One dilemma I've heard from parents again and again: If my T1D kid is acting out because of a high blood sugar, how do I parent that? Do I give my kid a pass because I know that their high affects their mood? Or do I parent the behavior as though diabetes isn't operating in the background? In this week's episode, Julie comes on the show to talk about how this issue is playing out for her 6 year old son, Ethan, when he's at school. It sounds like Ethan has an amazing teacher: She's paying attention to his diabetes and wondering if his high blood sugar might correlate to some disrespectful behaviors that she's seen. The problem is that Julie and her husband haven't seen that same correlation at home: Although they know that their son is very wiggly, has a lot of energy, and likely has a hard time sitting still — especially in the afternoons at school — they haven't seen disrespectfulness when his blood sugar is out of range. At the same time, when he's high — or has a big case of the wiggles — Julie will take him for a run to help him get his energy out or bring his blood sugar down, something teachers can't do for him at school. Whether diabetes is driving Ethan's behavior or not, we came to the conclusion that the teachers might be overly focused on Ethan's diabetes, giving a reason for his not-so-stellar behavior when it would be better to just treat it as a behavioral issue.Truth be told, we really never can know how our kid's blood sugars are affecting them. But in my experience working with T1D adults, I found that they often felt frustrated with their parents for attributing their behavior to highs when they were kids. And the reality is that we all need to strive to be our best selves, even under less than ideal circumstances. So in the show, we talk about different ways we can help our kids show up as their best, whether it's the high blood sugar getting in their way, or just the wiggles.Hope you enjoy the show!

In this episode of The Metabolic Classroom, Dr. Ben Bikman explores the metabolic implications of hyperglycemia, the condition of elevated blood glucose levels. He focuses on both the chronic effects, like those seen in diabetes, and the acute effects of glucose spikes, even from short-term dietary indulgences. Hyperglycemia, while often linked to insulin resistance and diabetes, also has a range of other negative consequences that can manifest even with occasional spikes.One of the key points Dr. Bikman discusses is the relationship between hyperglycemia and insulin. As glucose levels rise, insulin levels rise as well, which can lead to insulin resistance over time, forming a vicious cycle. He highlights studies showing that even a short-term increase in carbohydrate intake can significantly raise fasting insulin levels and triglycerides. These changes have direct consequences on metabolic health, including an increased risk for cardiovascular disease.Dr. Bikman also explains how hyperglycemia leads to oxidative stress, causing the overproduction of reactive oxygen species (ROS). This oxidative stress can damage proteins, lipids, and even DNA within cells. It also contributes to the formation of advanced glycation end products (AGEs), which are harmful compounds formed when glucose binds irreversibly to proteins, fats, or DNA. These AGEs are linked to various diseases such as retinopathy, kidney disease, and atherosclerosis.The lecture further explores how hyperglycemia damages the endothelium (the inner lining of blood vessels) and degrades the glycocalyx, a protective gel-like layer on the endothelium. This damage increases the risk of cardiovascular disease by making blood vessels more prone to atherosclerosis. 63: HbA1c Levels Explained: Why They May Be High Without High Blood Sugar

Are you finding that your HbA1c levels are elevated despite being on a low-carb, ketogenic, or even carnivore diet? Recently, my HbA1c test came back at 5.7 (just under the prediabetic range), and I knew something was off; after all, I've been following a relatively low-carb lifestyle for years, so there's no way my blood sugar should be high enough to suggest prediabetes.  In consulting with carnivore diet expert and medical doctor Shawn Baker, I learned that healthy red blood cells that live longer than average can actually skew HbA1c readings, making it appear as though you have higher blood glucose than you do. This happens because the longer your red blood cells live, the more glucose has time to attach to hemoglobin, artificially inflating your HbA1c levels. (Conversely, shorter-lived red blood cells can lead to lower readings, even in those with high blood sugar.) In this episode, I break down the science of the HbA1c test and explain how factors like red blood cell lifespan, iron deficiency, anemia, and other health conditions can all affect your hemoglobin A1c results.  