Dr. Miglietta is the Chief of Trauma & Critical Care at the new Wynn Hospital in New York. He is currently the Program Director of the General Surgery Residency Program, Chairman and Professor of Surgery, and Associate Dean at Touro College of Osteopathic Medicine. He is the former Chief of Acute Care Surgery at the New York-Presbyterian Hospital. Prior to that role, at Columbia, Dr. Miglietta served as Chief of the Division of Trauma & Critical Care at New York University School of Medicine and Director of Trauma and Co-Director of the Surgical Intensive Care Unit at the renowned and historic Bellevue Hospital Trauma Center in New York City.Dr. Miglietta founded and serves as Director of the Homeland Security Mobile Trauma Unit. This unit provided medical care to dignitaries and federal agents in conjunction with the United States Secret Service and Department of State - Diplomatic Security Service for over 10 years. He is an Honorary Police Surgeon providing expert trauma consultation for the New York City Police Department. From 2004 to 2014, he served as the Physician Advisor to the United States Secret Service. Dr. Miglietta is a Special Deputy U.S. Marshal assigned to the NY/NJ Fugitive Task Force.Dr. Miglietta spearheaded medical education in northern New Jersey by creating 11 residency programs. There comes a point in our life when we all need to take an adventure. For most that means a vacation to an exotic faraway continent. For others, that might mean taking a new job, or changing careers altogether. Or starting a family. All of these are adventures of one sort or another. All are wonderful. But then there is Howard Rodenberg. Howard decided at age 60 to take a real-life adventure. To the South Pole, where today he is serving as a physician at the Amundsen-Scott South Pole Station with the U.S. Antarctic Program. Prior to this monumental decision Howard served as a physician advisor to the CDI program at Baptist Health in Jacksonville, FL, and chaired the ACDIS regulatory committee. And took adventures of the mind playing the fantasy role playing game Dungeons and Dragons. All of which immediately endeared me to him and led to this interview. On this episode (conducted tenuously via satellite link) we cover: • Why do this? What did your family think? • What does an average day look like? Have you had any medical emergencies? • Do you spend much time outside? Can you? • What's the hardest part about life in the South Pole and what don't most people understand about the conditions and isolation? • Do you have to document and code your work? • What have you learned about yourself? • What's the first thing you're planning to do upon your return? While there is not as much CDI, coding, and health records management talk as you'd find on an ordinary episode of OTR (although we do get into this, and CDI may be part of Howard's future plans), I guarantee you'll be inspired by his story. And possibly find the courage to take an adventure of your own. Even if it doesn't lead to the South Pole.

Dr. Mauvareen Beverly has.  She has developed a care management program to get to the root cause of this patient population to reduce hospital readmissions.Dr. Beverly says “ We must treat the whole person and not just the patient” in the case of sickle cell patients who are the least addicted yet the most vilified when it comes to empathetic care.  She has a progressive training program that asks the question WHY to reveal the human values in a clinical care setting.Her cultural competency program targets patients that have been targeted as non-compliant and have reduced their readmission rates substantially.  Her nonjudgemental empathetic approach has proven to be a  very effective method for improving patient outcomes. Dr. Mauvareen Beverley is an executive-level physician, with 20 years of experience advocating for improving patient engagement and cultural competence for all populations, especially the geriatric, immigrant, and African American communities. As AVP, Physician Advisor for NYC Health + Hospitals, she sponsored the first Conference on Improving the Health of the Elderly Black Population. She implemented the concept of “The Bridge Team” whose role was to bridge the gap in care for the most complicated and vulnerable population and as a result improve care and health outcomes. As Deputy Executive Director of Kings County Hospital, her team decreased Congestive Heart Failure readmission from 30% to 18.7% in less than 2 years. Dr. Beverley is a Fellow at the New York Academy of Medicine (NYAM) and her Abstract “Health Disparities and Epidemics: Perception vs. Reality was selected for presentation at NYAM 12th Annual History of Medicine and Public Health Night. Dr. Beverley is also a Member of the American Medical Association (AMA) and Medical Society, State of NY (MSSNY), working in collaboration with Westchester County Medical Society, Westchester Academy of Medicine, and Putnam County Medical Society in developing Patient Engagement and Cultural Competence Training Program with CME credits Dr. Beverley received her bachelor's from Boston University and MD from University of Buffalo School of Medicine. Today we meet a hospitalist