In this episode of Insurance Shoptalk, Eric Stein sits down with David Prejeant, Vice President of Client Relations at Alacrity Solutions, to discuss one of the fastest-growing challenges facing the insurance industry: AI-powered fraud. With more than two decades of experience in claims operations, property adjusting, catastrophe response, and commercial claims management, David shares firsthand insights into how artificial intelligence is changing the way insurers, MGAs, MGUs, carriers, and claims professionals evaluate risk and investigate claims. The conversation explores how AI can be used to create convincing but fraudulent documentation, including weather reports, business interruption records, loss histories, and other claim-related materials. Eric and David discuss the growing need for new fraud detection strategies, enhanced adjuster training, and AI-powered verification tools that can help insurers identify inconsistencies before fraudulent claims are paid. They also examine how organizations can use AI responsibly to improve claims handling, streamline data analysis, connect fragmented systems, and provide better information to underwriters and clients. If you're interested in claims management, insurance fraud prevention, underwriting, AI-powered analytics, risk management, or the future of insurance operations, this episode offers valuable insights into both the opportunities and risks that AI brings to the industry. #InsuranceShopTalk #InsuranceFraud #ArtificialIntelligence #ClaimsManagement #Insurtech #Underwriting #RiskManagement #PropertyClaims #CommercialInsurance #ClaimsTechnology
In recognition of Alzheimer's & Brain Awareness Month, this episode of the Research Insights Podcast revisits "Dementia Neurology Deserts and Long-Term Care Insurance Claims Experience in the United States." The discussion explores how limited access to neurology specialists—often referred to as dementia neurology deserts—may correlate with long-term care insurance claims experience. Geography can influence the timing of diagnosis, access to treatment, and the progression of care needs, with meaningful implications for both health outcomes and financial security. As dementia affects individuals, families, and care systems alike, this episode offers important perspective on how gaps in specialty care intersect with long-term care planning. Listen now to revisit this timely and impactful conversation.
Show Notes What if you could catch a plumbing leak before it became a five-figure repair? In this episode of Be a Smarter Homeowner, Beth Dodson talks with Ian Greene about why leak detection is one of the smartest upgrades a homeowner can make. They break down how flow-based leak detection works, why hidden leaks are so destructive, and how smart systems can shut water off automatically before a small issue becomes a major disaster. Beth also shares her own experience with water damage and why even a "small" leak can create months of disruption, unexpected out-of-pocket costs, and a long repair process. Ian explains why toilets, hidden pipe failures, freezing conditions, critters, and faulty fittings are among the biggest leak risks homeowners face. He also shares how FlowLogic differs from simpler devices by detecting extremely low flow rates, offering battery backup, app alerts, and user-controlled settings that help prevent nuisance shutoffs. The conversation also explores the insurance angle: how water damage claims can be costly, why some insurers now require leak detection devices for certain homes, and how discounts may help offset installation costs over time. Beyond protecting your home, Beth and Ian discuss the peace of mind, water savings, and environmental benefits that come with catching leaks early. If you have ever worried about what's happening behind your walls, under your floors, or while you're away from home, this episode is a practical reminder that prevention is far easier than restoration. Sound bites "Recognize when a leak has been going on too long." "Nothing lasts forever, but technology can help." "Installing leak detection can significantly reduce risk." The Stark Reality Of Insurance Claims... ... and the feeling that you get when something wasn't covered that you thought was covered as we enter hurricane season we are preparing for the worst and hoping for the best damage to your home can come from a number of things wind tropical storms hurricanes floods termites vandals aliens when its time for a home owner to place a claim they will need to provide some documentation and then wait for an insurance adjuster to come out to inspect the property their findings will determine if your claim is approved even if your claim is approved, you may not get all of the money you may have a hurricane deductible that needs to be met the damaged part of the home may be subject to prorations based on its age loss of rent for investment properties may have its own challenges as far as flooding goes... if its due to a named storm you may be more likely to have a successful claim if its caused prolonged rains it may be more difficult if the home is in a flood zone other than zone x your claim may very well be dependent upon having a flood insurance policy I always recommend that homeowners ask their insurance agents a lot of "what if" questions tune in to todays show where I break down these scenarios
Deborah Schwartz, a healthcare attorney, saw how medical insurance companies put profit over patient and the impacts it had. While inside the health insurance industry, she tried to change the practices she saw which were antithetical to better health outcomes, but since leaving the field, she’s writing about in her new novel, “EmmaCare” and sounding the alarm about what she saw. By whatever measure, claim denials, coupled with prior authorizations and other practices, are growing and put the onus on the patient to fight for and appeal judgments from the insurers even when their doctor prescribes various drugs and procedures. It’s a maze that leads to financial strain and bankruptcy, delayed or denied care and mental and emotional stress. We all know that our health care system is broken. These practices are chief among the issues, though there are many more. So, what can we do? Aside from a universal health care model which is unlikely in this country at this time, how do we proceed? We’ll discuss on this podcast.

