At 25, Benny Schwartz was diagnosed with a rare autoimmune disease called NeuroMyelitis Optica, something only 4,000 to 8,000 people have in the United States. That caused Benny to lose his vision. Now at 35, he is legally blind, but he has turned to art as a way to deal with his blindness, mental health and emotional wellbeing. The Pittsburgh-based artists works under the name Benny Blindspots. He just had 50 pieces of his art included in a collection that is being displayed at Bank of America locations across the county.

Christine Ha, an award-winning blind chef and restauranteur, shares her experience grappling with neuromyelitis optica spectrum disorder (NMOSD). Facing relapses with the inability to walk and feed herself that challenged her independence, she leaned on the support from family and friends. As she lost her sight due to optic neuritis in both eyes, Ms. Ha had to embark on a journey of rediscovery in the kitchen, starting with the fundamentals. Winning MasterChef Season 3 marked a turning point, propelling her culinary career forward despite the obstacles posed by her disability. NMOSD is an autoimmune disease in which an antibody attacks water channels on astrocyte cells in the optic nerves, spinal cord and sometimes the brain. Attacks or relapses can be devastating and incomplete recovery from attacks is typical. Like Ms. Ha, some people living with the condition can be misdiagnosed with multiple sclerosis. A blood test for the aquaporin-4 antibody is key to getting diagnosed correctly early. Since 2019, highly effective treatment options have been FDA-approved that reduce relapses by 77-94%. Multiple sclerosis frequently causes visual impairment. 70% of people living with the disease can develop optic neuritis at some point and often the first sign of MS. The symptoms, medical evaluation, treatment and prognosis of optic neuritis are conveyed. Besides multiple sclerosis, other causes are discussed including MOG antibody-associated disease (MOGAD) and neuromyelitis (NMO). Double vision and shaky (or jumpy) vision are other concerning visual symptoms for people with MS. The reason for these eye movement abnormalities and detailed treatment options are covered. Experts share the latest advancements in vision research for those living with multiple sclerosis. Barry Singer MD, Director of The MS Center for Innovations in Care, interviews: Dr. Anneke van der Walt is an Associate Professor of Neurology at Monash University in Melbourne, Australia. She is the head of the MS and Neuro-ophthalmology Research Group. She completed her undergraduate work in South Africa and completed her neurology training and PhD at the University of Melbourne. She is also the Chief Operating Officer of MSBase Foundation. Dr. Tariq Bhatti is a neuro-ophthalmologist currently at Kaiser Permanente in Northern California. He completed his neuro-ophthalmology fellowship at Emory. Dr. Bhatti was most recently a Professor of Ophthalmology and Neurology at Mayo Clinic and previously Chief of Neuro-ophthalmology at Duke University.
Multiple sclerosis (MS) is a disease of the brain and spinal cord (central nervous system), which can cause a disability. In MS , a person's immune system attacks the protective nerve fiber covering and causes communication problems between your brain and the rest of your body to stop. Though this disease has been around for a long time, it doesn't seem to be as diagnosed or even acknowledged in our African community when compared to the common infectious diseases. Such diseases, though rare, are slowly plaquing our community and it is about time we talk about them. Today, our guests speak extensively on MS from living with it to the course of therapy. She is currently completing an integrated Master's by Research in Cardiovascular Biology and a Ph.D. in Cardiovascular Science at the University of Edinburgh. She has endured several surgical procedures and endured many hospitals stays. She has also been misdiagnosed for 15 years with Neuromyelitis Optica- which is a rare, severe central nervous system autoimmune illness that affects the optic nerve and spinal cord. Chelsey has a truly amazing story...here is the cliff note version ________________________________________________________________ “You don’t need sight to have a vision.” – Chelsey Tucker While she may not be able to see the vibrant colors of her stunning paintings, this does not stop Chelsey Tucker from sharing her beautiful collection that will debut at the O Gallery on April 18th. Diagnosed with a very rare autoimmune disease, Neuromyelitis Optica or NMO, which leaves her color blind and visually impaired, Chelsey uses emotion to feel the painting while creating. She paints as an expression of how she sees the world. This is a notion that is resonating directly with her rapidly growing clientele. Chelsey has been drawn to the creative arts since she was a child, singing and songwriting for several years. But it wasn’t until 2018 that she joined the arts community professionally. She began by posting photos of her paintings to her social media accounts, sharing only with family members and close friends. Falling in love with her paintings, her friends and family encouraged her to share these gifts with the world. Thus, kick starting her quick rise within the Nashville scene as an in-demand artist. “Do not let your circumstances derail your success. You can discover new gifts.” NMO doesn’t just affect Chelsey’s vision. During periods of relapse, those with NMO often lose feeling in their hands. While this may seem like a deterrent and a provocation to stop altogether, this is not an option for Chelsey. She needs to create. Her subjects and methods may change depending on her symptoms, but this certainly does not prevent her from maintaining the same level of creativity and professionalism for which she is known. When holding a brush is no longer an option, she will use a technique involving her fingers directly on the canvas and a special pouring method. “Elements of creativity are required in order to establish new norms, especially with how this disease progresses.” Residing just outside of Nashville TN with her husband and their 5 children, Chelsey prioritizes family above all things. She derives a great deal of inspiration from her family and they support her every step of the way. She is also a believer in giving back to the community. Proceeds from gallery sales will benefit the Guthy Jackson Foundation, which donates to those afflicted with NMO. Understanding Neuromyelitis Optica and Neuromyelitis Optica Spectrum Disorder: NMO-101
In this podcast, Dr Jeffrey Bennett discusses his review article “The Treatment of Neuromyelitis Optica”.
Neuromyelitis optica (NMO) predominantly affects women, some in childbearing age, and requires early therapeutic intervention to prevent disabling relapses. We report an anti-AQP4 antibody-seropositive patient who became pregnant seven months after low-dose (100 mg) rituximab application. Pregnancy showed no complications, and low-dose rituximab restarted two days after delivery resulted in neurological stability for 24 months. Remarkably, her otherwise healthy newborn presented with anti-AQP4 antibody and reduced B lymphocyte counts in umbilical cord blood, which normalized three months later. Confirming and extending previous reports, our case suggests that low-dose rituximab might be compatible with pregnancy and prevent rebound NMO disease activity postpartum.

