Pulmonary embolisms don't always announce themselves... sometimes they ambush. One minute your patient is walking with physical therapy, the next they're hypotensive, hypoxic, and coding. This re-released early episode dives deep into why PE patients can look deceptively stable… right up until they aren't.In this episode, I revisit one of my earliest case-based teachings on pulmonary embolism, updated with an added segment on vasopressin use in obstructive shock from PE. Through real bedside stories from my time as a rapid response and ER nurse, we break down the physiology behind PE-related collapse, why intubation isn't always the answer, and how to think through management when the right ventricle is failing in front of you. This is a sobering but essential refresher on one of the most dangerous diagnoses we encounter.Topics discussed in this episode:Why pulmonary embolism is a common cause of in-hospital cardiac arrest (even if it's not common overall)Classic and subtle PE presentations and why they're often missedA real-time rapid response case: stable to crashing in minutesRisk factors for PE and the anticoagulation double-edged swordObstructive shock explained: what's actually killing the patientRight ventricular failure, septal bowing, and the spiral of deathWhy intubation can worsen outcomes in massive PEVasopressors in PE: norepinephrine, epinephrine, and vasopressinThe unique benefits of vasopressin in obstructive shockThrombolysis vs. thrombectomy: when TPA helps — and when it's deadlyBedside echo findings that point to massive PEWhy PE patients can crash during transport (and what to always bring)Nursing vigilance, rapid escalation, and activating help earlyWhen perfect care still isn't enough and the heart of nursing in end-of-life momentsMentioned in this episode:CONNECT

Pod Crashing episode 428 with Maggie Freleng from the podcast Bone Valley Season 3 Mayfield is a small town in Kentucky with a population of only 10,000. It's the type of place where gossip spreads like weeds, where the residents know something about everyone. But as the Lava team digs in to learn more, it's as if the entire town withdraws and goes dark. There's an aura of fear and suspicion. But of what? Pod Crashing episode 427 with Bridget Armstrong from the podcast Curse of America's Next Top Model. When America's Next Top Model premiered in the early 2000s, it looked like a golden ticket: a glamorous TV show that could catapult aspiring models into fame. But two decades later, many contestants describe its aftermath as a curse. You'll hear from contestants who say ANTM saved their lives and from those who say it ruined theirs. The contestants also talk about how the body shaming they experienced on the show caused lifelong scars. Finally, we try to answer the question, is Tyra Banks the ultimate reality television villain? Y'all, my guest today, Toni Becker is absolutely incredible.Toni lives in South Africa and is a content creator with twelve years sober. Toni was plagued by disordered eating years before her addictions to meth and alcohol began. Years of partying led to homelessness, meth psychosis and severe illness and kidney failure. In sobriety, Toni was still struggling with disordered eating. Treatment in sobriety finally helped her with that struggle. Later in sobriety, she developed facial dysmorphia induced by social media expectations and filters. Talking about these issues in sobriety was fascinating and I learned so much from Toni!Connect with Toni on InstagramDM me on InstagramMessage me on FacebookListen AD FREE & workout with me on Patreon Connect with me on TikTokEmail me chasingheroine@gmail.comSee you next week!
