La maladie de La Peyronie est un trouble médical qui affecte le pénis et provoque une courbure anormale lors de l'érection. Cette affection, souvent source d'anxiété et de gêne, touche environ 1 homme sur 100, surtout après 40 ans, mais sa fréquence est probablement sous-estimée en raison du tabou qui l'entoure.Le nom de la maladie vient de François Gigot de La Peyronie, chirurgien du roi Louis XV, qui fut le premier à la décrire au XVIIIe siècle. Elle se caractérise par la formation de tissu cicatriciel, appelé plaque fibreuse, dans l'enveloppe du pénis, connue sous le nom de tunica albuginea.Comment cela fonctionne-t-il ?Le pénis est constitué de deux cylindres appelés corps caverneux, entourés par la tunica albuginea. Lors d'une érection, ces corps se remplissent de sang et s'élargissent, mais pour que le pénis se redresse correctement, la tunica doit rester souple et élastique.Dans la maladie de La Peyronie, des fibroses (zones de tissu durci) se forment dans cette enveloppe. Elles empêchent la tunica de s'étirer uniformément, ce qui entraîne une courbure, voire une déformation du pénis lors de l'érection. Plus la plaque est rigide ou étendue, plus la courbure est marquée.La direction de la courbure dépend de l'emplacement de la plaque :Si elle est sur la face supérieure : le pénis se courbe vers le haut.Si elle est sur le côté : il se penche latéralement.Dans de rares cas, la plaque se trouve tout autour, provoquant un rétrécissement en forme de "sablier".Quelles sont les causes ?La cause exacte reste inconnue, mais plusieurs facteurs de risque sont identifiés :Traumatismes répétés lors des rapports sexuels (même mineurs),Prédispositions génétiques,Maladies du tissu conjonctif, comme la maladie de Dupuytren (déformation de la main),Vieillissement naturel des tissus,Certaines carences en vitamines, comme la vitamine E, ont aussi été évoquées.Quelles sont les conséquences ?La maladie de La Peyronie peut provoquer :Douleurs lors de l'érection (surtout au début),Difficultés à avoir des rapports sexuels en raison de la déviation,Troubles de l'érection, dans environ 30 % des cas,Un impact psychologique important : honte, perte de confiance, dépression.Existe-t-il des traitements ?Oui, mais ils dépendent de la gravité. Dupuytren's disease is a chronic, progressive condition that primarily affects the palmar and digital fascia of the hand. Over time, fibrous cords form under the skin, thickening and contracting, which can cause the fingers (most often the ring and little fingers) to curl inward. This results in Dupuytren's contracture, limiting hand function and impacting daily activities. It often starts with the formation of painless nodules in the palm. As the disease progresses, these nodules develop into cords that restrict finger movement. While the condition is not life-threatening, it can significantly affect a person's quality of life and is a common presentation in general practice. La maladie du Viking, ou contracture de Dupuytren, est une affection chronique qui touche principalement les mains, entraînant une perte progressive de leur fonction. Son surnom, "maladie du Viking", vient de sa prévalence accrue dans les populations d'Europe du Nord, en particulier chez les descendants des peuples scandinaves. Cependant, cette condition peut affecter n'importe qui, bien qu'elle soit plus fréquente chez les hommes de plus de 50 ans. Le trouble se manifeste par un épaississement anormal et une rétraction progressive du tissu conjonctif situé sous la peau de la paume. Ce tissu forme des cordons qui tirent sur les doigts, les forçant à se plier vers la paume. Les doigts les plus souvent atteints sont l'annulaire et l'auriculaire. Avec le temps, il devient difficile, voire impossible, de les redresser complètement, ce qui peut perturber les activités quotidiennes comme serrer la main, écrire ou manipuler des objets. He serves as the Chief of Reconstructive Transplantation as well as the Medical Co-Director of Operation Mend which is a UCLA program aimed at providing complex reconstructive surgery for wounded service members injured in Operations Enduring Freedom and Iraqi Freedom.   Dr. Azari completed General Surgery and Plastic Surgery training at the University of Pittsburgh Medical Center, a Tissue Engineering Fellowship at Carnegie Mellon University, and a Hand and Microsurgery Fellowship at UCLA. Foot pain is a common issue that many of us face at some point in our lives. While plantar fasciitis is often the culprit, there's another- probably under-reported condition that deserves attention: plantar fibromatosis, also known as Ledderhose disease. In our latest podcast episode, Debra Schindler takes a dive deep into this lesser-known condition with MedStar Health podiatric foot and ankle surgeon, Dr. Paul Carroll.Plantar fibromatosis affects approximately 200,000 people in the United States and is characterized by the growth of fibromas—benign nodules—on the plantar fascia, the thick band of tissue that runs along the bottom of your foot. Unlike plantar fasciitis, which typically causes heel pain that improves throughout the day, plantar fibromatosis presents as a constant pain in the middle part of the foot. Patients often describe feeling a pea-sized bump on the bottom of their foot, which can become painful as it grows.Dr. Carroll explains that while this condition is generally benign, it can be quite painful and debilitating if left untreated. The demographic most affected includes males, diabetics, individuals with chronic alcohol use, and those who have experienced trauma to the foot. Interestingly, there is also a significant correlation between plantar fibromatosis and Dupuytren's contracture, a condition affecting the hands.So, what should you do if you suspect you have plantar fibromatosis? Dr. Carroll advises seeking medical attention as soon as you notice any unusual bumps or persistent pain in your foot. Early diagnosis can help manage the condition more effectively and may prevent it from worsening. While there are