There's no getting around the surges and potential chaos of an emergency department. The good news is that these events are predictable, you know they're going to happen, just not when. So how do you prepare and then navigate the storm once you're in the thick of it?In today's episode, we start with a discussion with Thom Mayer, MD exploring strategies to steel yourself for the inevitable chaos, underscoring the importance of maintaining a positive, agentic, proactive mindset. Drawing parallels to General Theodore Roosevelt Jr.'s decisive leadership on D-Day, the discussion highlights the critical need for quick, confident decision-making. A key focus is on the concept of "energy packets," a methodical approach to managing the cognitive overload in the ED by strategically allocating mental resources to specific tasks. We also delve into understanding individual stress thresholds and how to manage stress without succumbing to a sense of helplessness. In the second half of the episode, Chris Hicks, MD gives his approach to managing high patient volumes, communication while managing critical patients, and how to address a direct challenge in the resus bay. Guest Bios:Chris Hicks, MDChris is an emergency physician and trauma team leader at St. Michael's Hospital in Toronto. He has innovated in several areas of resuscitation and psychological skills, including mental practice, stress inoculation training. These days he enjoys a quieter life, free from the problematic corporate jib-jab of academic deliverables. In 2018, as a partial rebuke of the status quo, Chris co-created and chaired resusTO, an inter-professional simulation-based resuscitation conference in Toronto with international acclaim. In 2020, he co-founded Advanced Performance Healthcare Design, consulting with hospitals and industry using simulation to inform the design of systems, spaces and teams. Chris is an avid speaker and lecturer, staunch #FOAMed supporter, occasional runner and cyclist, fledgling boxer, semi-retired pianist, and proud father of three lunatic boys.Thom Mayer, MDDr. Mayer is the Medical Director for the NFL Players Association, Clinical Professor of Emergency Medicine at George Washington University and a Senior Lecturing Fellow at Duke University. “My cardinal rule – the rule that you cannot break is: don't be boring. Because you can have the perfect script that follows every screenplay formatting rule, but if you're boring, it doesn't matter. First and foremost, you've got to hook the reader,” says screenplay and short story writer Chris Hicks. Hicks is the author of a short story called “I Am Not Alone,” that recently was the subject of a five-party bidding war that came down to Warner Bros. and Netflix, with Netflix proving the victor. Genre writer Misha Green (Lovecraft Country) is set to write the screenplay and Jessica Chastain is attached to star. Hicks is part of a growing group of short story writers who are quickly making the jump from Reddit (r/NoSleep) to the big screen. But Hicks's success didn't come overnight. He talks about the long process, sometimes even years, it takes to perfect a short story. But it's clear he understands the relationship between reader and writer better than most. “You have a very limited window to grab somebody's attention. In the case of writing on Reddit, you have to have a clickbait title, something to entice somebody to click, ‘Oh, what is this?' And then you've got a paragraph to set the hook…The internet is a vast place and people are fickle with their time, so you have to make it worthwhile for them to hang around,” says Hicks. Design thinking, a human-centered design method, represents a potent framework to support the planning, testing, and evaluation of new processes or programs in healthcare. As opposed to traditional education needs assessment, design thinking takes the next step (beyond the impact on learning) to explore, diagnose, and test how new interventions will impact actual patient care and workflow. Andrew Petrosoniak, Chris Hicks, and Kari White from St. Michael's Hospital in Toronto will discuss how their team used design thinking to open a new emergency department. They employed end-user engagement and feedback to brainstorm and implement effective solutions to problems encountered before opening. The iterative steps and targeted use of simulation resulted in better designed departmental processes and actual clinical space while mitigating safety threats and departmental deficiencies. Design thinking, coupled with simulation, can be applied to current healthcare system challenges such as COVID-19. This session builds on this team's recent publication in Simulation in Healthcare to achieve the following objectives: Contrast traditional educational needs assessment with design thinking “customer empathy” Apply the steps of design thinking to create simulation interventions that best meet “end-user” needs Describe “use cases” of high impact design thinking-informed simulation education and quality and safety interventions

