry and the NHS.  For the last few months they (along with a few of their colleagues) have been helping us train. Specifically, they are debriefing and writing up human factors performances (overseen by clinical faculty) in a multi-specialty/disciplinary in situ simulation programme. We met John Kelly in episode 483 after he successfully broke the 31-year FKT on the 268-mile Pennine Way in Scotland earlier this summer. In this episode, John discusses his successful completion of The Grand Round, UK's big three fell running rounds run consecutively (192 miles, 84,000' elevation, 113 summits) AND riding a bike between the trail heads totaling 398 miles. This was John's second attempt at The Grand Round. We discuss why he decided to try again and lessons that he learned. Although most of us will never consider something this grandiose, we do like to set goals that push us to our own personal limits. The lessons that John learned can be transferred to our personal attempts at greatness. This year, John Kelly became a Spine Race winner and broke a Pennine Way FKT which had stood for over 30 years (setting a record which was rudely broken a week later by Damian Hall!). As if this wasn't enough achievement, he also recently completed his 'Hartley Slam' project by completing the Grand Round. This monstrous endeavour saw John take on all 3 UK grand rounds (the Paddy Buckley, Bob Graham and Charlie Ramsay) consecutively, whilst cycling the distances in between.  Each of those runs are around the 100km mark with stacks of ascent, and the rides between are around the 300km each! John Kelly is only the 15th person to finish the formidable Barkley Marathons since its creation 24 years ago.  Since then he has finished many of the toughest races in the world including the Laverado Ultra Trail and the Tor des Geant.  Last year John moved from the U.S. to England and began tackling the amazing EU trails.  This spring, before the pandemic, John won the Spine Race, a 268-mile, 39K elevation route following the Pennine Way National Trail in Scotland.  As races were cancelled, John decided to try and break the 31-year-old FKT for the Pennine Way. The Pennine Way is 268 miles long and one of the first National Trails in England. The FKT was held by Mike Hartley for 31 years until John broke it. Less than a week later, his friend Damian Hall broke it again! Both Mike and John in attendance at the finish - FKT action is really hot in the UK right now - will John have another go? I asked him - his answer is very interesting! John is a native of Tennessee and the last person to finish the Barkley Marathons. He moved to England last year - what is different? He's been getting after it in his new home - last year he tried the "Grand Round" - self-powered. This is the three biggest "rounds" or link-ups in the UK - the Paddy Buckley, the Bob Graham, and the Charlie Ramsay Round, each done consecutively in less than 24 hours, cycling in between them. That's 180 miles of running/hiking, with 85k of vert, 113 summits, and 400 miles between them. Yikes! What happened? In 2017, John Kelly became only the 15th finisher of the Barkley Marathon, an epic race through the rugged mountains of Tennessee. He battled through pain, exhaustion, sleep deprivation and three years of preparation in order to achieve one of the biggest accomplishments in ultra running. It was an incredible accomplishment that only someone with a solid physical foundation and mental stoicism can endure. An incredible athlete who has fallen in love with mountain running, over time John has discovered a lot about his limits, his motivations, and his fortitude. He's also developed what is an outstanding mindset as it pertains to WHY we pursue adventure and how to walk that fine line between pushing your limits and putting yourself to unnecessary risks (which he writes about on his fantastic blog post "Look How Tough I Am"). In this episode we chat about that fine line. When should you push to the limit (which...funnily enough when I typed that phrase into a thesaurus the first suggestion that popped up was "injury"...illustrating the complications of this discussion perfectly) and when should you come back to fight another day? What are healthy reasons to really challenge yourself physically and what are unhealthy reasons? When taking on an adventure why does your WHY matter so much? How does the Barkley find your weaknesses and how does one adjust and adapt? We also talk a bit about his next big adventure as he explores some big trails around his new home. He will be linking three big English "Rounds", running each of them and then biking in-between. It's something he's created and he's calling it the "Grand Round".
