Este es un debate fundamental en el ámbito de la nutrición, la medicina y la salud pública. Vamos por partes:1. ¿Obesidad o Enfermedad Crónica Basada en la Adiposidad (ABCD)? La AACE propone el término ABCD (Adiposity-Based Chronic Disease) para enfatizar que la obesidad es más que una acumulación de grasa: se trata de una condición crónica con múltiples factores subyacentes, incluyendo genética, metabolismo, entorno y comportamiento. Este cambio terminológico busca evitar la reducción de la obesidad a una simple cuestión de peso y destacar su impacto en la salud. 2. ¿Es la obesidad una enfermedad o un factor de riesgo? Impacto de la clasificación de la obesidadPacientes: Considerarla una enfermedad puede reducir la culpa y el estigma, promoviendo un enfoque más médico y menos moralista. Sin embargo, puede también reforzar la idea de que la única solución es médica o farmacológica.Políticas públicas: Puede favorecer la financiación de tratamientos, programas de prevención y acceso a profesionales de salud.Investigación: Mayor inversión en estudios sobre su fisiopatología, tratamientos y prevención.Percepción pública: Puede reducir el estigma de la obesidad como un "fallo personal", pero también puede reforzar la dependencia de tratamientos médicos en lugar de abordar causas estructurales (alimentación, sedentarismo, desigualdades sociales).4. ¿Comparte el enfoque tradicional factores de riesgo con los TCA? Sí. La visión pesocentrista y la cultura de la dieta pueden fomentar conductas alimentarias desordenadas y contribuir al desarrollo de trastornos de la conducta alimentaria (TCA).Se ha observado que adolescentes con sobrepeso u obesidad tienen mayor riesgo de desarrollar TCA debido a la presión social y médica por perder peso.Reforzar la idea de que la obesidad es una "enfermedad a erradicar" puede llevar a restricciones extremas y a ciclos de pérdida y ganancia de peso (efecto rebote).Enfoques no pesocentristas, como el HAES (Health At Every Size), buscan evitar este problema.5. Car oui on blâme beaucoup les personnes grosses à propos de leur santé et leur soit disant manque de soin pour être en bonne santé, soit disant laisser aller... mais qui a vraiment conscience de la trouille magistrale que représente un rendez-vous médical quand on est gros.se dans une société grossophobe où la grossophobie médicale est quasi la norme ?! L'idée ici, encore et toujours, n'est pas spécialement de pointer du doigt, mais de faire prendre conscience de la réalité ! et de dédramatiser et d'encourager les personnes grosses à ne pas laisser gagner la grossophobie au détriment de leurs santé ! Oui on a besoin de consulter, oui on a besoin que les soignant.e.s progressent, oui on a besoin de déculpabiliser, si ces rendez-vous sont si compliqués, c'est pas à cause de nous ! Notre corps n'est pas le problème ! Le système qui refuse de nous accueillir et de nous soigner comme tout le monde, c'est ça le problème ! You might be wondering what do we do in terminal cases of Anorexia. Terminal Anorexia is different than Severe and Enduring AN- where sufferers do often make a recovery after 20 years (research shows). Terminal AN is diagnosed when the sufferer will die from malnutrition within 6 months. This is an extremely small portion of AN sufferers that often falls through the cracks of dignified, whole person care. Dr. Gaudiani and I talk about how to cope and treat these cases in a way that centers autonomy, dignity and love for one's end of life. Jennifer L. Tasheon is a plus-size Personal Trainer and Coach located in Tacoma, Washington. The lack of diversity in the fitness industry pushed her to pursue movement as a career instead of a hobby. She believes that movement does not have to be about changing a number on the scale or measurements and instead practices a HAES (Health At Every Size) and size-inclusive approach. She loves creating programs for people who want to improve their strength and confidence with functional and intuitive movement. In 2019, she created BOPOMO, a Body Positive Movement class specifically for plus-size, fat-identified, people of color and queer folks of all abilities to find joy and empowerment through movement. Because of the COVID-19 pandemic, she began offering classes online. Tasheon is also a self-described Unlikely Hiker. She didn't really understand why people would climb mountains until 2017 when her brother died and hiking became a way to process her grief. I was delighted to spend some time talking with Tamara Greenberg about Intuitive Eating, a lifestyle choice that allows us to develop healthy relationships with our bodies, the movement we do, and the foods we eat. Tamara Greenberg is a certified professional life coach, intuitive eating counselor, speaker, author and corporate trainer. Her book, Hunger Satisfied – My Intuitive Eating Journey, shares her personal journey of dieting and beyond. In the book she explains why dieting does not work. Tamara started her intuitive eating journey in her 20's, but only really shifted into being an Intuitive Eater in her early 40's. Her book outlines a strategy to building a lifestyle based on healthy habits. The focus is to change the way people think about eating, exercise and their body image. Weight stigma is harmful to human health at all sizes. The medical community upholds weight stigma and harms the very people they should be helping by weighing patients without informed consent, blaming weight on non weight related concerns like earaches and wrist pain, and recommending weight loss based on the unscientific BMI measurement. Medicine must work from the inside out to eradicate the problems they have perpetuated.