In this episode we speak with Chris Hicks, Canadian emergency physician and trauma team leader who is a master of teaching the cognitive skills of resuscitation.  Chris shares several protocols from his hospital regarding code blue and intubation in the era of COVID-19 including: the pre-brief process, communication during a code in PPE, preparing for a code (when you have time), what to do when there’s a surprise arrest and the team isn’t in PPE, how to effectively use an airway checklist, and how to find your anchor when you’re stressed.    Many of us “pregame” and have psychological skills that prepare us for whatever stressors we may encounter during a shift.  Similarly, we have scripts for most of the things we do in EM. Our script for the process of resuscitating patients does not work in the COVID-19 era.  COVID keeps us in system 2 thinking, which is time-consuming, labor intensive, and not efficient when you have to make quick decisions. Everything around preparation, PPE, role designation, and process is now strange. Since it’s an unfamiliar situation, we have to understand process and structure in a scenario with no script.   When resuscitating COVID-19 patients who are code blue, an organized structure and consistency are essential. Here are some recommendations: Have a team based pre-brief. This is a deliberate discussion before you enter the room which stresses the importance of cross-monitoring and establishes the rules. The team gathers around a code blue protected airway cart which contains airway equipment and PPE. Above the cart is a poster-sized infographic which details the pre-brief process. One physician is assigned the team leader.  Roles are assigned. Inside the room are 2 MDs, 1 RN, and 1 RT. Outside the room are 2 RNs. 1 safety officer (MD or RN) Donning and doffing buddies are assigned. Team members check each other’s PPE. A safety officer observes the process and makes sure there are no lapses in the PPE protocol. Decisions are made in advance regarding what will be brought into the room (drugs, drips, other supplies). This whole process can take less than a minute. Communication during a code in full PPE must be deliberate, succinct, and directive. Use closed-loop communication techniques. Baby monitors can be an effective means of communicating to team members outside the resuscitation room.   What happens in the scenario where there’s an unanticipated arrest and you feel there’s no time to prepare? While it’s admirable and brave to want to rush into the room to care for the patient unprotected, that cannot be the case in this era. We must put staff safety ahead of patient care. Hick’s institution has developed a process for protected code blue which has 5 key messages. Ensure airborne PPE for all providers before initiating BLS/ACLS. If nobody is in PPE, then nobody responds. Consensus opinion is that if you don't have appropriate PPE, you cannot be providing high risk procedures. If 1 person is in PPE, they can enter the room, put a mask on the patient and start compression-only CPR while others don PPE. Apply a NRB mask with filter when starting CPR. No BVM ventilation prior to intubation. If BVM is necessary, it should be a two person, four handed technique. Prioritize intubation using a protected airway process. Airway is prioritized earlier in the process vs. a standard cardiac arrest. Endotracheal intubation is preferred over a supraglottic airway. Pause chest compressions during intubation.   We are cautioned against using the BVM as it is considered highly aerosolized. Where does that happen in the circuit? With BVM ventilation, you run the risk of the patient’s airway secretions getting out of the BVM unless you have an interposing mechanical filter. Their BVMs also have a side port where a PEEP valve is meant to fit. They routinely attach a PEEP valve to prevent any passive flow of secretions from escaping.   Another cognitive offloading tool is a detailed protected airway checklist. The airway checklist is run by the 2nd physician, not the airway operator. It’s a call and response process, double checking that all necessary supplies are available and a reminder of things that will NOT be done (“We will not be inserting an airway. We will not be topicalizing medications. etc”.)  For the intubator, it is reassuring to have somebody else supporting you to make sure the process is safe. See also Protected Airway Equipment Checklist   During one of Chris' first intubations, he found himself unusually stressed while he was waiting for the paralytic to take effect.  