Over thirty percent of individuals living with dementia living in the US each year die either of or with dementia – and almost half of those enrolled in hospice have dementia. As with so many other types of healthcare, there are disparities in both who enrolls in hospice as well as the type of care these individuals receive after enrollment. In a recent election, members of the Confederated Tribes of the Grand Ronde voted to amend their constitution and drastically limit tribal disenrollment. The CTGR is one of the few tribes to make a revision like this to protect its members. The new changes now mean only members who enrolled fraudulently would face disenrollment or those who were a member of another tribe when they received Grand Ronde citizenship. Cheryle Kennedy is the Chairwoman for the tribal council with the CTGR. She joins us to share the effects disenrollments can have and the protections will mean for both individuals and tribes.

Hospice patients with dementia are up to four times more likely to be disenrolled before death than patients without dementia (PWD). Additionally, PWD who are also racial minorities may receive lower quality of care while in hospice. Dr. Lauren Hunt, PhD, RN, FNP of UCSF, Department of Physiological Nursing, University of California San Francisco discusses these findings that were part of "Risk Factors for Disenrollment from Hospice in Persons with Dementia: A National Cohort Study", that was presented at the 2021 American Geriatrics Society General Meeting. Dr. Hunt has worked in healthcare for more than 20 years, serving as a research coordination, hospice nurse and palliative care nurse practitioner. The percentage of Native “blood” one possesses, the fraction listed on Certificates of Degree of Indian Blood, and a fraught concept that has its defenders and dissenters in our communities. Despite its colonial origins, many tribes still use blood quantum as a requirement for tribal enrollment, and these fictional fractions carry huge weight in the lives of Indigenous Peoples. $11.1 Billion. That's how much, according to the 217 CAQH Index, the healthcare industry could save by transitioning to fully electronic administrative transactions. The transactions are for things like claim submission, eligibility and benefit verification, pre-authorization, etc. These are clearly defined standards and many of them are widely adopted. This is real interoperability in healthcare and it's in place today. Yet, not everyone is participating. Why is that? How can we expand adoption and realize the tremendous value that's available for the taking? With richer data, CAQH can deliver increasingly useful insights, more precise measures of progress, and more accurate estimates of cost savings. All health plans and providers, regardless of adoption status, are encouraged to participate. Receive a Customized Benchmark Report. Health plans and healthcare providers that contribute data receive customized benchmark reports. CAQH prepares and delivers a detailed analysis of organizational performance alongside national results to each data contributor. Hey Medicare Nation! It's January 2018! I hope everyone made informed decisions regarding your Medicare Advantage Plans for 2018. If you missed the last episode, go back and listen to it! I discussed the Medicare Premiums, co-pays and co-insurance for 2018. Many of you have sent me emails "asking me" if you can change your Medicare Advantage Plan in January. The answer is......yes....with specific guidelines. Currently, it is the Medicare Advantage Plan "Disenrollment Period." The current Disenrollment Period runs from January 1st through February 14th each year. During this time, you can "drop" your Medicare Advantage Plan and go back onto Original Medicare. You do this by contacting MEDICARE by phone 800-633-4227.....and telling the Medicare representative that you would like to "Disenroll from your Medicare Advantage Plan" to go back onto Original Medicare. Medicare may also help you with a Part D prescription Drug Plan if you'd like. On Original Medicare, you are covered under Part A and Part B of Medicare. Under Part A....you are covered for Medicare benefits where you would stay at a location as an "inpatient." The most common location is .....The Hospital. Another location where you stay overnight as an inpatient is....a Skilled Nursing Facility (SNF). A SNF is NOT a Nursing Home. An SNF is a location where you are admitted as an inpatient to receive medical care and rehab 24hrs a day. Also..... if you are diagnosed with a terminal illness, your doctor may suggest you enter Hospice as an inpatient. All the services covered in the Hospital, SNF and Hospice are covered under Part A of Medicare. There is a "Deductible" each time you are admitted to the Hospital. The Deductible cost for being admitted as an inpatient in the hospital is $1,340.00 in 2018. The Deductible is due EACH benefit period you are admitted. Part B of Medicare is for "Outpatient Services." Benefits under Medicare for Outpatient Services covered under Part B include, but not limited to: * Doctor Vists * MRI's * Laboratory Blood Draws * Outpatient Same Day Surgery * Oxygen in your home There is an "Annual Deductible" for Part B of $183.00. After you pay your $183.00 annual deductible, you will be responsible for the remaining 20% Medicare Allowable Charges for services under Part B. What does that mean? Let's say you already visited your Cardiologist and had bloodwork drawn at Quest or Labcorp. We'll say your out-of-pocket costs for both cost a total of $183.00. That takes care of your annual Part B deductible for 2018. Now....let's say three months later.....you need to have an MRI. We'll say the Medicare allowable cost is $1,500.00. Medicare Part B covers 80% of the $1,500.00, which is $1,200.00. You will be responsible for the remaining 20%, which is $300.00. You will pay 20% of ALL Part B Medicare Allowable Charges. There is NO Cap! You may also need Prescription Drug Coverage. Prescription Drugs are NOT covered under Part A or Part B in general. Prescription Drugs will be covered while you are admitted to one of the facilities under Part A. If you want Prescription Drug coverage, you WILL need to enroll in a stand-alone-prescription-drug-plan. You can find which Prescription Drug Plan (PDP) is available in your area, by going onto the Medicare.gov website and "hover" over the FIRST Blue Box named "Sign Up/Change Plans." A column will appear and go down to where it reads..."Find Health & Drug Plans." "Click" on that box and it will bring you to the Medicare Plan