This week on the podcast I speak to Dr Catia Martins about metabolic adaptation and weight loss. Catia has spent her research career answering questions around why some people experience a greater reduction in basal metabolic rate (BMR) than others when they lose weight, and how this impacts on overall weight loss success and the timing of that success. She has also studied the effects of the ketogenic diet on weight loss and how ketosis may impact this relationship. This is such a great conversation and, while a bit scientific, is interesting for general audience and practitioner/scientist alike.Dr. Martins has been investigating over the past almost 20 years how exercise and energy restricted diets impact on energy balance and body weight homeostasis, in particular their impact on appetite control. She holds a first degree (honor) in Nutrition and Dietetics from Oporto University (Portugal), a M.Sc. in Clinical Nutrition (distinction) from Roehampton University, London (UK) and a Ph.D. from University of Surrey (UK). She has received an awards from the British Nutrition Society in 2007 and Association for the Study of Obesity (ASO) in 2008 in recognition of her research on the effects of exercise on appetite control. In 2008, after completion of her Ph.D., she was awarded a Post Doctoral fellowship (from FCT, Portugal) to investigate the effects of exercise-induced weight loss on appetite-related peptides and motivation to eat in individuals with obesity, at NTNU, under the mentorship of Prof. John Blundell (University of Leeds, UK). Two years later, in 2010, Dr. Martins was awarded a research grant from Central Norway Regional Health Authority/NTNU to lead a project on “High-intensity intermittent training to maximize metabolic and cardiovascular protection in individuals with obesity” in collaboration with Dr. Neil King (Queensland University of Technology, Australia) and Prof. Linda Morgan (University of Surrey, UK).She is at the moment at the University of Alabama at Birmingham (UAB) investigating the physiology of the reduced-obese state in collaboration with Dr. Barbara Gower, Dr. Gary Hunter and Dr. James Hill. Dr. Martins is particularly interested in understanding the phenomenon of  metabolic adaptation (a reduction in energy expenditure below predicted levels) and its clinical relevance.Dr. Martins has published several original papers and reviews on the impact of exercise and energy restricted diets (namely ketogenic diets) on appetite control and energy metabolism and is a regular speaker at the European Congress of Obesity.

Daniels injury and who might start at Quarterback this Saturday versus The University of Alabama at Birmingham (UAB) in Athens. “From what I understand I think they (UGA coaching staff) are going to hold him (J.T. Daniels) out against UAB. You don't really need him (Daniels) for this game and that's not taking away anything from UAB…UAB has a great secondary, two good cornerbacks that are going to be tough to beat but I don't know that you need him (Daniels) to win this game for you. So why not let him (Daniels) rest and get ready for South Carolina,” Nabulsi said when asked about what Georgia will do with J.T. Daniels this week. McMichael then asked Nabulsi, “We shouldn't be surprised if we see him (referring to Carson Beck) under Center on Saturday then?” “You should not be surprised; his trainer (Carson Beck's Trainer) has been telling everyone that he (Beck) is going to get the start…but I'm telling you guys I know Kirby Smart and I know he likes known quantities. He likes production he can count on and it would not surprise me if we saw Stetson Bennett under Center,” Nabulsi responded. See omnystudio.com/listener for privacy information.

