You will recall a previous podcast about preventing Neisseria gonorrhoea through an effective vaccine. Neisseria gonorrhoeae is a ubiquitous sexually transmitted bacteria that can cause both localised and systemic disease if left untreated. It may be transmitted to neonates. We also reported on a rise of the gonorrhoea incidence, as well as increasing rates of antibiotic resistance. Today we will revisit the implementation of vaccinations against gonorrhoea in the UK. Here is an exciting up-date on the advice provided by UK's Joint Committee on Vaccination and Immunisation, JCVI, on using the vaccine 4CMebB OMV against Meningococcus used off-label for the prevention of gonorrhoea infection • in patients at significant risk of infection with bacterial STIs who • attend specialist sexual health services. Host: Dr. Fabiola Martin, Sexual Health HIV, HTLV Specialist, Canberra Sexual Health Services & University of Queensland, Australia Participants: Dr. Suneeta Soni, Consultant in Sexual Health and HIV, University Hospitals Sussex and Chair of the Bacterial special interest group for BASHH *British Association for Sexual health and HIV) Dr. Yen Bui, Consultant in Vaccination and Travel Health, Institut national de santé publique du Québec, Canada A/P. Let's focus on women - in this Episode: What females need to know about themselves: Early sexual arousal rate will influence later addiction to porn. Females are more vulnerable to succumbing to sex addiction where they experienced frequent sexual arousal at an early age.Alcohol consumption raises both sexes testosterone levels up to a point; but testosterone levels increase much more in females, with alcohol consumption. Females can then do out of character behaviours and become a very different person when they have consumed alcohol. If females are not on birth control or near their ovulatory window (eg day 8 to day 14) and they have consumed alcohol, they could have the same testosterone level as a male. Even if she is not consuming alcohol during that ovulating window, she will still have a much higher sex drive than other females, during that ovulatory window. So, be aware of the impact of consuming alcohol during that period! Behaviour (sexually) may be very different. That may include dressing more provocatively or more flirtatious; yet, at the same time, be less self-aware. Females are much more vulnerable. Not a great combination!Female anatomy makes them more vulnerable to STIs. The vagina has a thinner layer of skin; is moister and warmer for harbouring bacterial growth and yeast infection. STIs are often asymptomatic. Females may then experience such conditions as pelvic inflammatory disease or endometriosis; which can eventually lead to infertility. On average, females have eight times higher HIV risk and four times higher Gonorrhoea risk. That risk significantly increases with anal sex. HIV risk goes up 17 times with anal sex. If they begin their menstruation early, it increases their risk of uterine cancer. Early promiscuity leads to increased risk of vaginal and cervical cancer. Non-breast feeding increases the risk of developing ovarian and uterine cancer. Therefore, we observe that sexual activity in younger years, affects sexual health. Neisseria gonorrhoeae, in short NG, is a ubiquitous sexually transmitted bacteria that can cause both localised and systemic disease if left untreated. NG may also be transmitted vertically from mother to baby. Over the last years, we have seen a rise in the number of people diagnosed with gonorrhoea, alongside growing rates of antibiotic resistance. New research makes us hopeful that a vaccine may soon become available to provide global, large scale benefits. In this podcast, we explore this possibility with three experts in the field: Prof Kate Seib (1), Dr Claire Dewsnap (2) and Dr Silvia Nozza (3). Would you like to know more about the topic? Please read the paper we recently published in STI: "Prior Neisseria meningitidis (Nm) proctitis does not prevent Neisseria gonorrhoeae (Ng) proctitis among men who have sex with men (MSM)": https://sti.bmj.com/content/99/3/215 (1) NHMRC Leadership Fellow and Associate Director for Research, Institute for Glycomics, Griffith University, Gold Coast, Australia (2) President of the British HIV Association for HIV & Sexual Health (BASHH) and Consultant in Genitourinary Medicine, Sheffield Teaching Hospital, Sheffield, UK (3) Consultant in Infectious Diseases, Head of Prevention Unit, San Raffaele Hospital (IRCCS), Milan, Italy

Gonorrhoea is causing trouble, with highly resistant cases showing up in Massachusetts. But fear not, some cases were tamed with ceftriaxone. Just a friendly reminder to stay safe and get tested if you think you've got the "clap". On a lighter note, a new male contraceptive called EF-4-177 has been developed, giving guys another option besides condoms or "the snip". This non-hormonal option can reduce sperm count by 45% in mice. Time to take control, fellas! Also! the Arctic blast in Nova Scotia is about to tick off... literally! Biologist Vett Lloyd says that it depends on the snow cover. If there's not much snow, all ticks will be ticked off... forever. But, don't celebrate just yet, the little guys are tougher. Oh by the way, sea spiders can regrow their anuses.