He handled this by visualizing every step of the procedure and quietly reminding himself to be “steady”. This mental rehearsal helped center himself. He went from feeling like he might pass out to feeling ready to go. This situation accentuated the importance of having psychological tools when you feel afraid and stressed. And of the need to have a sense of humility when confronted with new situations. Chris Hicks is @HumanFact0rz He is into Human Factors. He is reading about Human Factors. He is writing about Human Factors. He is on stage, he is on EMCrit, he is on fire. But it’s a smouldering kind of fire, a nice fire, not a raging forest fire. A raging forest fire would be a better metaphor for what drove Chris into Human Factors; a bunch of over heated but talented egos coming together, with the all the poise of a conflagration, in a trauma bay. I met Chris first in Dublin, with a bunch of other erudite, humorous and well grounded Canadians. There does seem to be a rich vein of talent up there. What’s happening in Canada? Is it all as good as it looks from afar? Or does Justin Trudeau just look handsome and apologise a lot? Maybe its just the contrast to that orange person to the south that some people call Mr President? Either way I keep meeting charming gifted Canadians. So that bit is good. The orange guy is a worry. In an effort to improve those trauma bay infernos Chris and his chum Andrew Petrosoniak have recently edited/curated a special EMClinics of North America. It is a trauma special and being Chris, you know the Human Factors stuff will be good. That’s just the start of it too. He has people from all over the place in there. Amal Mattu, George Kovaks, Katrin Hruska, Brian Burns, Paul Engels, Karim Brohi and Captain America. That’s a lot of FOAMed types and a few superheroes too. Some of the EMClinics chapters will available for free until the end of December 2017. You can get free PDF’s from https://www.sciencedirect.com/science/journal/07338627 I reckon we should all download it. If the EMClinics of North America that was free for a while gets double or triple the downloads that it normally achieves then the publishers would surely sit up and take notice. That might cause a little disruption. Chris is good at that disruptive stuff. It is worth remembering that all this #FOAMed stuff is not necessarily safe from harm. If we lost a few major protagonists for whatever reason then how would FOAMed change? It is all happening because a relatively small number of people put a relatively huge amount of effort in. I don’t think we can get complacent. We certainly have not achieved universally free medical education have we? #FOAMed is great but conventional medical education is not getting any cheaper. Whilst the skills of applying life support and resuscitation take up most of our training, they are relatively easier to master than the skills that allow us to become good at diagnosis, good at communication, and most of all good at being resilient over a whole career so we can satisfactorily work with others and deal with the stress of working in intensive care. Peter Brindley, a Canadian intensivist from Edmonton, thinks that these “human factors” are crucial for us to master, especially in the second half of our careers, when we should be striving to be simply “a good person”. In this episode Peter reflects, tells some stories, and invites us to consider many important topics that will help us become better people. These include reflection, simulation, mental rehearsal, debriefing, dealing with upset people and the feeling of being an “imposter”. Peter is a full-time critical care doctor at the University of Alberta Hospital. He is a Professor of Critical Care Medicine, Anaesthesiology, and Medical Ethics. He has published papers and given talks widely. He was a founding member of the Canadian Resuscitation Institute; and was previously Medical-Lead for Simulation, Residency Program Director, and Education Lead at the University of Alberta. He is proudest of his two children, neither of whom care one iota what titles he may or may not possess. He is convinced that happiness rests in finding meaning and showing gratitude - he occasionally succeeds. This is the second in a series of DasSMACC special episodes, where I interviewed speakers from the recent DasSMACC conference held in Berlin. In addition to the human factors described above, we also spoke at length about burnout, its relationship to resilience, and the potential benefits of both working less (like part-time) and of having eccentric hobbies or passions. Peter spoke on several other topics including: His life journey from growing up in the United Kingdom to becoming a Canadian for all of his adult life The country of Canada, it’s national identity and it’s intensive care system The hallmarks of good teams Rudeness and its iatrogenic effects (including when family members are rude) The components of communication – including verbal, paraverbal and non-verbal A situation when Peter was accused of unprofessional behaviour and how he dealt with that How Peter has used a mini-sabbatical to reflect and to think about the next part of his career Exercise and the risk that it, other hobbies and material things can become like fetishes How our careers can be broken into thirds of “learning, earning and returning” but that we should consider all three in even the smaller periods like weeks or months The benefits of having an identity that is more than simply being a doctor His time on a cruise ship where he worked as an anonymous doctor How he is more proud of his writing of poetry and travel-writing than some of the medical papers he has published. Ep. 9 - The Trojan Horse of Simulation

