In this episode of the Intelligent Medicine Podcast, Dr. Ronald Hoffman discusses the importance of hydration with Dr. Dana Cohen Integrative Medical Doctor and co-author of “Fuel Up: Harness the Power of Your Blender and ‘Cheat' Your Way to Good Health.” They explore the often-overlooked role hydration plays in maintaining health and preventing chronic diseases. Dr. Cohen emphasizes practical strategies to improve hydration through diet and lifestyle changes. They highlight the risks of both dehydration and overhydration, discuss the impact of processed foods, and offer realistic solutions to help listeners incorporate better hydration practices into their lives. Too often doctors are dismissive of pop culture exhortations to pay attention to hydration. Except for those with specific medical conditions, the conventional mantra remains “In ordinary healthy individuals, thirst is a good guide to how much fluid you should consume.” In medicine, we routinely query patients about their diets, their exercise, their sleep, their stress levels, drug, alcohol and tobacco use, their social interactions, their spirituality, even their sexuality. But so seldom do we ask “How's your hydration?” A recent review found that “Hypohydration is known to reduce mental and physical performance, and more recent evidence suggests hypohydration also impairs vascular function and cardiovascular regulation.” But adequate fluid intake is not merely a matter of optimization; a recent medical study sampling 1200 adults aged 51-70  suggests that it may literally be a matter of life and death: “. . . underhydration was significantly associated with increased prevalence of obesity, high waist circumference, insulin resistance, diabetes, low HDL, hypertension, and metabolic syndrome. Over 3–6 years of follow-up, 33 chronic disease deaths occurred in the sample, representing an estimated 1,084,144 deaths in the U.S. Alongside chronic health conditions, underhydration was a risk factor for an estimated 863,305 deaths. Independent of the chronic health conditions evaluated, underhydration was a risk factor for 128,107 deaths.” How prevalent is underhydration in a sample of American adults? According to the study: “Nationally representative data from the National Health and Nutrition Examination Survey (NHANES) indicate that over 65% of adults aged 51–70 years in the U.S. do not meet hydration criteria.” The episode also touches upon broader health issues, contrasting dietary extremes, and the importance of accessible, enjoyable healthy eating habits.

In this episode, we review the pharmacology, pharmacokinetics, adverse effects, monitoring, medicinal chemistry, and more of loop diuretics. Key Concepts Loop diuretics (furosemide, torsemide, bumetanide, ethacrynic acid) are the most potent type of diuretic and are used to relieve edema. Loop diuretics cause an increased loss of sodium, chloride, potassium, hydrogen, magnesium, and calcium ions into the urine. Excessive loss of these ions manifests as hypokalemia, hypomagnesemia, and metabolic alkalosis. Loop diuretics have an S-shaped dose response curve – a minimum dose is required for diuresis and a “ceiling” effect occurs at higher doses (leading to more ADRs). Doses should be individualized based on the clinical response of the patient. Ethacrynic acid is incorrectly used in patients with a “sulfa” allergy. The other loop diuretics contain a sulfa moiety but are safe for use in patients with “sulfa” allergy (e.g. allergy to sulfamethoxazole-trimethoprim). They also have a much higher rate of zinc-deficient soils. Zinc deficiency can cause hypertension. Doctors are often hyper-focused on reducing sodium to lower blood pressure, and people are surprised to find their blood pressure does not go down after reducing their salt intake. If you're deficient in zinc, your sense of taste and smell is often inhibited. This might cause you to add more salt to your food and crave junk food. The average American indeed consumes too much sodium from junk food and not enough potassium, but zinc deficiency is also a significant issue. ACE 2 inhibitors are the number one blood pressure medication. This type of medication depletes zinc, potentially causing more blood pressure problems. Diuretics, antibiotics, antacids, and birth control pills also deplete zinc. Zinc is a helper mineral for making nitric oxide, a vasodilator that helps relax the blood vessels. Zinc is required for nitric oxide to work. Zinc is also necessary to make certain antioxidants, including SOD, which helps to keep inflammation at bay. You can become deficient in zinc by consuming food grown in zinc-deficient soils or if you don't consume shellfish and red meat. Sugar, refined carbs, whole grains, alcohol, and fructose deplete zinc. Weak stomach acid interferes with mineral absorption, which can contribute to a zinc deficiency. Oysters and other shellfish, as well as red meat, are the best sources of zinc.

