Darrell has worked as an Emergency Manager for nearly 20 years at the local, regional, and state level when he was with Spokane Emergency Management and for the last 8 years at Providence. As an Emergency Manager for Providence Inland Northwest Washington, he was responsible for the Emergency Management Program for the five (5) Providence hospitals in Spokane and Stevens County that represent just over 1,000 licensed beds: a level 2 and 3 trauma centers for adults and pediatrics, two 25-bed Critical Access Hospitals (CAH), a specialty rehab facility, and stand-alone Ambulatory Surgery Center. He started working for Providence in September 2015. Prior to work at Providence, Darrell worked for the Spokane Department of Emergency Management for 11 years, as the Regional Coordinator for Washington State Homeland Security (HLS) Region 9 (6 years) and as a Program Coordinator (5 years). Region 9 is comprised of the 10 counties and three tribes of Eastern Washington. In that role, he supported regional collaboration, coordination, and an interagency approach to all-hazard emergency preparedness. He has worked in all phases of emergency management, supporting planning, training, and exercises, HLS grants, and grant-related projects. Darrell continues to support community-wide planning on behalf of Providence. Darrell has an undergraduate degree in Construction Science from Texas A&M University, a master's degree in business, and retired from the naval reserve as an Explosive Ordnance Disposal Officer December 1, 2015 (12 years active, 13 years reserve). He is most proud to be happily married (30+ years) and the father of three kids +1 (married daughter). Ryan Tuchmayer, MPH, CEM Director of Emergency Management Cedars-Sinai Medical Center Ryan is the Director of Emergency Management for Cedars-Sinai Medical Center. He also serves as a liaison between hospitals, clinics, and pre-hospital providers in the western region of Los Angeles. County. Quality and risk of bias were assessed for both data sources. Analyses were conducted for pre-/perinatal, postnatal, neonatal, and overall CP. For each region, trends were statistically classified as increasing, decreasing, heterogeneous, or no change, and most recent prevalence estimates with 95% confidence intervals (CI) were calculated. Meta-analyses were conducted to determine current birth prevalence estimates (from birth year 2010).Results: Forty-one regions from 27 countries across five continents were represented. Pre-/perinatal birth prevalence declined significantly across Europe and Australia (11 out of 14 regions), with no change in postneonatal CP. From the limited but increasing data available from regions in low- and middle-income countries (LMICs), birth prevalence for pre-/perinatal CP was as high as 3.4 per 1000 (95% CI 3.0-3.9) live births. Following meta-analyses, birth prevalence for pre-/perinatal CP in regions from high-income countries (HICs) was 1.5 per 1000 (95% CI 1.4-1.6) live births, and 1.6 per 1000 (95% CI 1.5-1.7) live births when postneonatal CP was included.Interpretation: The birth prevalence estimate of CP in HICs declined to 1.6 per 1000 live births. Data available from LMICs indicated markedly higher birth prevalence.What this paper adds: * Birth prevalence of pre-/perinatal cerebral palsy (CP) in high-income countries (HICs) is decreasing. * Current overall CP birth prevalence for HICs is 1.6 per 1000 live births. * Trends in low- and middle-income countries (LMICs) cannot currently be measured.* Current birth prevalence in LMICs is markedly higher than in HICs. * Active surveillance of CP helps to assess the impact of medical advancements and social/economic development. * Population-based data on prevalence and trends of CP are critical to inform policy.

We had the pleasure of interviewing The Hics over Zoom video.After rousing fans both old and new with their comeback last year, London-based multi-hyphenate duo The Hics return with their eagerly anticipated new EP HARMINE, out now on EMPIRE.A five track release conceived during the last 5 years and pieced together during the global pandemic, HARMINE opens with ‘Caught In A Lie', a deeply personal track first released as a single back in October 2021. For most of us, especially those in high-income countries, these experiences will be mild, bearable, and short. Others are not so fortunate. Millions suffer excruciating pain. Millions more suffer moderate or severe pain. They suffer despite the fact that cheap and effective treatments exist. This report briefly discusses the measurement of pain then explores three major causes of pain and what might be done to relieve them. The findings are relevant for individuals and organisations considering if and how to put their resources (i.e. their money and/or time) towards this global problem. Problem 1: Terminal conditions requiring access to opioids First, we consider pain from terminal conditions such as advanced cancer and HIV.[3] These cause intense suffering if untreated. If carefully treated with opioids[4], 95% of those with severe or moderate-pain report mild or no pain and quality of life greatly improves. In high-income countries (HICs), some 98% of this need for opioids is met; this figure falls to 5% in low and middle-income countries (LMICs). Cost is not the main barrier, however; a course of 90 days' opioids is as little as $8. This ‘access abyss' stems from an overestimate of the risks from opioids and a focus on life extending treatment (Knaul et al., 2017a). The specific issues include: restrictive regulation designed to stop the misuse of opioids; a lack of training and awareness among health professionals; and fragile supply chains. We discuss how organisations taking a multipronged approach have succeeded in improving access in India and Uganda. Problem 2: Headache disorders Second, we examine two headache disorders: migraines and cluster headaches. The former are common, affecting around one in six people, and sometimes debilitating. They impose a burden of disease roughly comparable with malaria or depression.