Even if you have healthy blood sugar and no signs of diabetes, your HbA1c might still be higher due to these factors. In my case, despite my elevated HbA1c, my continuous glucose monitor (CGM) showed average blood glucose levels between 85 and 89 mg/dL — well within the normal range. Episode #171 In this Fasting Q&A episode, hosted by Coach Lisa Chance, Dr. Jason Fung answers questions from the TFM Community: 1.  Imagine that I dress the carbs that I eat by adding acid and fat so that my BG spike is minimized. What happens to all the fat that I added? Don't the carbs send the signal for insulin to go up and, consequently, for all the fat I added go directly into body fat? If so, should I minimize the amount of fat that I dress carbs with and mostly just eat them with acid, like vinegar or lemon juice? [01:55] 2.  What eating strategy would you suggest for an insulin-resistant or pre-diabetic person? In other words, do they need to low carb to get out of the pre-diabetic range? [08:32] 3.  I am struggling with getting my blood sugars down to a normal range and am stalled in my weight loss. I have lost 150 lbs and gotten off insulin and other oral meds. I feel like my way of eating is on point 95% of the time and I am fasting 24-48 hours or longer 2-3 times a week. On my last extended fast, my blood sugar didn't come down until day 3 and then started going up every day. By day 5, my blood sugar was 189. I broke my fast with whole, low-carb foods and my blood sugar came down to the 140s where it likes to live these days. I'm wondering why, so far into my journey, things would start going wonky. I do have a lot of personal and family stress happening right now. Could that be generating enough cortisol to be negating my diet and fasting or are there other explanations that could be contributing? [16:44] 4.  Do you have any evidence (empirical or otherwise) on fasting, autophagy, and reduced carbohydrate intake for people with ADPKD (Autosomal Dominant Polycystic Kidney Disease)? [20:23] 5.  Can patients with gallstones fast? Does it increase the risk of gallstones? [24:57] 6.  I know seed oils are not recommended but what do you think about cold pressed niger seed oil? Avocado oil is not readily available in my country, and I cannot use butter or animal fat everyday as there are a minimum of 180 religious fasts a year. These fasts prohibit dairy products and meat, so we are basically vegans during these fasts. [30:36] 7.  Why do some people (who have never missed a meal in their life and suddenly start a multi-day fast) get very high ketones and have to break their fast? I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Diana: Hi Dr. Cabral. I started wearing a CGM two weeks ago and my blood glucose levels are constantly elevated above 100. It's not due to the way I eat; in fact, eating will raise my blood sugar about 20 points, but they'll then drop to healthy levels after two hours. Following this, my blood sugar will rise again to over 100. I've had ME/CFS for almost 30 years and have such an intolerance to exertion that I can't take a five minute walk without crashing, so I can't turn to exercise to help manage my blood sugar. What could cause this and what can I do to help bring my blood sugar levels down into a healthier range?     Mindy: Hi! I've been hearing a lot about how citric acid is horrible for you and is often made from mold. I know in other podcasts you've said that not all citric acid is bad and can be made from other things. Is there a way to tell what it's made from? Should we be looking for a non GMO label? Thank you!     Lisa: Hello Dr. Cabral, Thank you for all you do as you have provided me with a tremendous amount of knowledge in my wellness journey. I have a question I have not been able to determine the reason why and best approach. Historically I received an annual Papanicolaou (Pap) test but now informed recommendations have switched to every 3-5 years, depending upon the patient. I have always believed one of the tools to cancer prevention is screening. Do you have any reasoning why it would have been lengthened to every 3-5 years? Is this the best approach to this 'area' of cancer screening or do you have an alternative screening you recommend? I reviewed your podcast archives and unfortunately have not seen something specific to this topic, apologize if you already answered.     