We examine a groundbreaking 2026 study introducing AI2CCA, a deep-learning pathology model that evaluates routine digitized slides. The study forces us to ask a critical question: how can we safely deploy AI in the clinic? The answer lies in teaching the machine to measure its own uncertainty, drastically reducing the need for invasive, exclusionary tests and accelerating life-saving treatments.In This Episode, We Cover:• The Ultimate Clinical Bottleneck: Understanding the high-stakes diagnostic overlap between ICCA and metastatic adenocarcinomas. Because these tumors look functionally identical—sharing irregular glandular structures, mucin secretion, and fibrotic responses—patients often face weeks of invasive endoscopies and body scans to rule out an occult primary site before targeted treatment can begin.• The Foundation Model Bake-Off: Researchers pitted three advanced, self-supervised deep learning architectures against each other using retrospective data from 544 patients across five European centers: ◦ Ctranspath paired with HistoBistro. ◦ UNI paired with CLAM. ◦ CONCH paired with TITAN, which emerged as the winner by mapping gigapixels of tissue to pathology reports using multimodal visual-language training.• The Secret Sauce - Predictive Entropy: An initial AUROC of 0.840 is not safe enough for clinical deployment. We break down how the team used Generalized-ODIN (G-ODIN) to calculate "predictive entropy"—a mathematical measurement of the AI's internal confusion when tissue is highly ambiguous.• The Power of Saying "I Don't Know": By setting a strict confidence threshold and refusing to diagnose ambiguous slides, the AI2CCA model improved its AUROC to 0.958 and dropped its false positive rate to absolute zero. While it only retained 46% of cases for high-confidence predictions, it provides a safe "fast-track" that could essentially halve the clinical backlog for unnecessary gastrointestinal scopes.• The Global Stress Test: To prove the AI didn't just memorize European lab stains, the team prospectively tested 161 new patients across France, India, and South Korea. Despite navigating completely different disease backgrounds—such as heavy cirrhosis and endemic liver flukes—the model achieved near-perfect accuracy (AUROCs of 1.00 and 0.965) with only one single misclassification globally.Key Takeaway: True clinical AI doesn't need to replace the human diagnostic process; it just needs to know what it doesn't know. Nearing the end of January, I'm only beginning to feel the sinew of this new year. Here in the United States, we're reckoning with what seems like a sudden surge of authoritarianism—though, as Hemingway reminds us in The Sun Also Rises, collapse happens “two ways. Gradually, then suddenly.” The hubris we've unleashed from within now sends shockwaves through the world, unmooring the institutions we've depended on and unsettling the nervous system of our species.Staying human amidst the swirl has become a practice unto itself. We must maintain the pleasantries of our daily lives, yoke ourselves to the people and practices that organize and buoy the mind, and make actionable the indignance of our deeper values—all while sifting through the muck and shimmer of the collective unconscious.Of those in privileged circumstances, many are divesting themselves of accountability or arming up for an uncertain future. Even a question like “How's it going?” can land strangely if it feels insulated from the existential tremors of the moment.Winter, of course, is the barest season. It's a time when thin, long-shadowed light clarifies sight and stillness disciplines attention, when branches shiver as the wind exposes the decorative notions of warmer seasons.A few weeks ago, I sat down with two friends, David Keplinger and Lindsay Whalen, whose companionship is like wool wrapped around the cold turnings of life. Our purpose was to interview Lindsay about the poet Mary Oliver—the subject of her forthcoming biography from Penguin Press—and to trace the threads of synchronicity and coherence among us.I imagine that rendering anyone's soul requires discipline and sustained concentration. But Mary's life, as her poetry reflects, was singular, cloistered, and prolific, demanding of her biographer an uncommon devotion. In our conversation, Lindsay explained that she misses Mary less than she might another deceased friend, given that she remains in constant contact with her. Yet there's one quality of Mary's presence she said she misses: “When she looked at you, she really looked at you. On a windy summer day on the New England coast, the air is filled with the irresistible smell of frying oil and the saltiness of seafood. Thousands gather in weathered roadside stands and home-based seafood restaurants, all lured by the promise of a crispy, golden morsel—clam cakes. These humbly fried delicacies, tender and crispy on the outside, are more than a quick bite. More than a quick bite, these are a taste of history, a tradition woven tightly into the fabric of coast-based living for generations.I