Feline infectious peritonitis (FIP) occurs as a result of a viral mutation in cats infected with feline coronavirus.  For decades, FIP has been a devastating condition in cats with an extremely high (95%) mortality rate and no reliable treatment options. A few years ago it was discovered that the antiviral drugs remdesivir and GS-441524 are highly effective for treating FIP, but these drugs are very expensive and there was no legal means of accessing them in Canada or the US – until now!  As of 2024, Canadian veterinarians can access these life-saving drugs through an Emergency Drug Release (EDR).  As these drugs and the process to access them are new to many Ontario veterinarians, the Ontario Animal Health Network is thrilled to have Dr. Kelly St. Denis with us to walk us through some of the ins and outs of these new treatment options. Hvordan håndteres vold i skolen og hva er det som utløser den? Har skolene ressursene til å håndtere dette og det som måtte ligge bak? Hvordan møter skolen elevene som utøver vold og de ansatte og elevene som utsettes for dette? Hvordan ivaretar man alle de som skal være en del av skolen? Temaet for panelsamtalen var voldsutøvelse fra elever i skolen og skolens ressurser til å ivareta disse elevenes behov før det kommer dithen at de utøver vold. Synliggjøringen av vold fra elever i Bergensskolen kommer samtidig som det politisk sett settes stramme rammer for økonomien. Dette skaper en ytterligere utfordring for lærerne til å gjennomføre lovpålagte pliktene og å sikre elevenes trygge og gode psykososiale miljø. Dette er en lokal debatt, men tjener også som eksempel på en større utfordring på landsbasis hvor mange tall peker i samme retning som i Bergen. She was the first woman to complete a vascular surgery fellowship, which was in 2001. Her training, like most, was very arterial focused at the time. She then moved into the private practice space while still completing clinical research. She began to realize that more of her patients had venous disease than arterial. For example, she saw many more patients with varicose veins than abdominal aortic aneurysms. Pelvic venous disorders (PeVD) in particular, remain poorly studied and understood. She became interested in this patient population because she saw many women present with pelvic pain and varicose veins after multiple targeted saphenous vein treatments. She realized this was because the source of the problem, the pelvic veins, were being left untreated. Dr. Gibson developed a varicose vein classification that is being disseminated around the world, and has been translated into multiple languages. It is called the SVP Classifier (Symptoms-Varices-Pathophysiology). It was developed to be used in conjunction with CEAP (Clinical-Etiology-Anatomy-Pathophysiology) classification for venous disease. There is an app available, as well as a workbook that can be used to claim CME. It is a tool that can aid providers and vascular technologists alike when working up PeVD. Lastly, Dr. Gibson reviews her workup of a patient with pelvic pain. Before undergoing ultrasound and vascular workup, it is important to think of other causes of pelvic pain in women of certain ages. In young women, she always ensures patients have seen a gynecologist, as endometriosis is the most common cause of pelvic pain in this group. If they are post-menopausal and present with new onset pain, she also has the patient see a gynecologist to rule out malignancy. Finally, if the patient is postpartum, she loops in a pelvic floor physical therapist because myofascial pain from pregnancy can mimic pain from PeVD. For the vascular workup, she begins with an ultrasound performed by a vascular technologist. She meets with the patient to discuss symptomatology and impacts on quality of life. If PeVD is found on US but the patient has minimal pelvic symptoms, she does not pursue treatment. She treats the patient, not the imaging. If symptoms are bad enough, she will move