Welcome Dr. Pilgrim. 00:00:42 Dr. Pilgrim Thank you. 00:00:43 Host So growing up with a sister who had heart surgery as a child had a major influence in your life, and ultimately led you to your role here at Cook Children's as medical director of the ACHD program, compared to many other cardiac specialties, adult congenital as a specialty is relatively new, can you give us a little history. 00:01:03 Dr. Pilgrim So, as you mentioned, my sister was born with a congenital heart disease. And so growing up with this in my family, it became of interest to me at a very young age. But as a field, congenital heart disease is something that has been around obviously, for several decades, this field of adult congenital heart disease has grown primarily because of the success stories that we've had in pediatric cardiology, namely, the surgical outcomes and the longevity of patients related to improved surgical care, improved surgical technique, you know, recognizing disease, early on fetal diagnosis, and so on and so forth. And so the incidence of congenital heart disease really hasn't changed. But the lifespan of individuals growing up with congenital heart disease has tremendously improved. And so when we think about the growth of adult congenital heart disease in the country, it really has grown out of necessity, because these kids are now growing up and becoming adults. Congenital Heart Disease or CHD is the number one leading cause of infant related death and it wasn't that long ago when the cardiovascular community didn't have enough “congenital heart disease” specialists working with adult survivors of CHD. Today, with help from the Adult Congenital Heart Disease Association (ACHA) the cardiovascular community has helped educate cardiologists on these abnormalities of the human heart and build a network of specialists throughout the world. I spoke with ACHA accreditation manager Misty Sharpe, also a CHD patient and nurse at Vanderbilt, about what ACHA is doing to connect adult patients, encourage and educate families, as well as work closely with the medical community to ensure families are aware of CHD specialists in their area or where the closest clinic is to their coordinates. As a survivor of CHD, Misty and I discuss the challenges we experienced when we became adults and had outgrown the children's hospital's age limit. A mother's story typically begins with a love story - and Jae Day's love story may be unconventional to some, but she and her husband just knew they were meant for each other after getting married just four short months of dating. What inspired this now-mother-of-four to become a doula was wanting to provide services to women in the Canadian Armed Forces (CAF), specifically mothers, after she had been in the military for over 13 years and saw that there was a need for available resources and support to pregnant women.  After giving birth to her two boys and twin girls, Jae became a certified Maternal Support Practitioner (also known as a Doula), offering her doula services under the name "The Baby Days" and has worked as a birth and postpartum doula, mostly serving CAF women and spouses in the areas of CFB Borden and CFB Petawawa. Have you heard of congenital heart defects?   A Congenital Heart defect is an abnormality in the heart that develops before birth. Congenital heart defect is one of the most common types of birth defects. Symptoms include abnormal heart rhythms, blue-tinted skin, shortness of breath, failure to feed or develop normally, and swollen body tissue or organs. Treatments include medications to lower blood pressure and control heart rate, heart devices, catheter procedures, and surgery. Serious cases may require a heart transplant. Today’s guest experienced an infant loss due to congenital heart defect and had a miscarriage. Keivon Hardy is a childless mother who managed life with prenatal and postpartum depression, anxiety, and isolation after her infant loss and miscarriage.  When she was 20 weeks pregnant with her son she found out via a routine ultrasound that he had a congenital heart defect.  Because his case was unique the doctors weren’t able to determine exactly what his defect was until birth.  After birth, it was determined that his blood vessels were too small to be operated on and he would need to grow before they could operate.  He lived to be one month old and never came home from the NICU.  Keivon went on to conceive again only to be faced with another loss a miscarriage while on birth control.   In today’s episode, Keivon takes us back to when she found out her son had a congenital heart defect, how she dealt with prenatal and postnatal depression, and losing her son and then having a miscarriage while on birth control. Dr. C. Huie Lin is the Director of the Adult Congenital Heart Program at Houston Methodist Hospital. Dr. Lin earned his Ph.D. and M.D. from the University of Virginia. His residency was done at Beth Israel Deaconess Medical Center in Boston and he completed his fellowship work in Adult Congenital - Adult Interventional Cardiology at Washington University-Barnes-Jewish Hospital. Dr. C. Huie Lin specializes in the care of adults with congenital heart defects. He is certified in the care of adults with congenital heart defects -- which is a fairly new specialization. Dr. Lins’s research work is focused on surgical intervention in congenital heart disease and adult congenital heart disease programs. Along with medicine, Dr. Lin also helped to establish the Adult Congenital Heart Symposium, a regional Houston conference which is held every year. In this episode of Heart to Heart with Anna, Dr. Lin shares with Anna what to expect at the 5th Annual Adult Congenital Heart Symposium. He