My heart is yearning (yep, it's a NOFX song) in anticipation.Of what?Well, that's what the episode is, you jerk. Don't ruin the surprise.Coming on the heels of the tragic shooting in Sydney, Australia, the boys couldn't help but notice the commentary about how someone, somewhere, should have anticipated it.What does that have to do with Punk Rock Safety?Well, anticipation is one of what Erik Hollnagel calls the four potentials of resilient systems. Sounds great, but what - and how - are we supposed to anticipate? It doesn't seem possible to predict every possible failure or event, right? But what about conditions in the system?Instead of trying to 'Magic 8 Ball' everything, Ron, Dave, and Ben suggest that what organizations should anticipate is where systems, processes, or people may be stretched, stressed, or pushed to their limits. Like in the circle pit. Or pretty much any time you're forced to listen to the Misfits for very long. Or ska.Basically, we should focus on anticipating where there is less capacity to adapt and maneuver. Recognizing these vulnerable spots, organizations can then build their capacity to adapt, respond, and recover, even if it isn't a specific scenario.So, expecting a single, complex convergence into an unpredictable event is tough. Planning for degraded conditions in parts of the system without a lot of backup? That's the kind of anticipation that counts (somebody let the sound guys at Punk in the Park know, would ya?). The LA-Barstow-to-Vegas (LAB2V) is a famous, long-running annual two-day dual-sport motorcycle event that covers over 400 miles of mostly off-road desert terrain between Los Angeles and Las Vegas, and our very own Charley, Scottie and Hector decided to give it a go. This is not an easy ride, so most people take a light, nimble and capable off road bike something like the KTM EXC500. But of course Misfits don't take the easy ride, and opt for the challenge instead. Charley rode his Yamaha Super Tenere, Hector his KTM 1290 Adventure, and being the wisest among them, Scottie opted for her Husqvarna 701. It might not surprise you to hear that only one of them finished the ride (I wonder who?) but all three arrived safe and had a great time. They share their stories and lessons from the trip, and we talk about what would be the perfect bike for this challenging ride. And due to Charley's unfortunate situation, we take a moment to talk about bearings, how important they are, and how little they've changed in the last 100 years. It typically affects infants 60 days old Gestational Age: GA > 32 weeks (and Post-Conceptional Age > 45 weeks) Frequency: This is the first episode Duration: Lasted < 1 minute Intervention: No CPR performed by a trained professional Clinical Picture: Reassuring history and physical exam Management for Low Risk: Generally do not require extensive testing or admission. Prioritize safety education/anticipatory guidance. Ensure strict return precautions and close outpatient follow-up (within 24 hours). High Risk Criteria Any infant not meeting the low-risk criteria is automatically High Risk. Additional red flags include: Suspicion of child abuse History of toxin exposure Family history of sudden cardiac death Abnormal physical exam findings (trauma, neuro deficits) Management for High Risk: Requires a more thorough evaluation. Often requires hospital admission. Note: Serious underlying conditions are identified in approx. 4% of high-risk infants. Differential Diagnosis: “THE MISFITS” Mnemonic T – Trauma (Accidental or Non-accidental/Abuse) H – Heart (Congenital heart disease, dysrhythmias) E – Endocrine M – Metabolic (Inborn errors of metabolism) I – Infection (Sepsis, meningitis, pertussis, RSV) S – Seizures F – Formula (Reflux, allergy, aspiration) I – Intestinal Catastrophes (Volvulus, intussusception) T – Toxins (Medications, home exposures) S – Sepsis (Systemic infection) Workup & Diagnostics Step 1: Stabilization ABCs (Airway, Breathing, Circulation) Point-of-care Glucose Cardiorespiratory monitoring Step 2: Diagnostic Testing (For High Risk/Symptomatic Patients) Labs: VBG, CBC, Electrolytes. Imaging: CXR: Evaluate for infection and cardiothymic silhouette. EKG: Evaluate for QT prolongation or dysrhythmias. Neuro: Consider Head CT/MRI and EEG if there are concerns for trauma or seizures. Clinical Pearl: Only ~6% of diagnostic tests contribute meaningfully to the diagnosis. Be judicious—avoid “shotgunning” tests in low-risk patients. Prognosis & Outcomes Recurrence: Approximately 10% (lower than historical ALTE rates of 10-25%). Mortality: < 1%. Nearly always linked to an identifiable cause (abuse, metabolic disorder, severe infection). BRUE vs. SIDS: These are not the same. BRUE: Peaks < 2 months; occurs mostly during the day. SIDS: Peaks 2–4 months; occurs mostly midnight to 6:00 AM. Take-Home Points Diagnosis of Exclusion: You cannot call it a BRUE until you have ruled out obvious causes via history and physical. Strict Criteria: Stick strictly to the Low Risk criteria guidelines. If they miss even one (e.g., age < 60 days), they are High Risk. Education: For low-risk families, the most valuable intervention is reassurance, education, and arranging close follow-up. Systematic Approach: For high-risk infants, use a structured approach (like THE MISFITS) to ensure you don’t miss rare but reversible causes. Read More

First-Half Flames, Second-Half Fix The Detroit Lions survived a nail biter against the New York Giants. It was a big NFL win, but it started ugly. The defense looked disorganized. Misfits. Miscommunication. The Giants scored more in the first half than Philadelphia managed across four quarters. That set the tone. The week's theme inside Allen Park was firefighting. Defensive coordinator Kelvin Sheppard called his unit the firefighters. Dan Campbell leaned into it too. In the first half, everything burned. After halftime, the Lions put the fire out. Big yet somewhat vague news on Misfits. Watch out for the nasal sprays! Instead of doing something fulfilling on Halloween like "taking my children trick or treating," or perusing LA Halloween whores, I went home and watched GI Joe: The Movie (from 1987) Subway got rid of their flatbread for breakfast sandwiches, because of course they did.  I went to see a myriad of old movies around town in the fortnight since we last spoke: Bullitt, Fitzcarraldo, and Annie Hall (RIP Diane Keaton)I did stand up for the first time in 3 months, and I went to see Michael Knowles speak at the Nixon Presidential Library, but I forgot most of what he said because I was having a full blown panic attack for most of it. (What fun it is to be me)

We're joined by Nancy Barile, award-winning author of "I'm Not Holding Your Coat", an insightful chronicle of her life as an OG Philadelphia/Boston punk who lived through the golden age of hardcore music as it evolved in real time before her eyes. The Ramones, Bad Brains, Misfits, Minor Threat, Black Flag... You name it, she saw it. We talk about the legendary brawl in Kensington, booking the infamous "Gathering of the Tribes" show, cold calling CBGBs to get bands booked, and bumping elbows with just about every legendary figure in hardcore punk, and much more. Her husband, the late great Al Barile of SSD, was a pioneer of the straight edge movement and a name brought up often on HardLore throughout the years. He sadly passed away in April of 2025, and we talk all about their chance meeting, his impact on us, and share stories of Al through the years and look back at his perception of how straight edge and hardcore music have grown since his early days in SSD.