forward with stenting (for obstruction) or embolization (for varicosities). For embolization patients, there is no routine follow up unless there is a complaint. For stenting in NIVL (non-thrombotic iliac vein lesion) patients, she follows patients with annual US for a couple years. Prepodcast 112 días, casi 4 meses.Dejé mi puesto como responsable BIM en Aelca.2 semanas de trabajo intenso para que la transición con mi sucesora fuera lo mejor posible.Hice varias entrevistas para encontrar un puesto como consultor pero en remoto.Estuve 15 con COVID, 10 de ellos ingresado con oxigeno.Hice una mudanza de Madrid a Tenerife.22 cajas y coche lleno hasta el techo.600 km en ese coche y 1400 km en barco (dos días).Preparé e impartí un curso de 18 horas sobre CDEs en Butic.Empecé a trabajar en mi nueva empresa.Berrilan, con David Barco e Iñaki Maiza al frente. Tuve ponerme al día de una implementación BIM bastante grande ya empezada, que tenía que liderar.Me sobraron unos días para irme de vacaciones.Todo esto hizo que tuviera que retrasar el podcast un mes.Ahora ya episodio semanal hasta final de año. Tema del episodio Volvemos a hablar del IFC (y van 5)010 Entendiendo el ifc023 Exportando a ifc035 Exportando la i a ifc041 Criticando el IFCLeyendo en LinkedIn: ¿Por qué debería molestarme en utilizar IFC en un proyecto donde todas las disciplinas se modelan con Revit?Supongamos que al cliente le da igual.Si el cliente exigiera IFC como entregable, el quipo de diseño podría colaborar directamente en Revit, y exportar a IFC sólo para las entregas.Aparentemente no hay necesidad del IFC por temas de interoperabilidad.Ni tampoco de neutralidad tecnológica al no ser una licitación pública.Tener un formato que no caduque, o tener un formato "no editable" tipo pdf, parecen argumentos de poco peso o que no compensan "el esfuerzo" de trabajar con IFC.Apostar por el Open BIM por el simple hecho de aportar nuestro granito de arena en la lucha contra el monopolio de Autodesk... Esta si que no le importa a nadie.Hoy vamos a ver 3 motivos puramente prácticos para utilizar IFC aunque todos modelen con Revit. Patrocinador No he tenido tiempo de buscar patrocinador. No admite plugins.Autodesk Viewer, es un visor online gratuito, pero muy limitado.https://viewer.autodesk.com/BIM360 (ahora Autodesk Construction Cloud), con más opciones (sin llegar a lo que puedes hacer dentro de Revit), cuesta (para la mayoría de las empresas) 200-600€/usuario/año.En resumen, poder abrir un RVT implica en la mayoría de los casos 5000€/año en SW+HW si queremos tener funcionalidades completas de revisión, medición y extracción de información. Las últimas exportan mejor a IFC.Gestión de IT más sencilla (instalación, licencias, renovación de hardware...)Cualquier PC destinado a ofimática puede mover modelos medianos/grandes. Ejemplos Ejemplo 1: gran promotora que exige Revit a todos sus proveedores:Si tienen 40 técnicos que revisan modelos, necesitarán 40 equipos de 2000€ y 40 licencias de Revit LT (si no necesitan plugins).Ejemplo 2: Estudio de arquitectura con 2 arquitectos y un aparejador.Si el aparejador no modela ni modifica los modelos, se pueden ahorrar una licencia y centrarse en herramientas basadas en IFC con las que pueda medir y hacer seguimiento de la obra. 2 Productividad Si trabajamos con RVT, nos autolimitamos a trabajar con Revit, Naviswork y BIM360.No cubren todas las necesidades de un proyecto de forma optima.Ejemplos: mediciones y presupuesto, visor ágil.En cuanto te sales de las disciplinas y tareas "mayoritarias", Autodesk ofrece soluciones pobres, o soluciones no conectadas a Revit, o no ofrece soluciones.Media y alta tensión.Cálculos acústicos, incendios, sostenibilidad...Podemos usar plugins o desarrollos propios que expandan las funcionalidades, pero estamos limitados a lo que nos permita hacer la API y a tener una sesión abierta del software de Autodesk.Con IFC, podemos modelar en Revit, y luego, tenemos libertad para quedarnos dentro