In this podcast, we're going to talk about potassium on the carnivore diet. You need 4700 mg of potassium each day, more than any other mineral. Potassium is important for the sodium-potassium pump present in all of your cells, which allows the contraction and relaxation of your nerves. Too much potassium is known as hyperkalemia, and too little is known as hypokalemia. Symptoms of high potassium include diarrhea, muscle pain, and muscle numbness. Symptoms of low potassium include constipation, muscle cramps, and fatigue. Potassium is vital for exercise performance. If you don't have enough, you'll probably feel weak or tired. One of the causes of low potassium is low magnesium. Kidney disease can cause high potassium. If the kidney is not functioning properly, it won't be able to get rid of potassium. It's very rare to develop too much potassium in the blood from dietary intake. Blood pressure medications or side effects of an adrenal condition known as Addison's can also cause high potassium. Diuretics and steroids can cause low potassium. Surgeries and injuries can also cause low potassium. The following vegetables are some of the best sources of potassium: •1 cup of beet leaves 1300 mg •1 cup of Swiss chard 900 mg •1 cup spinach 840 mg •1 avocado 500-700 mg •1 cup of beets 520 mg •1 cup of salad 350-500 mg Bananas contain around 300 mg of potassium but have too much sugar. Here are some of the best sources of potassium on the carnivore diet: •1 cup of bone broth 500 mg •3 ½ oz beef 350 mg •3 ½ oz octopus 650 mg •3 oz salmon 300 mg •1 large egg 63 mg •3 oz beef liver 125 mg •3 oz chicken 300 mg

Today we're going to talk about how potassium affects diabetes and high blood pressure. Diuretics are commonly used to treat high blood pressure. But, they come with side effects, including an increased risk of developing diabetes. On the flip side, ace inhibitors, also used to treat high blood pressure, protect against diabetes and lower the risk of diabetes. I believe the big difference between these two drugs, when it comes to diabetes risk, is what they do to potassium. Diuretics deplete potassium, and without enough potassium, insulin can't be released very well. A deficiency of insulin can then lead to diabetes. Ace inhibitors increase potassium by helping to prevent the loss of potassium. So why do we even need a drug—why aren't we just consuming more potassium? Many people support healthy blood pressure and healthy blood sugar levels by taking potassium. Benefits of potassium: • It protects the kidneys • It protects the heart • It helps protect against stroke • It helps protect against diabetes • It helps power the nervous system in connection with the muscles • It helps regulate sodium The RDAs for potassium are 4700 mg. It might surprise you, but bananas aren't a good source of potassium. John Powell was at a loose end when he returned to Australia in 2021 after calling time on his long riding career in Singapore. It wasn't long before Racing NSW enlisted his services as a mentor for northern NSW jockeys, and more recently Gary Kliese has snapped him up for a co-hosting role on Sky Thoroughbred Central's coverage of northern rivers racing. It's almost forty years since John Powell began his apprenticeship with Helen Page at Warwick Farm. In a 37 year riding career he rode 1400 winners in five jurisdictions, more than half of them in Singapore. We were able to pin him down recently for a lengthy chat about his fascinating journey in racing. John begins by explaining how his opportunity with Sky Thoroughbred Central came up.  He says his main focus is on the characteristics of horses and tactics employed by jockeys. John says he'd barely settled back in Australia when he got a surprise phone call from Corey Brown on behalf of Racing NSW. The former top jockey takes us back to childhood days at Campbelltown and talks about the family friend who introduced him to Helen Page. He recalls his delight when Warwick Farm trainer Frank Bacon gave him his very first race ride.  