Episode Summary: In this episode of Your Hope Filled Perspective, we honor National Caregivers Month by shining a light on the often unseen challenges and rewards of care-giving. Whether you’re a caregiver yourself or know someone who is, this conversation will provide encouragement, practical tips, and faith-based hope to navigate the emotional, physical, and spiritual toll of caregiving. Carole Leathem and I discuss the misconceptions about caregiving, the importance of self-care, and ways communities can better support caregivers. Join us as we explore how faith and community can sustain caregivers through their most challenging moments while also revealing the unexpected blessings caregiving can bring. Quotables from the episode: There are 55 million caregivers in the world. Caregiving often comes with, as you've alluded to, a range of emotions. You called it a roller coaster of emotions, which I think is a beautiful picture of just exactly what's that like. It comes with grief, it comes with guilt. Sometimes it even comes with resentment. How can caregivers process these feelings in a healthy way? Well, for me, it really boils down to what I call asking the right questions. Because early on, I was asking, why in the heck is this happening? What did I do to cause this? I would cry out to God and say, why are you allowing this? I would say all of these why, why, why, why, why questions. And the why questions would get me nowhere. And one day, God, in my quiet time, said to me, “Carol, you're asking the wrong question.” And the question I needed to be doing asking was “okay this is happening, now what?” What I have now done is, I have taken the focus off the why which is the focus on me and what I'm going through and what I'm feeling now I'm not diminishing those emotions but when I am focused on me and my hurt and my pain and my resentment, then I'm not doing anything to get myself off that hamster wheel or that roller coaster. When I ask the right question, which is, I make a statement to God, “okay, this is happening. Now, what?” Then I can take a step back, my eyes are us back on God. My heart is now open to the mistakes that I'm making. And the emotions now can one by one be dealt with. How do I deal with the resentment? Well, when I say to “God, you know, I didn't sign up for this.” God says to me, “Oh girl, yeah, you did. Remember those vows that you took 48 years ago.” And I'm like, yeah, you're right. And all that does is it just kind of puts it back into a perspective, because the perspective of focusing on me is not as healthy as the perspective of focusing on God. God didn't answer Job. When Job said why, God had something else for Job to learn. And when you say now what that offers God the opportunity to teach you either something about yourself, something about your spouse, or something about him. Yeah,… those are hard fought lessons. But I'm convinced that sometimes the only way we learn those lessons is walking through the hard times. I agree. I agree with you 100%. “ I have this space created so that when I walk into this room, it's me. It's my safe place. It's my place where I do my quiet time. It's my place where I work. It's my place where I do my podcasts. It's where I do everything in this room. I had to create a space. And fortunately, I had a room that I could do that with. So I come in here and if life is particularly hard, I have to tell myself 15 minutes is not going to change anything outside these walls, but it can make a big difference in me. We make time for the things that are important. We can always find time if we make it important enough. The real bottom line is, and I deal with a lot of caregivers, we make time for what's important, and that is the honest truth. … we can always find time if we make it important enough. So we have to make it important. And I hope that you're listening because that is such a valuable nugget because we really do make time for what's important. The number of times that I hear people say, oh, I just don't have time. You know, we all have 24 hours in a day, And we are all going to spend every minute of those 24 hours, but we will spend it on what we deem is important. And so if your loved one that you are caring for, if you deem them as important, you need to also deem your own self -care as important. You can still experience joy even in the midst of sorrow. It comes down to perspective and how we’re looking at it. It’s not about me, even though it feels like it’s about me. We have to know our limits. There are some things when we’re dealing with caregiving there’s things I can do, and then there’s things I can’t do. The hardest thing is to understand where those boundaries are. What am I responsible for? What am I not responsible for? Spasticity is one of the most common symptoms of MS, impacting up to 80% of the people living with MS. It can