Finder site. Type in your zipcode and follow the instructions. If you are comfortable with the costs associated with Original Medicare Parts A & Part B.....then that's all you need to do. If you'd like to add additional coverage to protect you against the on-going out-of-pocket costs associated with Original Medicare, you can purchase a Medicare Supplement (a.k.a. Medi-Gap) Plan. A Medicare Supplement Plan is an Insurance Policy, where you pay the insurance carrier a monthly premium and the plan will pay Medicare out-of-pocket costs that you have pre-determined. Medicare Supplement Plans "VARY" in coverage and in premiums. The "Medicare Benefits" they pay for you, are the SAME, no matter where you live in the U.S. So.....if you chose a Supplement Plan "F," which is the policy which pays ALL your out-of-pocket costs for Medically Necessary services under Medicare, and you live in Seattle, WA.......you will be covered for the EXACT SAME Medicare benefits as a person living in Tampa, FL. What is different you ask? The difference is in the PREMIUM you pay. Insurance Carriers that offer Medicare Supplement Policies charge DIFFERENT Premiums! You NEED to know what the difference in Premiums are by EACH Insurance Carrier for the SAME TYPE OF PLAN. Here's an example: Mary is turning 65 in March of 2018. Mary has a history of heart problems and would like to remain on Original Medicare and purchase a Medicare Supplement Plan "F" so that she can see ANY Cardiologist that is contracted with Medicare.... in ANY State. Mary also wants to have a budget for her out-of-pocket health costs and having a Medicare Supplement "F" plan will allow her to do that. Mary lives in Miami, FL and calls her Medicare Specialist Diane. Mary discusses purchasing a Medicare Supplement with Diane and asks for her expertise and guidance. Diane tells Mary that the 3 lowest premiums in her zipcode have the following montly premiums: 1. $239.00 From Acme Insurance Co. 2. $250.00 From Beta Insurance Co. and 3. $275.00 From Delta Insurance Co. These premiums are for the EXACT same Plan with the SAME benefits! Why would you pay Delta insurance company $275.00 a month, when you can pay Acme Insurance Company $36.00 a month less....for the SAME benefits! That's why it's soooo important to speak with a Medicare Specialist or Medicare Consultant like myself. I speak MEDICARE! I care about YOUR best interests! I have NO loyalties to ANY Insurance Company! You can also STAY on the Medicare Advantage Plan you are enrolled in. The neighborhood and the town that you spent your childhood. How familiar does it feel to you? The friends you grew up with, other family members that you spent time with regularly. That comfortable and secure feeling. Now imagine one day that you are told that you may no longer live in your home and that in fact, you no longer have a right to stay in it, or the right to even live in your neighborhood. Imagine how you would feel if you suddenly were faced with having to move away from that home, those surroundings...all because someone (else) said you are no longer welcome. Rick is a descendant of Paulina Hunter, an original allottee to the Temecula Indian Reservation in California, who was disenrolled from the Pechanga Band of Luiseno Indians in 2006. Paulina Hunter was disenrolled 107 years after her death, and ALL descendants, living, dead and unborn, had their heritage eliminated. To bring awareness to this problem and to the very subject of dis-enrollment, He founded "Original Pechanga" a Blog which chronicles cases surrounding tribal dis-enrollment for members of his own people as well as news and information on other dis-enrollment actions around the country. The Original Pechanga blog was started in March of 2007 and continues to bring awareness about this modern form of Tribal Genocide. “Look for those opportunities that may not be obvious to you.” Gabe Galanda is a member of the Round Valley Indian Tribes. Galanda is also a partner at the law firm Galanda Broadman. He joined NextGen Native previously to share his personal journey. If you haven’t heard his story, I recommend listening to his personal journey. It’s a perfect example of how one can overcome challenges to succeed in your own personal way, at a high level. His personal journey is prologue to his work for clients facing disenrollment. Through his law practice, Galanda emerged as one of the most vocal critics of disenrollment. For several years, he has represented clients fighting disenrollment. During that time he experienced many trying moments and challenges in his fight for his clients. At the time, not many people in Indian Country were openly discussing disenrollment, let alone fighting against the movement. But the tide may be turning. We spoke not too long after a #stopdisenrollment day of action and also following the decision by the tribal council for the Robinson Rancheria of Pomo Indians to re-enroll members that were previously disenrolled. Now, you’re seeing people speaking out against disenrollment, and taking action against it, in larger numbers. Much of that can be attributed to the work of Galanda and others who took on the fight several years ago. We discussed the fight against disenrollment, how Gabe managed the challenging battles on a personal and professional front, and what’s next in the fight. This includes an upcoming symposium called “Who Belongs” at the University of Arizona Rogers College of Law. The event includes a list of heavy hitters that will attend including tribal leaders and leading legal minds. This is a huge topic of the moment for Indian Country, and it’s one of the most heavy and intense topics, too. But this podcast creates space to discuss these issues in an in-depth, nuanced fashion, even if the fire still burns hot. This is one of those conversations, and I hope it sparks more across Indian Country.

The show’s topic is about tribal disenrollment, and how disenrollment is detrimental to individual and tribal existence and identity. When tribes decide to remove people from their rolls, individuals who have been on tribal rolls for many years, decades, the action(s) of removing tribal members, if the reasons are other than to maintain the safety of tribal members, is a move to weaken your tribal base.Has the tribal body been party to the removal of some individuals, or are the actions taken by a few individuals who may have an agenda? In some cased, the disenrollment of an individual, or families is initiated by individuals on tribal councils, and the tribal body may not know, or, may not agree with the council.
