Episode #452: “We still had a lot of ideals… but we had some illusions, so to speak.” François Nosten has spent decades on the Thai-Myanmar border, where war, disease, and displacement overlap endlessly. He arrived in the 1980s, a young doctor from Toulouse with Médecins Sans Frontières, drawn by a sense of purpose. What he found was devastation: malaria sweeping through camps, killing faster than bullets. “There were more of the student dying of malaria than from the fighting,” he recalls of the post 1988 movement. Nosten met British scientist Nicholas White, and their work helped pioneer artemisinin-based treatments, which transformed malaria care worldwide. “If you test and treat systematically, early, quickly, then the people don't die anymore,” he explains. For a moment, it seemed victory was possible: “One year later, malaria was gone from the Thai side.” But the disease returned, mutating and persisting through poverty and conflict. When Myanmar's 2021 coup collapsed its health system, millions were displaced. Aid stopped, clinics closed, and outbreaks flared again. “Tuberculosis is still very serious worldwide… more than HIV,” he warns. “If funding is being cut… I think that tuberculosis will explode again.” Nearby, scam compounds now imprison thousands in unsanitary, lawless towns. “They are like towns,” he says, almost as big as Mae Sot itself.” Nosten still reflects on the conviction and purpose that drove his early ambition as a young doctor. “I did my medical school to be able to travel and to do something that I think was useful,” he says. Now, decades later, he continues that same work, even as the border he serves teeters once again on the edge of collapse. “If you have a stable country… you can control malaria,” Nosten says. “But here, everything conspires against stability.”

In this episode, we hear how Generative AI is making it into the consultation room - but not through NHS endorsed routes - surveys suggest that ⅔ of doctors are using AI, for backoffice tasks - but also increasingly for information and diagnosis. David Navarro, a research fellow in generative AI at the Australian Institute of Health Innovation, Charlotte Blease, associate professor at the Participatory eHealth and Health Data Research Group at Uppsala University, and Marcus Lewis, GP in London, reflect on what we know about the real way in gen AI is being used - and what “triadic care” (doctor, patient and AI) will mean for the future of the therapeutic relationship. We also hear from Teppo Järvinen, professor of orthopaedic surgery at Helsinki University, about surgical subacromial decompression - a 10 year follow up of a double blinded placebo controlled trial, confirms that surgery is no more effective than standard care. Yet surgical interventions continue - we hear why. Finally, we go to a Cholera clinic in Nigeria, where Médecins Sans Frontières are running cholera treatment centres, which you can help by donating to our Christmas appeal. Civil society groups in South Africa have launched a high court application to force authorities to stop what they call xenophobic vigilantes blocking access to healthcare facilities in Gauteng. The groups, including Médecins Sans Frontières and the Treatment Action Campaign, say patients are being turned away from clinics due to their nationality or lack of identity documents. They're calling for the authorities to ensure safe access to healthcare and remove anyone obstructing medical services. The organisations accuse the state of failing to uphold the Constitution and protect the right to healthcare. Elvis Presslin spoke to Director of Advocacy and Analysis at Médecins Sans Frontières (MSF), Claire Waterhouse

Katie Treble grew up crying at about how all the king's horses and men couldn't put Humpty Dumpty together again. It was that compassion that made her the perfect candidate for doctoring during war as an adult. When Dr Katie Treble decided to swap the good vibes and beautiful beaches of Byron Bay for work with Médecins Sans Frontières (MSF) she knew she would be in for a shock. Nothing could have prepared her for the desperate need she encountered in the Central African Republic (CAR) in the midst of a civil war.But Katie was even more affected by the courage and kindness of her colleagues. She came away from her months in Bria, CAR knowing that her time as a humanitarian doctor would change her own life in deep ways, and so when she got back to Australia she started the work of trying to make sense of it all.Field Notes from Death's Door is published by HarperCollins. This episode of Conversations was produced by Meggie Morris, executive producer is Nicola Harrison. It explores medicine, university, war, civil war, Africa, humanitarian crisis, Gaza, Israel, Palestine, MSF, doctors without borders, access to medicine, hospital, conflict zones, PTSD, malaria, defence, navy, Kenya, France, Jamaica, Haiti, natural disaster, murder, infant mortality rate, vaccination, religious war, Islam, Christianity, genocide, MDMA therapy, psychology, recovery, healing. To binge even more great episodes of the Conversations podcast with Richard Fidler and Sarah Kanowski go the ABC listen app (Australia) or wherever you get your podcasts. Dr. Mohammed Abu Mughaisib, Deputy Medical Coordinator for Médecins Sans Frontières in Gaza, discusses his experience treating patients amid a dire humanitarian situation in Gaza.
In 1971, 13 men sat down in a Paris office to launch what would become one of the world's best known humanitarian organisations: Médecins Sans Frontières, or Doctors without Borders. The men were among hundreds of volunteers responding to an appeal by the French medical journal, Tonus, after a major cyclone devastated East Pakistan. The campaign sparked the idea for the charity. The 13 founders – two journalists and 11 doctors – drew up a charter aiming to provide medical care regardless of race, religion or politics. MSF's first missions included helping victims of a Nicaraguan earthquake and a hurricane in Honduras. And in 1999, the charity won the Nobel Peace Prize in recognition of their work. Dr Xavier Emmanuelli, one of the founders, talks to Jane Wilkinson about setting up MSF, and his early humanitarian work.Eye-witness accounts brought to life by archive. Witness History is for those fascinated by the past. We take you to the events that have shaped our world through the eyes of the people who were there. For nine minutes every day, we take you back in time and all over the world, to examine wars, coups, scientific discoveries, cultural moments and much more. Recent episodes explore everything from the death of Adolf Hitler, the first spacewalk and the making of the movie Jaws, to celebrity tortoise Lonesome George, the Kobe earthquake and the invention of superglue. We look at the lives of some of the most famous leaders, artists, scientists and personalities in history, including: Eva Peron – Argentina's Evita; President Ronald Reagan and his famous ‘tear down this wall' speech; Thomas Keneally on why he wrote Schindler's List; and Jacques Derrida, France's ‘rock star' philosopher. You can learn all about fascinating and surprising stories, such as the civil rights swimming protest; the disastrous D-Day rehearsal; and the death of one of the world's oldest languages.(Photo: Doctors from Médecins Sans Frontières in Chad, 1981.