John hasn't forgotten his very first winner at Gosford for his boss Helen Page. He admits the ride wasn't pretty. The chief steward spoke to him on the day about an entirely different matter. He fondly remembers his magical first city winner at Canterbury in 1987. Powell recalls multiple wins on the Guy Walter trained Prince Invader- a very important horse to the young rider in those early days. He has never forgotten a pick-up ride on brilliant sprinter Show County. He got the ride during a jockey's strike when every runner at a Warwick Farm meeting was ridden  by an apprentice. The recently retired jockey looks back on his affinity with Kembla Grange racecourse. In one season in the late 80's he was champion Kembla apprentice. He talks of Shane Edmonds, another talented apprentice who was with Helen Page at the same time. John talks about his addiction to diuretics, the demon pills used by many jockeys in days gone by to help them excrete water through the kidneys. Diuretics gave jockeys a simple way of losing weight on race days. They were very dangerous long term. He looks back on several short term contracts in Mauritius- a unique racing environment. John talks of his very successful stint in Macau. It was here a Panamanian jockey helped him to fine tune his riding style. He looks back on some of the outstanding horses he got to ride in Australia including a few of Bart Cummings' topliners. John talks of his association with John Hawkes which led him to a full season in Brisbane. The champion trainer once entrusted John with a barrier trial ride on champion Octagonal. He still talks about it. He looks back on his appointment as stable jockey for Don Baertshiger in Singapore- a partnership that would last for many years. John says he got away to a “flyer” in Singapore by winning the Gr 1 Derby on Hello And Goodbye. It was Kiwi trainer Bruce Marsh's first Singapore win. He talks of his first win in the Kranji Mile and a second Singapore Derby. John reflects on a Kranji Mile win for Laurie Laxon and a QE2 Cup success. The former ace jockey talks openly about his cancer scare. The discovery of a lump on his neck during a routine medical check led to further scans. Thyroid cancer was detected. Covid enforced travel restrictions delayed John's return to Sydney for crucial surgery. He says the thyroid complication sent his weight on a spiral, and he knew his riding days were numbered. John remembers the shock he received to read about his retirement on social media, long before he'd made any announcement. It's a laid back chat with a talented jockey who spent 25 of his 37 riding years out of Australia.

Dr. Escalante has secured over $150,000 of internal funding to support his teaching and research as well as over $5 million in external funding as principal investigator or co-investigator through various businesses and agencies, inclusive of the US Department of Education and the US Department of Veteran Affairs. Dr. Escalante holds a BS in Athletic Training with a Biology minor and an MBA with concentrations in Marketing and Healthcare Management from the University of La Verne. He also received a Doctor of Science in Athletic Training from Rocky Mountain University of Health Professions and is certified as an athletic trainer through the National Athletic Trainer's Association (NATA), a strength and conditioning specialist through the National Strength and Conditioning Association (NSCA), and as a Sports Nutritionist through the International Society of Sports Nutrition (ISSN). In 2021, Dr. Escalante became one of only 36 fellows (out 36,000+ members) of the ISSN for his contributions in the field of sport nutrition. Dr. Escalante is an active member of the NATA, NSCA, and the ISSN. He serves as a reviewer and associate editor for the Journal of the International Society of Sports Nutrition (JISSN), is the chair of the NSCA Bodybuilding and Fitness Special Interest Group, and is on the executive board for the NSCA Nutrition and Metabolism Special Interest Group. He was also an instructor to third year Doctor of Physical Therapy students at Western University of Health Professions between 2004-2018. Prior to going into higher level education, Dr. Escalante owned and operated an outpatient orthopedic physical therapy and personal training business for 18 years and was an owner/operator of 2 World Gym franchises.  He also served as interim head athletic trainer for East Los Angeles Community College, was a fitness/health consultant for the US Department of Defense, and was the Chief Operating Officer and Sports Medicine Coordinator for a large physical therapy/sports medicine practice in Southern California. Over the course of his 20+ year career, Dr. Escalante has had the opportunity to train, treat, coach, and/or consult with thousands of athletes and non-athletes alike inclusive of professional athletes in the NFL, MLB, NBA, LPGA, Olympians, and IFBB bodybuilders/physique competitors.Dr. Escalante prides himself in his commitment to life-long learning and professional development.
