For patients suffering from melasma, titanium oxide is another step up.  Calluses build in response to external pressure. Adjusting footwear and habits should come FIRST. A general "clean up" of calluses at the nail salon is not recommended. A gentle approach while keeping some protection from the cushioning callus is recommended if the inciting stimulus remains. Also, the cuticle (little skin fold attaching the skin to the nail where the lunula is (white crescent) is super important to seal the nail unit from bacteria crawling in. What are ingrown toenails and how do we recognise them? They're common, typically the corner or side of toenail grows into the surrounding soft skin. It often occurs on the big toe, although it can occur on any toe. It causes pain along the nail edge, redness, swelling, and is tender when touched. What causes them? Commonly caused by a badly cut toenail – too short, or rounded edges. Wearing tight fitting footwear can increase pressure on the nail. Injuring the toe, such as stubbing it, or recurrent pressure from something like running. Sweaty feet, genetics causing curved toenail shape, and occasionally medication. How do you treat them? Mild, early Warm saltwater soaks. Lift the nail edge – put some cotton wool under the edge to it lift out. Wear more comfortable footwear, take panadol. Moderate It can sometime become infected. Requires antibiotics. Severe Requires nail removal under local anaesthetic. Often needs a podiatrist or GP. Can you prevent them? The main thing is to cut nails straight, square across, and not too short. Avoid tight shoes. Avoid picking at nails. Keep feet clean and dry. When we encase them in restrictive shoes, we develop lifelong bad habits, dysfunction, weakness, and atrophy—no wonder we have these ridiculous statistics, such as the 78% of Americans who complain of chronic foot pain. “Solutions” to their chronic foot pain range from orthotics to prescription medication to surgery. And did you know that Medicare covers only two pairs of shoes per year for elderly Americans that are in the Medicare system? Isn’t that kind of ridiculous? Of all the things that your standardized, nationalized healthcare policy should cover, they threw in a couple pairs of orthopedic shoes because everyone, of course, is going to experience foot problems and foot pain. But there is a way to escape from this disastrous decline, and that is to rebuild your foot functionality very carefully by transitioning over to a more barefoot inspired lifestyle. In this episode, I share how to get started, why you want to go barefoot as much as possible (and how do it safely!), why your big toe must be allowed to operate independently from the other toes and in multiple planes of motion, why a truly barefoot style shoe must have a zero drop sole, what experts have deemed as the single most destructive feature in the modern shoe, and much more! TIMESTAMPS: What happens to your feet in the modern day restrictive padded athletic shoe? [00:56] You need to rebuild your foot functionality very carefully. [02:21] You must have the articulated five toes. [03:16] It is very important how and where your heel hits the ground in the middle of your stride.
