Chris Tench is a producer/songwriter/multi instrumentalist & foodie. Tench started out his professional career in the mid 1980's as a Drummer and guitarist in cover bands. He relocated from Hudson Ohio to Atlanta Georgia with original band Tabula Rasa where he also began working on short films and television. Moving to Nashville Tennessee in 1993, Chris joined up with long time friend/musician/puppeteer/writer and directorLiam Lynch to form the band Owens Ashe's. Idea: An improv show format which is an absurdist parody of a rigid agenda-based speech of a Toastmaster public speaking course. The name "toady mustard" is based on a funny autocorrect re-spelling of the word "Toastmaster". Idea: A comedic performance (either on stage or on TikTok) in an over-the-top costume representing one of the seven chakras which shows both love for and criticism of gurus and the culture of yoga, meditation, and "New Age"/"Eastern" religion/spirituality. The Booker Prize was announced earlier this week - it went to Irish writer Paul Lynch for his dystopian novel "Prophet Song" - but Paul Lynch wasn't the only Irish writer shortlisted for the award. In fact, he wasn't even the only Irish writer named Paul to make the shortlist - The Bee Sting, by Paul Murray, was also on there. Two other Irish writers made the Booker LONG-list. And that's quite apart from other writers like Sally Rooney, who's pulling up trees wherever she goes Of course, Ireland has an incredibly rich literary tradition - from George Bernard Shaw to James Joyce, to Oscar Wilde, Samuel Beckett and WB Yeats, to name but a few. So what is it about this little island nation north-west of Europe that allows it to produce such lyrical, erudite authors? Matthew Ryan is an expert in Irish Literature at the Australian Catholic University in Melbourne.

The British pound has fallen to a record low against the US-dollar as markets react to the UK's biggest tax cuts in 50 years. The sterling fell close to $1.03 early on Monday before regaining some ground to stand at $1.06 The pound, and the euro, also plummeted in early Asia trade. The Bank of England says it won't hesitate to raise interest rates by as much as needed to return inflation to the 2 percent target. Matthew Ryan Davies is the author of the young adult novel This Thing of Darkness, which was highly commended in the Victorian Premier's Literary Awards. Today I've brought in his debut adult novel Things We Bury.Transporting us to the town of Pent, Matt offers up a portrait of life in a regional town through his central family, the Hardings.It's a sad fact that tragedy brings us together.Dane, Jac, and Josh don't get together as much as they should. Spread out across the country; Jac in Sydney, Josh all over the country as his TV work takes him and Dane, who never left Pent. It just never seems like there's enough time.Now the siblings must converge on their hometown after their father is in a car accident that leaves him in a coma.It's not just the accident. Each of the siblings has their own drama eating away at their peace. The reunion isn't destined to go smoothly. Pent is no longer the same town they grew up in, but the ghosts of the past still rest there.While their father remains in a coma, questions emerge that have them wondering if there is more to his accident that they first assumed. He left the town and changed his name to become someone in entertainment. Now having realized his dream he is dogged by an incident that has blown up on social media and left him hiding his face beneath a baseball cap.Jac has returned with a surprise. She's engaged, but doubting herself, and the person she'd most like to tell is lying in a coma. Jac was always their father's favourite, which makes it harder to deal with the possibility he did this to himself.Dean has had to step up in his dad's absence and is questioning more than ever the path he chose. Can he really ignore his own dreams any longer to run their father's company.In each of the siblings stories and in the hovering questions around their father's crash we have the specter of mental health that too often gets ignored, especially amongst men.The title Things We Bury hints at secrets and these drive the narrative forward but also remind us of the difficulties we can face when we feel like we have to go it alone.Through the shifting perspectives, as each chapter moves through a different character point of view, we see the siblings' struggles and also hear as they question what might have been on their father's mind. Together they question how he was feeling and come up against the usual suspects of stoicism and a fear of seeming weak in front of others.While the central mystery of why drives the siblings to try and uncover their father's state of mind we are also offered parallel narratives of their own struggles. It's an effective and expansive look at the fact that mental health is a concern we all share and that we deal with it best when we deal with it together.Things We Bury is a page turner that offers us valuable insights. Biologics are monoclonal antibodies that block T2-mediated immune responses (IL-3, IL-4, IL-13, IgE). They are administered subcutaneously and follow various dosing regimens. Biologics are a viable treatment option in patients with recurrent nasal polyps who have failed conventional therapies, such as high doses of antihistamines, topical steroids, and systemic steroids. It is still considered as a last line treatment because of the high cost associated with production of monoclonal antibodies. However, not all patients with recurrent nasal polyps are good candidates for biologics. The patient must present with a specific endotype––the T2-mediated etiology. T2-mediated patients can be identified through their high responsiveness to steroid therapy, positive history for allergic asthma and atopic dermatitis, and high peripheral eosinophil and serum IgE levels on a CBC with differential. In a surgery-naive patient with a temporary steroid response, it is best to perform sinus surgery first in order to widen the nasal mucosal surface area for efficient delivery of topical therapies. However, if post-surgical intranasal steroid sprays and saline irrigations are ineffective, biologics should be considered. It is best to avoid surgery and skip straight to biologics in patients with comorbid conditions that prevent surgery, patients with severe asthma, and patients with high peripheral IgE counts (>1000). Once the decision to start biologic therapy is made, many factors have to be considered, such as insurance pre-authorization, administration methods, and frequency of dosing. Each biologic manufacturer has a “hub” that assists physicians and patients in navigating biologic dosing, delivery, and insurance paperwork. They will often have co-pay assistance programs for patient benefit as well. Common side effects observed in biologic trials are arthralgia, injection site inflammation, oropharyngeal pain, and headaches. However, all the doctors agree that these side effects are more mild than those of long-term systemic steroid use, which include avascular necrosis, cataracts, sepsis, and thromboembolic events. Picking which biologic to prescribe is a clinical decision because they have not been subjected to comparative trials yet. The three biologics currently on the market are: Dupilumab (anti-IL-4 receptor), Omalizumab (anti-IgE), and Mepolizumab (anti-IL-5 receptor). All work to prevent T2 immune signaling by targeting different receptors. Factoring in comorbid conditions, payer systems, and dosing regimens can help a physician choose the best biologic for a nasal polyps patient.

The methods of allergy testing (prick test, intradermal test, and blood draw) are clinically interchangeable, with skin tests being the cheapest. Unfortunately, there are nuances in interpreting allergy test results, and a patient's symptom complex may have a great deal of variability. There may not be correlation between a patient's symptoms and their allergy test results. While an allergy test can be helpful, chronic rhinitis can be diagnosed and treated without testing. As a first-line therapy for allergic rhinitis, Dr. Ryan will prescribe a combination of a nasal steroid spray and a nasal antihistamine spray, and then follow up with the patient to see if these help with symptom relief. The doctors also talk about treating allergic rhinitis and medication considerations in subgroups such as children and patients with nasal polyps, asthma, and hypertension. For patients who still struggle with symptom control, the doctors discuss two types of immunotherapy, subcutaneous immunotherapy (SCIT) and sublingual immunotherapy (SLIT).
