For years, Medicare has paid hospitals and their affiliates more for services than it has to others. The reasons are complex, but this policy significantly incentivizes hospitals to absorb independent practices, creating “monopolies” that reduce competition and increase prices for everyone. Dr. Richard Kube, M.D., founder and CEO of the Prairie Spine and Pain Institute—an independent practice in Illinois—experiences first-hand how this policy and other top-down government regulations work against patients. Kube, an advocate for “site-neutral” payment, recently discussed this topic in Newsweek. “Site-neutral payment would end the unfair policies promoting consolidation and encouraging higher prices,” Kube writes. “Such proposals have bipartisan support in Congress. Several congressional committees are currently debating a health reform package, including provisions to establish site neutrality under limited circumstances. This would be an essential first step, one that physicians nationwide hope will soon extend to other services. Akuamoah, M.D., M.P.H is originally from Chicago. She is a board-certified anesthesiologist and pain management physician at The Spine and Pain Institute of New York. She graduated from The University of Rochester with a Bachelor of Science in molecular genetics prior to earning her medical degree and master of public health at Tulane University School of Medicine. She completed her residency in anesthesiology at The Mount Sinai Hospital in New York City followed by a fellowship in interventional pain management at the University of Pittsburgh Medical Center. A complete evaluation of each patient is performed by Dr. Akuamoah. She uses an evidence-based multimodal approach often recommending behavioral therapy, physical therapy, medications, and cutting edge minimally invasive procedures, when necessary. Dr. Akuamoah has expertise in the treatment of painful conditions of the back and neck, including sciatica, spinal stenosis, degenerative disc disease, arthritis, and continued pain after spine surgery. She also specializes in treating chronic joint pain, headaches, and neuropathic pain. Dr. Akuamoah strives to offer her patients new and targeted treatment options that take into account their diverse needs and lifestyles. Dr. Marcos Lopez PT, is a Physical Therapist in San Antonio, Texas. He is a board certified orthopedic clinical specialist. He completed the Therapeutic Pain Specialist program through International Spine and Pain Institute in the Fall of 2016 and completed Evidence in Motions Fellowship in Manual Physical Therapy program in the Summer 2019. Dr. Lopez has spoken at State and National conferences regarding physical therapy, integrating pain neuroscience into clinical practice, and evidence-based practice. In the past he has lectured for Modern Pain Care and facilitated their online mentoring program. Now, he is Adjunct Faculty for Bellin College where he developed the curriculum for the pain science course he teaches for the OMPT Fellowship Program. He is also Faculty with Evidence in Motion in which he leads weekend intensive courses focusing on orthopedic manual physical therapy and pain sciences. He has served on various leadership positions within the Texas Physical Therapy Association at the district and State level. Welcome back to Pain Reframed!  This week, Dr. Kory Zimney joins us from the University of South Dakota (Tim’s old stomping grounds where he grew up!).  Dr. Zimney has been practicing physical therapy since 1994, following his graduation from the University of North Dakota with his Masters in Physical Therapy. He completed his transitional DPT graduate from the Post Professional Doctorate of Physical Therapy Program at Des Moines University, Class 2010. He moved into full time academics in 2013. Past clinical work experiences have been with various community-based hospitals working in multiple patient care areas of inpatient, skilled rehab, home health, acute rehab, work conditioning/hardening and outpatient. He also was department manager of rehabilitation services at St. Luke's Health System in Sioux City for 8 years. He still maintains clinical practice one day a week at Mercy Business Health Services in Sioux City, IA working with outpatient work related injuries and providing on-site loss prevention and ergonomics services for local businesses. He also teaches nationally with International Spine and Pain Institute. Kory discusses the teaching of pain and how we can get better at managing patients with persistent pain.  Tim, Jeff, and Kory also have an important discussion about building trust in therapeutic alliance. The increasingly widespread use of opioid painkillers for chronic pain has created a public health crisis as many people are abusing these highly addictive medications. Aiming to reduce the number of opioid prescriptions doctors write, the Centers for Disease Control and Prevention recently issued guidelines that recommend doctors try pain relievers like ibuprofen before prescribing the highly addictive pills, and that they give most patients only a few days’ supply. But pain intervention specialists actually have a number of other alternative treatments that may address chronic pain with little or no medication. Today, Dr. Erik Shaw of Shepherd Spine and Pain Institute addresses the opioid crisis and explains some pain interventions that are helping patients manage their condition without addictive painkillers.