de Revit usando Cost-it o el plugin de Arquímedes, pero también usar TCQ, Open Quantities, Mamba (cuando salga) o Presto, sin necesidad de plugins, pasos intermedios, ni el Revit abierto.Lo mismo para revisión. Hay opciones más potentes o más baratas que Navisworks y BIM360, que sólo podemos usar si exportamos a IFC.Solibri, bimspot, bimvision...Software BIM más específico suele usar IFC.Hay muchas soluciones que han creado plugins para Revit:SynchroVico OfficePrestoCYPEDaluxProcore...Si usas varias de estas, tienes que tener los plugins de todas, exportar de forma independiente a cada plataforma, no sabes si estás exportando la misma versión a todas, etc.Y aunque usemos alguna con conexión directa como Synchro o Dalux, con IFC sólo exportamos una vez para todas ellas y ya está. 3 Estandarización Información (gracias por al idea a Álvaro Sánchez) Que todos modelemos en Revit, no significa que todos usemos los mismos nombres de parámetros ni las mismas categorías para modelar los mismos elementos.Podemos "inventar la rueda" y persuadir a nuestros colaboradores de que usen "nuestra rueda" y no la suya.O podemos usar todos las clases, tipos y atributos IFC.Hay que dedicar tiempo a conocerlo y configurarlo.Está todo en inglés.Estándar eCOB https://ecobject.com/Plan BIM Chile https://planbim.cl/Pero sigue siendo mejor que lo de inventar la rueda. Estándar más robusto y también más productividad.Los EIRs y BEPs serían más sencillos de redactar. Flujos de trabajo Es poco probable que siempre se de un "full Revit" y entonces tendrás que modificar los flujos de trabajo para que funcionen con IFC.Tendrás que investigar y mantener flujos de trabajo RVT, pero también flujos IFC. Doble trabajo.En los flujos IFC serás menos productivo porque tendrás menos práctica.Si, aunque modeles siempre en Revit, usas siempre IFC, tendrás los mismos estándares para el 100% de los proyectos, no sólo para el 80%.También puede pasar que un cliente te pida IFC y entonces ya estarás preparado. Conclusiones No se trata de usar IFC por usarlo, porque nos han dicho que es lo correcto y somos defensores del OPEN BIM.El IFC tiene ventajas pero también inconvenientes:Es complejo para el usuario medio.Es fácil que se pierda información en la importación/exportación. Tenemos que decidir cuando usarlo.Por mucho que mejore Revit en el trabajo con IFC, la interoperabilidad 1 a 1 siempre será más cómoda y sencilla que el uso de IFC. Entonces se podría pensar que el IFC se usa sólo cuando esta interoperabilidad 1 a 1 no existe.Pero cuando todos modelen con revit y existan plugins con conexión directa con terceros software, el IFC sigue siendo interesante para:No quieres depender de tener una licencia de Revit y un pc que lo mueva, para tareas que no son de modelado.Quieres poder hacer esas tareas de" no modelado" con un software mejor o más barato que la oferta de Autodesk.Necesitas IFC en uno o dos flujos concretos, pero por productividad y estandarización, extiendes su uso a todo el proyecto. ¿Quieres escuchar otro episodio? Prepodcast 112 días, casi 4 meses.Dejé mi puesto como responsable BIM en Aelca.2 semanas de trabajo intenso para que la transición con mi sucesora fuera lo mejor posible.Hice varias entrevistas para encontrar un puesto como consultor pero en remoto.Estuve 15 con COVID, 10 de ellos ingresado con oxigeno.Hice una mudanza de Madrid a Tenerife.22 cajas y coche lleno hasta el techo.600 km en ese coche y 1400 km en barco (dos días).Preparé e impartí un curso de 18 horas sobre CDEs en Butic.Empecé a trabajar en mi nueva empresa.Berrilan, con David Barco e Iñaki Maiza al frente. Tuve ponerme al día de una implementación BIM bastante grande ya empezada, que tenía que liderar.Me sobraron unos días para irme de vacaciones.Todo esto hizo que tuviera que retrasar el podcast un mes.Ahora ya episodio semanal hasta final de año. Tema del episodio Volvemos a hablar del IFC (y van 5)010 Entendiendo el ifc023 Exportando a ifc035 Exportando la i a