Most church customers who believe in Jesus have been taught they are just sinners (saved by grace) with unclean hearts ... and need to keep seeking a renewed forgiveness from God whenever they fail or fall short. This often becomes a heavily embedded routine that isn't exactly going to strengthen one's true identity as a person who is declared to be righteous and holy, apart from works. It keeps Christians in a state of a sin consciousness—similar to Jewish people under the sacrificial system of the Old Covenant—bringing a continuous reminder of sins day to day and year to year. The gospel of grace has revealed forgiveness by God has been completed through the blood of Jesus, once for all, and many centuries ago. God remembers sins no more in a new and better covenant. They were taken away! Now we begin to see the inconsistencies of the 'Christian religion' insisting we remind God for personal sins that He said would no longer be remembered. Christ fulfilled forgiveness forever, having thrown sins as far as the east is from the west. With that in mind, how does one 'kick the habit' of confessing every single wrongful thought or action that they can't possibly keep up with? How does a believer in Christ respond when tempted ... and is it possible to overcome unhelpful habits that are not really a part of who you are in Him? TR is an 88-year-old man who is disabled and in chronic pain from spinal stenosis and knee osteoarthritis. He has multiple other medical issues, including coronary artery disease and COPD.He has been through years of treatment for his knees and back, including physical therapy, multiple joint injections, and an in-office procedure for spinal stenosis. He has been on multiple regimens of oral pain medications. He is not interested in surgery (and probably would not be considered a reasonable candidate if he were).He comes to a visit and asks what can be done.Sensible Medicine is a reader-supported Substack. If you appreciate our work, consider becoming a free or paid subscriber.We are taught never to say, “There is nothing I can do for you.” We may be out of surgical or medical options, but we can always continue to care for and support our patients. This is not part of the Hippocratic Oath, but it is so integral to our training that one might think it belongs there.There are good reasons to caution doctors from saying some version of, “I am sorry, I don't think there is anything more I can do for you.” It is true that in 21st-century medicine, the act of caring for patients has been so trivialized that we need to remind ourselves that caring is not just a last resort but our first responsibility.Hope in the face of illness is exceedingly valuable. Robbing this hope is not only cruel, but probably bad for the health of patients.Then there is the truth that when one doctor has nothing left to offer, medicine often still has more to offer. I am still haunted by at least one case when I mistook my lack of knowledge for a shortcoming in the field.A doctor saying he has nothing left to offer may be taking the easy way out. Further research or a considered referral often suggests effective treatments, if not cures. There is always a risk that when we say, “We can't help,” we are wrong.All this being true, I don't think we admit that there is nothing left to be done often enough. The Modern Hippocratic Oath does include the line:I will not be ashamed to say “I know not”, nor will I fail to call in my colleagues when the skills of another are needed for a patient's recovery.Is it that much of a stretch to imagine the oath saying:I will not be ashamed to say “I have nothing left to offer beyond my ongoing care and commitment”, nor will I call in my colleagues, knowing they have nothing to offer beyond protecting me from admitting medicine's limitations.We no longer conceal bad diagnoses from patients; we consider this unethical. It is time that we consider it unethical to conceal that we have nothing to offer beyond palliation.It is hard to tell patients that the best we can do is control pain, especially when we have been trained not to. It takes work to be sure that neither you nor your colleagues have something to offer beyond a hamster wheel of care to provide hope but nothing more. It also takes some guts; we can never be 100% sure that nobody has anything to offer.Yet there are downsides to never saying we are done. Many patients accept that further care is futile, not after an honest conversation, but after being worn down by my endless visits, treatments, and promises that lead nowhere. All the while, time and money are wasted; time and money that could be better spent on life. The useless medical care leaves patients and their families increasingly frustrated by a medical system that refuses to be honest and caring.There are patients who welcome an honest assessment of their options and shift their focus to living with their disease and disability. There are also patients who do not. I've had patients fire me for saying there was nothing to do. They left me to get on that hamster wheel of care. This decision did not make them better, but maybe it made them happier and more ready to accept the inevitable. Eventually.TR calls now and then and comes to see me every three or four months. He tells me about his symptoms and the things he wishes he could do. I tell him there are things he could try and people he could see, but I let him know I don't think any of these options would be productive. He tells me he's not interested. I advise