Today, we welcome back Ian Kinsella to the podcast. Ian is the first person to appear on the podcast for a second time and his story definitely justifies this distinction. In 2023, only a few months after Ian appeared on ‘Irish in Toronto' and at the age of 41, he suffered a widowmaker heart attack. Following his heart attack in 2023, Ian underwent life-altering surgery. In November 2024, he set a Guinness world record for the fastest marathon with a coronary artery bypass graft, running the New York City marathon in 3 hours and 42 seconds. Ian is clearly grateful for running post-surgery but it was poignant to listen back to our conversation from three years ago and hear how much he appreciated the sport before he ever had any heart issues. Ian is upfront and honest in this conversation. His recovery is a winding process and he pulls no punches in going into the details here. Trigger warning, there will be mentions of suicidal ideation and substance abuse. I consider Ian a good friend and a role model for being honest with oneself.

This week we review a recent report from the team at Amrita Institute in Cocchin, India about their use of extended reality and virtual reality as well as heart model printing to aid in planning for complex intracardiac baffles for the treatment of complex congenital heart defects. What was the process used to provide actionable advice in the operating room during surgery? How has the team in southern India created a workflow that can accurately predict this complex anatomy and the patches needed to successfully septate complex hearts? We speak with the director of the 3D imaging group at Amrita, Professor Mahesh Kappanayil about this remarkable achievement of imaging in collaboration with surgery. DOI: 10.1016/j.jtcvs.2026.03.616

Heart Surgery | Joel 2 & Acts 2 In this Pentecost sermon, Rev. Dr. Lewis Galloway reflects on the story of Pentecost and the radical transformation that takes place when the Holy Spirit gives us a new heart. Drawing from Joel 2 and Acts 2, this message explores repentance not as guilt or shame, but as a turning — a reorientation of the heart toward God and toward one another. Through baptism, community, worship, and the power of the Spirit, ordinary people are invited into a new way of living together. From stories of literal heart surgery to young people inspired to change the world, this sermon reminds us that spiritual transformation does not happen simply by trying harder, but by learning to trust more deeply in the work of God within us. Again and again, Pentecost breaks open closed doors, dismantles fear, and builds bridges between strangers. The Holy Spirit creates a community where people from every language, nation, and background are welcomed together in Christ. Before Pentecost, the disciples gathered in fear behind closed doors. After Pentecost, the church became an open community filled with courage, joy, and hope. This is God's theater of operation — where anyone and everyone can receive a new heart.

Tom Foxley is having heart surgery. Fully awake. No sedation. A cardiologist will thread a wire through his groin, navigate it to his heart, and burn a small section of tissue — while he lies there conscious for two hours. And there's a small but real chance things could go wrong. He's a little bit terrified. In this episode he shares what that experience has surfaced — and specifically, the old toolkit he noticed himself reaching for. The one from Royal Marines training. Ignore it. Push through. Don't feel it. Be a man. That toolkit is extraordinary when you're being prepared for combat. It's what gets you through the unsurvivable. But when you're running a complex business, training for ultras, trying to be a present husband and father — it becomes the handbrake. And most high-performing business owners are driving with it on. This episode is about what he's been doing instead — and what shifted when he stopped suppressing the fear and started training his capacity to feel it. Topics covered: - Why suppress-and-push-through works short term and costs you long term - What unprocessed fear actually does to business performance - How to train emotional capacity the same way you train physical capacity - What changed when Tom stopped ignoring the surgery and started working with the emotion instead - What this means practically for how you show up this week
Tahlia was born with a complex congenital heart condition requiring three open-heart surgeries and a pacemaker. She was told early on that if she wanted children, sooner would be better than later. After conceiving through IVF, her high-risk pregnancy led to an emergency caesarean at 30 weeks, followed by an intensive 78-day NICU stay with her daughter Sunny.Tahlia's story is one of extraordinary resilience, the power of expressing breast milk as connection, and finding hope in the darkest moments. She shares candidly about the emotional toll of leaving hospital without her baby, the grief of missing ten weeks of pregnancy, and how the NICU community became her lifeline. This episode also highlights the incredible work of the Humpty Dumpty Foundation, whose donated equipment - including hospital-grade breast pumps - made such a difference to Tahlia's journey.Today's episode is brought to you by iL Tutto. Minimally invasive cardiac surgery (MICS) is one of the techniques used by cardiac surgeons to repair or replace valves, including the mitral valve. While the procedure has a shorter recovery time than the traditional sternotomy, it is still an open heart operation, and not the same as the minimally invasive techniques used by interventional cardiologists. SMH Chief of Cardiovascular Surgery Jonathan Hoffberger, DO, explains the different options, and how the heart team approach helps to pair the right patient with the right procedure. This week we replay an episode from nearly 2 years ago about an editorial commentary from multiple congenital heart surgical leaders in the US. The topic of their expert opinion piece is the concept of public reporting of results and their 'amplification' through organizations such as US News and World Report, to name one. How has the use of the database from STS been sometimes misinterpreted through public reporting or ranking systems? What was the initial intent of the STS database and how does the present usage of these data differ from the initial intent? Do STS risk adjustment models capture all aspects of risk for patients undergoing surgery and how do deficiencies in this result in unfair 'rankings' of programs? Can we 'take control' of our data and interpret it more clearly and accurately for the public and reduce misusage or misinterpretation of the data? How can public reporting result in improved outcomes? How can 'gaming' of the system be reduced? Dr. Emile Bacha, Professor of Surgery at Columbia University shares his deep insights into a complex and challenging topic.DOI: 10.1016/j.jtcvs.2023.03.022

tackles some of the most urgent and controversial issues shaping our world today—from the cutting edge of medical technology and the AI revolution to gender policy, cultural change, and the debate over reparations. Kicking off with a wild breakthrough in robotic heart transplants, the conversation quickly expands into the rapid-fire disruption of artificial intelligence, how deepfakes and digital mimicry are challenging reality, and why no one should expect the pace of change to slow down. The episode dives headlong into the Supreme Court's ruling on gender-affirming care, questions about the true legacy of feminism, and whether modern societies are undervaluing parenthood and family. With candid commentary on Chicago's reparations plan, the immigrant mindset, and the pitfalls of race-based policy, this episode asks the hard questions, highlights what's at stake, and offers a blueprint for thriving amid chaos and change. If you want to stay ahead of the curve—and challenge your own assumptions—don't miss this one.
