In this episode of Nutrition Pearls: the Podcast, co-hosts Megan Murphy and Jen Smith speak with Megan Horsley on her involvement with an intensive tube feed weaning program.  Megan is a pediatric dietitian at Cincinnati Children's Hospital Medical Center. She has 18 years of experience in pediatrics with a focus in cardiology and currently practices as a Lead dietitian as well as a cardiac float, serving all areas of cardiology (the Critical Cardiac Intensive Care Unit, the Acute Care Cardiology Unit and Outpatient Cardiology). Her most recent work has involved establishing a tube wean program in 2020 for heart patients which focuses on optimizing oral skills and getting them safely off their feeding tube.She is a Certified Nutrition Support Clinician and in addition maintains her Certification as a Specialist in Pediatric Nutrition. She is involved in many projects, research, and quality improvement work, including the National Pediatric Cardiology Quality Improvement Collaborative (NPC-QIC), the Advanced Cardiac Therapies Improving Outcomes Network (ACTION) and the Pediatric Cardiac Intensive Care Society (PCICS). She has also served as a board member of the Congenital Heart Alliance of Cincinnati since 2017 and is a member of the Prolacta Clinical Advisory Board (PCAB) since 2019. Megan has eighteen years of professional practice in pediatric nutrition and twenty-one years of personal experience as a heart mom herself. Some years ago, after exchanging heated words, Carolyn and I resolved our conflict through compassion and love for each other. I confessed my wrongdoing, and she prayed for me, referencing Ezekiel 36:26: “I will give you a new heart and put a new spirit in you; I will remove from you your heart of stone and give you a heart of flesh.” I sensed that God was conducting a form of spiritual heart surgery on me, taking away my fears and bitterness as He enfolded me in His love. God delights for us to engage with the Scriptures as I did back then, but it’s important to note the passage’s original context. Ezekiel spoke on God’s behalf to His people with the promise that He would cleanse them. Why? “It is not for your sake . . . I am going to do these things, but for the sake of my holy name” (v. 22). God would do His purifying work among His people so that all the nations would revere Him and know that He is God. God works His purposes in our lives not only to help us flourish and grow, but so we would bring Him honor. Even as He gave the Israelites the promise of a new heart and a new spirit, so through the inward work of the Holy Spirit, God replaces our cold and hardened hearts with those that receive and share His love and life.

Dr. Smith describes the excitement of helping to pioneer the institution's heart transplant program in the 1980s, when it was just one of only three hospitals in the country practicing heart transplantation. Dr. Smith also explains how a unique collaboration with Columbia's cardiology team led to the first of several groundbreaking trials, called PARTNER (Placement of AoRTic TraNscatheteR Valve), which paved the way for a monumental treatment for aortic stenosis — the most common heart valve disease that is lethal if left untreated. During the trial, Dr. Smith worked closely with Dr. Martin B. Leon, Professor of Medicine at Columbia University Irving Medical Center and Chief Innovation Officer and the Director of the Cardiovascular Data Science Center for the Division of Cardiology. Their findings elevated TAVR, or transcatheter aortic valve replacement, to eventually become the gold-standard for aortic stenosis patients at all levels of illness severity …

In this episode, Billy shares about how drinking too much coffee on Denali led to a dual diagnosis of Septal Defect and Partial Anomalous Pulmonary Venous Return resulting in open-heart surgery instead of a trip to Pakistan. A year later, he began to experience Severe Vena Cava Occlusion leading to the placement of a stent.