Kelsey: hi dr. cabral, i'm noticing i have insulin resistance fairly bad. i wake up with a fasted glucose between 100-110 almost daily. i eat a low carb diet with healthy fats and protein primarily but any time a starch / sugar is involved… it takes hours, sometimes days, to return to my baseline of 100. this is with healthy carbs like potatoes and fruit. some background is i have SIBO (currently on CBO protocol and PCOS w/ high androgens). ive experimented with longer fasts and i can go days without food with my blood sugar only reaching somewhere in the 90's. i feel very good with no energy issues and i would have no idea i had this issue without deciding to get a CGM one day. can you please tell me the steps you'd take in reducing my blood sugar over time? thank you.     Barbie: Dr Cabral, I desperately need your help. Could it be a nervous system issue? Maybe a result of some kind of trauma? This person would insist there has never been any sort of trauma. Maybe it’s not an issue at all and there’s just a portion of the population who don’t dream? I find it very strange as I dream every night very vividly. Looking forward to hearing your experience with this! Thanks!   Katie: Hi Dr Cabral, Could you please help with some advice for my teen sons acne. After seeing a Naturopath he has been off sugar, dairy and processed foods for 4 months with not too much change. He currently takes zinc and a probiotic and does clay masks.The only option he has been given from his gp is 3 to 6 months of antibiotics and Accutane which we want to avoid due to side effects and long term damage. As well as the pimples he already has alot of scarring and he is only 14. Thankyou for your time we appreciate it :)   Lindsay: I’d love to hear your thoughts on a shortened follicular phase. This is something I have been struggling with for the last 7-8 years. I wear an Oura ring and can clearly tell when I ovulate based on temperature change, vaginal discharge, and mittelschmerz. I typically bleed for 5 days at the beginning of my cycle and ovulate around day 10, resulting in a 24 day cycle. My luteal phase is always 13-14 days in length. In November I ovulated on day 7! I couldn’t believe it, I bled for 5 days, ovulated on day 7, and then had a 14-day luteal phase for a 21-day cycle. No history of birth control or health struggles to speak of, and not on any medication. No obvious hormonal issues: clear skin, healthy blood sugar levels, healthy weight, sleep well, and exercise regularly. 39 years old.   Kelsi: hi dr. cabral! a question about sauna - is it still beneficial for someone who experiences elevation in blood sugar during / after a session? thank you.   Aaron: I've noticed that I'm more prone to tendonitis and muscle strain as I get older. I try to lift weights 2 to 4 times a week when I'm not injured. I'm interested to hear your thoughts as a physical trainer on how to increase muscle and strength without overstressing tendons. I'm especially interested to hear how you balance the workout advice with the DESTRESS protocol and supplementation. I try to eat whole food, try to exercise whole body workout daily, trying to reduce stress (big problem for me), I'm working on reducing toxins, worst sleep (my biggest problem I'm working on), very vata/anxious, can't take DNS because I'm sensitive to B6, but I do maintain my methylation, vit D, and magnesium, healthier now than I have been. I'm hoping you have some thoughts or suggestions on this one. Background: I'm a lean, 56 yo male who follows a lower carb (50-75g daily) / higher protein diet (1+g/body weight). I'm very in tune with my diet given my wife is a T1D following Bernstein. I lift weights 3x/week and run about 20 miles/week (because I enjoy it). Admittedly, my work stress is high and I work about 50-60 hours/week. I get about 7.5 hours of sleep nightly and do all the sleep hacks to ensure I'm getting restful sleep. I've been wearing a CGM in hopes to better understand a recent A1c test of 6.0. I also had my fasting insulin level checked and it was 3 - so I don't think I'm insulin resistant. The CGM consistently shows fasting glucose around 115 and staying there through mid afternoon, when I'll typically drop into the 90s. I see spikes for exercise as high as 160, but come back down within 1-2 hours. I'm trying berberine (even though I don't have a carb load) and l-theanine for the stress spikes, but so far, I'm not seeing much change Could this all be stress related? Any suggestions on how to fix this? Could this be gluconeogenesis from too low calorie? I don't think I eat too few calories and am about to embark on some tracking to see where I am.   