Send us Fan MailOn our Inside Geneva podcast this week, we host a debate on a key question: what's the point of foreign aid?“What's the point of mobilising taxpayers' money and sending it to faraway countries to assist vulnerable people? And how do you sell that to your own taxpayers?” says Gilles Carbonnier, former Vice-President of the International Committee of the Red Cross (ICRC).There are more conflicts and greater needs, yet donor countries want to spend less.“What's the point of mobilising taxpayers' money and sending it to faraway countries to assist vulnerable people? And how do you sell that to your own taxpayers?” says Gilles Carbonnier, former Vice-President of the International Committee of the Red Cross (ICRC).There are more conflicts and greater needs, yet donor countries want to spend less.Do we think some people's tragedies are less important than others? If a child is in pain in Khartoum, that's exactly the same as a child being in pain in New York or London. If a mother is grieving in Gaza, that's exactly the same as a mother grieving in Geneva,” says Chris Lockyear, former Secretary General of Médecins Sans Frontières/Doctors Without Borders (MSF).How can aid agencies better explain what they do?“What defines us is humanity, and humanity means we cannot stay idle when we see others suffering. We have to act and do something to try to protect them, assist them and prevent suffering, regardless of where this happens,” says Carbonnier. Listen to the full episode on Inside Geneva.Get in touch!Email us at insidegeneva@swissinfo.chTwitter: @ImogenFoulkes and @swissinfo_enThank you for listening!

Attacks on the medical mission: identification of issues and good practices

ICRC Humanitarian Law and Policy Blog

Play Episode Listen Later May 6, 2026 15:55

During the last decade, attacks against hospitals have been a hallmark of almost every conflict. What humanitarian medical practitioners have witnessed and denounced for years has become alarmingly routine. International humanitarian law (IHL) is sometimes criticized for failing to protect the very purpose that justified its own existence, particularly when the states responsible for its enforcement remain incapable or unwilling to stand up for the protection of medical facilities in armed conflicts. In this post, Claude Maon, Legal Director for Médecins Sans Frontières (MSF), highlights the need to build consensus around the specific protection of the medical mission under IHL. She calls for good faith interpretation in applying these rules to ensure the effective protection of hospitals by all actors in real situations of attack. In doing so, she underscores that attacks on hospitals are not inevitable and argues that this persistent trend of violations can only be reversed if states respect the law by adopting good operational practices to protect the medical mission, alongside ensuring accountability for attacks affecting medical facilities in armed conflict.

An estimated four million Canadians currently live abroad. So what compels someone to ditch their life in Canada and start over in a different part of the world? From Bogotá to Tokyo, Cape Town to València, Now or Never crisscrosses the globe with expats living out some big, bold adventures right now.Joshua Will dreamed of living in Japan since he was 14-years-old, so when he got a job with the Japan Exchange and Teaching Programme (JET) he was ecstatic. But he never expected to find himself on tiny Yuge Island, navigating the wild world of Japanese real estate, and renovating a 100-year-old house with no experience and few language skills. At age 75, Janet Skog-Reid moved to Spain with her children and grandchildren. But when they returned to Vancouver, Janet decided to stay. How this grandmother who spent her life caring for others, is now chasing a life that is finally her own. When Calgarian Aaron Cresswell bumped into Laura Fajardo at a crowded bar in Bogotá, the two instantly hit it off — so much so, that he ended up moving to the country to be with her. But it soon sunk in that while Aaron found his forever person, he definitely didn't find his forever home in Colombia. Barb Deck is no stranger to intense situations — when she's at home, she's a pediatric ICU nurse at Sick Kids in Toronto. But since joining Médecins Sans Frontières, Barb spends most of her time tackling even bigger challenges, flying to conflict zones around the world to help deliver care.Four months ago, Chloe Wilde and her partner quit their jobs, sold their Toronto home, and bought a one-way ticket to South Africa with their two-year-old son. Chloe tells us why she feels a responsibility and pressure to live a "big, bold life," and what it's really like backpacking with a toddler.

Global Health Matters is the monthly podcast from TDR, if you like our content then please subscribe. We embrace the fact that public health and global health are intertwined and through our podcast we hope to engage with broader topics bringing together experts from across the globe providing a platform for wider engagement.      For this episode we bring you a topic that rarely reaches the top of the global health agenda, that is the subject of the health of refugees and migrants.  This podcast episode lays out the key issues for the listener and through our guests it builds an awareness to ensure this topic gets better attention in the future. Global Health Matters host Garry Aslanyan speaks with the following guests:  Eugen Ghita Human Rights Monitor and President, Roma Lawyers Association, Romania RomaJust  Reem Mussa  Humanitarian Advisor and Coordinator of the Forced Migration Team, Médecins Sans Frontières  During this podcast we hear the health experiences of Eugen Ghita, who was a migrant and he is now working as a human rights monitor on behalf of the Roma community in Europe.  Eugen identified that there is a lack of information dedicated to the refugees and many language barriers.    Reem Mussa from MSF is able to give us context in relation to the policies of the different types of refugees and migrants.  Many countries do allow for access to health care regardless of the legal status of people, but not all, some may only provide for emergency healthcare.  MSF promotes the importance of a separation between border control/ immigration schemes and healthcare access as some migrants without right to remain may fear authorities or deportation and so not access healthcare for that reason.    Kindly note that this podcast discusses mental health for refugees and migrants, TDR would like to refer you to WHO guidance for those seeking further support. Related episode documents, transcripts and other information can be found on our website. The global landscape of humanitarian aid is facing unprecedented challenges, marked by increasingly complex, protracted crises and massive funding disruptions. How do organizations like Médecins Sans Frontières (MSF), or Doctors Without Borders, navigate this new reality, especially when core principles like neutrality are tested and government support is pulled? In this powerful conversation, Sana Bég, Executive Director of MSF Canada, offers a compelling perspective on leading through global crisis and funding disruption. She details the profound ripple effects of crises and cuts—such as the disruption in USAID funding—even on independently funded organizations like MSF.Sana shares her insights on:The critical mission to “bear witness” to suffering, balancing impartiality with the responsibility to speak out.The moral hazards of having to stretch resources to fill gaps left by others, and challenging the assumption that “growth equals impact”.The role of “hopeful optimism” in leadership, and the urgent need to disrupt entrenched mindsets to ignite a new era of truly equitable humanitarianism.Dive into this essential discussion about strategic leadership, maintaining integrity under pressure, and how MSF Canada is mobilizing global action in a world that is attempting to normalize the removal of humanitarian norms.