  The dasSMACC (social medial and critical care) conference was recently held in Berlin. Simulation was a theme woven throughout the conference, and we thought a simulcast episode recapping on some of the messages and themes was timely. Smacc has arguably redefined the way we think about medical conferences – great speakers, great messages, hard core critical care in an engaging format. This year, simulation activities were embedded in main stage talks, workshops, panel discussions and the ‘Sim Haus’ we previewed in a recent Simulcast episode. A series of linked main stage talks from Clare Richmond, Chris Hicks and Jon Gatward gave us a framework for thinking about simulation modalities and matching method to objective. Following the journey of a head injured patient, we saw performances by actor Renee Lim, the incredibly realistic manikin made by Lifecast, and how end of life discussions, organ donation, cognitive biases can be addressed using simulation just as well as the action sequences involving airway management of a head injury. Also on the main stage - Brian Burns opening talk wasn’t about simulation, but rather was a futuristic trauma simulation in which he demonstrated the ways technology might improve our pre-hospital trauma care in the not too distant future, including drone delivered blood products. Jenny Rudolph gave us a practical insight and skills for dealing with ‘WTF’ moments, by seeking out the underlying frames for behaviour we find annoying or disappointing. She led a whole of audience exercise in resetting our response from unhelpful emotion to curiosity, bringing to life her longstanding work in double loop learning and debriefing using advocacy inquiry.     The interprofessional panel discussion (aka ‘the tribalism panel’) sparked conversations about how simulation can be an agent of culture change, but also how deliberate our strategies need to be in making our educational outcomes truly interprofessional. Both Jesse and I were involved in the education panel on Day 3 where simulation was again a core theme, especially as it pertains to preparing learners for the future, connecting with quality improvement in hospitals, and integrating into everyday work. Walter Eppich’s work on how to take our debriefing skills into the clinical area through coaching conversations received a lot of attention. As the Twitter moderator, Jesse wonderfully captured some audience questions and responses in this storify. Pre-conference workshops on Debriefing, Leave the Sim Lab behind and Stress Inoculation were a chance for attendees to deep dive with internationally respected faculty. Jan Schmutz presented his recent work on team reflexivity in the debriefing workshop (an enlightening read, but you will need to concentrate). The last of those workshops connected with a presmacc meeting in London – the Performance Psychology in Medicine Seminar. This fantastic program connected the worlds of high performance in pre-hospital care, other healthcare, elite sport and on stage. The event highlighted work like Mike Lauria’s Psychological skills to improve emergency care providers performance under stress, and others like Vicki Leblanc.   The Sim Haus was a dedicated physical space within the conference venue, which housed industry displays and ‘meet the experts’ session. There was some fascinating new technology and an atmosphere of sharing sim ideas and challenges.   Where to for more? In this episode on Cognitive Decision Making & Medical Error, Dr. Doug Sinclair, CMO of St. Michael's Hospital and Dr. Chris Hicks show us that, while the ED physician's knowledge base may play a small part in predicting medical error, more important might be how we understand and reflect upon our decision-making processes, how we communicate with our staff and patients, and how we cope with the ED environment and shift work. Medical error is the 6th leading cause of death in North America, and despite huge advances in imaging technology and lab testing as well as an explosion of EM literature in recent years, the misdiagnosis rate detected through autopsy studies has not changed significantly over the past century. Studies on diagnostic error in emergency medicine have shown error rates between 1 and 12%, and it's been suggested that cognitive error, or some flaw in the decision making process (as apposed to a lack of knowledge), is present in about 95% of these cases. Dr. Sinclair and Dr. Hicks elucidate for us how to identify and understand cognitive decision making and medical error, and how we can improve our decision making, reduce medical error and optimize the care of our patients.