Can an insurance company make repair the default choice across the entire supply chain? Søren Frederiksen from Tryg explains how a repair-first claims policy was built and scaled across Denmark, starting from a market where nine out of ten damaged car bumpers were replaced instead of repaired. What you'll hear in this episode: * Why repair in insurance claims is rarely more expensive than replacement, and often reduces cost, CO2 emissions, and improves customer satisfaction at the same time. * How cross-country benchmarking data created the proof of concept that got workshops and suppliers on board. * What it took to change an industry default: financial incentives per repaired item, access to training, and continuous follow-up. You'll also learn why making repair financially attractive for suppliers, not just environmentally preferable, is what moves adoption in practice. This is the second episode in the series Enabling Circularity Through Insurance. The series looks at the concrete levers insurance companies hold, from risk assessment and advisory services to product design and claims policies, and how these can enable circularity.

New rules introduced by the Central Bank should make it easier for customers to deal with insurance claims and in turn, help consumers reach fairer insurance settlements. In addition, the new rules around insurance quotes will make it easier for customers to understand how certain actions, such as accumulating penalty points – or paying insurance premiums monthly rather than upfront – could make their insurance premiums more expensive. This is according to the insurance broker Gallagher, which is advising consumers to get up to speed on the new rules being introduced under the Central Bank's revised Consumer Protection Code (CPC), which came into force on March 24, 2026. The ultimate aim of the new code, which covers a wide range of areas, is to protect consumers of financial products today and in the future. Ban on insurance auto-renewals Gallagher is also drawing the attention of consumers to a major change in the new rules, whereby there is now a ban on the auto-renewal of travel insurance, gadget insurance, dental insurance and pet insurance policies. Geraldine Kelly, head of personal lines with Gallagher, explained: "The Central Bank's new Consumer Protection Code (CPC) includes a number of new rules which will put insurance customers on a better footing. The ban on the auto-renewal of travel, gadget, dental and pet insurance is a positive development as it reduces the risk of consumers paying for products that they no longer need or that aren't suitable for them. This ban also reduces the risk of consumers missing out on opportunities to shop around to find the most suitable or cost-effective product. However, it's important that holders of travel, gadget, dental and pet insurance policies, which are due to expire in the coming months, get up to speed on the steps they need to take if they wish their policies to be renewed. In particular, those who wish to renew their policies will need to engage with their insurer or broker prior to the expiry date to ensure their policy is renewed on time." More transparent claims process The revised CPC also has a series of new rules which aim to enhance the transparency of the process for those making insurance claims – and to improve the experience of customers in this regard. Commenting on this aspect of the new rules, Ms Kelly said: "It can be daunting for consumers to make insurance claims, particularly if they have just been in a car accident or have experienced trauma. In addition, consumers whose homes have been badly damaged by a storm, flood, fire or other event could be under a lot of emotional pressure and stress, and so need the claims process to be as straightforward as possible. So, the new rules around the transparency of insurance claims are welcome as they should make it easier for customers making insurance claims to consider, engage and respond to settlement offers – or the decline of a claim." 10 ways the new Consumer Protection Code will help insurance customers In the wake of the new CPC rules kicking in, Gallagher has put together 10 must-knows on how the new requirements will impact insurance customers: 1. Auto-renewal of travel, gadget, dental or pet insurance is now banned Under the new CPC, consumers will no longer have their policies for travel insurance, gadget insurance, dental insurance or pet insurance automatically renewed unless they have provided their explicit consent for the automatic renewal in advance. 2. No ban on automatic renewals of health, home or car insurance The new opt-in rules around automatic renewals will not apply to all types of insurance products, such as health insurance, home insurance or car insurance as for these particular products, consumers could be at a substantial disadvantage if they didn't renew their policy in time and their policies lapsed as a result. 3. Insurers must give 20 days' notice if car or home insurance is to expire Where a home or car insurance policy is due to expire or be renewed, the insurer must pro...

In this informative episode on Radio Medium, host Laura Lee chats with legal expert Todd Bresney, senior partner and trial attorney at Kanoski Bresney, to break down one of the biggest myths in personal injury law: whether having a preexisting condition prevents you from winning an injury claim. Many accident victims are told they don't have a case because of prior injuries or health issues, but Todd explains why that belief is often incorrect and how the legal concept of aggravation of a prior injury allows victims to still pursue compensation when negligence worsens an existing condition. Through a real-life case example involving a devastating truck accident and delayed spinal injury diagnosis, Todd reveals how insurance companies attempt to minimize claims by blaming past medical history and how experienced trial attorneys challenge those defenses with medical experts and legal strategy. This episode offers essential insights for anyone navigating personal injury claims, accident liability, medical documentation, insurance negotiations, and legal rights after an injury, reminding listeners that understanding the law can help protect their health, financial recovery, and future well-being.

