I'd been wanting to do this podcast for a while. Professor Alessandra Lemma is one of the most experienced, and insightful, specialists I work with. Every word she shares is a gem. But Pr Lemma's diary is packed and so this took a while to organise. I hope you think it was worth it.The idea for this episode - the first in Series 9 - came to me last year but in reality, it had probably been bubbling away for some years. Alessandra and I were working on a problem for my Guardian column. The problem was from a reader who said that, despite them being an adult, they were still being body shamed by their parents for being overweight, this is sadly by no means an unusual problem to arrive on my desk. In our chat Alessandra said a line which stayed with me all of last year. It was this: you can't delete developmental history it is always there even if you change the surface of the body. From this we went off topic a little, as my chats with my specialists sometimes, to talk about how - with some caveats - people who modified their bodies through things like cosmetic or plastic surgery, often didn't feel happier because whatever developmental history that had led them to be unhappy with themselves, lay within then and beyond the reach of any sort of body modification. It got me thinking about how our body image is arrived at. Do we make it, or do others shape it? Why are so many people unhappy with they way they look? In this episode we look at how our body image is formed, who shapes it and how we can try to change our internal dialogue about it. Children are waiting up to three and a half years to see a pediatrician in WA's public health system.  Perth mum, Louise Thurlow, told Gary Adshead on Mornings she's been waiting 18 months for an appointment after her son was waitlisted for an autism assessment. This time I welcome Simona Stokes, Counselling Psychologist and the Founder of Menopause CBT Clinic, an independent psychology practice which aims to support the psychological wellbeing of women going through the perimenopause and menopause. Based on her extensive clinical experience in this area, Simona has developed EMBERS® Menopause CBT Model which is centered around the psychological principles supported by the best evidence-based research and interventions in the field of mental and emotional wellbeing. She holds professional registration and accreditation with several bodies including the BPS, BABCP, and BACP and has been working in the mental health field for over 20 years.  We begin with an explanation of what CBT is and how it works for anyone who is not familiar with Cognitive Behavioural Therapy.  Simona explains little more on the different types of CBT, and her experience is that ‘third wave' CBT approaches seem to work best with those experiencing perimenopause symptoms. The draft NICE guidelines issues at the end of 2023 caused quite a media stir with the increased focus on CBT as a treatment for menopause. We discuss why the headlines were misleading and how CBT can help with hot flushes and sleep disturbances, as well as psychological and emotional symptoms. As Simona outlines, even individuals who are taking HRT sometimes struggle with their mental health and access to CBT can be helpful. Medication alone cannot address struggles with confidence, self esteem, self-worth, and identity. We delve deeper into menopause as a psychological developmental stage - just as pregnancy and puberty (see Lisa Mosconi's book The Menopause Brain for more on this). In all these life stages our brains are remodelling themselves - we are updating who we are and our perspective on the world…a process some of us will navigate on our own or with support of our own networks of support, and some of us may need a little extra help and support from professional - space where we can safely explore these topics. Simona explains that the way we think about physical symptoms can amplify the symptoms themselves…CBT helps us look at the kinds of stories we tell ourselves and write new ones. It can support us to make meaningful change in areas that help us regulate our bodies and thereby mitigate symptom severity.  Unfortunately as Simona highlights, the types of CBT available on the NHS are not always tailored to the needs of women going through the menopause and that can lead to sub-optimal experiences when people do choose to pursue CBT as an option. In this episode I'm joined by Dr James Munnik and Prof Leon Van Niekerk. James is a Counselling Psychology Lecturer at the University of KwaZulu-Natal in South Africa. James has worked with a number of athletes in his own private practice as a high performance mental coach in psychotherapy and counselling. He has special interests in sport/exercise psychology research and Rational Emotive Behavioural Therapy (REBT) theory and practice. Leon is a Professor of Psychology at University of Fort Hare in South Africa. He is a registered Counselling Psychologist and has his own psychology consultancy practice where he focuses on the provision of sport psychological services to individual athletes and teams. Leon's research interests are in the psychological well-being of athletes. This includes topics such as the development of mental toughness, sport identity and life roles, burnout, stress and anxiety among athletes, the effectiveness of mindfulness during competition, mental health in sport and the psychological health benefits of exercise. In this episode we discuss a paper entitled “Recommendations for Integrating Psychological Skills Training (PST) into Rational Emotive Behavioural Therapy (REBT)” which was led by James and co-written by Dr Martin Turner and Leon.