Background: The diagnostic and pathophysiological relevance of antibodies to aquaporin-4 (AQP4-Ab) in patients with neuromyelitis optica spectrum disorders (NMOSD) has been intensively studied. However, little is known so far about the clinical impact of AQP4-Ab seropositivity. Objective: To analyse systematically the clinical and paraclinical features associated with NMO spectrum disorders in Caucasians in a stratified fashion according to the patients' AQP4-Ab serostatus. Methods: Retrospective study of 175 Caucasian patients (AQP4-Ab positive in 78.3%). Results: Seropositive patients were found to be predominantly female (p < 0.0003), to more often have signs of co-existing autoimmunity (p < 0.00001), and to experience more severe clinical attacks. A visual acuity of 1 myelitis attacks in the first year were identified as possible predictors of a worse outcome.

In neuromyelitis optica (NMO), the monoclonal B-cell antibody rituximab is a therapeutic option. Little is known about the course of NMO and the safety of rituximab during pregnancy. In this study, we report the clinical course of a patient with NMO after application of rituximab 1 week before inadvertent conception. Mother and child did not experience any adverse event, and the postpartum development of the baby was completely normal up to 15 months. Clinical course of NMO was stable during the entire pregnancy. This case illustrates a favorable outcome in a pregnant NMO patient and her child after therapy with rituximab.
Background: A polyspecific, intrathecal humoral immune response against neurotropic viruses such as measles, rubella and varicella zoster virus (MRZ reaction, MRZR) is present in 80--100% of patients with multiple sclerosis (MS), but has not to date been evaluated in patients with neuromyelitis optica (NMO).Aims: To evaluate whether MRZR distinguishes NMO and MS.Methods: 20 patients with NMO and 42 with MS were included. The intrathecal synthesis of antibodies against measles, rubella and varicella zoster virus was detected by calculation of the respective antibody indices (AI).Results: A positive MRZ reaction, as defined by a combination of at least two positive AIs, was found in 37/42 MS, but in only 1/20 NMO patients (p
Es werden zwei Fälle von Neuromyelitis optica beschrieben, die jeweils eine hochgradige Erhöhung der Proteine im Liquor aufwiesen. Während ein Patient nach 6monatigem Krankheitsverlauf starb und pathologisch-anatomisch untersucht werden konnte, überlebte der andere Patient mit erheblichen Residuen. Dieser Fall zeichnete sich dadurch aus, daß deutlich erhöhte IgA- und IgM-Werte sowohl im Liquor als auch im Serum vorlagen. Es wird diskutiert, ob die gefundenen Liquorveränderungen möglicherweise eine Abgrenzung entsprechender Fälle gegenüber der multiplen Sklerose zulassen.