ifc041 Criticando el IFCLeyendo en LinkedIn: ¿Por qué debería molestarme en utilizar IFC en un proyecto donde todas las disciplinas se modelan con Revit?Supongamos que al cliente le da igual.Si el cliente exigiera IFC como entregable, el quipo de diseño podría colaborar directamente en Revit, y exportar a IFC sólo para las entregas.Aparentemente no hay necesidad del IFC por temas de interoperabilidad.Ni tampoco de neutralidad tecnológica al no ser una licitación pública.Tener un formato que no caduque, o tener un formato "no editable" tipo pdf, parecen argumentos de poco peso o que no compensan "el esfuerzo" de trabajar con IFC.Apostar por el Open BIM por el simple hecho de aportar nuestro granito de arena en la lucha contra el monopolio de Autodesk... Esta si que no le importa a nadie.Hoy vamos a ver 3 motivos puramente prácticos para utilizar IFC aunque todos modelen con Revit. Patrocinador No he tenido tiempo de buscar patrocinador. No admite plugins.Autodesk Viewer, es un visor online gratuito, pero muy limitado.https://viewer.autodesk.com/BIM360 (ahora Autodesk Construction Cloud), con más opciones (sin llegar a lo que puedes hacer dentro de Revit), cuesta (para la mayoría de las empresas) 200-600€/usuario/año.En resumen, poder abrir un RVT implica en la mayoría de los casos 5000€/año en SW+HW si queremos tener funcionalidades completas de revisión, medición y extracción de información. Las últimas exportan mejor a IFC.Gestión de IT más sencilla (instalación, licencias, renovación de hardware...)Cualquier PC destinado a ofimática puede mover modelos medianos/grandes. Ejemplos Ejemplo 1: gran promotora que exige Revit a todos sus proveedores:Si tienen 40 técnicos que revisan modelos, necesitarán 40 equipos de 2000€ y 40 licencias de Revit LT (si no necesitan plugins).Ejemplo 2: Estudio de arquitectura con 2 arquitectos y un aparejador.Si el aparejador no modela ni modifica los modelos, se pueden ahorrar una licencia y centrarse en herramientas basadas en IFC con las que pueda medir y hacer seguimiento de la obra. 2 Productividad Si trabajamos con RVT, nos autolimitamos a trabajar con Revit, Naviswork y BIM360.No cubren todas las necesidades de un proyecto de forma optima.Ejemplos: mediciones y presupuesto, visor ágil.En cuanto te sales de las disciplinas y tareas "mayoritarias", Autodesk ofrece soluciones pobres, o soluciones no conectadas a Revit, o no ofrece soluciones.Media y alta tensión.Cálculos acústicos, incendios, sostenibilidad...Podemos usar plugins o desarrollos propios que expandan las funcionalidades, pero estamos limitados a lo que nos permita hacer la API y a tener una sesión abierta del software de Autodesk.Con IFC, podemos modelar en Revit, y luego, tenemos libertad para quedarnos dentro de Revit usando Cost-it o el plugin de Arquímedes, pero también usar TCQ, Open Quantities, Mamba (cuando salga) o Presto, sin necesidad de plugins, pasos intermedios, ni el Revit abierto.Lo mismo para revisión. Hay opciones más potentes o más baratas que Navisworks y BIM360, que sólo podemos usar si exportamos a IFC.Solibri, bimspot, bimvision...Software BIM más específico suele usar IFC.Hay muchas soluciones que han creado plugins para Revit:SynchroVico OfficePrestoCYPEDaluxProcore...Si usas varias de estas, tienes que tener los plugins de todas, exportar de forma independiente a cada plataforma, no sabes si estás exportando la misma versión a todas, etc.Y aunque usemos alguna con conexión directa como Synchro o Dalux, con IFC sólo exportamos una vez para todas ellas y ya está. 3 Estandarización Información (gracias por al idea a Álvaro Sánchez) Que todos modelemos en Revit, no significa que todos usemos los mismos nombres de parámetros ni las mismas categorías para modelar los mismos elementos.Podemos "inventar la rueda" y persuadir a nuestros colaboradores de que usen "nuestra rueda" y no la suya.O podemos usar todos las clases, tipos y atributos IFC.Hay que dedicar tiempo a conocerlo y configurarlo.Está