talks about why the symposium was created by him and Dr. Ari Cedars. The doctor spent over an hour analyzing my baby's heart during the 31-week ultrasound. This was our second child and I knew something was not right. Turns out Abby had a congenital heart condition. Nothing can prepare you for that bit of news. Nothing. Don't waste time thinking about the what ifs. Save your energy for getting ready. Decisions need to be made. Which hospital to deliver at? Which hospital would be best for Abby post-birth? What level of care can its NICU provide? Which OB should deliver? My OB did not have admitting rights at the hospital of our choice. So many decisions. Don't think about the what ifs. I was afraid before the birth. So many things can go wrong. I knew they were going to take my baby from me. Our three year old, our dog, our home all fell on my husband's shoulders. He is the more rational one. Just be strong and patient. Abby had an abnormality in her heart. They had to stop Abby's heart before they could operate. She looked "dead" just laying there. Nothing can prepare you for that. Her chest was kept open for 24 hours after the surgery. Nothing can prepare you for that. Then you wait. That's all you do in the hospital. Just wait. Waited for 56 days in the NICU. I wasn't in control of anything. I needed ownership of something. I am her mother after all, but feedings and diaper changes were on the nurses' schedule. The nurses' strength carried me through. They were my rock. They understood me as a person, and took the white coat out of it. She has had three major surgeries. This surgeon has never nicked an artery before, but he nicked one during one of Abby's procedures. She was losing blood as quickly as they were giving it to her. They replaced her blood volume twice. She made it. She is a fighter. The stress and aggravation had to go somewhere. Small things can lead to big arguments, but our marriage survived. We promised to be at church every Sunday. Every Sunday. Over 50% of couples with seriously ill children don't make it. That wasn't going to be us. Our story is documented on our Facebook pages. One of our posts had over a million views. We used that momentum to raise money and help the NICU get more sound machines. Have a late term ultrasound. That's our advice. Just schedule it. Even if you don't think you need it. It will buy you time to prepare if something is not right. Three operations and 10 months later Abby is fine. Abby should live a relatively long and healthy life. We are fortunate. Mark Donne, best man at my wedding and friend since age 5, significantly impacted my attitude towards my bipolar disorder. Mental illness is chronic in nature, and patients don't truly achieve a "cure," but rather, a reduction in symptoms. Mark was born with Tetralogy of Fallot, a congenital heart condition that has required him to undergo multiple open heart surgeries. Despite these constant visits to doctors, I never heard Mark complain. His attitude towards his condition is inspiring, because he never feels like a victim. Mark takes advantage of everyday he has, and continues to pursue his dreams. Mark and I discuss his experience living with his heart problems, and also the mental health of entrepreneurship.

Mark Donne, best man at my wedding and friend since age 5, significantly impacted my attitude towards my bipolar disorder. Mental illness is chronic in nature, and patients don't truly achieve a "cure," but rather, a reduction in symptoms. Mark was born with Tetralogy of Fallot, a congenital heart condition that has required him to undergo multiple open heart surgeries. Despite these constant visits to doctors, I never heard Mark complain. His attitude towards his condition is inspiring, because he never feels like a victim. Mark takes advantage of everyday he has, and continues to pursue his dreams. Mark and I discuss his experience living with his heart problems, and also the mental health of entrepreneurship.

CHAMP (Cardiac High Acuity Monitoring Program) is a multi-disciplinary based team that has been designed to meet the needs of the most complex cardiac population with single ventricle heart disease. The program closely monitors patients with single ventricle heart disease between the critical first and second stages (before stage II Glenn surgery). The team at the Ward Family Heart Center at Children's Mercy has also developed a tool that makes it easier for families to report their single ventricle child's vital signs from home and provides the hospital team with frequent updates needed to keep these patients well between visits. Amy shares her story about her son who was not diagnosed with CHD until after he was discharged from the hospital.  She talks about his heart surgery and his time in the hospital.  She also talks about caring for her son once he was home.  She talks about the potential future heart procedures that her son may need.  Further, she discusses how finding a good support network is vital.  Amy then reveals how she worked with the Mended Little Hearts National Organization, the American Heart Association, and the Children's Heart Foundation. She discusses how her work with these organizations led her to start her own organization - the Pediatric Congenital Heart Association!   In addition, Amy talks about what helped her the most while her son was in the hospital.  She talks extensively about how she is working with the hospital to provide transparency within their pediatric heart program when dealing with the parents of children with CHD.  Plus, she explains the differences between her organization and the other national heart organizations.   Join us for this podcast episode as Jennifer shares her story!
