In Part 2 of our Resistant Hypertension videos, I'm looking at hyperaldosteronism, or high aldosterone levels, as a cause for treatment-resistant hypertension — and an underrecognized one at that. So what is high aldosterone? Where does it come from? When do you check for it? What causes it? If a patient has high aldosterone, what do you do next? Let's dig in!Note that assessing for this may be out of your practice comfort zone — and that's totally fine! But it's still important to know what hyperaldosteronism means for your patients. In this video, I touch on:✅ What distinguishes primary and secondary aldosteronism - and why it matters✅ Symptoms to keep an eye on when looking at the different ideologies for high aldosterone✅ And how to navigate that always-tricky dance of testing and referral as a nurse practitionerLooking for more guidance on CKD? Ultrasonic Nerve Destruction Treats Drug-Resistant Hypertension Vidcast: https://youtu.be/5fSwIDc3Tcs Zapping nerves entering the kidneys with pulsed ultrasound can significantly lower elevated blood pressures in those who fail traditional drug therapy with a triple combination medication. An international, multi-center study headed by Columbia University researchers enrolled 136 patients, and 69 of them underwent ultrasound renal denervation with the 67 controls undergoing sham procedures. The ultrasound treated group enjoyed a significant 5.8 mmHg blood pressure reduction compared with the controls. There were no significant complications from the procedure. Larger clinical trials will be needed to confirm the safely and effectiveness of ultrasound renal nerve ablation. If proven effective, ultrasound will be added to radiofrequency energy as a means of renal nerve ablation. https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(21)00788-1/fulltext #hypertension #ultrasound #renalnerve

We all know that hypertension is a significant risk factor for cardiovascular disease and stroke. I have two questions for you. Did you know that most of the decisions your doctor makes about your hypertension treatment are based on your 10-year cardiovascular risk score? What are your chances of having a heart attack or stroke within the next ten years? Dr. Tonya will tell you how to calculate your 10-year cardiovascular risk score, why it is essential to understand, and how to improve your score. According to extensive research that has been done since the 1960s, this calculation is fairly reliable if you are between 40-79 years old, have cholesterol less than 320, and without a history of cardiovascular disease. Listen to our first ever discussion with @kidney_boy, Joel Topf MD. It’s a classic episode with a fresh intro as we prepare for @NephMadness 2019 with our friends from Twitter and @AJKDonline Dominate leg cramps, diuretic therapy, and resistant hypertension with tips from @kidney_boy, Joel Topf MD @kidney_boy, Chief of Nephrology Kashlak Memorial Hospital, co-creator @NephMadness. We start with basic renal physiology and build up to the treatment of resistant hypertension. Corrections: Dr Topf posted the following corrections on his blog PBfluids.com 3/22/2017 “I enjoyed the experience immensely, but in an hour of talking off the cuff I made some embarrassing mistakes: In describing water reabsorption I said it occured in the cortical collecting duct rather than the medullary collecting duct. In describing my cure for cramps I tell the story of Gitelman’s and say it is like congenital loop diuretics rather than congenital thiazide diuretics I mucked up the story about the MRFIT story and how it allowed a head to head comparison of HCTZ and chlorthalidone. I really oversold what happened.” Full show notes at https://thecurbsiders.com/podcast. Join our newsletter mailing list.