Episode #508: Damian Lilly, a veteran humanitarian and human-rights specialist, who has worked in conflict zones across the world, believes assistance must be joined with protection and accountability. “We can't just be there to assist people—we also need to be there to protect them.” He formed this conviction through his work with Médecins Sans Frontières, documenting sexual violence in places such as Afghanistan, South Sudan, and the Congo and turning testimony into pressure on governments. Working with the UN, he returned to South Sudan later as Senior Advisor on the Protection of Civilians. The civil war there drove more than 250,000 people into UN compounds, and although his work helped shelter so many people, he looks upon it as a failure because in the end, there was no justice or redress. Protection without justice, he says, “really loses sight of what we're trying to do.” Later on, he was posted in Gaza in his role as UN Relief and Works Agency for Palestine Refugees: during that time, there were three Israeli military incursions into the territory between 2008 and 2014, with no real repercussions, which only reinforced to Lilly how impunity fuels repeated wars. Accountability, he says, fails not for lack of law but ofpolitical will. When his wife, also a UN employee, received a posting to Myanmar, Lilly and his family moved to Yangon. This was shortly before the coup. He reports watching the tanks roll down the streets. In response, Lilly co-founded the Myanmar Accountability Project (MAP) to pursue prosecutions abroad through what is called “universal jurisdiction,” where crimes against humanity in one country can be prosecuted in another country's national courts. MAP is seeking cases in countries like Indonesia, the Philippines, and Turkey. Lilly critiques the International Criminal Court and UN Security Council for their paralysis, timidity in engaging the junta, and reluctance to recognize the NUG as the people's legitimate representative. While the UN employs the claim of neutrality to justify continued relations with the junta, Lilly argues that true neutrality means fidelity to humanitarian principles, not moral equivalence. Despite bureaucratic inertia and shrinking aid budgets, Lilly insists that localization, persistence, and creative legal action can still advance justice. “It is a complex area,” he says, “but an important part of how we address many of these situations.”

What does it take to deliver high quality medical care in the middle of war, displacement and disaster? We gain a behind the scenes understanding from Chris Lockyear, Secretary General of Doctors Without Borders / Médecins Sans Frontières (MSF). This conversation offers a rare look inside one of the world's most recognised humanitarian medical organisations and the complex system that allows it to operate in some of the most dangerous and hard to reach places on earth. With around 70,000 staff working across more than 70 countries, the organisation provides emergency medical care to millions of people affected by armed conflict, disease outbreaks and natural disasters. In the past year alone, teams carried out more than 16 million outpatient consultations, alongside trauma surgery, treatment for malaria, tuberculosis and HIV, vaccination campaigns, and mental health support. Yet behind every clinic or hospital lies an intricate global operation that combines medicine, logistics, diplomacy and risk management. In this episode, MSF's Secretary General explains how humanitarian medicine works in practice. Teams must negotiate access with both state and non state actors, often in highly polarised conflict environments. Medical professionals work alongside logisticians, analysts and coordinators who ensure that drugs, equipment and staff can reach remote locations safely and reliably. The scale of the logistics alone is extraordinary. Medicines and vaccines must travel through complex supply chains while maintaining strict quality standards and often requiring temperature controlled storage. Equipment for surgery, sterilisation and treatment must arrive on time in places where infrastructure is limited or damaged. In many cases, care is delivered through mobile clinics operating from the back of a vehicle. Security is an ever present concern. Staff operate in environments where shelling, crossfire or kidnapping are real risks. Rather than promising safety, the organisation focuses on understanding risk, training staff and ensuring informed consent about the conditions in which they work. In 2025, eleven colleagues lost their lives while carrying out humanitarian work. The conversation also explores how knowledge gained in these extreme settings travels across the global health system. Experience with epidemic response, infection control and contact tracing developed in Ebola outbreaks later helped support hospitals and health ministries in Europe and the United States during the early stages of the Covid-19 pandemic. A defining feature of the organisation is its financial independence. Around 98 percent of funding comes from private donors, with more than 7.3 million donors contributing. This allows operations to be guided primarily by medical need rather than political priorities. Beyond funding, these contributions represent something deeper: a global expression of solidarity between people who will likely never meet but are connected through a shared commitment to helping others in crisis. For listeners interested in humanitarian medicine, global health, logistics, crisis response or international cooperation, this discussion offers an inside perspective on what it really takes to bring medical care to the front lines of human suffering.
