Yvonne chats with Nicola O'Brien from The Social Media Animal Cruelty Coalition (SMACC) which was formed in May 2020 by members of the Asia for Animals Coalition. Their main goal is to unite organizations and address the issue of animal cruelty content on social media platforms. SMACC aims to expose and eliminate all forms of animal cruelty content and work with social media platforms to ensure its eradication. The coalition was created due to numerous public inquiries about cruelty content and the lack of progress by individual organizations working independently. SMACC collects data on animal cruelty content, writes reports to expose it, engages in ongoing communication with social media platforms, seeks dialogue with other platforms, raises awareness among the public and media, provides guidance on encountering cruelty content, and represents a unified voice through its network of diverse organizations.

Held captive as pets, dressed in human clothes, forced to perform tricks, and in some cases, even brutally tortured and killed. These are just some of the despicable ways that primates and other wildlife are being exploited online for likes, shares, and cash. The rise of social media has unfortunately made it easier for people to share photos and videos of animal cruelty. And according to research, primates make up an overwhelming majority of the abuse cases we find online. What's perhaps even more disturbing is the growing online community who not only seek out this kind of content, but are willing to pay to see horrific acts of abuse committed. What's worse - the social media platforms, like Facebook, Instagram, and TikTok, allow revenue to flow into the pockets of the content creators thanks to the popularity of these videos. 'I've watched some of these videos and I can't always watch them all the way through because they're so awful...what's worse are the comments from the community watching and enjoying them.'The range of types of online animal abuse is vast, varying from obvious and outright acts of violence to the more subtle and insidious fake rescues and wildlife as pets. Cruelty disguised as cuteness fuelling and perpetuating the illegal wildlife trade. Nicola O'Brien has been on a 20-year mission to ensure that vulnerable and defenseless animals are treated with respect and dignity. Previously at SMACC, Steve talked about NonSTEMI that needed the cath lab immediately and showed many ECGs which represented acute coronary occlusion (Myocardial Infarctions) but present on the ECG as very subtle findings (http://hqmeded-ecg.blogspot.com/search?q=subtle), particularly as subtle ST segment elevation that does not meet “STEMI” criteria and is diagnosed as NonSTEMI. Now, he builds on that idea and challenges the whole idea of a dichotomy between STEMI and NonSTEMI. These are NOT distinct pathologies, but rather exist on a continuum of intracoronary thrombus. Nevertheless, this false dichotomy is rarely recognised by emergency physicians or cardiologists, and patients suffer because of it. There are obvious STEMI, which always need the cath lab emergently, and for which time is myocardium. On the other hand, there are patients whose symptoms are resolved, ECG is non-diagnostic, shows no active ischemia nor subtle ST elevation, but whose troponin is positive and their resolved chest pain is due to an MI with an open artery and no ongoing myocardial cell death. These are NonSTEMI that can be treated with antiplatelet and antithrombotic therapy and get their angiogram the next day. And then there are the patients who have subtle ST elevation representing acute coronary occlusion, or who have active symptoms and/or persistent ECG ischemia. These patients do NOT have STEMI but do need the cath lab now. 25% of occlusions do NOT have diagnostic ST elevation and they do not get their angiogram until 24-36 hours later; their outcomes are worse: they have worse LV function, higher biomarkers, and higher mortality than NonSTEMI whose arteries are open at next-day cath. Steve advocates for an end to another dichotomy: activate the cath lab (Pathway A) or do NOT activate. Instead, he advocates what they, at Hennepin County Medical Center, call “Pathway B”: emergent consultation with cardiology, including a high quality emergency contrast echocardiogram to look for wall motion abnormality. Two inspiring and innovative educationalists, Dr Roger Harris and Dr Oliver Flower, are featured in this episode. Both of these Sydney intensivists are the force behind the recently created educational initiative named Coda, having previously been two members of the successful triumvirate who set up SMACC (Social Media and Critical Care). To my mind Oliver (or Oli) and Roger have led an educational revolution by utilising speakers with high level presentation skills and encouraging community engagement, through a blend of real life events, internet technology and social media, to bring us the type of innovative and entertaining educational platform we haven’t previously seen in intensive or critical care. Dr Roger Harris is a senior staff specialist in the ICU at the Royal North Shore hospital and the Sydney Adventist hospital. He is dual qualified in Emergency Medicine and Intensive Care. He is passionate about education, his five children and especially his wife Georgie. He’s on Twitter @RogerRdharris where his bio states he is “privileged to care for the critically ill.” Dr Oli Flower is also an intensivist at the Royal North Shore Hospital in Sydney. He’s steered the creative side of SMACC & Coda, from the graphic design elements to the style, marketing, website and social platform strategies. He has an interest in neurocritical care and is on Twitter @oliflower where his bio states he is a “believer in the power of the big medical community”. We recorded this in February 2020; just before the COVID-19 pandemic scuppered Oli and Roger’s plans to release nearly 4000 tickets