In this episode of the Brawn Body Podcast, Dan is joined by Dr. Gabriel Petruccelli from Greater Washington Orthopaedic Group to discuss rotator cuff injuries, surgical considerations, reverse total shoulder, & more! Gabriel L. Petruccelli is an accomplished board-certified orthopaedic surgeon specializing in arthroscopy, sports medicine, shoulder replacements and general orthopaedics. Dr. Petruccelli joined Greater Washington Orthopaedic Group, PA in 2012, with three locations: Rockville, Silver Spring and GermanTown, Maryland. Dr. Petruccelli is a Maryland native, who graduated from The Heights School in Potomac, where he played basketball and soccer. Dr. Petruccelli graduated from the George Washington University with a Bachelor of Science degree in Exercise Science before receiving a certificate in Physiology at the Virginia Commonwealth University. He completed his medical degree from Ross University School of Medicine in Barbados in 2006 before relocating to New York City to complete his Internship and Orthopaedic residency at New York Medical College-St. Vincent's Catholic Medical Center/ Kingsbrook Jewish Medical Center in 2011. In 2012, Dr. Petruccelli relocated to the west coast to complete his fellowship with the San Diego Arthroscopy and Sports Medicine Fellowship. There he honed and specialized his skills within the field of sports medicine and open shoulder surgery. Also, during his time there, he worked closely with Major League Baseball's San Diego Padres. He also assisted the team physician for the San Diego State Aztecs. Given the opportunity to research, he published surgical technique videos for national orthopaedic society meetings that year. Dr. Petruccelli has received the Top Doctors Washingtonian Magazine Award in 2017, 2018 & 2019. Since 2016, he has been the Section Chief of the Department of Orthopaedics at Adventist Healthcare Shady Grove Medical Center. Dr. Petruccelli supports the athletic programs as the team physician at Georgetown Preparatory School and supporting his high school alma mater, The Heights School. He also gives valuable educational talks throughout the community. Dr. Petruccelli's main focus is to help each individual get back to their normal daily life and activities as soon as possible with top quality and very personalized orthopaedic care. Bone stress injuries (BSIs) occur in up to 20% of runners and military personnel. Typically, after a period of unloading and gradual return to weightbearing activities, athletes return to unrestricted sports participation or military duty approximately 4 to 14 weeks after a BSI diagnosis, depending on the injury location and severity. However, the time course of the recovery of the bone’s mechanical competence is not well-characterized, and reinjury rates are high. Bone density declined in both the injured and the uninjured legs and, on average, did not return to baseline for 3 to 6 months after a tibial BSI diagnosis. The observed time to the recovery of baseline vBMD, coupled with the high rate of recurrent BSIs, suggests that improved return-to-sports and military duty guidelines may be in order. I was recently on a zoom call with one of the members of The Soft Tissue Revolution and was asked a very straightforward question on why I am giving all this information away for free and what my real goal was with all of this. After spending the last episode airing out some of my grievances about what is going on in the world and calling people out for shaming others I decided its time to switch gears and rid myself of that negativity. I personally over the years have been a very self-motivated person, I didn't need a lot of hype and rah-rah to get fired up for the day and excited to get after that, I guess finding my purpose helped that out a lot for me. I do understand some of us do need more motivation and a polite kick in the ass to get us out of our funk and start focusing on doing the work and finding your passion and purpose. I decided to go a bit off format on this one and really just speak from my mind and hopefully help you learn from some mistakes I made when dealing with people throwing some hate and shade. There is a lot going on in our world right now, 99 percent off it we really have no control over. People are mad, sad, scared, hostile, and downright reactive in the "new" world we are living in. Many people are mad at their own situation, want to blame others for their business not being what it should be or flat out just want to be negative and pissed off. Instead of keeping their frustrations to themselves, they decide to attack others and throw lots of hate over what went wrong for them, blaming others and staying a victim in everything. In this episode, I share an encounter I had this week with someone on social media on the merits of being open or staying closed and really learned a lot about myself, what other people think, and some mistakes I made with getting emotional with other people that are angry. Sales are the lifeblood of your business, without sales you don't get to stay in business for very long. The issue with sales for most of us is feeling fearful about it, feel bad for asking for money, and really don't want to focus too much on getting better at sales. Most soft-tissue practitioners just want to focus on delivering treatment, getting better at treatment, and trying to get more clients in the door, the idea of selling more can really give us some deep visceral discomfort. This was our first week being back opened in our clinic after deciding to stay shut down for the past 8 weeks. My clinic is still under the designation of being an essential business, but we wanted to do our part to help flatten the curve and stay closed during the COVID-19 outbreak. After about 5 weeks we were anxious to get back, had a lot of patients reaching asking when we were going to open, and was really ready to get back to work. I wasn't too sure of how it was all going to go and even had a sleepless night before our first day back. Needless to say it was great to be back, it was challenging, it was rewarding, a little fearful at first and really gave me some new perspectives on the post-COVID-19 world we are living in and running a business. We are living in some truly chaotic, frustrating, and worrisome times. COVID-19 has completely shifted and altered the way we live our lives, run our businesses, and interact as humans. None of us were expecting this, none of this is anyone's fault and many people are worried about not only losing their businesses but losing their ability to provide for their families and their own livelihood. We now live in a world where the government can flat out tell us who is considered "essential" and who is considered "non-essential." Like it or not, that's just the way it ended up being, and unfortunately, we had no control of it. It's sad and disappointing to me that good quality and effective soft tissue practitioners, specifically massage therapists, were unfairly deemed "non-essential" and were one of the first groups to be shut down. The problem we are seeing is that the first businesses to be shut down will also be the last ones to get to open up, that just flat out sucks, but it is reality. It's going to be quite a tough road going forward for massage therapists, especially these