Struggling with motivation lately? You're not broken and you're definitely not lazy. In this episode, I share a real-time reflection on why motivation feels shaky during seasons of massive identity transition, especially when life, work, and motherhood are all evolving at once. When our identity is clear, action feels easy but when we're questioning who we are becoming, everything can feel heavy and confusing. I break down why motivation is an unreliable driver and why waiting to “feel ready” keeps you stuck. We dive into the three things that actually create follow-through: commitment, identity, and systems. I'll challenge how you think about commitment (with a comparison that might sting a little), and explain why saying you're committed isn't the same as acting like it. We'll talk about how anchoring your actions to who you want to be, not just what you want to achieve, changes everything. I'll also walk through practical ways to reduce friction by building systems that support you on low-energy days. If you're in a season of transition, questioning your next chapter, or frustrated with your lack of consistency, this episode will help you reframe motivation and rebuild alignment. Stop waiting to feel motivated and start building something that works even when motivation is gone. Lindsee's first birth was a fairly typical hospital experience—and it left her feeling there had to be a better way to bring life into the world. When it came time to choose where her second baby would be born, she opted for a midwife-run birth center, and the difference was profound. The experience was respectful, empowering, and deeply positive. Like so many after a redeeming birth, Lindsee became fascinated by all things birth and eventually went on to become a doula herself. Pregnant with her third, she eagerly began planning a home birth. Tragically, that pregnancy ended in a devastating second-trimester loss due to a rare genetic anomaly. Much of how the hospital handled both the birth and death was deeply disappointing, with the exception of one compassionate nurse who stood out during an otherwise excruciating experience. Lindsee longed for another baby, but it was a long road before she finally saw a positive pregnancy test again. This time, she was able to welcome her baby at home, experiencing the birth she had always hoped for. Certified Nurse Midwife Roxanne explains the significance of the second trimester anatomy scan, typically performed between 18 to 22 weeks of pregnancy. She delves into the primary objectives of the scan, which include assessing the baby's organs, limbs, and overall growth, as well as evaluating the placenta's position and condition. Roxanne also covers common reasons for follow-up scans and helps alleviate anxiety by discussing the low likelihood of identifying true abnormalities. Feeling more energized and less nauseous? It's the perfect time to prepare for a smoother birth and postpartum. Trish shares a practical, no-fluff checklist of 11 actionable steps to take in the second trimester - from mobility exercises to crucial conversations with your provider - that can reduce interventions, ease labor, and boost your confidence. Whether you're a first-time mom or a seasoned parent, these tips are designed for real, busy mamas to help you feel prepared and in control.Your Essential Second Trimester Checklist:#1: Anatomy Scan (Around 20 Weeks): Go beyond the gender reveal - understand checks for placenta location, baby's growth, heart, and fluid levels. Bring questions and follow up with your provider for clarity.#2: Gentle Pelvic and Mobility Routines: Dedicate 5-15 minutes daily to movements like pelvic tilts, squats, cat-cow stretches, and glute bridges to reduce tension, improve baby's positioning, and open your pelvis by up to 30% during labor.#3: Nutrition for Energy and Baby's Development: Focus on high-protein snacks, iron-rich meals, choline from eggs, DHA from supplements, and hydration. Balance cravings while preventing anemia and supporting brain growth.#4: Mindset and Breath Practice: Combat fear and tension with 5 minutes of daily breathing (inhale for 4, exhale for 6) and visualization to activate your parasympathetic nervous system for easier labor.#5: Build Your Birth Team and Define Roles: Educate your partner, family, and friends on your birth goals. #6: Screening Tests and Vaccine Timing: Research and discuss upcoming tests with your provider. Ask about alternatives and risks.#7: Fetal Positioning Moves: Adopt habits like forward-leaning positions, birth ball sitting, side-lying releases, and pelvic rocks to encourage optimal LOA (left occiput anterior) positioning and avoid back labor.#8: Maternity Leave and Logistics: Plan now - talk to your boss, HR, and insurance about leave, work handoffs, and paperwork to reduce third-trimester stress.#9: Baby Gear Essentials vs. Convenience: Prioritize must-haves like a car seat, safe sleep surface, diapers, thermometer, and basic clothes. Borrow or skip gadgets; survey mom friends for realistic advice.#10: Advocacy Scripts and Being Informed: Practice simple phrases like "What are the risks of waiting?" or "No, thank you" to navigate decisions calmly and firmly during labor.