Happy Hump Day everyone, and welcome to your weekly dose of Sex On The Peach. Today we'll be talking about a topic in the sexual space that literally no one wants to address or discuss. The matter of STIs. In my opinion, it's time we turn our attitudes about STIs away from shame and disgust, and toward support and education. It's a minefield of unnecessary stigma and negativity, and so having awareness and being informed is key. Words like "clean" and "dirty" are for your laundry, NOT for your sexual health, and if you contract an STI it does not then mean that you are less deserving of the fulfilling sex life that you desire. So today, we're going to discuss the STIs themselves, how to feel safer in communication surrounding them, and as always I'll be answering some of your questions.Sex On The Peach is a completely inclusive, open, judgement-free zone. However you identify - both in your gender and sexuality, whatever race, age, size, ability, religion you are - everyone is welcome here, and I hope that at some point in an episode we will touch on something you can relate to and resonate with. Please like, follow, share, review and subscribe to stay in the loop of all future episodes, and you can also follow me on Instagram, TikTok and YouTube @sexonthepeachcast. Mkurugenzi Mkuu wa WHO Dkt.

There are plenty of things that separate us from the animal kingdom. But one of the weirdest and most inexplicable is that we can grow so bloody old – way past reproductive age. And one theory is that we owe our longevity to none other than the sexually transmitted infection, gonorrhoea. How? In this week's episode, Dr. Oakley and Holly dive into an often stigmatized topic related to our gynecological health. STIs are shrouded in loads of shame, secrecy and regret. And it’s not just young people getting them—women in their 30s and 40s get them, too. In fact, the number of older people getting them is actually growing. We smash the stigma and learn that women living with an STI aren’t alone. The Lady Bod Podcast is presented by St. Elizabeth Healthcare and Physicians.

Australia’s rising number of gonorrhoea cases has been described as a potential ‘perfect storm’, with the emergence of extensively drug resistant strains overseas. University of Queensland Associate Professor David Whiley is leading a clinical study to determine the most appropriate treatment for patients by analysing drug resistance from the DNA of the organism causing the infection. He explains what is happening and what UQ scientists are doing to address the issue.
Helen Petousis-Harris discusses a proof of principle study which suggests that a type of meningococcal B vaccine could protect against gonorrhoea.
Kit Fairley discusses The Lancet Infectious Diseases' new Commission on sexually transmitted infections.
How can burdens and trends of gonorrhoea and syphilis be estimated using surveillance data routinely collected in low- and middle-income countries? Eline Korenromp, epidemiologist from the Avenir Health, Geneva, Switzerland, tells STI Editor Jackie Cassell how the Spectrum-STI tool, developed at request of the World Health Organization, facilitates standardised, country-level estimation of trends in adult prevalence of sexually transmitted infections. The Spectrum-STI is an epidemiological framework which facilitates data review, validation and strategic analysis, prioritisation of data collection needs and surveillance strengthening by national experts. It has so far been applied in Zimbabwe, Morocco and Mongolia. Read the full details of the study, “Estimating prevalence trends in adult gonorrhoea and syphilis in low- and middle-income countries with the Spectrum-STI model: results for Zimbabwe and Morocco from 1995 to 2016”, on the STI website (http://dx.doi.org/10.1136/sextrans-2016-052953).
Prevalence studies indicate approximately 10% of patients in HIV clinics may be infected with Neisseria gonorrhoeae (NG) and/or Chlamydia trachomatis (CT) at any given time, and a study recently published in STI by Burchell and colleagues reveals that a modest increase in testing in their clinic did not improve detection of CT and NG. Khalil Ghanem, STI associate editor, asks Stephen Berry, Division of Infectious Diseases, Johns Hopkins University School of Medicine, to comment on the paper and what can be done to improve detection of NG and CT. Yonatan Grad discusses the genomic epidemiology of antibiotic resistant gonorrhoea in the USA.
Cathy Ison discusses a Review about antibiotic resistance in gonorrhoea.
Some things can hurt!
Cathy Ison (director of the Sexually Transmitted Bacteria Reference Laboratory, HPA) talks to David Barlow (consultant physician at the Department of Genito-Urinary Medicine, St Thomas’ Hospital) about the future of treating and testing for gonorrhoea.