In this episode, Michael sits down with Reid Zeising, CEO of GAIN, the largest revenue cycle management organization specializing in litigated and complex healthcare claims. The conversation pulls back the curtain on how healthcare providers struggle to get paid for services already delivered, and how technology and AI are being used to push back against decades of insurance-driven denial strategies. Reid explains how the insurance industry fundamentally changed in 1994, when Allstate introduced Colossus, a system designed to standardize and often suppress claim payouts in favor of shareholder value. That shift, he argues, still shapes today's reimbursement environment, leaving providers underpaid and patients caught in the middle. Drawing on Michael's background in primary care administration, the discussion highlights a stark reality: many healthcare organizations collect only a fraction of what they bill, even when care is medically necessary and properly delivered. Reid compares this to asking professionals to do full work for partial pay and explains why this model is unsustainable, especially for providers serving uninsured and underinsured populations. The conversation then turns to how GAIN is using AI, predictive analytics, and technology-enabled workflows to reverse that imbalance. By focusing on litigated and complex claims, GAIN helps providers recover fair compensation, improve cash flow, and continue offering care to communities that need it most. Reid also shares why his company intentionally shifted away from higher-margin claim financing toward a service-driven model built around access, transparency, and long-term system impact. Michael and Reid also explore the broader healthcare landscape, including the financial strain on providers, the coming “silver tsunami” of aging patients, and the consequences of tort reform on patient access to care. Reid challenges common insurance-industry narratives around “frivolous lawsuits,” explaining how language and lobbying efforts have been used to restrict legitimate claims and reduce accountability. The episode closes with Reid's advocacy work through Americans for Patient Access and Americans for Responsible Consumer Funding, organizations focused on protecting access to healthcare and helping individuals navigate overwhelming medical and financial challenges. This is a candid, systems-level conversation about healthcare economics, AI-driven disruption, and what it will take to ensure providers get paid and patients get care. https://gainservicing.com/ https://www.linkedin.com/in/reidzeising/

Claims ops keeps buying “automation” that quietly adds work: more review, more disputes, more cycle time. The real villain is verification overhead—when you still have to check everything, “80% automated” is a new cost center disguised as progress. Ralph von Grafenstein, Founder/CEO, DocuSketch, has lived the messy middle of property claims: documentation fights, estimate friction, and payout delays between restorers and carriers. His model is blunt: shrink cycle time by redesigning the workflow and centralizing quality—not by selling buzzwords. Join us if you're wrestling with vendor ROI, cycle-time targets, or post-cat documentation chaos. #Insurance #Claims

A large insurance bill is looming from the latest weather-related disasters. The landslide at Mount Maunganui combined with flood damage in Northland, Coromandel and Gisborne means that the claims are set to roll in. Insurance and Financial Services Ombudsman Karen Stevens told Ryan Bridge that people affected can expect delays given the high number of claims being made. "I think everybody understands that with a significant weather event, there are going to be more delays than if it's just business as usual." On this episode of the Scouting For Growth podcast, Sabine VdL talks to Arvind Sontha, COE and co-founder of Kyber, an AI startup redefining how carriers handle claims correspondence. The insurance industry is undergoing a seismic shift as carries face mounting pressure to deliver faster, more transparent, and compliant communications to policy holders and clients, so the need for digital claims transformation has never been greater. KEY TAKEAWAYS If you think about insurance and tailoring insurance, the underlying model for risk is effectively a ‘user personal model'. We started with an obscure line of insurance that didn't exist yet – or did around personal cyber insurance. We wondered what it would look like, rather than SMB or commercial cyber insurance, as individual underwriting and risk modelling. We got lucky finding a great partner in branch insurance very early on. Over the course of our time engaging with them we ended up turning into an extension of their team. We were able to work closely with them, they trusted us to quickly understand their problems and iterate to give them quick solutions, while at the same time they understood that there are going to be quirks with products that aren't fully fleshed out which they could iron out over time. It was a symbiotic relationship. If an adjustor has to take an hour to put a document together you have to clear a 1.5-hour space in your calendar to do that. Life is hectic, you have meetings and other tasks to do and so that 1.5-hour block keeps getting moved back, same thing happens to managers. If you can take it from 1.5 hours to 30 seconds for a high-quality letter and a one-click process to approve, you can slip that into any part of your calendar. That's a really underrated part of the process. Some of the things we want to do in the future is include things like managed parameters. We think it's obtuse for all the carriers to manage all the fraud language individually all the time, for example. Kyber could manage that for you to make sure everything's automatically compliant and good to go. Statutory language really enables the full organisation to be prepared to catch each other. BEST MOMENTS ‘Kyber is an AI native, document generation and delivery platform made for claims teams, that's what we do.' ‘Nobody doubted that I could build the complex AI to underwrite and quantify the risk, what they needed to figure out was could I sell insurance, which is why I got my broker's licence!' ‘The results have been better than I expected, we've seen 65% faster drafting times, 80% consolidation of their templates across a 50-state operation, and 5x reduction in letter cycle times for documents.' ABOUT THE GUESTS Arvind Sontha is co-founder and CEO of Kyber, an AI startup that is redefining how carriers' NTPAs handle claims correspondence. Arvind is at the forefront of digital transformation, leading Kyber's mission to automate and streamline the entire lifecycle of claims forms and letters. Kyber's clients report the impact of AI automation is undeniable: Claims teams using Kyber have reduced letter drafting time by up to 85%, cut review time by 60%, and achieved a 3x faster outreach to policy holders.
