This week we delve into the world of the single ventricle when we speak with Dr. David Hoganson, Assistant Professor of Surgery at Harvard Medical School about a recent work he co-published with investigators from Boston Children's Hospital on computational fluid dynamic modeling in the planning of the Fontan operation. Single ventricle heterotaxy patients with interrupted IVC are at enhanced risk for the development of pulmonary AVM's due to flow maldistribution to the pulmonary arteries from the hepatic veins in a Fontan. Can a computational fluid dynamic model predict which operative approach would result in the most balanced hepatic venous flow distribution? Can this reduce the incidence of pulmonary AVM's? How well did the models predict the actual pulmonary flow measured after surgery on CMR? Who beyond the heterotaxy patient might benefit from this approach? Dr. Hoganson offers us a peek into the world of personalized surgery in this week's exciting episode. DOI: 10.1016/j.jacadv.2024.101057

Andrew "Hoges" Hoganson fills in for a traveling David to discuss the first-place Minnesota Twins' disappointing split of a home series with the Cleveland Guardians, two games apiece. The series featured a marvelous comeback and shutout followed by two offensive duds, leaving both Hoges and Dan to ponder just how opposite this series felt from the games between these teams last year. They discuss how Rocco Baldelli's hands were tied due to injuries to Byron Buxton and Carlos Correa, and they also look at how the reliability of bullpen arms has shifted just over the last couple of weeks. Dan asks again when the Twins will simply move on from right fielder Max Kepler, while Hoges wonders who exactly could be counted among the team's "A" lineup right now. They both heap praise on the heroics of Royce Lewis, who seems to be OK after a scary Game 4 injury, and they look ahead to the next series -- a key three-game set at Tampa Bay. The Minnesota Twins only managed to take 1 of 3 from the Angels in LA and the boys lament what's turning out to be a familiar script for the Twins this season. Andrew "Hoges" Hoganson is in studio filling in as Dan is traveling around California. Hoges is wondering if it's time give the 7th inning to someone other than Griffin Jax, while David wants to know if the Twins are bummed that they put Royce Lewis on the 60-day IL as he's been mashing at AAA, but is unable to join the MLB club until June. The boys are in agreement on the series grades for a change, and Hoges has some further thoughts about Dan's new series grading rubric. The guys talk about Trevor Plouffe being back with the Twins in a broadcast role, and the DFA'ing of Aaron Hicks from the Yankees. Dan continues his reign of terror in Puckett's Picks, but David and Hoges have made a pact to stop the madness. Thanks for listening, and as always, go Twins! The Minnesota Twins seasoning opening win streak comes to an end at 4 games, losing two of three to the Marlins. The starting pitching performances from Kenta Maeda and Pablo Lopez were all for not as the Marlin's reigning Cy Young winner Sandy Alcantara shut down the Twins offense in game two, and Luzardo continued the dominance in game three. Dan is still traveling so David is joined in studio once again by Andrew, "Hoges" Hoganson. The boys try and keep their spirits and optimism high after two lackluster offensive performances. Hoges offers pennance to Joey Gallo for not picking him for Puckett's Picks, David is upset that Rocco didn't have Buxton pinch hit at the end of game two, and Hoges wants to know how often we'll see Pagan in medium to high leverage situations. The guys discuss how much playing time Ryan Jeffers will be given if he keeps hitting well, as well as who is next up from AAA if Kepler lands on the IL. The boys finish by grading the series and giving their Puckett's Picks for the upcoming Astros series. Thanks for listening, and as always, go Twins! The 2023 Minnesota Twins season has officially begun! The Twins sweep the season opening three game series against the Kansas City Royals. Andrew "Hoges" Hoganson joins David in studio filling in for Dan as he travels the world. The boys are riding high and try their best to keep their optimism in check. Hoges gloats over his Joey Gallo Puckett's Picks selection and David continues to be amazed at the abilities of Byron Buxton. The guys discuss Rocco's nearly flawless management in the first series and the stellar pitching performances given by the starting rotation and back-end bullpen. Hoges is having trouble adjusting to the pitch clock and also takes issue with David's grade for the series. The guys wrap up giving their Puckett's Picks for the upcoming Marlines series. Minnesota Twins baseball is back! David and Dan are joined in studio by Andrew "Hoges" Hoganson as the boys offer up their season predictions for 2023. The guys discuss their attitudes towards the World Baseball Classic, as well as the Twins rebranding efforts this offseason. They talk through the finalized roster to start the season including Polanco and Kirilloff starting on the IL. Carlos Correa's new house purchase is reviewed which leads to the boys taking a few shots at A-Rod's shoveling abilities. The season schedule is broken down with David contending the first half of the season will be much more difficult than the second half. The boys wrap up