todo en inglés.Estándar eCOB https://ecobject.com/Plan BIM Chile https://planbim.cl/Pero sigue siendo mejor que lo de inventar la rueda. Estándar más robusto y también más productividad.Los EIRs y BEPs serían más sencillos de redactar. Flujos de trabajo Es poco probable que siempre se de un "full Revit" y entonces tendrás que modificar los flujos de trabajo para que funcionen con IFC.Tendrás que investigar y mantener flujos de trabajo RVT, pero también flujos IFC. Doble trabajo.En los flujos IFC serás menos productivo porque tendrás menos práctica.Si, aunque modeles siempre en Revit, usas siempre IFC, tendrás los mismos estándares para el 100% de los proyectos, no sólo para el 80%.También puede pasar que un cliente te pida IFC y entonces ya estarás preparado. Conclusiones No se trata de usar IFC por usarlo, porque nos han dicho que es lo correcto y somos defensores del OPEN BIM.El IFC tiene ventajas pero también inconvenientes:Es complejo para el usuario medio.Es fácil que se pierda información en la importación/exportación. Tenemos que decidir cuando usarlo.Por mucho que mejore Revit en el trabajo con IFC, la interoperabilidad 1 a 1 siempre será más cómoda y sencilla que el uso de IFC. Entonces se podría pensar que el IFC se usa sólo cuando esta interoperabilidad 1 a 1 no existe.Pero cuando todos modelen con revit y existan plugins con conexión directa con terceros software, el IFC sigue siendo interesante para:No quieres depender de tener una licencia de Revit y un pc que lo mueva, para tareas que no son de modelado.Quieres poder hacer esas tareas de" no modelado" con un software mejor o más barato que la oferta de Autodesk.Necesitas IFC en uno o dos flujos concretos, pero por productividad y estandarización, extiendes su uso a todo el proyecto. ¿Quieres escuchar otro episodio? Becky Taylor has been dedicated to her career as a Registered Veterinary Technologist for over 25 years. In that time, she has worked in mixed animal practice, in education, and in corporate veterinary medicine. She has participated in hundreds of CE opportunities, sat on national and provincial boards and considers herself a lifelong learner. A pinnacle of this learning was in 2016, when Becky completed a Master of Arts in Professional Communication through Royal Roads University. Her final project was authoring a paper on the role of RVT's with clients and resulted in the creation of a communication model for RVT's. Becky currently has 3 professional roles. Her full-time gig is the Co-Founder of BS Communication Strategies, where she provides communication coaching to individuals, companies and teams in a practical, skills-based way. She is a Co-Founder in the tele-triage company, AnimalHealth Link and in addition to those roles, she contracts to and supports Mosaic Veterinary Partners in the management and building of their veterinary practice teams. These endeavours fuel her communication nerdiness and her entrepreneurial spirit. As we observed NAIDOC Week in November, many RVTS registrars and staff used the opportunity to attend (mainly online) events to celebrate the history, culture and achievements of Aboriginal and Torres Strait Islander peoples.  RVTS Acknowledges the traditional custodians of the lands and seas on which we live and work, and pays respect to elders: past, present and future.  Events during NAIDOC week remind us about our commitment to consider a First Nation perspective in all training activities at RVTS. As you may be aware, according to the strategic plan of RVTS, the purpose of RVTS is to improve Australian health by training and retaining medical practitioners working in Rural, Remote and First Nation communities by facilitating access to medical education and training in circumstances where it may otherwise not be possible, providing flexibility and innovation in meeting the medical training needs of diverse communities, and, as mentioned, to consider a First Nation perspective in everything we do.

