What does it take to earn the nickname, ‘The Leonardo da Vinci of heart surgery'?That's the moniker given to today's guest - a man who pioneered high-profile and often controversial procedures, but also helped drive huge medical progress; carrying out around 2,000 heart transplants and 400 dual heart-lung transplants during his 60-year career.Sir Magdi Yacoub is Emeritus Professor of Cardiothoracic Surgery at Imperial College London, and Director of Research at Harefield Hospital's Magdi Yacoub Institute. Inspired by a surgeon father and impacted by the tragic early death of his aunt from a heart condition, his medical career includes various surgical firsts alongside numerous research projects, to further our understanding of and ability to treat heart disease. He headed up the teams that discovered it is possible to reverse heart failure, and that successfully grew part of a human heart valve from stem cells for the first time. But it hasn't always been plain sailing. At times, his work – such as early, unsuccessful transplant attempts, or using a baboon as a life-support system for a baby – attracted serious public criticism.Speaking to Professor Jim Al-Khalili, Sir Magdi reflects on the highs and lows of his cardio career, and offers his advice to the next generation of surgeons and researchers hoping to make their mark in heart medicine. However, thanks to innovations led by surgeons such as Prof. Donald Nuss of Eastern Virginia Medical School, these procedures have undergone a remarkable transformation. Such work has shifted the paradigm from radical surgery to minimally invasive solutions, changing lives and restoring confidence for countless patients. Many significant medical discoveries were accidents, such as Fleming's discovery of penicillin in 1929. Similarly, in 1958, Dr. Mason Sones from the Cleveland Clinic accidentally injected contrast into a patient's right coronary artery, leading to a cardiac arrest. However, this incident allowed the first detailed visualization of the coronary artery, eventually leading to the development of coronary angiography, which remains the gold standard today. We discuss risk factors, screening criteria from the U.S. Preventive Services Task Force, National Comprehensive Cancer Network, and the American Cancer Society, and why screening isn't universal despite its importance. Additionally, we explore current research, recent updates to guidelines, barriers to screening in underserved communities, and the potential of emerging technologies, such as AI, to enhance lung cancer screening in the future. Dr. Kozower, a thoracic surgeon, discusses the role of surgery in lung cancer treatment and the use of surgery in the diagnosis of lung cancer. The conversation then shifts to the PCORI study on lung cancer surveillance, where Dr. Kozower explains that the intensity of surveillance, such as the frequency of CT scans, does not impact survival rates for lung cancer patients in retrospect. We also discuss the future of lung cancer treatments.  This episode was supported by the Patient Centered Outcomes Research Institute (PCORI) and features this PCORI study by Dr. Kozower. Key Highlights: 1. Surgery is the primary treatment for early-stage lung cancer, serving both curative and diagnostic purposes. 2. Surveillance, including regular follow-up visits and imaging, is crucial for detecting new lung cancers, especially in individuals with a history of the disease. Screening, on the other hand, aims to detect cancer in its early stages in individuals without symptoms. 3. The narrative around lung cancer is changing due to advancements in molecular diagnostics, targeted therapies, and immunotherapy, offering more hope and optimism for patients. About our guest: Benjamin D. Kozower, MD, MPH is a Professor and Vice Chair of Surgery at the Washington University School of Medicine in St. Louis, MO. He completed his General Surgery training at the University of Connecticut in 2004 and his Cardiothoracic Surgery residency at Washington University in 2006. Dr. Kozower worked at the University of Virginia in Charlottesville, VA from 2006-2016 until returning to St. Louis in 2016. He is a General Thoracic Surgeon with a focus in thoracic oncology and directs the Thoracic Robotic Program at Barnes Jewish Hospital. He is also a clinical outcomes and health services researcher with funding from the Agency for Healthcare Research and Quality, the Patient Centered Outcomes Research Institute and the National Cancer Institute. Key Moments: At 7:29 "Surgery is the traditional treatment for early-stage lung cancer, patients who have small tumors typically confined to the lung when hopefully it can be curative." At 14:50 "Everybody's followed after their cancer treatment. What's not clear is how often should we follow people?" He is best known for developing the Cox-Maze procedure, the first cure for atrial fibrillation, which he performed for the first time at Barnes Jewish Hospital in 1987. The selected fellow spends up to four weeks studying at the Texas Heart Institute (THI) and Baylor St. Luke's Medical Center (BSLMC), learning from its team of world-renowned physicians and researchers. The Denton A. Cooley Fellowship provides an educational experience for North American cardiothoracic surgeons in their final year of residency or who recently completed their residency. The AATS established the program to honor those cardiothoracic surgeons who have demonstrated long-standing leadership and dedication over the course of their careers. The 2022 Denton A. Cooley Fellow is Dr. Clauden Louis, MD, MS, MPH, a recent University of Rochester Integrated Cardiothoracic Surgery Residency graduate. He is also a graduate of Howard University Medical School and Johns Hopkins Bloomberg School of Public Health. Dr. Andrea Wolf is Director of the New York Mesothelioma Program at the Mount Sinai Health System, which provides comprehensive, multidisciplinary clinical care for patients with suspected or diagnosed malignant pleural mesothelioma. She has expertise in surgery for pleural mesothelioma and VATS lobectomy, and research interests in mesothelioma, health care disparities, and lung cancer. Dr. Wolf graduated Cum Laude from Princeton University and earned highest honors and her medical degree at Harvard Medical School. She trained in General Surgery and served as Chief Resident at Massachusetts General Hospital in Boston. Dr. Wolf earned a Master in Public Health with a focus on Clinical Effectiveness at Harvard University School of Public Health while researching malignant pleural mesothelioma and early stage lung cancer as a Thoracic Oncology Research Fellow at Brigham and Women's Hospital, also in Boston. Post-cardiotomy shock carries a high mortality rate. However, early recognition and prevention strategies can help mitigate the risk for developing post-cardiotomy shock. Pearls and Quotes - Post-cardiotomy Shock Weaning from cardiopulmonary bypass is an intricate process that includes: rewarming the patient, de-airing the cardiac chambers, ensuring a perfusing heart rhythm, confirming adequate ventilation and oxygenation, removing the intracardiac catheters and cannulas and slowly reducing the blood diverted to the cardiopulmonary circuit and returning it small aliquots to the patient. Much to monitor during the process! Assessing the risk for post-cardiotomy shock prior to going to the OR is important. Consider left ventricular, right ventricular, and valvular function, and don't forget about the value of hemodynamic assessments (pulmonary artery catheter evaluations) to ensure patients are adequately compensated. Close peri-operative monitoring of hemodynamics, hemo-metabolic derangements, and acid/base status can help identify patients who are failing therapy and may require upgrade to temporary MCS. RV assessment is challenging. Utilizing both imaging and hemodynamic evaluations can help understand which RV's will require more support.