Keto and xanthelasma Fredrik writes: Hi Robb and Nikki, I have a question about a condition I heard you mention once on the podcast, xanthelasma. Basically yellow spots around the eyes. I have been on paleo since 2009 and keto since 2012 and you were some of the first people I found on my journey. It really has changed my life for the better. Back in 2009 I was training 5-6 days a week as a 28-year old with good performance but still had a less than optimal body composition and generally did not feel great. Now many years later on keto, as you guys often say, I look, feel and perform much better. There is just one nagging issue, yellow spots around my eyes. They started to occur after about three years on keto and I had a couple of them removed surgically but they are now coming back slowly. Obviously I cant keep on having surgery, so what to do? I have theorized that dairy might be a contributing factor but the problem of just trying to remove something is that I will not know if it is effective until years later. I have tried but it is very hard to find any useful advice for this online. Some people mention that supplementing with bile salt could help, what do you think? Physically and mentally I feel my absolute best on keto bordering to carnivore so I really want to maintain that. Dairy has also been really helpful for me to get enough calories for my workouts (three days a week of heavy weights and two days of either running or cross country skiing). So how do I know what to do and is dairy a likely contributor? Also, what else, other than dairy, could I eat to get in the calories for not just maintaining but building muscle? Fredrik   Weight loss/mind change Mark writes Robb, Long time follower, my wife and I went to Nutrition seminar in 2009 at crossfit Monrovia. I like listening to yall. I am the former football/rugby player that eats too much food and sits at a desk. It has come to a point now that Life Insurance is requiring me to lose 60 pounds. I need to get to 231 pounds at 6'0". Currently i range between 285-295. I have tried diets the last couple of years for challenges, clean it up for a month and lose 30 pounds, super focused, lift, train without burning the joints. Skip meals, Skip Carbs. I lose the weight. Then i go back to normal life and eat my normal food which is too much. The question is how do i reset my need to eat and take in extra calories. Should I consider counseling? I eat as a feeling. I eat the same as I used to when I was playing rugby. I do also follow Dan John and I try to use Easy Strength for lifting. But I have not found a balance where i steadily lose. I have been maintaining my weight at about 285 with what i normally eat. My sleep is ok, Normal is 10:30 pm to 6 am. I sit at a desk 30 hours a week. I lift 15 minutes once a week. I play/run/yard work one time a week also. I have heard you talk about this before where we have been trained to gain and grow. Now that I am not trying to keep muscle mass on I have trouble shutting off eating. I still have not written my food down, like in the ketogenic reset. I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Kelsi: hi dr. cabral! i started tracking my blood sugar because i wanted to see if coffee was knocking me out of a fasted state. turns out my fasting glucose levels are elevated (consistently 100-110 upon waking and after prolonged fasts). i won't go into everything i already do health - wise but i am very active, have no energy issues and eat well. i run a little anxious, high cortisol, etc. i have normal insulin and A1C levels. would the stress, mood and metabolism test be your go to here?   Kelsi: hi again dr. cabral! i know you have a million podcasts on blood sugar but i didn't find any on myparticular question. what're your thoughts on longer fasts for someone who consistently has slightly elevated blood sugar levels (100-105)? even after 24-36 hour fasts my blood sugar will remain around 100. curious on whether or not fasting would be helpful in this situation. lastly, is if possible for someone's ‘normal' blood sugar to just be higher than the standard?   Nana: Hi Dr. Cabral, Thanks to your content and your team's support, i have been able to resolve a lot of my pre existing conditions and i am currently now doing the cbo finisher after completing the cbo protocol to heal my gut. However, one thing i havent been able to resolve over the years is my liver detox capabilities. I have trouble detoxing food chemicals like salicylates and even though taking the daily activated multivitamin helps, i am still unable to eat high salicylate foods. I can't even get through the equilife detox even when i try to titrate. My salicylate bucket fills really fast and i get super inflammed all over again. How can i support my liver to detox better? Should i minimize fat like ghee and red meat? What can i do to finally heal?   