#11: The "What If" Conversation: Schedule a detailed chat with your provider about induction thresholds, failure to progress, pain management, VBAC policies, and newborn care like delayed cord clamping. We're diving into the top functional and conventional tests to consider after the second trimester or late pregnancy loss, especially if you've been diagnosed with low AMH, high FSH, diminished ovarian reserve (DOR), or premature ovarian insufficiency (POI). If you've had a second trimester loss or late pregnancy loss after 20 weeks and been told “everything looks normal,”this episode is for you. We dive into what's often overlooked in conventional care and how a functional fertility approach can help uncover underlying imbalances that impact conception, hormone health, and pregnancy outcomes. In this episode we cover 7 categories of testing to consider after late loss especially if you have low AMH, diminished ovarian reserve, autoimmune issues, recurrent miscarriage, or unexplained infertility. You'll learn: The top clotting and thrombophilia markers to test (including Factor V Leiden and antiphospholipid antibodies) Which inflammatory and immune markers (hs-CRP, ANA, cytokines, NK cells) are often missed and why they matter How the vaginal microbiome and hidden infections like ureaplasma can cause second-trimester loss The role of chronic stress, adrenal hormones and the HPA axis in pregnancy outcomes Why a comprehensive blood chemistry panel can reveal nutrient deficiencies and hormone imbalances that are missed by conventional labs Key methylation and genetic SNPs (like MTHFR) that impact detoxification, clotting and hormone metabolism The impact of gut health and stool testing on immune tolerance, estrogen balance and inflammation We'll also explain how this whole body functional lens can guide your next steps in preconception planning, whether you're trying again naturally or preparing for IVF. This episode is for you if: You've experienced a loss after 14 weeks of pregnancy and are seeking deeper understanding and support. You want to explore both conventional and functional medicine approaches to uncover underlying causes. You're looking for practical lifestyle, testing, and healing strategies to improve future pregnancy outcomes. --- TIMESTAMPS [00:00:00] Introduction: Late term pregnancy loss overview, compassion, and what to expect in this episode [00:02:30] Functional fertility testing for late term loss thrombophilia panel, immune markers, inflammation, and infections [00:06:00] Stress hormones, nervous system support, and comprehensive blood chemistry for improving pregnancy outcomes [00:09:00] Blood sugar, insulin, and comprehensive thyroid testing in pregnancy loss [00:12:00] Genetic testing, including MTHFR mutations and the importance of body healing before conception [00:14:30] Role of gut health, infections, and estrogen metabolism in pregnancy loss --- RESOURCES
Amniotic Fluid Sludge (AFS) has been theorized to be sonographic evidence of an underlying infection/inflammation. Others have proposed it may represent an organized clot from the placental surface. At the same time, the finding of AFS may be more common as a benign finding especially at/after 40 weeks as the amniotic fluid accumulates shed skin cells, vernix, and possibly meconium past 41 weeks. What can be tell the patient when we identify AFS in the early second trimester? What do we do with this? In this episode, we will summarize the data on second trimester AFS and review the evidence-based “next steps” in care. Does this require empiric antibiotic therapy in the asymptomatic patient? Listen in for details.

Today's episode is a two part episode where we discuss adoption and second trimester loss. Today's guest Dr. Tatiana Height wanted to become a mother through private adoption as she was single at the time. During this time she faced alot of judgement from people about trying to adopt instead of carrying a biological child. If you haven't listened to part 1 please go back to Episode 383 to listen. In this episode we discuss her finding out she was pregnant, her second trimester and miscarriage loss story, her 3rd failed adoption, and her grief and healing journey. We discuss how the third mom changed her mind the day the baby was due. Her due date was right at the same time as Tatiana was entering her second trimester, so she put her adoption plan on hold to focus on pregnancy. At 19 weeks and 3 days, her water broke and she gave birth to a baby girl, but she didn't survive. 10 months later she got pregnant again, but miscarried around 6 weeks, and she is now trying to adopt again. What I have figured out so far during pregnancy is that your attitude and how you approach the changes in your mind and body are of utmost importance. In many seasons of life we get lost in comparing ourselves to others, all that does is take away from the joy of being our unique selves. Maybe it's the pay at your job, the lavish trips your neighbors go on, the car your boss drives, or your partner's sickening motivation to workout every day. No matter what, comparison is the thief of joy. Today we deep dive into the raw truth of my second trimester. From the saggy forehead skin, to how tight my pants are, let's be honest about what women go through during pregnancy.

Tracing women's experiences of miscarriage and termination for foetal anomaly in the second trimester, before legal viability, shows how such events are positioned as less ‘real' or significant when the foetal being does not, or will not, survive. Invisible Labour: The Reproductive Politics of Second Trimester Pregnancy Loss in England (Berghahn, 2024) by Dr. Aimee Louise Middlemiss describes the reproductive politics of this category of pregnancy loss in England. It shows how second trimester pregnancy loss produces specific medical and social experiences, revealing an underlying teleological ontology of pregnancy. Some women then understand their pregnancy through kinship with the unborn baby. This interview was conducted by Dr. Miranda Melcher whose new book focuses on post-conflict military integration, understanding treaty negotiation and implementation in civil war contexts, with qualitative analysis of the Angolan and Mozambican civil wars.