several treatment options available, including conservative measures like custom orthotics and steroid injections, surgical options are often considered when other treatments fail. However, surgery comes with its own set of challenges, including a high recurrence rate of the fibromas.One of the more promising treatments discussed in the episode is the use of hyaluronidase injections, which help to shrink and soften the fibromas, making them less symptomatic. External beam radiation is probably the most common type of radiation therapy used in cancer treatments. Using x-rays and gamma rays are types of external beam, and that is the most common and what everybody thinks of when we talk about radiation therapy. Also, particles would be another type. Particles would be electrons and protons. Then there's brachytherapy. That's internal radiation, which is a technique that is sealed radioactive sources placed directly into or adjacent to the tumor.” TS 3:13  “First step [in the treatment coordination process] is that consult—getting the patients in the door. Quite honestly, this consult can be a long day for the patients. They may just have a consult with the radiation oncologist. [But if they're coming from a distance,] they might seeing a multidisciplinary clinic, so they are seeing all the physicians all in one day: the medical oncologist, the radiation oncologist, a surgeon.” TS 6:31  “We have a clinic team that oversees a lot of the clinical nursing side of things with rooming and then anesthesia recovery and things like that. And then we have the nursing working with the physicians and care coordination. We kind of have two different nursing roles within the Cleveland Clinic. So, education from both sides, you know, doing education, providing care for the patients and the patients' families.” TS 16:28  “This is one of the many phone calls that we get, I should say, almost daily. We get several phone calls from patients who say, ‘I've looked at my chart, I don't see my radiation treatment. Why are they no longer there? I don't see them. What's going on?' And it is because your radiation treatments ... do not interface with [the electronic health record]. You will be given a handout when you come because there is an issue with the system we use. We use a different system for the computerized radiation treatment, and then we use a different system for our computer charting. And they do not interface, they do not like each other. So, all of their radiation treatments do not show up in their [electronic health record]. They do not show up in their computer system.” TS 19:09  “Within the first two weeks, at least at the Cleveland Clinic, our plan is to give those patients a call back, see how they're doing, how they're doing with their side effects. Have they got scheduled for their follow-up? Do a check-in. Some of our patients have tox visits at six weeks with their nurse care coordinators, and that's just to check and see if they're having any lingering side effects, as well. And then we continue to get calls.” TS 26:31  “Listening to tumor board if you have the patience, so you know what patients that are going to be coming down the pike, because you've heard all the physicians discussing these cases. So, you know the plan because you've heard the surgeon, the radiation oncologist, the medical oncologist discuss the case. So, you know kind of what the plan is, then you can kind of get an idea, ‘Hey, I this one might be coming to me soon, and maybe I should be watching out for this patient or discussing this with my physician if I haven't seen it.'” TS 34:46  “In reality, it can be those days afterward, after they finish, that actually can be the worst. Letting the patients know that and that we're still only a phone call away and, you know, we're there for them. So, you know, continuing to educate also on when to call us—when to call, when to show up in clinic. We're there. We will get them an appointment. We will get them hydrated. We will do whatever they need.” TS 39:16  “Radiation therapy is not only used to treat cancers and malignant conditions. It is also used to treat quite a few benign conditions: arthritic knees, V-tach [ventricular tachycardia] in cardiac patients, Dupuytren's contractures—if you've watched the commercials that they're showing all over now—so, the Dupuytren's contractures of the hands and even plantar warts. So we use a lot of radiation therapy to treat these benign conditions, so it's not just malignant cancers.” TS 42:42

Welcome to 60 Questions. In this series, we will be interviewing different medical specialists to give our audience a better idea of what it is like to specialize in a certain field within the workers' compensation industry. In this episode we speak to Dr. David A. Blum, Dr. Blum is board certified by the American Board of Orthopaedic Surgery and holds a Subspecialty Certificate in Surgery of the Hand. He received his undergraduate degree (B.S., 1990) and medical degree (M.D., 1994) from the University of Florida. After completing a surgical internship at Orlando Regional Medical Center, he did an orthopaedic surgery research fellowship which involved publishing research papers and presenting at orthopaedic meetings. Dr. Blum completed an orthopaedic surgery residency at UMDNJ-New Jersey Medical School in 2001. He completed his hand and upper extremity fellowship at the University of Pittsburgh in 2002. Before joining Orthopaedic Center of South Florida, Dr. Blum had a very successful solo practice. He began practicing in Plantation in 2002. He is known in the community for his excellent and compassionate care for his patients. He won the Patients Choice Award and Most Compassionate Doctor Award, as voted on by patients, several times. ​Dr. Blum is on the cutting edge in minimally invasive surgeries and procedures such as ultrasound-guided trigger finger release, endoscopic carpal tunnel release, collagenase injections for Dupuytren's contractures, as well as wrist and finger arthroscopy.