Going through engineering school is hard, let alone passing the FE exam that comes along with it. Some engineers have taken the strategy of taking the FE exam right after school as the best option. In contrast, others get so busy with family- and work-related stuff that devoting hours to studying for the exam daily seems a barrier in itself, which causes them to postpone it — most of the time indefinitely. Today's guest is the living proof that being out of college for a bit and in an exam setting is not that much of an obstacle. Today's interview is with D. J. Leslie, a civil engineer who graduated from the University of Alabama at Birmingham (UAB) and has been working as a construction inspector at Volkert, Inc. for about 5 years. He started out doing some basic paving jobs, working closely with the Alabama Department of Transportation (ALDOT). He then moved to a much larger project: a three-phase intersection redesign between the two major interstates in Birmingham, the I-65 and I-59/20. Earlier this year, as an engineer with a 5-year experience in the field and away from exam settings for some time, he decided to take the FE exam in October. As a way to help him recall all the content that is tested on exam day, as well as to keep him on track to get a passing grade and connected with people who know how the exam works, he enrolled in the Ultimate Civil FE Review Course. D. J. mentions some features of the course that stood out to him, such as the huge bank of problems available and the practice exams that give a realistic preview of how exam day looks like. This act will lead to greater opportunity. Ask yourself if you can achieve your dreams without extending what you are capable of? Build relationships so that when a new opportunity arises, you are first person who is considered for the role. Do this by asking where you can help and proclaiming where your skill set can be beneficial. Don't believe everything that is said about you; listening to the advice or criticism of others is not always to your benefit. Meaning, don't carry that critique around with you and turn it into negative self-talk. Evaluate where you can improve, do the work, and let go what's not serving you. Marjorie Lee White and Andres Viles, an Emergency Physician and Emergency Nurse from the University of Alabama-Birmingham join us to discuss how their organization has worked to integrate simulation into the health system at every level and in every program. The story begins when the simulation program was put in charge of the response to the 2017 Ebola outbreak. Now people say “we need to simulate first” when new issues emerge. We discuss how the simulated run-through of complex plans can show the flaws in systems before they can potentially harm patients. We also talk about how preparing for Ebola management through simulation gave expertise in donning and doffing and other PPE measures, including the technique of creating a remote doffing expert who helps tired providers stay safe at the end of procedures. Andres Viles is a Simulation Coordinator, Senior and Director of Immersive Simulation in the Office of Inter-professional Simulation for Innovative Clinical Practice at UAB in Birmingham. He also holds the title of Training Coordinator for the UAB Serious Infectious Disease (SID) Team. What's the role of geriatrics and palliative care in the care of individuals with COPD? We talk this week with Anand Iyer, the lead author of this weeks JAMA IM article on this subject. It's a little off from our ongoing COVID topics, but given that his along with his co-authors (Randy Curtis and Diane Meier) JAMA IM piece just got published, we figured now is the right time to highlight #PalliPulm. What is #PallPulm? #PalliPulm is something that Anand Iyer founded, and is an online community of clinicians, investigators, patients, and others interested in the intersection of palliative care and geriatrics. Anand is also an Assistant Professor in Division of Pulmonary, Allergy, and Critical Care Medicine at the University of Alabama at Birmingham (UAB) and faculty at the Center for Palliative and Supportive Care at UAB. Anands research on this subject has shown a lot of things, including that COPD patients are often referred too late to palliative care and hospice, pulmonologists often equate hospice with palliative care, there is high symptom and emotional needs in COPD patients, and that patients and family members feel palliative care should be integrated early on in the care of COPD. Ira Pastor, ideaXme life sciences ambassador, interviews Dr. Randy Cron, MD, PhD, Professor of Pediatrics and Medicine at the University of Alabama at Birmingham (UAB) and the Arthritis Foundation, Alabama chapter endowed chair in Pediatric Rheumatology. Ira Pastor Comments: As we are a few months into the COVID-19 pandemic, a medical term we’ve been hearing mentioned quite a bit it in the press in terms of the pathophysiology of the infection, is that of Cytokine Release Syndrome (CRS) or Cytokine Storm Syndrome (CSS) is a form of systemic hyper-inflammatory response that can be triggered by a variety of factors such as infections, as well as certain drugs. CRS / CSS occurs when large numbers of white blood cells are activated and release inflammatory cytokines (which are a broad category of proteins important in cell signalling) which in turn activate yet more white blood cells, and the cycle repeats in a very dangerous and deadly cascade which can harm and kill patients via systemic hyper-inflammation, hypotensive shock, and multi-organ failure. Just a few months ago, the Springer publishing organization released a text entitled "Cytokine Storm Syndrome" - a first