Friends, this episode is a departure from my usual offerings. I will return to meditations and occasional interviews after my mini book tour, but today I'm talking about my novel DAWNLAND, and what IT has to do with meditation, particularly the question: Do we sometimes meditate by accident? And if so, what's happening there? Inadvertent meditation might seem like an oxymoron. We usually associate meditation with intention and focus. Even devotion. But I think there are moments when life meditates us. And the more we practice, the more we open ourselves for this to happen spontaneously. The characters in DAWNLAND don't meditate in a traditional sense, but they each have their own way of connecting inwardly. I describe examples in case they resonate with you. This talk invites you to relax your grip on "solving" and "fixing," to be radically open to the silences that emerge in your day, to notice when life meditates you. Savoring those moments, even for a breath, allows a sense of spaciousness to seep into your mind and your body, a spaciousness that allows the responses you need in any given moment to arise on its own. Highlights From This Episode “More frequently oncology nurses are using intramuscular injection techniques when giving certain hormonal therapies for cancer treatment and for cancer symptom management. Some examples of those are fulvestrant for treatment of hormone receptor–positive, HER2-negative breast cancer, leuprolide as androgen deprivation therapy in prostate cancer. This is also used off label for breast cancer management. It's used for premenopausal ovarian suppression and also in noncancerous conditions like endometriosis and uterine fibroids.” TS 2:04 “Inadvertent injection into the sciatic nerve is one of the most common causes of sciatic injury. It has significant morbidity associated with it. And even for drugs like fulvestrant, the prescribing information notes reports of sciatica, neuropathic pain, neuralgia, peripheral neuropathy—all related to dorsogluteal injection.” TS 6:09 “When administering an IM injection to someone who is cachectic, you don't want the subcutaneous tissue to bunch up. So you can kind of stretch this over with your nondominant hand, as in the Z-track method, and then grasp the muscle between your thumb and index finger. That's going to help you ensure that you're getting that muscular injection.” TS 11:47 “Z-track is a way that you inject so that there's no leakage back out into the subcutaneous space. Clean your area as usual. You displace the skin and the subcutaneous tissue that's over that muscle, and then you inject slowly into the muscle. Once you remove the needle, then you release that tissue. And it kind of seals it over and prevents that leakage back up into the subcutaneous space.” TS 14:19 “I think ventrogluteal injection is less commonly done. There are documented issues with confidence in landmarking and giving it to that site, so practice is necessary. A great way to identify the ventrogluteal site on yourself to start is to stand up and put your hand at your side. You feel for the top of that iliac crest. Place your hand right below the iliac crest and then just start marching in place. You're going to feel that muscle contraction right away. This also works when you abduct your leg. Abducting the leg is helpful when a patient is at a side-lying position to give a ventrogluteal injection—you feel that muscle contraction.” TS 17:06 “I wish it would be front of mind to encourage adverse event reporting related to any injection you're giving. These types of reports—they drive improvement measures and monitoring. And then when things are underreported, it leads us to anecdotal reports. So really monitoring any change, trying to get some baseline data on adverse events with injection is really important.” TS 26:32

Also, the process takes much longer because of the backlog in the courts. The records are 100% public, so there's no privacy whatsoever. If you don't want scammers to harass your children by knowing how much they received, make sure you have an estate plan.While avoiding expensive lawyers and keeping your final financial information private and away from the eyes of scammers are great reasons to have an estate plan, the primary reason is to preserve family unity. As I mentioned in the previous episode, family unity is the most important legacy you leave behind. Without an estate plan, your heirs may have some strong disagreements. Relationships could be ruined over a simple thing like money.Will vs. trustSome law firms prefer wills over trusts because the result is more lucrative. That's why some law firms charge so little for wills; they want the probate business. Changing a will requires certain steps; the same witness must sign the updated will. Trusts are easier to change than wills, and assets will be distributed based on an attached document. That document can be periodically updated, and distributions are based on