Bettina: Hi Dr. Cabral. Thank you for a great podcast and products. My husband is from Italy and when we gp there to visit family we are typically there for a minimum of 7 days. This means my mother in laws amazing pasta dishes lunch and dinner. What should I do in order to keep weight off? I bring my nutribullet so that I can make a smoothie with the DNS in it. But my mother in law is amazing in the kitchen and it is not well seen to day no to her food or even to eat very little food as there is often more dishes and they are amazing :-) I bring the digestive support but what else should I do? Even if I eat a smaller portion of the pasta and the eggplant lasagna etc. I am quite sure I will gain weight as this has happend each year so far. Thank for you advice in advance.   Nicole: Have any natural recommendations for broken capillaries on the face? Feel like I have tried it all except laser at this point. I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Carolyn: Hi, I stumbled across this show last week while searching the podcast app for podcasts about my problem. I feel like I found a treasure trove and am busily binge-listening to see what I can learn. My question is, how can I diagnose and address the root causes of my health issues if I live in a country where I can't buy Equilife testing kits or find functional medicine practitioners? I live in China. I can test blood nutrient and hormone levels in a local hospital, but that's about it. My symptoms include poor sleep quality (waking up at night), bloating after meals, chronic loose stools (no matter what I eat), susceptibility to yeast infections, difficulty losing fat (despite an active lifestyle and 95% whole-food diet), and chilblains on my hands. What should I do?   Vanessa: I am very thankful for the support and education you provide to so many. You are truly a blessing. My question is about Mycoplasma Pneumonia. I tested high for this infection over a year ago and have been on a few natural supplements to help lower it, biocidin being one of them. My numbers went down but it never went away. I have been working on healing from Autoimmune, Hashimotos, Mercury Toxicity and working on drainage, detoxing, balance deficiencies, ect. After coming off my supplements for a few months because I felt so good, the Mycoplasma pneu. increased quickly. Then, I started itching, coughing anytime I would laugh, and increased inflammation again. I could believe how quick it happened. I just finished the Organic Acids Test to check my gut as well. I was wearing a continuous blood glucose monitor and found that the daily shake kept my blood sugar elevated (120s-130s) 2-3 hrs post morning and afternoon shake. Check out today's questions:    Rob:  Hi Doc, me again. Sorry forgot to ask about my blood sugar. I've been wearing a CGM for almost 2 months now and I was shocked how I was basically pre-diabetic. Fasting glucose was about 120 and it could spike all the way up to 180 after eating my prep meal which could involve something like lentils, sweet potato and peas + olive oil. I have gone to a completely starch free diet where I'm eating plenty of cruciferous veg, pulses. 20g+ protein in every meal and healthy fats. No fruits, grains or starches such as sweet potato. I do have a cheat meal per week and anytime I try to add a starch, my blood glucose spikes again. Am I supposed to be on a starch free diet forever? Why is my fasting glucose still around over 100 when I'm not eating any starches? This is so confusing. Bearing in mind that I already get my 10k steps and I'm generally moving after my meals (washing dishes). Please help, I'm so lost.   Anonymous: Hello, I'm 38 years old and my Dexa Scan came back with upsetting results. I have -2.0 T score in my spine which is significantly decreased from 2 years ago. I am wondering if you know anything about CalZbone (Cissus quadrangularis) supplement or IMD supplements (which have an Oseto product containing this). I am worried about putting things into my body if I don't know about any potential side effects, especially long term. At the same time i'm working on my nutririon, i'm working with an endocrinologist for on my hormones and thyroid (which have been a mess for years), i'm also working on increasing the amount of calcium in my diet and making sure I have adequate Vitamin D and K.