the show with their season predictions for the 2023 season and make their first Puckett's Picks of the season for the opening series against the Kansas City Royals. The Minnesota Twins season has officially come to a close and David and Dan welcome Andrew "Hoges" Hoganson in studio for their annual season wrap show. The boys revisit their preseason predictions and take solace in the fact that they weren't the only ones in the sports world to be a little off with their guesses. The guys discuss the Puckett's Picks results from the season and then work through their segments with the whole season in mind. Carlos Correa receives high praise and all three of the guys hope to see Correa return next season. David and Hoges pull no punches regarding their feelings towards Rocco's management of the pitching staff and Dan wonders if the Twins trainers made a mistake in how they handled Buxton early in the season. The hosts offer up their grades for the season and give their predictions for the MVP for the Twins next season. Thanks so much for listening, and as always, go Twins! The Minnesota Twins are swept at home by the visiting Cleveland Guardians. Andrew "Hoges" Hoganson fills in for Dan and the boys do everything in their power to avoid talking about this series. The boys breakdown the areas where the Twins struggled in the series, as well as some odd happenings involving an ump giving an aggressive head massage to a Cleveland pitcher. Hoges keeps the listeners in the Puckett's Picks race coming away with the victory after taking Correa. David wants to ensure that Varland gets another start with the Twins this coming series, and Hoges is tired of keeping track of all the injured Twins players. David wants to know if Twins fans' mindset would be more optimistic if the Twins had been trailing in the AL Central all season, and Hoges wants to know which sport's championship David would take if he could choose one of his favorite teams to win it. The boys breakdown the 2023 MLB rules changes approved by the MLB, and Hoges finishes the show poking some fun at Dan Thompson while Dan is away. Dan is away and so Andrew "Hoges" Hoganson fills in with David as the boys discuss the Twins three game series against the LA Angels. The boys discuss if it's time to put Kepler back on the IL after extending his hitless streak to 28 at-bats. Hoges wants to know why Rocco insists on using Pagan so much and David wants to know why Barcher's starts aren't being supplemented with Cole Sands for additional length out of the pen. David defends the merits of the "MN for the Win Unofficial Win-Loss Stat," but Hoges still isn't on board. The boys discuss Tatis Jr.'s suspension and how steroid use should be viewed when it comes to statistical records. The guys wrap up with a heartwarming story of a minor leaguer finally getting the callup to the majors after 10 long seasons in the minors. The boys finish up making their picks for the upcoming Royals series. Thanks for listening, and as always, go Twins! Music: "Minnesota Twins Theme" (1961) written by Ray Charles and Dick Wilson. Dan is away and so Andrew "Hoges" Hoganson fills in with David as the boys discuss the Twins three game series against the San Diego Padres. Hoges is upset that Rocco didn't let Sonny Gray go deeper in his start and David is upset that Rocco waited one batter too long to pull Dylan Bundy from his. The guys both praise Buxton who looks to be at 100% and carried the team offensively in this series. David wants to know if we'll ever see Sano wear a Twins jersey again, and Hoges is dying to know if the Twins will make any moves at the deadline, knowing that any moves will not be cheap. Hoges takes his shots at David and Dan for not being optimistic enough about the Twins chances to make a run in the post season this year, and he's lost all respect for David due to David's lack of enjoyment of musicals. While the $52 billion of federal funding in the https://www.congress.gov/bill/116th-congress/house-bill/7178 (CHIPS for America Act) has strong bipartisan support, it's caught in a larger fight between the two legislative chambers over an attempt to reconcile the bill with other Democrat spending priorities. Senate Majority Leader Mitch McConnell (R-KY) has built a coalition against the larger bill that will stop it from passing unless the process is focused exclusively on semiconductors. Commerce Secretary Gina Raimondo has also agreed that the CHIPS Act should be passed on its own immediately. The Minnesota Twins lose 2 of 3 against the visiting Cleveland Guardians and now sit tied atop the AL Central with the Guardians. Andrew "Hoges" Hoganson fills in as Dan makes his way to Minnesota. The boys discuss the Twins inability to put away games when they are up in the late innings, and the roller coaster of emotions the Twins took their fans on throughout the series. The guys both express their frustration at Dick Bremer's non-home run home run calls, and David puts pitchers in his beast and bench spots in honor of Dan Thompson. Hoges wants to know if the Twins should act early before the trade deadline to bring in some relief help, and David is furious that Rocco didn't ask Buxton to bunt in extra innings in game one. David asks Hoges how much he believes in this team, and Hoges pulls no punches calling out David and Dan's downplaying of the Twins success thus far. Opening Day is only