the latest version. A trust is much more flexible than a will to make those changes. Trusts are great when you have several beneficiaries on accounts. You won't need to update your accounts if you've named the trust as the beneficiary. With a will, you can't control the distributions. With a trust, you can for some beneficiaries. Smoothly transferring assetsCertain situations almost require a trust, as inadvertently disinheriting children is too easy, as with blended families. For example, if a husband and wife each have kids from a previous marriage, and the husband were to pass away, the wife may inherit everything. Then, when she passes, only her children may inherit all of the money. Inadvertently, this would disinherit the husband's children from the previous marriage. Because of gift taxes and other complications, the solution isn't as simple as inheriting children giving a portion to the others.One important thing to note about the transferring of assets is that property transfers first by title, then by beneficiary designation, and finally by probate or will through the courts. Some assets can't have a beneficiary named, like a house. So if your will states that your 401k will be split between your kids but names only one of your kids as the beneficiary, that beneficiary will supersede the will. The dust that settles on the various surfaces in your house may seem innocuous - a pesky inconvenience that disrupts the aesthetic of your home. However, dust carries many microscopic particles and organisms that can impact human health. Dust control is vital in creating healthier living spaces. What is dust? House dust is a heterogeneous mixture of substances from sources such as soil particles, clothing fibers, atmospheric particulates, hair, allergens such as mold and pollen, microorganisms including bacteria and viruses, insect fragments, ash, soot, animal fur and dander, skin particles, residues from cooking and heating, and bits of building materials. A reproducible house dust sampling is challenging and highly dependent on the method. The Wikipedia article on dust cites the 1981 book "House dust biology: for Allergists, acarologists, and Mycologists," - "dust in homes is composed of about 20–50% dead skin cells". However, according to this newer 2009 study from the American Chemical Society, 60% of household dust comes from the outdoors, specifically soil resuspension, and track-in. Even more recently, the Australian Microplastic Assessment Project asked members of the public to collect house dust in specially prepared glass dishes, which was then analyzed by 360 Dust Analysis. They found 39% of the deposited dust particles were microplastics. It is likely that dust composition is very heavily location-dependent and has changed noticeably with time. Effects on human health Respiratory damage Particles that evade elimination in the nose or throat tend to settle in the sacs or close to the end of the airways. The macrophage system is a crucial part of our body's immune defense. When we breathe in dust or foreign particles, macrophages engulf and 'eat' these invaders, helping to keep our lungs clean. However, there's a limit to how much a macrophage can handle. If there's too much dust, the macrophages can become overwhelmed and not clear it all out. When this happens, the excess dust particles can accumulate in the lungs, leading to inflammation or other lung diseases. Cooking, open fireplaces, and smoking indoors add fine dust to your home and contaminants of concern, which are associated with poor health outcomes. Each year, 3.2 million people die prematurely from illnesses attributable to household air pollution caused by the incomplete combustion of solid fuels and kerosene used for cooking (see WHO's household air pollution data for details). Particulate matter and other pollutants in household air pollution inflame the airways and lungs, impair immune response and reduce the oxygen-carrying capacity of the blood. Allergies Dust mites, tiny organisms that feed off house dust and air moisture, are among the most common indoor allergens. In addition to allergic rhinitis, dust mite allergy can trigger asthma and cause eczema to flare. Mold, pollen, and animal hair in dust can also trigger allergies. These allergens permeate our indoor spaces and become part dust. Exposure to them can lead to various allergic reactions, from mild symptoms such as sneezing, itching, and nasal congestion, to more severe responses like asthma attacks. Furthermore, constant inhalation of these allergens can lower one's immune response over time, leading to chronic allergic conditions. Toxicity Dust can transport toxic substances, such as heavy metals and persistent organic pollutants, contributing to various health issues over time. Chemicals used in pesticides, clothing, and furniture can combine with dust in our homes. Toxic flame retardants are used in countless domestic products and can make their way into dust.