feel like mild muscle tightness that you wish would just go away or it can be so painful and debilitating that it keeps you from moving. Dr. Michelle Cameron joins me in this episode to discuss what spasticity is and how to best manage it. Dr. Cameron is a neurologist and physical therapist, a professor in the Department of Neurology at Oregon Health & Science University, Interim Chief of Neurology at the VA Portland Healthcare System, and Co-Director of the VA MS Center of Excellence West. We're also talking about a new framework for thinking and talking about MS that's been proposed by an international panel of MS experts. This “fabulous” interview with Loretta Veney builds on a caregiving relationship spanning 16 years. During those years my guest discovered a unique approach to enhance the relationship between herself and her mom who had dementia. LEGO had been a part of their lives before caregiving and turned out to be an effective and enjoyable communication tool through which Loretta continued to communicate with her mom. This interview was recorded on International Women's Day – which turned out to be an appropriate day to learn about the significance of the day as it relates to “Wonder Woman”. Loretta Woodward Veney is an inspirational speaker who has delivered more than 300 speeches and presentations on dementia and caregiving since 2014, offering a wealth of information, encouragement and humor to her audiences. Loretta is the author of Being My Mom's Mom, Refreshment for the Caregiver's Spirit, and Colors Flowing from My Mind. In 2006 after her beloved mother Doris was diagnosed with dementia, Loretta began learning everything she could about the disease becoming a fierce advocate for her Mom in the process. Loretta and her Mom have been featured in articles in the Wall Street Journal, Psychology Today, The Washington Post, The NY Times, AARP Caregiver Stories, as well as a PBS special. In 2019, Loretta was selected as Trailblazer of the Year by Johns Hopkins Medicine. In November 2021 Loretta and her Mom were featured in Episode 6 of Season Four of Robin Roberts Thriver Thursdays series, which previewed on Good Morning America during National Caregivers Month. Loretta's Wonder Woman Mom's 16-year journey ended on January 31, 2022.

We met Dr. AJ Joshi, Chief Medical Officer at Atara Biotherapeutics, at ECTRMS 2019, when he gave us our first update on ATA 188, Atara's investigational cell therapy for MS. After years of tests, after countless of late-night WebMD searches, and a sea of referrals … after hours on the phone with insurance companies and payment plans. After all of that. That is when you FINALLY receive the words: We think your son has a rare condition. And just when you thought you would get answers. That — for patients with rare and orphan conditions — is only where your journey begins. At least that is where the journey began for Monica Weldon, our guest for this episode of Rare Voices. Monica was able to absorb those words and the emotional roller coaster attached to them and create a network of support and a vision for what advocacy can mean for patients with rare and orphan conditions. In 2014, Monica founded Bridge the Gap: SYNGAP Education and Research Foundation. While most patient advocacy groups focus on an illness, Monica focused her foundation on the research involving a rare gene mutation. As a result, she has been able to build a coalition with other advocacy groups, research institutions, and, most impressively, with manufacturers. Today, Monica is a force for change in patient advocacy, an author, and keynote speaker at countless conferences even beyond health care. I'm excited to bring you her story during National Caregivers Month, because, most importantly, Monica has redefined what it means to be a caregiver with what she has accomplished on behalf of her son, Beckett.

To round of National Caregivers Month, we feature Sarah Verardo. Sarah is the CEO of Independence Fund and a caregiver for her husband, an Army Veteran. Sarah also authored the children's book, Hero at Home. Founded in 2007, The Independence Fund is committed to empowering our nation’s catastrophically wounded, injured, or ill Veterans to overcome physical, mental, and emotional wounds incurred in the line of duty. We are dedicated to improving the lives of both our Veterans and their families. Through our Mobility, Caregiver, Adaptive Sports, Advocacy, and Family programs, The Independence Fund strives to bridge the gap of unmet needs for Veterans and their caregivers. Sarah also recently announced Independence Fund's new program, Operation Resiliency. The program will team with VA to host suicide prevention reunion retreats with the Veterans of high suicide rate military units. Sarah joins this week's podcast to discuss her experience as her husband's caregiver and her efforts at Independence Fund.
