I have been told by my podiatrist that I have a grade one possible border lane, grade two sprain/tear, and I am struggling to get it to heal while I keep running.” What should a 53-year-old runner with a plantar plate sprain do? One of the most common runners' injuries I see for second opinion is a plantar plate sprain. A plantar plate sprain happens usually on the bottom of the second toe. It is a little bitty ligament that gets sprained or partially torn. Most of those runners with a plantar plate injury want to know how long is it going to take to get back to running. Recently a few runners told me they asked chat GPT, or some AI program to try to figure this out. I often get some questions from listeners I would never imagine. I just got one of those questions from a runner who has a mallet toe. Her doctor suggested surgery. The runner was worried because she been researching hammer toes and mallet toes. During that search she learned a lot about the plantar plate ligament as it relates to crooked toes. Plantar plate injuries in runners are a common topic at medical conferences because they cause a lot of pain in the ball of the foot. I just spoke to a runner who had an interesting question about plantar plate injuries, which was a question I never would have imagined. She wanted to know whether or not a "mallet toe" might lead to a plantar plate sprain. She didn't really understand the difference between a "mallet toe" and a "hammer toe." But she did understand that one or both are in some way related to plantar plate injuries. Does a mallet toe lead to a plantar plate sprain in runners? Well, that is a good question and that is what we're talking about today on the Doc On The Run Podcast.

The plantar plate ligament that gets injured in runners is typically located on the bottom of the foot, at the base of the second toe. It gets inflamed, it gets irritated, it may feel weird at first. The plantar plate may not even feel sprained, or sore, or injured. It might just feel like this weird sort of fullness sensation. I got a question from a runner who had those same strange symptoms. He was trying to figure out what treatment would best address the "fullness" sensation in and around the plantar plate. Which is better for an inflamed plantar plate in a runner who wants to run? Icing, cryotherapy, NSAIDs, which one is going to help the most? Well, great question and that is what we're talking about today on the Doc On The Run Podcast.
When you have a plantar plate injury, it can be frustrating. The whole problem stems from a tiny little ligament right at the base of the second toe that is getting irritated, stretched, and strained. If its really bad, you might even have some tiny little tears in the plantar plate ligament. I was just talking to a runner who asked me about other exercises. Because, she had been told... "stop running, do other exercises to get stronger." So, she signed up for a boot camp training class. Although she wasn't running, shewas doing lots of exercises that were potentially harmful to the plantar plate and slowing her recovery, and eventual rturn to pain-free running. So, which is better for a healing sprained plantar plate, running more or doing exercises that make you stronger like planks, push-ups, and burpees? Well, that is a great question and that is what we're talking about today on the Doc On The Run Podcast.

If you are a runner and you got diagnosed with a plantar plate sprain, you probably already figured that out plantar plate sprains can be frustrating. The plantar plate ligament is on the bottom of the foot, at the base of the second toe and it helps hold the toe down against the ground. What is crossover toe deformity? Well, if you look at the foot from the top, typically crossover toe deformity means the big toe tilts this way and the second toe forms a hammer toe and tilts that way and goes over the big toe. When you have the second toe over the big toe, it causes lots of problems. Today on the Doc on the Run podcast, we're talking about when a plantar plate tear can cause or contribute to the development of a thing called crossover toe deformity.
Plantar fasciitis that you can eat; who knew.
If you are a runner, and you have pain in the ball of the foot, and you have been diagnosed with a plantar plate sprain, you probably already realize you are in for a lot of trouble. It is very difficult to get the plantar plate ligament to heal while you are still running. If you caused a plantar plate injury by hyperextending the toe, then obviously, you need to stop hyperextending the plantar plate by stretching it and pulling on it. At least, if you want to get back to running faster. Today on the Doc on the Run podcast, we're talking about how hyper extension can lead to a plantar plate injury.
I was just on a webcam call with a runner who had a plantar plate injury. We were reviewing her MRI. And one of the first things I did was check to see how big the slices were on the MRI. That's because there can be a wide variation on how tight those cuts are when the MRI imaging machine is taking those pictures. Believe it or not, there's research that shows how big the plantar plate ligament usually is. How big is the plantar plate ligament? Well, great question and that is what we're talking about today on the Doc On The Run Podcast.

I was recently on a second opinion web cam call with a runner who had a plantar plate injury. One of the things we were talking about as was managing the inflammation in the joint next to the sprained plantar plate ligament. The plantar plate ligament is basically a thickened plate of collagen that helps hold the toe down against the ground and reinforces the bottom of that joint capsule. Inflammation in the joint which doctors call "synovitis" can damage the plantar plate. The chronic inflammatory fluid surrounding the plantar plate ligament is going to slowly degrade the collagen in the plantar plate ligament and make it more prone to stretching out, or worse, getting little plantar plate ruptures. How does chronic synovitis lead to plantar plate injury? Well, that is what we're talking about today on the Doc On The Run Podcast.

Anything the runners does to heal faster will get the runner back to running faster. Even simple things you can do at home can help change the course of an injury. On of the oldest treatments for musculoskeletal injury, and one any runner can do themselves, is massage. A runner on webcam just asked me whether or not it might help her plantar plate injury recover faster. The short answer is: Yes, massage can help as along as you do not injure the plantar plate ligament further in the process. How does massage really help when you have an injury? Yes, there are a couple of ways massage can help, as long as that massage does not make the injury worse.