who transitioned to work as a remote physician advisor. Dr. Robert Craven completed his medical degree at The University of Tennessee Health Science Center and his Internal Medicine Residency at Carolinas Healthcare System. He worked as a hospitalist for nine years. He was offered a position as a physician advisor for utilization management unexpectedly at a previous employer. He took the job and has completed a year of full-time work in that position for a large hospital system, mostly working remotely. Today he tells us what his job entails, why he likes what he is doing, and how you can land a similar position. Rob explained the details of his job as a physician advisor. It is somewhat unique in that he is working remotely for a large system, with occasional opportunities to go on-site if needed. The job is more flexible than a full-time hospitalist position, and it allows him to spend more time with his family. It is intellectually stimulating. One of the greatest ways to build your influence and leadership abilities is to lead when you have no official authority. We most often encounter this situation when working with volunteers. Host, Laura Prisc, wasn't expecting to discover this framework within a hospital, however, that's exactly what she uncovered when she engaged in a meaningful conversation with Brent Jackson, MD, VP and Chief Medical Officer of Dignity Health, located in Sacramento, CA.   While he was a practicing physician and general surgeon for many years, he decided to step away from his practice and shift into an administrative role. While he's a member of the health center's Executive Team, the physicians he advocates for, and guides do not report to him in the typical sense. Turns out, they are contracted through the hospital, rather than actual employees.   As such, Brent had to approach leading them a bit differently than we imagine it working. As the liaison between the Executive Team and the Medical Staff, he sees himself functioning as the voice of the physicians, advocating, supporting, and serving them.   To do that, he focuses on relationship building, which allows him to earn credibility and build trust over time. They come to see him as transparent, reliable, and having their best interests in mind, and that's how it works.   Not surprisingly, these particular skills – what many would frame as ‘soft skills' – were not taught in medical school. Medical school teaching was more aligned with the command & control model of leadership, which really doesn't work well long-term.   It wasn't until Brent went back to earn his MBA that he saw what he'd been missing. He built new and deeper listening skills, came to understand how much it matters for people to understand why things happen the way they do, and recognizes that each of the people with whom he interacts is having a slightly different experience, with a different perspective about how things work.   It's a powerful exploration that touches on all things leadership, embracing growth, relationship building, and self-care (hint: one of his practices, which he shares openly, is playing the guitar).   Dr. Jackson's Bio  Brent C Jackson, MD, MS, MBA, FACS — Dr. Jackson is currently the Chief Medical Officer for Mercy General Hospital, a Dignity Health facility in Sacramento, California.  He was a practicing General Surgeon for 15 years before retiring from clinical practice to pursue a career as a physician leader.  He went back to school for his MBA while working as a Medical Director for River City Medical Group, a Medi-Cal IPA.  He then worked as the Physician Advisor for Mercy General Hospital before being promoted into the VP/CMO role.  He finished his MBA in 2020.  Dr. Jackson also holds a B.A. from UC San Diego in Biochemistry and Cell Biology, an M.S. in Neuroscience from Case Western Reserve University, and an M.D., also from CWRU.  He also served for eight years in the US Air Force Reserves and earned an Honorable Discharge at the rank of Major.  In his spare time, he plays guitar, participates in high-performance driving events, and spends time with family, including his four grandchildren.  Connect with Brent:  On LinkedIn

In this episode, I interview Dr. Ralph Wuebker, CMO at Optum 360, a revenue cycle company offering end to end solutions for providers (including hospitals, physician group, labs, etc.). Ralph’s current primary responsibility is building Optum’s On Site Physician Advisor (OSPA) program, including: sales support, recruiting, training and oversight of physicians at over 70 hospitals Prior to that he worked as VP at Executive Health Resources, and was Medical Director at Great West Healthcare. He received his MD from the University of Missouri – Kansas City School of Medicine and completed a pediatric residency at St Louis University. He later completed an MBA from Washington University in St. Louis. What I like about today's interview is learning how Ralph transitioned from pediatrician, to physician adviser, to chief medical officer of a large provider support organizaton. And, we spend a few minutes discussing the cost/benefit analysis of pursuing an MBA We get into many other topics, with lots of useful advice along the way.