him to push up on his pain medications and to pretreat his symptoms; I know he will not take this advice. I know we will repeat this visit in the future; I actually hope we will, for years. “I often feel like the work is kind of a disintegrative work that we’re doing, we’re allowing the patterns and the structures to release, so that there’s sort of nowhere left to go, because there’s nothing left to lose.” – Julia Bodkin Beloved Hoffman Process facilitator and psychotherapist, Julia Bodkin, first took the Process in India in her very early adulthood. After living quite a bit of life, she took it again in Australia. Now, as a trained professional in many healing modalities, Julia facilitates the Process in the U.K. She brings a lifetime of experience, wisdom, and expertise, along with her generous, vulnerable heart. In her youth, Julia wondered if there could be more to life than just getting a job and settling down. She grew up in a middle-class British family that, to her, seemed very “dull.” So she left the U.K. and travelled around India until she came to Pune, India, where she studied on and off for ten years. Along her journey, having worked within many of the great schools of spiritual transformation, Julia knew she was seeking enlightenment and an “everlasting blissful state.” Through study, teaching, and much meditation, she’s, over time, found herself settling into an awakening and the embodiment of her true nature. Julia now sees it as “settling into what is.” In teaching the Process, Julia shares how beautiful it is to watch students soften into themselves. Send us Fan MailYou pull into the driveway after a full day. Patients, staff, meetings, inbox, all of it. You open the door and immediately there are more people who need something from you. And you have nothing left.If you have ever sat in your car in the driveway just a little longer than you needed to, you are not broken. You are not someone who doesn't love the people waiting for you inside. You are someone who has been on stage all day with no time off stage. And today we are going to fix that.What we cover in this episode:On Stage vs. Off Stage: What It Actually MeansOn stage is any time your energy is going outward. You are performing in some kind of role, whether that is physician, leader, parent, partner, or PTA secretary. It includes clinic, charting, inbox, staff interactions, and yes, family time counts too if you are managing emotions and playing a role.Off stage is genuine time where there is no performance element. You are just a human being allowed to exist. A solo walk, reading for pleasure, crafting, time with friends who know all your secrets and expect nothing from you. It is not scrolling social media while you tell yourself you are resting. It is actual restoration.The Gray ZoneSome time feels like it should be off stage but is actually draining you. Social media is the big one. If you are reaching for your phone out of habit and putting it down feeling worse, that is gray zone time, not off stage time. We have to be honest about this or the audit will not work.Introversion, Extroversion, and What You Actually NeedThis is a spectrum, not a binary. More introverted physicians need quiet solo time to recharge. More extroverted physicians might actually need more time with people they genuinely choose, not just the people at work. Neither is better. Both require intention. And where you fall on that spectrum can shift depending on stress, illness, life season, and what you are carrying right now.The AuditThis is a doing episode. Here is what we are walking through together:Step 1: Map one to two weeks of your actual schedule. One week if your schedule is fixed. Two or more if you are a hospitalist, nocturnist, or work variable hours. Include work time AND home time. Both matter.Step 2: Label everything. On stage, off stage, or gray zone. Be honest. Family time that feels like a performance goes in the on stage column even if you love your family deeply.Step 3: Look at the ratio. Where is your off stage time? Is there any? How does that balance feel?Step 4: Reflect. Does this match what you actually need given where you are on the introversion/extroversion spectrum? What would need to shift, even just slightly, to give you more of what you need?Making Changes Without Flipping the TableSmall shifts create sustainable change. Before you decide you need to leave medicine, consider whether you have actually had any off stage time lately. Not as a reason to stay if you truly need to go, but because we do not make our best decisions from a completely depleted state. Protect one or two off stage blocks per week. In today's Daily Fix:Crimson Desert has been out for less than a month, and hardcore fans have not only put in hundreds of hours, but they've killed everything they can and are now left with nothing to do. Over on reddit, fans are complaining of lack of things to kill once you've completed everything. They've compared the game to other open-world titles where enemies can respawn, but that's apparently no the case with Crimson Desert. In other news, The Super Mario Galaxy Movie is crushing it at the box office, like Mario would crush koopas. Not accounting for print and marketing, the movie cost Universal and Nintendo around $110 million, and has now made around $370 million worldwide in five days. And finally, the new Darth Maul series on Disney+ in earning rave reviews, and Maul voice actor Sam Witwer thinks it's time to finally give Maul his own video game.