Today, I am happy to have the privilege of talking to Angie Arora and Kirsti Clarida, two professionals who I got introduced to through the Veterinary Social Work Program. Angie is a registered social worker from Toronto, Canada. She gears much of her practice around pet loss bereavement and building compassion resilience in animal care professionals. Kirsti is a registered veterinary technician in Ontario, Canada, and she has done a lot of advocacy work to increase awareness around the importance of the roles of Registered Veterinary Technicians in veterinary medicine. Kristi and Angie got together, and they have been doing a lot of research and putting some hard work into compiling a veterinary training manual to help people deal with any challenges related to their relationship with animals.  Although working with animals can be fulfilling and rewarding, it can also be physically and emotionally exhausting. Angie and Kirsti are joining me today to discuss the work they are doing in the field, which includes pet loss and bereavement, compassion fatigue, and strengthening compassion resilience. Angie and Kirsti's Bios: Angie Arora is a registered Social Worker from Toronto, Canada, with over 15 years of experience in the areas of pet loss and veterinary compassion resilience. She has worked as a hospital Veterinary Social Worker, facilitated community-based pet loss support groups, and engaged in media campaigns to reduce the stigma associated with pet loss. She is on the inaugural Board of the International Association of Veterinary Social Work and serves as a Pet Loss Support Group Facilitator with VetVine. Angie is a Professor with Seneca College's Social Service Worker Program. Her social work practice has also focused on issues of gender-based violence with a specific focus on immigrant and refugee communities. Also, she is a Certified Compassion Fatigue Specialist with the Traumatology Institute and works with the veterinary industry to build wellness through strengthening compassion resilience. Angie obtained her Bachelor of Social Work from Ryerson University and Masters of Social Work from York University.   Kirsti Clarida has been a proud Registered Veterinary Technician since graduating from Seneca College in 1999. With service in small animal, emergency, and locum veterinary practice, she brings a wealth of experience to her current role as Coordinator of the Veterinary Technician Program at Seneca College. Recent besloot het kabinet om het advies van de SER over een quotum van 30% vrouwen (en 30% mannen) in de raden van commissarissen van beursgenoteerde ondernemingen te gaan doorvoeren Daarnaast is er een pakket maatregelen dat er op is gericht ook de andere circa 5000 vennootschappen die onder de Wet bestuur en toezicht vallen in beweging te krijgen. Wordt dit een succes? Of moet er dan meer veranderen binnen organisaties in Nederland? En: heeft het zin om je alleen op RvCs van beursgenoteerde ondernemingen te richten? In hoeverre weerspiegelen de verhoudingen in het bedrijfsleven de nog altijd weinig geëmancipeerde verhoudingen in de Nederlandse samenleving? In de studio is Babette Pouwels van Bureau Pouwels, zij heeft meegewerkt aan onderzoek ten behoeve van het SER-advies en is ook al jaren betrokken bij het samenstellen van de monitor die sinds 2013 jaarlijks in opdracht van het ministerie van OCW wordt gemaakt om in de gaten te houden hoe de samenstelling van RvBs, RvCs en – soms – RvTs zich ontwikkelt. Ook professor Belle Derks is te gast. Zij is hoogleraar Sociale en Organisatiepsychologie aan de Universiteit Utrecht en doet veel onderzoek naar onder andere genderstereotypering.

We’re excited to share this special edition of RVTS4GPs with you, featuring the recording of the recent 2019 RVTS Grand Round "Stump the Chump" with Dr Casey Parker.  The case presented is a 16 year old girl who comes in with her Aunty. Casey is asked to unravel the puzzle. Dr Casey Parker is based in Broome in WA and is well known for his blog and podcast broomedocs.com. He discusses various aspects of this case that need to be considered, with input from Marlene Drysdale (RVTS's Cultural Educator) and Dr Jacki Mein (a previous RVTS registrar based in Cairns). It was a challenging case, with an interesting discussion, and plenty of learning points.

As GPs, we aim to relieve pain and minimise harm, but occasionally patients get addicted to medications we prescribe. How can we become part of the solution? One of the RVTS supervisors, Ian Kamerman, speaks about this in a very practical and real way in the webinar recording excerpt this month. This area is certainly where the “art” of general practice comes into play. Registrars are encouraged to discuss this topic further with their supervisors to refine their skills in managing opioid addiction in the community.

Uncertainty in general practice is unavoidable. We deal with uncertainty in more than half of our consults. Coping with the uncertainty is an essential skill for GPs. This month we again listen in on a webinar about Dealing with Diagnostic Uncertainty with Marlene Pearce, a GP based on the Sunshine Coast, QLD. This is the final part of a 3 part series, based on the webinar delivered to RVTS registrars by Marlene. In Part 1of ‘Uncertainty in General Practice’, Marlene spoke about using clinical reasoning to assist, what to do when we’re stuck, as well as what pitfalls to avoid. In Part 2, we consider what contributes to the uncertainty we experience. In this final part of the webinar, we look at how best to deal with this uncertainty.

This month, Dr Sonia Singh discusses cultural communication in general practice. Dr Singh co-ordinated a session at the last RVTS workshop on “Cultural Diversity”. During the session, consults were presented by registrars in a traditional setting in their own language. Guessing the context of the consult from the dynamics and body language revealed implicit biases. The consultations were then spoken in English, followed by insightful discussions. Sonia speaks about the lessons learned:
