First do no harm: Universal Mental Health Interventions in Schools

Summary In this episode Dr. Carolina Guzman Holst is a NIHR Research Fellow at the University of Oxford, discusses her recent paper titled Scoping review: potential harm from school-based group mental health interventions.  Where the authors ‘examine three areas: the types of potential harms and adverse events reported in school-based mental health interventions; the subgroups of children and adolescents at heightened risk; and the proposed explanations for these potential harms.'The conversation explores the rising mental health issues among young people in schools, focusing on universal interventions like mindfulness and CBT. Carolina Guzman discusses her scoping review on the potential negative effects of these interventions, emphasising the importance of understanding their impact on vulnerable groups. The discussion highlights the need for careful implementation and monitoring of mental health programs in educational settings, advocating a tailored approach rather than a one-size-fits-all solution. Links: Guzman-Holst C, Streckfuss Davis R, Andrews JL, Foulkes L. Scoping review: potential harm from school-based group mental health interventions. Child Adolesc Ment Health. 2025 Mar 18. doi: 10.1111/camh.12760. Epub ahead of print. Universal interventions aim to help all students, regardless of need. Mindfulness and CBT are common school interventions. Negative effects of interventions can occur, especially in vulnerable groups. High quality implementation quality is crucial for intervention success. Long-term benefits of interventions are often not well-studied. Schools should monitor the effects of mental health programs. A hybrid approach may be more effective than universal interventions alone. Teachers should consult evidence before implementing programs. Tailored support is essential for students with specific needs. A parent whose child is suffering from anxiety can feel utterly helpless. They are desperate to help them but simply don't know what to do. For a parent whose child is on the CAMHS waiting list, the waiting time can feel interminable or even catastrophic. But there is something a parent can do to help their child - and themselves. ISPCC provides three separate but complimentary Digital Mental Health and Wellbeing programmes, two of which are available to parents/carers of either anxious children or teens while the third is aimed at teenagers experiencing low to moderate levels of anxiety. ISPCC offers these programmes to all, it isn't necessary to be on a CAMHS waiting list to avail of them. The online programmes, which are all free, are based on the principles of Cognitive Behavioural Therapy, or CBT, and are created by SilverCloud, a leading digital mental health provider. For Susan Nicholson, who completed the Supporting an Anxious Child programme after her GP recommended that her daughter Lauren attend CAMHS, the ISPCC programmes were invaluable. "I wanted to do something that would help our family to support Lauren. The programme encouraged me to look at the positives rather than focusing on the effects of her anxiety. Going through the programme, you could see what other parents were going through, and you're able to relate to their challenges. To know that other people are going through is very helpful. As a parent, you're supposed to know all the answers, and it's scary. "Starting the programme was such an easy process. I liked the fact that it was anonymous, nobody knew me, and I didn't have to go to a meeting in person. It was such a personal experience - in a good way. I didn't feel like I was just another number. ? Supporting an Anxious Child and Supporting an Anxious Teen are programmes that will help users to understand anxiety and better support their child or young person. Space from Anxiety is aimed at 15-18-year-olds, and it is designed to empower young people who experience low to moderate anxiety. An adult can refer themselves or their child to the programmes. The programmes includes interactive tools, activities, mood monitors and journals to encourage users to apply CBT to their own lives. Users have 12 weeks in which to complete the programme under the guidance of a volunteer, known as a supporter, and can avail of an unsupported version of the programme for one year after this time. "Chloe [my supporter] was very empathetic," says Susan. "When you're in this situation, it's great when you know someone else cares. I felt as if she cared about Lauren." Dee Higgins, an ISPCC volunteer who works on its Digital Mental Health and Wellbeing programmes, believes that the programmes are empowering for users of all ages. "It gives parents and young people a chance to deal with their anxieties before it becomes a huge issue," she said, adding that "if parents can understand what their child is going through, that's a huge step." In this episode of Alimentary! I am delighted to welcome Penny Sharkey (and yes we are related!) to discuss how Cognitive Behavioural Therapy (CBT) can be used either by itself or with other therapeutic tools in therapy to support mental health and wellbeing. Penny is a professional counsellor and psychotherapist, and a member of the Irish Association for Counselling and Psychotherapy (IACP). She holds a First-Class Honours Degree in Integrative Counselling and Psychotherapy, a Degree in Social Studies, and a master's Certificate in Cognitive Behavioural Therapy (CBT). Currently, she works in a private practice in Celbridge and Maynooth, in Co. Kildare. From an early age, Penny developed a curiosity in how the power of human connection and the resilience of the human spirit worked. This passion was sparked by witnessing a friend struggle with severe anxiety and depression and seeing how effective therapy was in transforming their life. That experience motivated her to help others navigate their own challenges and find healing. In her practice, she strives to create a warm, accepting, and honest environment where clients feel safe to explore their feelings. Her therapeutic approach is flexible and client-centered, emphasizing empathy and tailored interventions to meet each person's unique needs. She believes in the power of active listening, congruence, and guidance to help clients discover meaning, build resilience, and achieve personal growth. She primarily works with Person-Centred Therapy, Choice Theory/Reality Therapy, and Cognitive Behavioural Therapy (CBT) to support clients on their journey towards well-being. She said that it has been so fulfilling to work in supporting those who are seeking support, helping them find their own strength and clarity in times of distress. Celissa and Melissa noticed the clear need for mental health services in Ontario over the pandemic, and decided to team up to put their expertise and skills to use to serve the community.  As a team, Celissa and Melissa were able to balance starting a practice between the two of them while continuing their social work positions that they love. Their Canadian private practice has now grown into a successful group private practice where they have the benefits of running their own practice while still working jobs that they cherish.  In this podcast episode, I chat with the two of them about their progress, their ups and downs, and what they recommend to you for your Canadian private practice career! MEET CELISSSA AND MELISSA  Celissa and Melissa are the owners of Virtual CBT, a private psychotherapy practice specializing in Cognitive Behavioural Therapy for clients in Ontario that are struggling with symptoms of depression, anxiety, phobias, OCD, and trauma. Between the two of them, they had dabbled in different sectors trying to find what suited them both best before specializing further.  Melissa thoroughly enjoyed working within CBT and she found it effective and helpful for both her services as a therapist and for her client's needs.  For Celissa, the path was a little different in that she only knew that she wanted to become a social worker after she graduated.  Celissa's expertise has become more narrow as the years have passed, from CBT and DBT to specialized CBT and others. Working as a team  Over the pandemic, both Celissa and Melissa were working for a not-for-profit organization when the public mental health system was being flooded with referrals.  Also, over the pandemic they went from working completely in person to completely virtual almost overnight.  One day, a doctor connected with Celissa asking if she had a private practice, and she ended up texting Melissa to see what she thought about it. They decided to work together to launch their own private practice since they could see that the need was there.   Using Jane App  Melissa recommends to all practice owners that Jane App is a great investment to commit to early on in your private practice journey since it streamlines so many processes in a way that they complement one another, helping you to stay organized and helping your clients to easily book appointments.  Jane's intuitive and automatic systems that you can customize to your needs allows it to help you with what you want help with.  The future of Virtual CBT Since September 2023 their practice has grown so much, which is why they have decided to take a step back and rest a bit before continuing to expand their practice together.  Diversifying your practice is also a good idea to consider because it can take away some of the stress and allows for the owners and staff to engage with clients and modalities in new and innovative ways.  Melissa and Celissa's advice to listeners Be willing to invest in your business, whether it is finances or time and effort, if you want to have a strong foundation and a successful practice. This time I welcome Simona Stokes, Counselling Psychologist and the Founder of Menopause CBT Clinic, an independent psychology practice which aims to support the psychological wellbeing of women going through the perimenopause and menopause. Based on her extensive clinical experience in this area, Simona has developed EMBERS® Menopause CBT Model which is centered around the psychological principles supported by the best evidence-based research and interventions in the field of mental and emotional wellbeing. She holds professional registration and accreditation with several bodies including the BPS, BABCP, and BACP and has been working in the mental health field for over 20 years.  We begin with an explanation of what CBT is and how it works for anyone who is not familiar with Cognitive Behavioural Therapy.  Simona explains little more on the different types of CBT, and her experience is that ‘third wave' CBT approaches seem to work best with those experiencing perimenopause symptoms. The draft NICE guidelines issues at the end of 2023 caused quite a media stir with the increased focus on CBT as a treatment for menopause. We discuss why the headlines were misleading and how CBT can help with hot flushes and sleep disturbances, as well as psychological and emotional symptoms. As Simona outlines, even individuals who are taking HRT sometimes struggle with their mental health and access to CBT can be helpful. Medication alone cannot address struggles with confidence, self esteem, self-worth, and identity. We delve deeper into menopause as a psychological developmental stage - just as pregnancy and puberty (see Lisa Mosconi's book The Menopause Brain for more on this). In all these life stages our brains are remodelling themselves - we are updating who we are and our perspective on the world…a process some of us will navigate on our own or with support of our own networks of support, and some of us may need a little extra help and support from professional - space where we can safely explore these topics. Simona explains that the way we think about physical symptoms can amplify the symptoms themselves…CBT helps us look at the kinds of stories we tell ourselves and write new ones. It can support us to make meaningful change in areas that help us regulate our bodies and thereby mitigate symptom severity.  Unfortunately as Simona highlights, the types of CBT available on the NHS are not always tailored to the needs of women going through the menopause and that can lead to sub-optimal experiences when people do choose to pursue CBT as an option. We end by agreeing that there is certainly scope for more practitioners to be trained in menopause-informed CBT practices.  You can find Simona at https://www.menopausecbtclinic.co.uk/ and https://www.instagram.com/menopausecbtclinic  If you enjoy the podcast and would like to help me keep it running (on a shoestring!) please consider buying me a ‘virtual coffee' at Ko-fi.com/middlingalong - or you can support me in a non-monetary way by sharing this episode, or writing a short review online!  If your workplace wants to become more ‘menopause friendly' then please let them know about the work I do at http://www.managingthemenopause.com  You can also find me over on Instagram https://www.instagram.com/middlingalong_podcast/ and https://www.instagram.com/managingthemenopause   Join our newsletter, The Messy Middle, for fortnightly(ish) goodness into your Inbox: https://dashboard.mailerlite.com/forms/323784/90772270045202190/share  We're delighted to be listed as one of the Top 25 podcasts for midlife and menopause here: https://www.lattelounge.co.uk/podcasts-about-the-menopause/

In this week's episode Chartered Clinical Psychologist, Dr. Lydia Stone, joins Alexa Terry to discuss the topic of trauma. Lydia specialises in working with fostered and adopted children and families, she also runs a choir and is currently on the BAST Training Level 5 qualification course. Lydia explains the potential impacts of trauma on the singer, and helps us understand how we can conduct more trauma-informed singing lessons.  KEY TAKEAWAYS When conducting trauma-informed singing lessons, it's essential to approach each session with empathy and sensitivity towards the experiences of the singers. While it's important to acknowledge the potential presence of trauma, it's equally crucial not to assume or pry beyond professional boundaries. Using recording and analysis tools during lessons can provide valuable feedback without overwhelming the singer or detracting from the session's focus. Concentrating on physical cues, such as mouth movements, can enhance communication and instruction. Familiar exercises like scales aid vocal technique. Gradually introduce minor scales for challenge. Connecting major and minor patterns enhances musical understanding. Encourage musical literacy and instrumental skills for independence. Positive reinforcement fosters confidence and growth while avoiding criticism preserves the singer's emotional well-being. Patience is key, allowing singers to progress at their own pace and respecting their boundaries. It's crucial to have resources available if singers require further support outside of the lesson. This could include referrals to counselling or other mental health services. By offering a holistic approach to singing education, teachers can empower their students not only as musicians but also as individuals capable of overcoming challenges and reaching their full potential.   BEST MOMENTS "Trauma responses impact muscle tension and vocal control" "Accept and empathise but maintain professional boundaries" "Self-care helps teachers support singers' mental health" EPISODE RESOURCES   The Body Keeps the Score by Bessel van der Kolk Singing Teachers Talk Podcast Ep.147 TV Vocal Coach: Behind the Scenes on ITV's Mamma Mia with Claire Delaney ]Dr Shannon Coates: Dr Shannon Coates Singing Teachers Talk: Eps. 155 & 156 - Building Neurodiversity-Inclusive Voice Studios with Dr Shannon Coates PACE: Playfulness, Acceptance, Curiosity, Empathy Vocal Health Education: Vocal Health Education The work of Petra Raspel Borzynski NSPCC: nspcc.org.uk UK Trauma Council: UK Trauma Council Beacon House: Resources Attach Team (Oxfordshire): The ATTACH team | Oxfordshire County Council   Contact Dr Lydia Stone directly: drlydiastone@gmail.com   BAST Book A Call   ABOUT THE GUEST   Dr Lydia Stone is a Chartered Clinical Psychologist, who started her career with a choral scholarship at Cambridge University before completing her doctorate at Surrey University. He is well-known here in the UK as a passionate advocate for PANS/PANDAS, and has played a large part in many people's recovery. He has one of the largest cohorts of PANS/PANDAS patients and in this episode he talks about how he believes Strep is the trigger - even in PANS, a course of action for treatment, and the growth of his clinic to include Cognitive Behavioural Therapy for OCD and EMG for treating tics. Responding to therapeutic needs of refugees: Many refugees from war areas flee to Europe. Due to traumatic events they experience in their home countries and on perilous journeys, and finally due to adverse conditions in exile, a large proportion of refugees present with posttraumatic stress disorder and depression. However, the current theories and treatments of trauma-related disorders are not designed to respond to refugees' presentations. Moreover, the current way the treatment is provided is not well suited to meet the needs of refugees. Experiences of refugees from war-torn countries help to challenge some core assumptions and prompt us to adapt the current thinking about psychopathology, therapy, and treatment delivery. The lecture is followed by a discussion hosted by Agnieszka Popiel, M.D., Ph.D. / Associate Professor, co-founder and co-head of SWPS University's Clinic of Cognitive-Behavioural Therapy. We focus on the following issue: If evidence-based practice (EBP) is defined as the integration of the best available research with clinical expertise in the context of patient characteristics, culture, and preferences (according to the American Psychological Association) - delivering treatment to refugees presents a challenge for a (quite idealistic) equilibrium of EBP. People who have suffered so much deserve the best available treatment. Our discussion focuses on questions that are emerging when clinicians equipped with evidence-based treatments face people fleeing wars.

1. Take time out It's impossible to think clearly when you're flooded with fear or anxiety. The first thing to do is take time out so you can physically calm down. Distract yourself from the worry for 15 minutes by walking around the block, making a cup of tea or having a bath. 2. Breathe through panic If you start to get a faster heartbeat or sweating palms, the best thing is not to fight it. Stay where you are and simply feel the panic without trying to distract yourself. Place the palm of your hand on your stomach and breathe slowly and deeply. The goal is to help the mind get used to coping with panic, which takes the fear of fear away. Try this breathing technique for stress 3. Face your fears Avoiding fears only makes them scarier. Whatever your fear, if you face it, it should start to fade. If you panic one day getting into a lift, for example, it's best to get back into a lift the next day. 4. Imagine the worst Try imagining the worst thing that can happen – perhaps it's panicking and having a heart attack. Then try to think yourself into having a heart attack. It's just not possible. The fear will run away the more you chase it. 5. Look at the evidence It sometimes helps to challenge fearful thoughts. For example, if you're scared of getting trapped in a lift and suffocating, ask yourself if you have ever heard of this happening to someone. Ask yourself what you would say to a friend who had a similar fear. 6. Don't try to be perfect Life is full of stresses, yet many of us feel that our lives must be perfect. Bad days and setbacks will always happen, and it's important to remember that life is messy. 7. Visualise a happy place Take a moment to close your eyes and imagine a place of safety and calm. It could be a picture of you walking on a beautiful beach, or snuggled up in bed with the cat next to you, or a happy memory from childhood. Let the positive feelings soothe you until you feel more relaxed. 8. Talk about it Sharing fears takes away a lot of their scariness. If you can't talk to a partner, friend or family member, call a helpline such as Breathing Space on 0800 83 85 87 or Samaritans on 116 123. You could also try a Cognitive Behavioural Therapy approach over the telephone, with a service such as NHS Living Life. If you would like to find out more about this appointment-based service, you can visit the Living Life website or phone 0800 328 9655 (Mon to Fri, 1pm to 9pm). If your fears aren't going away, you can ask your GP for help. GPs can refer people for counselling, psychotherapy or help through an online mental health service, such as Living Life to the Full. 9. Go back to basics Lots of people turn to alcohol or drugs to self-treat anxiety, but this will only make matters worse. Simple, everyday things like a good night's sleep, a wholesome meal and a walk are often the best cures for anxiety. 10. Reward yourself Finally, give yourself a treat. When you've made that call you've been dreading, for example, reinforce your success by treating yourself to a massage, a country walk, a meal out, a book, a DVD, or whatever little gift makes you happy. --- Send in a voice message: https://podcasters.spotify.com/pod/show/creative-habits/message