[5] The latter are agonising and experienced by 0.1-0.2% of the global population. Inexpensive relief exists for migraines, such as aspirin, triptans, and propranolol. The main barriers appear to be: patients do not seek help; and if they do, they are often misdiagnosed by doctors who generally receive little training about these conditions. Education campaigns for physicians, patients, and policymakers seem to be the order of the day. Temporary relief exists for cluster headaches in oxygen and triptans, while preventative agents can be used to reduce the frequency of headaches. Like migraines, patients are often misdiagnosed and receive the wrong treatment. Yet, even with access to the right treatments, many people with cluster headaches endure a great deal of suffering. In addition to education campaigns, possible solutions include increasing access to proven treatments for people in low-resource settings or expediting the development of, and access to, novel therapies, such as psychedelics. Problem 3: Low back pain The third and final issue assessed is low back pain. This is the leading cause of years lived with disability globally. Little is known about what causes low back pain or how to treat it. Efforts to increase access to existing treatments are therefore unpromising. However, there may be a high value of information in medical research into the nature of and intervention for this problem. Conclusions and limitations Our report closes with suggestions for further research and identifies some promising career and donation opportunities. Abstract: Central nervous system (CNS) hyperexcitability is a clinically significant feature of acute ethanol withdrawal. There is evidence for a genetic contribution to withdrawal severity, but specific genetic risk factors have not be identified. The gene glyoxalase 1 (Glo1) has been previously implicated in ethanol consumption in mice, and GLO1 inhibition can attenuate drinking in mice and rats. Here, we investigated whether genetic and pharmacological manipulations of GLO1 activity can also mediate ethanol withdrawal seizure severity in mice. Mice from two transgenic lines overexpressing Glo1 on different genetic backgrounds (C57BL/6J [B6] and FVB/NJ [FVB]) were tested for handling induced convulsions (HICs) as a measure of CNS hyperexcitability during acute ethanol withdrawal. During withdrawal from an acute injection of 4 g/kg alcohol, both B6 and FVB mice overexpressing Glo1 showed increases in HICs from their wild type littermates. We also administered daily ethanol injections (2 g/kg + 9 mg/kg 4-methylpyrazole) to wild type B6 mice for 10 days and tested them for HICs on the 10th day, following treatment with either vehicle or a GLO1 inhibitor (S-bromobenzylglutathione cyclopentyl diester [pBBG]). Treatment with pBBG reduced seizure activity, although this effect was only statistically significant following two 10-day cycles of ethanol exposure and withdrawal. Taken together, these results provide converging genetic and pharmacological evidence that GLO1 can mediate ethanol withdrawal seizure susceptibility. Abstract: Objective: To identify, characterize, and automatically classify hypoperfusion-related changes in the blood oxygenation level dependent (BOLD) signal in acute stroke using spatial independent component analysis of resting-state functional MRI data. Methods: We applied spatial independent component analysis to resting-state functional MRI data of 37 stroke patients scanned within 24 hours of symptom onset, 17 of whom received follow-up scans the next day. All patients also received dynamic susceptibility contrast MRI. After denoising and manually classifying the components, we extracted a set of temporal and spatial features from each independent component and used a generalized linear model to automatically identify components related to tissue hypoperfusion. Results: Our analysis revealed "Hypoperfusion spatially-Independent Components" (HICs) whose BOLD signal spatial patterns resembled regions of delayed perfusion depicted by dynamic susceptibility contrast MRI. These HICs were detected even in the presence of excessive patient motion, and disappeared following successful tissue reperfusion. The unique spatial and temporal features of HICs allowed them to be distinguished with high accuracy from other components in a user-independent manner (AUC = 0.95, accuracy = 0.96, sensitivity = 1.00, specificity = 0.96). Interpretation: Our study presents a new, non-invasive method for assessing blood flow in acute stroke that minimizes interpretative subjectivity and is robust to severe patient motion. Paediatric training and the practice of paediatrics is built on the bedrock of ensuring best possible health outcomes for all children, optimising opportunity for those without full health and contributing to a voice, in advocacy, for children. In the world’s high-income countries (HICs), child health outcomes are comparatively good, and the paediatric workforce well trained to manage the health issues of children. Paediatric training and continuing professional development is generally under the authority of paediatric societies or colleges. Such organisations are well funded and supported, have long traditions of curriculum and resource development with supervisors trained in postgraduate training and supervision. They have a history of matching training needs to the health needs of their children. Some attempt at supporting advocacy and a voice for children is made, and a sense that paediatricians do all they can for the underprivileged is instilled during training. Such approaches are right and proper, but do they miss something fundamental? The question is the starting point of this podcast.
