This episode features Richard Keagan-Bull (Faculty of Health, Science, Social Care and Education, School of Nursing, Allied and Public Health, Kingston University London, London, UK).   What is already known about the topic? Deaths of people with intellectual disabilities are often unanticipated, unplanned for and poorly managed. Service providers in residential settings are at the forefront of supporting people with intellectual disabilities at the end-of-life, but staff lack skills, knowledge, confidence and support within the area. In order to develop end-of-life care planning interventions that are welcomed by people with intellectual disabilities, their families and support staff, it is important to understand their perspectives, preferences and needs for end-of-life care planning.   What this paper adds? Participant perspectives provided a detailed view of end-of-life care planning involving four areas ([i] funeral planning, [ii] illness planning, [iii] life planning and [iv] talking about dying); what area participants focused on affected their views on whether, when and with whose input they thought end-of-life care planning should happen. Barriers to end-of-life care planning with people with intellectual disabilities included: reluctance to consider death and dying, with a preference to focus on ‘planning for living'; issues around understanding and communication; not knowing how to initiate conversations; dying being an abstract concept; and lack of staff training. Planning for illness (the last phase of life) was particularly abstract and difficult to do in advance.   Implications for practice, theory, or policy The definition of end-of-life care planning may be ambiguous, particularly for non-healthcare professionals. It should be clear what is meant and understood by people when using the term. The development of future interventions, resources and approaches for end-of-life care planning with people with intellectual disabilities should address the need for clarity around what is being planned for. Highlights From This Episode “There's a saying in this field: It's never too early to have the conversation until it's too late. And like I said, when my children turned 18, we completed advance care plans with each of them. … Thankfully, they were, and they still are, healthy, and they didn't need an advance care plan imminently, but that's actually the perfect time to do it. So, we had this conversation when there was no emotionality really attached to it, and that's the best time.” TS 7:31 “So, trying to give them as much autonomy as possible is really important. I always like to ask, ‘Would you like to have a conversation? Do you think that you can handle a conversation about advance care planning?' Or ‘What you would want someone to do for you if you're not able to speak for yourself?' They may say no, you know, and we have to respect that too.” TS 11:28 “I like to use an acronym called WAIT, W-A-I-T—Why am I talking? And frequently, I talk because I'm nervous. I'm so anxious at such, you know? Exactly. Just because we have a little training in this doesn't make it an easy conversation to have. So I often notice that when I'm feeling anxious, I fill the room with words. So saying to yourself, ‘Wait, why am I talking?' And if you realize ‘I'm talking because I'm nervous; I'm uncomfortable with this conversation,' remind yourself to stop because a little silence is not bad. It actually gives the patient a little time to think about the question.” TS 12:25 “Some young adults are very on top of this planning. You know, I think it's slow progress, but there has been some progress in that young adults are much more comfortable than a lot of us older people in having really difficult conversations. So we're the ones that are afraid to bring it up, but some of them are much more comfortable. So we have to remember that each of these people are individuals, and they may be very on top of this kind of planning or feel more comfortable having the conversations than we are. So it's important that we follow their lead and not make assumptions that because they're young, that they haven't thought about their own death.” TS 16:44 “I think a great question to ask them is just like, ‘What is your understanding of your diagnosis and prognosis?' Because they may have heard it already. They may not have absorbed all of the information. They may not be ready to talk about it. So asking them what's their understanding—if they say, ‘I'm dying; I know that,' that makes the conversation a little bit easier, right?” TS 18:30 I actually think [it's] more important—the healthcare proxy—than the forms, because you're never going to be able to possibly come up with every single scenario that could happen. So you're not going to be able to document like, ‘If this happens, do this,' for everything—but having a healthcare proxy who you've had conversations with about what your values are, not necessarily about every scenario.” TS 25:19 “Whatever your value is, you want to be able to have that conversation with your healthcare proxy so they can speak—I'm not even going to say for you—I'm going to say as you, so they can really advocate for you as if they were you and making sure your values and wishes are respected.” TS 25:54

To help you learn more about this field, Dana Bissontz, who has more than 25 years of nursing experience and 15 years in the medical-legal arena, shares her experiences in the area of life care planning. Life care planning involves the life expectancy of a plaintiff and calculating the cost to provide him or her with the ability to meet anticipated needs. These are often greater than a layperson would realize. Equipment, such as wheelchairs, need to be serviced, altered, and replaced. Misdiagnosis can sometimes be over whether the individual has Type 1 or Type 2 diabetes. Since different treatments are required for the two major forms of diabetes, misdiagnosis can be a major problem. When the possible presence of diabetes is not tested for in emergency situations, life-threatening crises may develop. A legal nurse consultant working in this field needs to bring the same degree of precision and detail orientation to examining the medical records as does an expert witness. The LNC considering this specialty may find this an attraction. Dana emphasizes that the expert witness, trained to look for causation, needs to focus, instead, on results. From her experience, she advises that if you get involved in this kind of work, you must know the expected side effects of a drug. Basically, know everything that drug is expected to do, understand its interactions with other drugs, and meticulously determine whether there could be other causes for a condition. She emphasizes that the field isn't limited to LNCs. Doctors, physical and occupational therapists, and other professionals may work in it, either in primary roles or as consultants. Dana, however, that LNCs can bring the greatest expertise to this field. She advises that if it interests you, to immerse yourself in the subject. You can learn by studying the work of a number of professional organizations devoted to this field and question practitioners. For her, this work fulfills her desire to help others in a very practical way. The same may be true for you. Wellness programs often offer simple health solutions or use words that may be triggering for employees with adverse childhood experiences. How can we influence employee health when we don’t have the full picture of what an employee has been through in their life?  Today’s guest, Diana Bishop, has done an extensive amount of work to understand the implications of Adverse Childhood Experiences (ACEs), how they affect adult health and wellness, and what strategies wellness professionals can use to work through ACEs rather than against them. Diana is a Senior Learning Consultant, certified Health and Wellness Coach, and Life Care Planning instructor trainer for Kaiser Permanente, San Diego. With 20 years of experience working with ACEs, as well as personal experience understanding and working through childhood trauma, she is an industry expert on developing programs for people with high ACE scores and is dedicated to meeting employees where they’re at in their emotional journey, not just their health and wellness.  In today’s episode, Diana shares not only her professional background but also her experience growing up as a child in a volatile home. She explains the origins of the Adverse Childhood Experiences Study and breaks down its implications including a higher risk of chronic disease and premature death. Diana encourages wellness professionals to take a look at the language surrounding health initiatives and discusses some helpful strategies to avoid isolating and disengaging employees with high ACE scores.
