Episode 436: In June 1967, two of northern Saskatchewan's most respected bushmen, Métis political organizer James Brady and Cree band councillor Absolom Halkett, were dropped off by float plane to prospect for uranium near La Ronge. A week later, their employer found the camp standing, the beds slept in, the canoe still tied to shore. Both men were gone. The RCMP searched for three weeks, found a trail of unexplained clues, and closed the case without finding either man. More than fifty years later, a small team of researchers and family members went looking for answers the original investigation never sought, and what they discovered at the bottom of a northern lake raises more questions than it resolves. America spends more on health care than any other wealthy nation—and yet millions of Americans struggle to afford care, medical debt remains widespread, and our health outcomes often lag behind those of countries spending far less. So what happened? Did American health care simply become too complicated? Did insurance companies become too powerful? Did hospitals become too corporate? Did politicians fail us? Or did something much bigger happen? Something changed in American political and economic thinking beginning in the early 1980s—and according to Harvard health-policy professor and former Massachusetts legislator John E. McDonough, that transformation fundamentally reshaped American health care. His new book, America's Wrong Turn, argues that the rise of neoliberalism, deregulation, consolidation, and shareholder-driven economics created a health-care system that increasingly rewards financial performance rather than human health. Today, we're going back to the moment when America took that wrong turn—and asking the most important question of all: Can we find our way back?

Beginning in the 1970s and 1980s (think, e.g., Hayek, Friedman, Reagan, Stockman and Clinton), US policymakers decided healthcare should be defined like any other commercial or market commodity. It's not, think: Ken Arrow. Therefore, national policymakers under the mantra of the financialization of everything or privatization included healthcare (think: Medicare Advantage); largely per Robert Bork's book, “The Anti-Trust Paradox” policymakers freed healthcare of anti trust enforcement and increasingly deregulated healthcare (think: the employer-sponsored insurance (EIS) system). Today, US healthcare is arguably the largest industry in the world's largest economy, with a ~$7.5 trillion market cap, and is highly concentrated. As Prof McDonough cites, “America's health care crisis is,” per Phil Longman, “brought to you by monopoly.” Think: UNH that controls approximately 2,700 global subsidiaries and affiliates. We know the results. In his introduction Professor McDonough appropriately cites Maya Angelou's observation, “if you don't know where you've come from, you don't know where you are going.” Possibly more optimistic, James Baldwin phrased the idea as, “If you know whence you came, there is really no limit to where you can go." In this episode of the Explaining History Podcast, we are joined by John McDonagh, professor of practice at Harvard T.H. Chan School of Public Health, former Massachusetts state legislator, and key architect of Obamacare. He is here to discuss his new book, America's Wrong Turn: U.S. Healthcare in the Neoliberal Era – a searing diagnosis of how a revolution in economic thinking turned the American healthcare system into a global outlier.The United States spends more on healthcare than any other advanced nation – by a staggering margin – yet its health outcomes rank near the bottom. Life expectancy has been declining since 1980. The infant mortality rate is embarrassingly high. And while every other wealthy country covers 99–100% of its citizens, the U.S. still leaves millions uninsured.John explains how a single chart – showing U.S. healthcare spending as a percentage of GDP – reveals the story. In 1980, the U.S. was the highest spender but still in the pack. Then something happened. Around 1982–83, the U.S. "jackrabbited" away from every other nation. The culprit? The neoliberal revolution – the ideas of Milton Friedman, Friedrich Hayek, and the Mont Pelerin Society – that captured American policy under Ronald Reagan and transformed healthcare from a public good into a site of value extraction.John traces how monopoly capitalism, private equity, and the mantra of "maximising shareholder value" have hollowed out the system. Hospitals are bought by private equity firms, stripped of their land and buildings, and turned into tenants paying exorbitant rents. Two companies now control 90% of dialysis care. Marianne Apostolides is the award-winning author of eight books, four of which have been translated and published internationally.