to the inaugural large-scale Coda event. Coda is aimed to be more like a festival than a conference and is not only for critical care practitioners but for a wider healthcare audience. The overall goal of Coda is to cover vital broader healthcare subject themes and to assist the assembled community to take meaningful actions as a result of what is discussed, the climate emergency being one of the first of these. When Coda was one of the earlier healthcare conferences to be postponed in the pandemic, I also held this podcast back until there could be firmer plans about any live Coda events. A meeting named Coda Zero Live has now been announced for 16-18 February 2021. This is an event in Sydney with both a live and a virtual audience. It now seemed appropriate to release this episode to help you know about Coda and to encourage you to attend or tune in to Coda Zero Live. The SMACC Opening Ceremony is an integral part of the conference welcoming delegates to an educational experience unlike any other. Inspiration for SMACC Sydney included the inverted triangle: the alchemical symbol for water which is steeped in history and meaning to represent the City of Sydney and a celebration of diversity and resilience. Retrofuturism influences the design and theming of SMACC Sydney by playing on different aspects of 80’s and 90’s popular culture. Using music as a storytelling medium the show presents a mesmerising and unique soundscape combining elements of classical, electronic and popular music deftly created by Composer and Music Director Jake Meadows. This year’s opening ceremony echoes the program through four stems and performances each illustrating the unique and essential role we play as critical care professionals: Learning and Culture, People and Planet, Bedside Critical Care and Science and Innovation. The show opens with a spoken word performance by Nardi Simpson, an Indigenous performer telling an electrifying story of the land and the importance of Country to Indigenous culture. This is then followed by a vocal ensemble performing Queen’s Bohemian Rhapsody, a cheeky throwback to an iconic piece of music and an homage to the importance of humour and levity in our line of work. A change of scene brings a dramatic dance performance performed by ICU Registrar Khairil Musa and his dance partner Sarah King reflecting on the tragic final moments leading to a cardiac arrest. The finale blends classic 80’s pop with the iconic sound of the synthesiser with a toe tapping performance of Van Halen’s Jump led by vocalist Maya Weiss as an uplifting tribute to creativity and innovation. Performed in the Sydney International Convention Centre Theatre to over 3000 delegates we present to you our best and most memorable opening ceremony yet.

Climate change is a conversation we need to be having in Intensive Care circles. Right now. If the environmental catastrophe that is unfolding around us continues unabated there may no longer even be Intensive Care Units. The rising global temperatures, the melting ice, the extreme weather events, and their impact on agricultural crops and human habitation may well lead to such a fall in the economy that our healthcare system may not have the financial resources it does now. And given ICUs are the most expensive part of our hospitals, have a guess what might disappear first. So who is there better to listen to about the climate crisis than British intensivist, Professor Hugh Montgomery, a deeply passionate and highly intelligent man, who was a founding member of the UK Climate and Health Council, and who has helped raise awareness about climate change for over 2 decades. In this episode Hugh outlines some simple things you can do today to help fight climate change. Hugh is a Professor and the Director of the Institute for Human Health and Performance at University College London. He obtained a 1st class degree in cardiorespiratory physiology and neuropharmacology before graduating from the Middlesex Hospital Medical School in 1987. He has since obtained specialist accreditation in Cardiology, General Internal Medicine and Intensive Care medicine, and practices as a Consultant Intensivist in North London in the UK. Hugh obtained his MD(Res) in 1997, is known for his pioneering genetic research, and has published over 450 peer reviewed papers, three in Nature. Hugh is also an author of children's books, and most recently of the medical thriller ‘Control’. Hugh has a such diverse array of interests and expertise that it’s hard to understand how he fits it all in. He has presented on television shows, climbed tall Himalayan mountains, run ultra-marathons, jumped naked from a plane at 14,000 feet, holds the world record for underwater piano playing, and also consults as a medical expert on Artificial Intelligence. Despite all of this, he continues to work as a clinical intensivist, publishes in the medical literature about important critical care topics, and is working to raise patient awareness about their end of life wishes. I caught up with airway educator, innovator and self-described enthusiast Dr. Richard Levitan at SMACC in Chicago this past June. In this Best Case Ever on Airway Strategy and Mental Preparedness in EM Procedures, Dr. Levitan uses a great save of his in a penetrating trauma case as a basis for discussion on mental preparedness and how we've been thinking about our general approach to emergency procedures the wrong way. Rather than fixating on the final goal of a procedure, which can often be daunting and lead us astray, he suggests a methodical incrementalized and compartmentalized approach to EM procedures that reduces stress and fear, improves confidence and enhances success. He runs through several examples including intubation, cricothyrotomy and initial approach to hypoxia to explain his Simple Incremental Approach to EM Procedures. Could this be a paradigm shift in the way we think about procedures in EM?....