of its kind book to addresses all aspects of this unique disorder, from genetics, pathophysiology, and ongoing research related to clinical presentations, risk factors, and treatment. Dr. Randy Cron: Today we are joined by one of the co-authors of this book, Dr. Randy Q. Cron, MD, PhD, Professor of Pediatrics and Medicine at the University of Alabama at Birmingham (UAB) and the Arthritis Foundation, Alabama chapter endowed chair in Pediatric Rheumatology. Dr. Cron is also the Director of the Division of Pediatric Rheumatology and the Pediatric Rheumatology fellowship program at Children's of Alabama, a private not-for-profit, which provides specialized medical care for ill and injured children, and is ranked among the best pediatric medical centers in the U.S. Dr. Cron received his medical degree from UCLA and a doctorate in Immunology from the University of Chicago, with Pediatric residency training at Stanford Children’s Hospital in California and Pediatric Rheumatology fellowship training at Children’s Hospital and Medical Center in Seattle. Dr. Cron is board-certified in Pediatric Rheumatology, is the past president of the Alabama Society for Rheumatic Diseases, and a member of American Academy of Pediatrics, Society for Pediatric Research, American Association of Immunologists, American College of Rheumatology and numerous other professional organizations. His major research interests include: dysregulation of CD154 protein that is primarily expressed on activated T cells in systemic lupus erythematosus; CD4 T cell host factor regulation of HIV-1 LTR transcriptional activity and latency; genetic mutations leading to defective lymphocyte cytolytic activity; diagnosis and treatment of temporomandibular joint arthritis in childhood arthritides, and the diagnosis and treatment of Macrophage Activation Syndrome (MAS), a severe, potentially life-threatening, complication of several chronic rheumatic diseases of childhood, and a component of the umbrella of conditions that make up CRS/CSS. On this episode we will hear from Dr. Cron about: His background: he developed an interest in pediatric medicine, immunology, and the very specialized pathology of CRS/CSS. An overview of CRS/CSS and conditions under the CRS/CSS umbrella including Macrophage Activation Syndrome and Hemophagocytic Lymphohistiocytosis. Treatment modalities for CRS/CSS. The connection between CSS and Acute Respiratory Distress Syndrome (ARDS); a type of respiratory failure characterized by rapid onset of widespread inflammation in the lungs, and responsible for many deaths in COVID-19 patients. The level of acceptable risk was way too high compared to how safety was managed in other areas. He started a quest to reduce the risk to people and advance the practice of electrical safety. In the last 20 years of his 45 years with DuPont he was able to reduce the number of electrical fatalities to zero. He and his team were able to achieve breakthrough performance in managing the risk with electrical energy contact and sustain that for 25 years. However, he is not satisfied with this number because there are still electrical fatalities around the world and he sees a global trend of increasing electrical fatalities. Lanny uses the concept of flying to the moon and how humankind achieved what seemed impossible. He describes the importance of stretching the capability of how we currently think and looking beyond the artificial barriers, what we perceive to be real barriers and think about things differently. This type of thinking may be used to help us work towards greater goals. It is extremely important to help instill a level of curiosity in others as we teach. Expanding the ability to think critically about what is possible is an extremely important skill for leaders as they lead others. As leaders, it is extremely important to help develop other leaders and improve the way leaders think about problems and how they engage others about thinking differently. Pushing beyond what we think is possible is important. We need to think beyond basic compliance and work towards achieving excellence. Compliance is important, but will only take us so far in terms of organizational performance. The analogy of heroes in history who have changed the world comes to mind, such as Thomas Edison, Nelson Mandela, Mother Theresa, Michael Jordan, The Beatles, and Hank Aaron. These were all people who pushed the boundaries. They didn’t get to where they were by seeking average compliance. They “kicked the boxes,” sought excellence and opened the way for people to come behind them. Just like those heroes, leaders need to build the case for being excellent, not just average. Virginia Sparks Volker was born in Jasper, Alabama in 1940. An instructor of Biology at the University of Alabama at Birmingham (UAB) since 1967, Volker has degrees from the University of Alabama at Tuscaloosa and Harvard University. Actively involved in the Unitarian church and in many community organizations, Volker also directs an annual summer intensive course on the Civil Rights Movement. As a college student in the early 1960s, Volker participated in inter-racial discussion and social groups with students from Stillman College. Because such activities were illegal, Volker and her friends were strongly encouraged by university administrators to cease participation. However, even after she enrolled in graduate school at UAB, she continued to cross boundaries and participate in groups such as the Alabama Council on Human Relations and Friendship in Action. Listen to Virginia Volker explain how she and other White students came to socialize with Black college students in Tuscaloosa, Alabama in 1960 and '61.