encountered these often heavy, oil-dense snacks while working at Ballard's on Block Island in the summer of 1973. As the youngest kitchen apprentice, my duties included frying food all day and night. Of course, every clam cake I burned ended up either in my stomach or in the flocks of hungry seagulls that roamed the nearby shore behind the kitchen, confronting me with my new experience of the American diet. Coming from Italy, and seldom eating deep-fried food, I soon realized that clam cakes were an indulgence to savor with moderation.The history of clam cakes dates back long before seafood huts and summer tourists poured in. Native tribes in the Northeast first harvested clams from the rich coastlines, adding them to fried breads. With the arrival of European colonists, the new residents adopted their methods of preparation, blending classic concepts with innovative styles to create a unique, tasty dish. In the 19th century, clam cakes had become a regular part of the diet along the Rhode Island and Massachusetts coasts, where clamming had become not just labor but an integral daily routine. Billerica, Massachusetts Hotel PTAC ambience. Enjoy hours of gushing hot air on a cold winter night in your Boston-area hotel room. The TV is off, so no local TV news to slog through. I used to watch local news in this area. Mostly the NBC station, back in the aughts. Their promotions featured station characters referring to themselves as “the neeeews station.”I also worked nearly two decades in local news. Take it from me: you could give up commercial local TV entirely and not miss a beat.Aside from what you think of Ralph Nader from a political perspective, he had the commercial news industry dead to rights in the summer of 2000: “Look at your late-evening news… It's 30 minutes. Nine minutes of ads; three minutes of street crime right at the beginning, never corporate crime, very superficially covered; one minute of impromptu chit-chat between the anchors; four minutes of weather; four minutes of sports — and that's what happens in your town tonight.”Nader didn't mention that our weather studios were named after local florists, and sports were “powered” by local Toyota dealerships.At one job, a befuddled new anchor approached me in the hall.“Do you know where the ‘Terrorism Desk is?”“Oh, for sure,” I said. “You want the lobby.”The lobby had an open window to the station's master control setup, flashing with over thirty monitors showing color bars, live cams, satellite feeds, and other inputs (looks impressive). And that camera station had other monikers: the “Breaking News Desk,” “Hurricane Whomever Desk,” and “We Have a New Baseball Team in Town Desk.”Still just the lobby.Nader also didn't mention sweeps week, the designated ratings period when stations try to attract the largest possible audience. Viewership is collected from a small sample of homes with Nielsen boxes — sometimes just hundreds — that determine a region's TV habits. Sweeps weeks set advertising rates, deciding how much a law firm or Buffalo Wild Wings has to pay to appear in a commercial break.Sweeps week is also a time of intrigue, danger, and sensationalized threats — online predators, out-of-control crime, spikes of spammers. I'm not being facetious: in Albany, I saw a promo claiming drinking water could be dangerous (the water is piped in from the Helderbergs, some of the cleanest water a small city could hope to access — you could eat off the floor in the Helderbergs).Sweeps week is also when favorite network TV characters die. J.R. was shot during sweeps. Brad Pitt showed up on Friends during sweeps.At one station, a producer said, “If Oprah has a Dancing with the Palins…” we'd beat our rival in the 5pm slot. It was the last day of sweeps, Oprah had Bristol and Sarah Palin gab it up on her program. We did hit #1 for the 11 that sweeps period due to the Bristol Palin-led Dancing with the Stars. Sweeps also judge station performance. If you watch local television and see a “We're #1 in something” ad, that's what that is all about. Those ads are specifically for station management, no one else gives a ****.Speaking of — once, walking into a station bathroom, I heard a toilet flush, and a colleague walks out of the stall holding his bag of Chipotle. These are folks you could stand to listen to less, is all I'm saying.Postscript-ish story: when I worked for a station that shared a newsroom with Politico.One morning, I'm walking into my department through the Politico sales area, gabbing with an awesome lady I worked with. Because I'm a stupid klutz, my hand bangs the side of a desk and dislodges my lunch. Which was soup in a Tupperware bowl. And it didn't just spill — it exploded. Clam fragments and sad potatoes amongst a red ooze splashed and soaked into the carpet (which, I'm not embellishing here, was new and cream-colored).I don't know what smells pleasing to you at 8:57 AM — I'm positive it isn't canned Manhattan Clam Chowder hit with 27 spluts of Tabasco.Awesome lady grabs my elbow and is like, “Go, go, go, go.”
