Host: Darryl S. Chutka, M.D. Guest: James T. Gaensbauer, M.D., M.S. Each year, we recognize World TB Day on March 24. This annual event commemorates the date in 1882 when Dr. Robert Koch announced his discovery of Mycobacterium tuberculosis, the bacillus that causes tuberculosis (TB). World TB Day is a day to educate the public about the impact of TB around the world.  The CDC, along with our partners and colleagues around the world, share successes in TB prevention and control and raise awareness of the challenges that hinder our progress toward the elimination of this devastating disease. Tuberculosis continues to be a global health concern.  Despite being preventable and very treatable, this disease continues to cause suffering and death.  It accounted for an estimated one and a quarter million deaths worldwide in 2023. In the United States, cases have been increasing over the past several years, and this is also true in children, especially in those under the age of 5 years. What are the common risk factors for tuberculosis in children? Is the disease different in children compared to adults? What should we, as a primary care provider do when we suspect tuberculosis in a young patient? These are some of the questions I'll be asking my guest for this podcast, James T. Gaensbauer, M.D., M.S., a pediatric infectious disease specialist at the Mayo Clinic as we discuss “Tuberculosis in Children”. This Thursday, 24 March is World Tuberculosis Day. An estimated 9.9 million people fell ill with TB in 2020 and 1.5 million people died from the disease. TB continues to be a blight in the Indo-Pacific region, particularly in Papua New Guinea, Indonesia, Kiribati and the Republic of the Marshall Islands. Papua New Guinea bears the largest burden of disease in the Pacific, including a heavy burden of multidrug-resistant TB. In 2020 almost 30,000 cases were notified there, including 600 cases of multidrug-resistant and extensively drug-resistant TB.But TB is preventable and curable. So why is it still one of the most infectious and deadly diseases on the planet? On today's episode we are joined by three experts who are on the front line of trying to bring an end TB.Dr Mel Spigelman is the President and Chief Executive Officer of the Global Alliance for TB Drug Development – the TB Alliance – and a Member of its Board of Directors. In his time at the TB Alliance, Dr Spiegelman has been a leader in developing a regimen-based paradigm of TB drug development and leading the TB drug research field.Dr Margaret Kal is the director of the PNG National TB Program (NTP). In this role, Dr Kal is responsible for the development of all national TB guidelines, policies, research, national program reviews and all other plans and policies for TB prevention and care in PNG. Australia's Department of Foreign Affairs and Trade supports the work of the NTP.And finally, Dr Philipp du Cros is an Infectious Diseases Specialist in TB Elimination and Implementation Science at the Burnet Institute. He has extensive experience in the management of TB/HIV programs and has worked with MDR TB programs across Tajikistan, Uzbekistan, India, Myanmar, Uganda, Swaziland and Zimbabwe. The theme of World TB Day 2022 - ‘Invest to End TB. Save Lives.' We talk to all of our guests on this episode about practical ways the global community can invest to end TB and the urgent need to invest resources to ramp up the fight against TB.We hope you learn something from the conversation and join the fight to end TB @TBAlliance @CentreHealthSec

It's described as PNG's 'number one killer' with thousands acquiring the disease every year. Tuberculosis has long posed a serious threat to the people of Papua New Guinea but the Covid-19 pandemic has made it worse. This coming Thursday is World TB Day but the civil society group, Businesses4Health, or B4H, want to see a greater emphasis on raising awareness about the disease. Project manager for B4H, Ann Clarke, told Don Wiseman that they want to see a year long focus on the deadly disease. She explained how Covid-19 had affected TB treatment.

Every year, TB kills an estimated 1.4 million people and infects another 10 million people, with about 400,000 of those cases resistant to two or more drugs. Covid-19 has disrupted health systems and displaced funding and frontline staff away from TB, which models estimate will cause an additional million cases per year over the next five years. In this episode, we talk to Adrian about the grave impact the pandemic is having on global TB efforts, how to build local health security capacities that control TB and respond to future outbreaks, how international scientific partnerships speed up R&D and create platforms with broad applications, and the recent emergency use authorization by the FDA of J&J's Covid-19 vaccine.   Adrian Thomas is Vice President for Strategy and External Affairs at Johnson & Johnson Global Health. We at Otsuka remember this every day. It is the driving force behind our commitment to discovering new tools to eliminate TB in our lifetime. As the world’s leading private investor in TB research and development, it has been our goal for the past 40 years to find new, effective solutions to eliminating this disease that claims 1.5 million lives a year. We know that TB can affect anyone, anywhere in the world. That is why this year, we focus on those who are truly on the front lines of this disease – the healthcare workers, the patients, and their families. Each day this week we will profile stories of those who are making a difference in this fight and the inspiration they provide to others here. People such as Dasha and Grisha Lavrushiny, a young married couple from Russia, who have both struggled with TB, including the extensively-drug resistant form of it, but in the end have won the battle. Today, they are helping others receive correct diagnosis and treatment. Li Lulu, a young mother from China, developed TB meningitis, a particularly dangerous type of TB, right after giving birth. The encouragement and support of her family had given her the strength needed to complete treatment and today she is cured. The support of loved ones and the dedication of healthcare workers on the frontlines of the battle, such as Stuart Pancho, a registered nurse from the Philippines who leads trainings for other healthcare workers. The personal bonds they have established with their patients have proven essential to their treatment success. Though treatment for TB may be complicated and lengthy, it is a curable disease. Yet an estimated 3 million people a year go undiagnosed or untreated. Much progress remains to be done until every person is correctly diagnosed, treated and cured. We at Otsuka are committed to continuing our efforts so that, ultimately, we can live in a world free from TB.