Everyone needs a break from time to time. Borne the Battle - and, more importantly, its host - decided to take a short break from production. It's been a month since our last episode was published, but I was still recording interviews in that span of time. A couple of the interviews I did were with caregivers. With November being National Caregivers Month, and with this week being Thanksgiving, I decided to bring the podcast back to full form with two episodes featuring caregivers. Not only to spotlight their efforts, but to show our gratitude for the role they play in Veterans' lives. First up is Liz Rotenberry. Liz joined me at VA Central Office after the Elizabeth Dole Foundation showed interest in collaborating on a podcast. I didn't know much about Liz before we started the interview, but by the end of it I felt I had just recorded one of the more valuable interviews in the podcast's two year history. Liz, is a military caregiver for her husband, a Marine Veteran of Operation Iraqi Freedom & Operation Enduring Freedom Veteran. As an Elizabeth Dole Fellow Alumna, Liz works to advocate not only for herself and her family, but for all Military and Veteran Caregivers. Liz joined the Elizabeth Dole Foundation as their Caregiver Community Program Coordinator and is now the Foundation’s Fellows Program Coordinator. Prior to becoming a military caregiver, Liz graduated from the University of Delaware with a BAS in Art and Art History. Liz worked in the architectural field as a designer for schools, medical, offices to government and military facilities, including the test sites on Proving Grounds in Arizona. The work Liz experienced with the Proving Grounds in Arizona provided an eye opening experience to what our Military train for in real life theater every day. Liz is honored to have been a part of the training and implementation of the Joint Experimentation Range Complex. It is important for Liz to represent not only the Veteran but especially the Caregiver. Liz and I talk about being a military spouse during her husband's service and how she experienced transition when her husband separated from the military. That, plus plenty of insight on her role as a caregiver.
As stroke survivors, Emergency Room physicians play an important part in our survival and rapid treatment, but we often don't think about them. We build ongoing clinical relationships with our neurologists, physiatrists, OTs, PTs, Speech Therapists. Rehab nurses, CNAs, and more, but not the ER docs. When they see us, our brains are in full crisis/panic mode, our loved ones are terrified, and the doctors are busy making rapid assessments, decisions, and referrals. By the time we get a chance to calm down and assess our new landscape, they are long gone and have transitioned off our care team. That's one reason I enjoyed talking with Dr. Kimberly Brown. It was a chance to get some insight into a field I knew little about. Among the things I found interesting was how she talk about the impact of technology in medical education -- listening to a heart murmur on the computer during school work and later tying that into the real world in listening on an actual patient. We also talk about the role of ER Pharmacists, the role Methodist University Hospital has had in treating folks with the clot busting drug tPA, and some of the challenges around public health. We do have some discussion of the politics of healthcare in this episode. Regardless of how you feel about the Affordable Care Act, the Americans with Disabilities Act, and the future of Medicare and Medicaid, the political decisions around these programs will directly affect the lives of millions of stroke survivors and potential survivors in the US, and it's important to be aware of what's going on and express your views to your elected officials. We reference the FAST signs for stroke. If you're not familiar with them they are: We also talk about sepsis or being septic. Sepsis can be the result of the body's attempt to fight an infection. Basically, the infection results in the body dumping an excess of chemicals into the blood stream to fight the infection, but it instead leads to inflammation in other organs. Symptoms of sepsis can sometimes mirror stroke. Sepsis is a life threatening condition. Fortunately whether you come into the ER with Sepsis or Stroke, folks like Dr. Kimberly Brown are there to take care of you. Dr. Kimberly Brown is an emergency physician in Memphis, Tennessee. She is a native of Milwaukee, Wisconsin and earned her undergraduate degree from Fisk University. Loving warmer weather, Dr. Brown earned her Master of Public Health degree from the University of Florida in Public Health Management and Policy. She attended Ross University School of Medicine and recently completed her emergency medicine residency as a member of the inaugural graduating class of the University of Tennessee Health Science Center in Memphis, Tennessee. Dr. Brown’s clinical interests include neurologic emergencies, critical care, sepsis, and education. In her free time, she enjoys traveling, volunteering, watching too much reality tv and trying new restaurants. Improving Care in the Stroke Belt Dr. Brown serves patients in the Stroke Belt of the US. This is the region of the country that since the early 60s has had a significantly higher about of stroke patients and higher mortality from stroke than the rest of the country. It includes: Tennessee Virginia Kentucky North Carolina South Carolina Georgia Alabama Mississippi Louisiana Arkansas Making sure all patients have access to coverage Decrease food desserts Programs to incentive physicians to go to under-served communities in Appalachia and urban centers to ensure access to care Make sure schools are good so kids can read and understand their health Medical professionals have to do a better job at educating patients about how their body works and what's going on with it. Hack of the Week When you arrive at the ER, especially as a previous stroke survivor or person with disabilities. It's important to provide the team with as much information as possible. The need to know about the medications (legal AND illegal) you are taking, any previous stroke or medical issues, what disabilities you already had, and more. Any video or recent photos of you prior to this incident can also be extremely helpful to the staff so they can get a better sense of what's changed. Every Branch and Leaf -- National Caregivers Month November is National Caregivers Month. These people make a huge difference in our lives, and it's important to recognize and thank them for their support.