a day or two away depending on the weather, and David and Dan are joined by Andrew "Hoges" Hoganson to give their predictions for the Minnesota Twins 2022 season. Hoges thinks there are too many "ifs" to have any optimism for the Twins starting rotation, while Dan has nothing but optimism for Miguel Sano. Dan's love for Byron Buxton is still as strong as his hate for Max Kepler and David proves himself to be the homer of the group with his season record prediction. Hoges believes the defensive upgrades will play a role in improving the pitching staff's ERA, and both he and Dan can't wait to gush about Joe Mauer while David is traveling later this season. Dan is traveling back home so once again Andrew "Hoges" Hoganson joins David in studio. The boys commiserate the lackluster performance the Twins put on display at Target field and Hoges finally admits the season is lost. Both David and Andrew are disappointed in the performance of Simmons on the season, and David feels vindicated in his prior defense of Max Kepler. When Kristin L. Hoganson arrived in Champaign, Illinois, after teaching at Harvard, studying at Yale, and living in the D.C. metro area with various stints overseas, she expected to find her new home, well, isolated. Even provincial. After all, she had landed in the American heartland, a place where the nation's identity exists in its pristine form. Or so we have been taught to believe. Struck by the gap between reputation and reality, she determined to get to the bottom of history and myth. The deeper she dug into the making of the modern heartland, the wider her story became as she realized that she'd uncovered an unheralded crossroads of people, commerce, and ideas. But the really interesting thing, Hoganson found, was that over the course of American history, even as the region's connections with the rest of the planet became increasingly dense and intricate, the idea of the rural Midwest as a steadfast heartland became a stronger and more stubbornly immovable myth. I’m speaking to Hoganson today, who is author of the book The Heartland: An American History. She tracks both the backstory of this region and the evolution of the idea of an unalloyed heart at the center of the land.

The term "the Heartland" is often used by politicians trying to connect with people, and is also used to define a national identity, often in a way that excludes some people within the country. The heartland has become a term of mythology that defines a place and a people that inhabits it, and in the US, it evokes ruralness, or small town values, agriculturally or Main Street oriented, overwhelmingly white, and suspicious of the outside world. University of Illinois historian Kristin Hoganson decided to investigate this idea of 'heartland' through a deep examination of her home, the college town of Champaign-Urbana in Illinois. By taking this micro-view of a community, she digs down through layers of community history and found story after story that belies this mythology surrounding the heartland, even as that myth becomes more enshrined as belief. Hoganson's book, "The Heartland: An American History" was an acclaimed book last year and is now out in soft-cover.

For children with a history of penicillin allergy, new evidence suggests a vast majority of kids could be “de-labeled” after undergoing a risk assessment and oral challenge in the pediatric emergency room. George Hoganson, MD, a Washington University emergency medicine fellow at St. Louis Children’s Hospital, is collaborating on a two-year, multi-center study to test children described as penicillin allergic, but who very likely do not have a true penicillin allergy in the St. Louis Children's Hospital Emergency Department. Dr. Hoganson joins the show to discuss the study, which involves a penicillin oral challenge with observation in the Children’s emergency room.
The Great West. Middle America. Flyover Country. The expanse of plains, lakes, forests, and farms, between the Appalachian and Rocky Mountains has carried many names. Beginning in the twentieth century, Americans began calling it The Heartland, a term that Dr. Kristin L. Hoganson argues carried a specific meaning that has changed across time. In The Heartland: An American History (Penguin, 2019), Hoganson tracks the global history of Champaign, Illinois – a small place with a large history, and, as a professor of history at the University of Illinois, Hoganson’s home for nearly two decades. The Heartland makes a strong case for the Midwest not as a provincial, isolated, region but rather as a place defined by global connections, diasporas, and a wide array of cultures. The book covers a lot of ground, from Kickapoo history to the story of high-bred cattle to a foray into the history of long-distance ballooning. Throughout, Hoganson maintains that just as scholars study the West and the South, the Heartland is deserving of its own status as part of the American regional canon, not because it looks inward, but because of its long history of affecting historical change and being affected by global events. Stephen Hausmann is a doctoral candidate at Temple University and Visiting Instructor of history at the University of Pittsburgh. He is currently writing his dissertation, a history of race and the environment in the Black Hills and surrounding northern plains region of South Dakota, Wyoming, and Montana.
