Dr. Black is the current president of the National Pressure Injury Advisory Panel (NPIAP) and has a passion for pressure injuries! In this first episode, Dr. Black is joined by fellow NPIAP Board Member, Kathleen Vollman. Kathleen has many years as an expert in the field of critical care nursing and during this episode, Dr. Black and Kathleen discuss pressure injury development in the critically ill and how to properly turn said patients. Kathleen dives into how she assists others in the knowledge of turning patients and how to quelch the anxiety of the bedside provider. Evidence supports those errors and poor quality are often the result of faulty system or processes versus individual behavior and are impacted by the work culture. Over the past couple years she has completed full distance triathlons, half ironmans, and multiple sprint races. She had placed in top 10 age group seven times and qualified for the 2020/2021 ironman 70.3 world championships. Her passion for this sport has discovered a new potential in these moments.

In the year of the nurse, it is important to remember a famous quote by Florence Nightingale; “It may seem a strange principle to enunciate as the very first requirement in a hospital that it should do the sick no harm”. In our current work cultures, some basic nursing care activities designed to prevent harm, are frequently seen as just tasks to be completed before the end of the shift or just one more project. With health care infections a worldwide problem we can make a difference in preventing the invasion or halting the spread of microorganisms by implementing basic care strategies to reduce the source of the infection. This session provides an in-depth examination of the science of bathing critically ill patients and the impact it has on infection. This session will seek to dispel any myths and address the evidence-based practice and provide suggestions for successful implementation leading to a change in care.

Prone positioning has been utilised to recruit alveoli to improve oxygenation while preventing complications of ventilator-induced lung injury in patients with ARDS for over 30 years. Recently with new studies, the use of prone positioning is now considered front line therapy and utilisation of the technique has increased significantly since COVID 19. With the mortality rate of the ARDS patient remaining at 40%, we need to implement evidence-based practices that work. This session discusses the physiological mechanisms of the prone position for reducing lung trauma and improving oxygenation. How a team can successfully build and implement a proning protocol is outlines. An exploration of the evidence used to define the patient likely to respond to prone positioning, identify the appropriate time to initiate therapy, and time spent in the prone position are discussed. Evidence-based strategies for turning and sustaining the patient in a prone position are outlined to ensure safety for the patient and healthcare worker. As practitioners, we have the potential to influence patient outcomes through a safe non-invasive positioning technique.
Evidence supports that we are in the midst of a significant nursing shortage and it is expected to worsen in the next ten years. Ms Vollman is a Critical Care Clinical Nurse Specialist, Educator and Consultant. She has published & lectured nationally and internationally on a variety of topics including critical care, pulmonary medicine, sepsis. From 1989 to 2003 she functioned in the role of Clinical Nurse Specialist for the Medical ICU’s at Henry Ford Hospital in Detroit Michigan. Currently, her company, ADVANCING NURSING LLC, is focused on creating empowered work environments for nurses through the acquisition of greater skills and knowledge. In 2004, Kathleen was inducted into the College of Critical Care Medicine in 2009 she was inducted into the American Academy of Nurses. In 2012, Ms Vollman was appointed to serve as an honorary ambassador to the World Federation of Critical Care Nurses.
There is no magic bullet that will reduce the mortality rate of the ARDS patients. It is the early and consistent applications of evidence-based supportive therapies that results in improve short and long-term outcomes. Understanding the ARDS Berlin definition that differentiates the syndrome based on severity of hypoxemia to be mild, moderate or severe, helps the experienced nurse target the correct supportive care. Our knowledge of the diffuse injury to the alveoli and capillary membranes caused by inflammatory mediators and ventilator-induced injury helps for greater understanding of the clinical signs/symptoms and why certain therapies support the ARDS patient better. Due to the complexity of the patient, it is critical that the skilled frontline nurse have a user-friendly structure (8 P’s) to trigger implementation of evidence based care practices. The goal is to PREVENT further injury of the lung, reduce ventilator-associated events and physical and cognitive decline. The team must utilize evidence-based ventilator strategies (PEEP) while ensuring adequate volume for the heart to work at maximum efficiency (PUMPS & PIPES) to help deliver oxygen to the tissue. The critical care nurse plays a key role in minimizing oxygen demand and balancing brain recovery through appropriate use of sedation and paralytic agents (PARALYSIS). The timing and implementation of the prone position (POSITIONING) can determine its effectiveness on oxygenation and prevention of lung injury. Once prone position is no longer useful, early progressive is critical to improve long-term cognitive and functional ability. Appropriate nutrition (PROTIEN) to promote healing and reduce inflammation is an essential component of the care plan. Incorporating evidence based (Protocols) such as the ABCDEF can impact overall outcomes in the ICU patient. It takes all our knowledge and skill to ensure the ARDS patient not only survives but is able to return to a meaningful life as soon as possible.

In 2011 a magnitude-9 earthquake shook northeast Japan and wracked the coast with a massive tsunami, devastating towns, destabilizing economies nationwide—and causing meltdowns in nuclear power plants. Nonfiction author William T. Vollmann joined us to share firsthand accounts of the fallout from these disasters, bringing excerpts from his newest book No Immediate Danger, whose title sardonically co-opts the reassuring mantra of official Japanese energy experts. Vollman cautioned against nuclear power, drawing parallels to other sprawling practices such as fossil fuel extraction and industrial manufacturing as contributors to climate change. He invited us to examine the ramifications of nuclear power and evaluate whether the risks it poses are outweighed by the economic demand for electrical power and the justifiable yearning of people all over the world to live in comfort. To illustrate the realities of these risks, Vollmann recounted visits made at significant personal risk over the course of seven years to the contaminated no-go zones and ghost towns of Fukushima, Japan. Equipped first only with a dosimeter and then with a scintillation counter, he measured radiation and interviewed tsunami victims, nuclear evacuees, anti-nuclear organizers, and pro-nuclear utility workers. Vollmann shared the powerful and sobering object lesson of Fukushima—and brought us into a broader conversation on the factors and human actions that will define our relationship with the environment for generations to come. William T. Vollmann is the author of ten novels, including Europe Central, which won the National Book Award. He has also written four collections of stories, including The Atlas, which won the PEN Center USA West Award for Fiction. He is the recipient of a Whiting Writers Award and the Strauss Living Award from the American Academy of Arts and Letters. His journalism and fiction have been published in The New Yorker, Harpers, Esquire, Granta, and many other publications. Tom came in to the studio on a cold wintery day and both of us had head colds. Milwaukee based, Tom put out a hell of a good show even with the cold.