next 90 days. In this episode, I give some real predictions based on facts and looking at the data of what the massage therapy profession will look like. The key take-home is to always look at the facts, make real judgments and decisions based on what is going on, and understand that things are going to change, regardless if you like it or not. During my time away from the clinic and during the lockdown from the COVID-19 virus I have had some amazing opportunities to talk with many massage therapists about their practices, their worries, their businesses, and the changes they want to make in the world. I have really learned a lot and it continues to solidify my stance on the importance of good hands-on soft tissue treatments being what is really going to make a difference in solving the pain pill epidemic. I still firmly believe that massage therapists have the best skillset for that and have the opportunity to deliver the best treatment out there. I have been spending the past few weeks having conversations with other soft tissue practitioners, specifically massage therapists about what is happening and what they think will happen in the post-COVID-19 world. Many people are scared, frustrated, sad, pessimistic and just going crazy from not being able to do the work they love and also dealing with the long-term financial consequences. I have been spending the past few weeks trying like hell to be a voice of truth, reason and full transparency about what is happening with all the chaos of COVID-19 and what the market will look like once things settle down. Some have listened, others stayed in denial and the rest are just going to wait and see how it plays out. We are now full-blown in the middle of this crisis and it really is a crisis we are all experiencing. Whenever there is a crisis there are always opportunity vultures out there trying to sell some type of program or transformation to help fix everything. These people make lots of lofty claims and give you business "advice" without any real data about what is going on in the market and what to expect. Here's the thing... Nobody knows how long this is going to last, what the long-term financial implications are going to be with all this and really what the world will look like. So specifically what I have seen is a lot of self-appointed "gurus" out there telling massage therapists to go all-in on shifting to an online business model and giving lofty claims that they are still able to charge just as much and people are gladly paying for that. Don't fall victim to these people or believe their nonsense. The truth behind all this is that you are going to have to drastically change how you provide treatment, how you market and how you run your business, but shifting completely to online is not a smart strategy long-term and I cover why it isn't in this episode. I don’t know about the rest of you but I have only been out of normal life and working for two weeks, but it feels like it’s been forever. I have found myself with more time on my hands than I ever could imagine and like a lot of you, I wasted the first few weeks sitting around and feeling sorry for myself. I got into bad habits, I slept too late, ate shitty foods and had more to drink than I really ever do. I think I was in denial at first about all this, it doesn’t help that the state where I practice Florida was really slow on bringing down stay at home orders. Needless to say, I got out of my routine, out of momentum and it really messed with me. We are all creatures of habit and have all been thrown for a loop here. Never in history has everyone been really focused on their overall health and wellness. People are starting to wake up on the importance of taking care of their health from all aspects. Nobody wants to be in the “high risk” category that makes them immune-compromised and once the dust settles on all this pandemic they will begin to re-evaluate what their true priorities are. There is going to be a huge opportunity as people that normally spent money on stupid stuff, like vacations, travel and going out to eat are now going to direct those funds to get healthier and taking care of their bodies. It’s going to take some time for people to come around to new changes, but once they do you need to be ready with a different, unique and value-driven product/ service that isn’t like anything you offered before. I had to take a hiatus from the podcast last week as the rigors and challenges that come with still running a very busy clinic got the best of me. I am AMAZED at how many people I talk to there about their business, practice and even skillset go along the line of, "I'm all good." It's not easy doing what we do, delivering the best treatment possible, trying to manage a business and often dealing with some really difficult patients. We get beat up, not just physically, but mentally and on a spiritual level. I have shared some of my struggles over the past few months and it really put me in quite a rut. When this happens it feels like the whole world is just giving us the middle finger and it really starts to spiral out of control. We end up being negative, too reactive and really start to dislike what we do. That really can put us even more in the negativity spiral. This week in practice I was starting to find myself getting into the negative headspace and it seemed to be getting worse until I had a real-life kick in the butt from a patient I have helped to really remind of WHY I do this and the impact/change I want to make. I don't know if you have noticed this as much as I do, but it seems every person out there is some type of coach or self-proclaimed "guru." I see my social media feed littered with these "imposters" saying they are the new and greatest thing out there, but if you actually do some research on these people they aren't worth a damn. Price is what you pay for something, value is what you truly get. There are many potential patients out there lurking that are always looking for the best deal or cheap solution to their injury. I made this mistake for the early part of my career and it really goes into our own human nature of just wanting to be liked by others. There are two types of data we collect when trying to see if the patient is improving with care: 1. Subjective ( Solely based on the patient's interpretation and their own data.) 2. Objective ( Solely based on things you can visualize, see or feel.) We are ALL guilty of relying too much on the subjective side of things and not getting enough real objective data to measure true progress. This can lead to many problems in care and I go over all of that today on this episode! One of the biggest lessons I learned in business and communication was a simple statement.... People buy based on EMOTION and justify it with LOGIC. As advanced soft tissue practitioners that spent the time learning anatomy, physiology and the complexities of the human body we all tend to sway towards the over-analytical side when selling and communicating with patients and it can lead to BIG problems on so many levels. Cynthia's husband was 47 years old when he suffered a cardiac arrest and subsequent traumatic brain injury. Cynthia and their two sons learned to overcome obstacles and make the most of what was left and re-envision life with disability.
