February 14, 2025 ~ Yesterday, RFK Jr. was confirmed as Secretary of Health and Human Services. Mark Fendrick, Professor in the Division of General Medicine, Department of Internal Medicine and Department of Health Management, and Policy at the University of Michigan, joins Kevin to react to this.
Hosted by RAPM Editor Eric Schwenk, MD, this episode of RAPM Focus welcomes RAPM Associate Editor Mark Bicket, MD, PhD, and medical student Sama Ramo following the recent publication of their original research paper, “Risk Factors for Persistent Postoperative Opioid Use: An Entity Distinct from Chronic Postsurgical Pain,” included in the February 2025 special edition of RAPM, titled “Tackling the Challenge of Chronic Postsurgical Pain.” Persistent postoperative opioid use results when patients continue to take prescription opioids well beyond the time their pain after surgery would be expected to resolve. In some cases, this is a couple days; in other cases, this is several weeks. There are many challenges associated with prescription opioid use, including the lack of a truly safe dose, so there is risk across the continuum. Patients facing persistent postoperative opioid use potentially confront worse outcomes, higher rates of health care utilization, and higher rates of mortality compared to patients not enduring persistent postoperative opioid use. Dr. Mark Bicket is an associate professor of anesthesiology with tenure at the University of Michigan and a joint appointment in the Department of Health Management and Policy at the School of Public Health. Additionally, he is co-director of the Overdose Prevention Engagement Network (OPEN). A clinician-scientist and practicing physician anesthesiologist, Dr. Bicket has pioneered evidence-based approaches to reduce opioid-related harms and improve the treatment of persons who experience pain. He currently leads an NIH R01 study examining pain-related outcomes for persons with opioid use disorder who have surgery and serves as PI for two pragmatic trials funded by the Patient-Centered Outcomes Research Institute including the CARES study, an international randomized clinical trial examining commonly prescribed treatments for postoperative pain. He previously served on the National Academy of Medicine ad hoc committee on evidence for opioid prescribing guidelines in 2018-2019 and a workshop on opioid disposal in 2023. Dr. Bicket has advised the White House, FDA, CMS, state governments, and national organizations on pain and opioid issues. His research, which includes 120 peer-reviewed articles, has appeared in JAMA, NEJM, and BMJ, and has been supported by the Foundation for Anesthesia Education and Research (FAER), SAMHSA, and CDC. Sama Ramo is a second-year medical student at Oakland University William Beaumont School of Medicine. She has earned a doctorate in Education from Columbia University in Behavior Science/Health Education, a Master's of Science in Psychiatric Nursing from The University of Medicine and Dentistry of New Jersey, and a Master's of Science in Policy and Health Management from the Wagner Graduate School of Public Service at New York University. As a Fulbright Scholar Specialist in Mental Health Dr. Marshall lived and taught psychiatric nursing in Malta. Dr. Marshall was the principal investigator on the US Department of Education's Model Programs on College Campuses grant (2006-8), conducting research at Montclair State University related to reducing alcohol use by freshmen students, and investigating the relationships between attitude and behavior and parental engagement and student drinking behavior. More recently, Dr. Marshall has had her research funded in the areas of evaluating outcomes of psychotherapy using fMRI imaging, investigating depression in parents of children with ASD, assessing the physical effects of depression on African Americans with diabetes and heart disease, examining the impact of Mental Health First Aid (MHFA) Training on healthcare workers in a post pandemic world, MHFA training for nursing students,and MHFA training for teachers, and providing education on Substance Use Disorders to all health care providers. Dr. Marshall has authored five books, numerous chapters in psychiatric nursing text books, and scores of articles. Minimum wage workers, especially those with children, face barriers to affordable child care. Child care costs can prevent working parents who earn minimum wage from participating in the labor market. Alternately, many child-care workers also face financial barriers because they, too, earn minimal wages. Therefore, increasing the minimum wage would alleviate financial burdens for both parents and child-care workers. In this episode, both Dr. Anna Godøy and Dr. Jennie Romich discuss their research on minimum wage and its effects on parental labor supply and the child care sector. Anna Godøy is a Senior Research Fellow at the Ragnar Frisch Center for Economic Research and an Associate Professor at the Department of Health Management and Health Economics at the University of Oslo, Norway. Her research interests include empirical labor economics, health economics, and policy evaluation. Dr. Jennie Romich is an Associate Professor of Social Work at the University of Washington, director of the West Coast Poverty Center, an active member of the Center for Studies of Demography and Ecology, and an IRP affiliate. Romich studies resources and economic well-being in families with a particular emphasis on low-income workers, household budgets, and families' interactions with public policy. On this week's episode of Rural Health Leadership Radio, we kick off a series focused on NRHA's 2023 Rural Health Fellows and their policy papers. The first group we chatted with includes Lisa Rantz, Kevin Lambing, and Dr. Carrie Shaver who had the task of examining Medicare Advantage. In our conversation we discuss the intricacies, impacts, as well as personal experiences that the Fellows uncovered in their research. “Get involved and advocate and educate on these issues. It will require all of us working together to create the change in policy and funding mechanisms that we need to ensure that our rural Healthcare is supported and continues to be strong.” – Dr. Carrie Shaver Kevin Lambing serves as the Senior Program Officer for Health at the T.L.L. Temple Foundation. In this role, he helps ensure East Texas children, adults, and families are healthy and have access to quality healthcare. He organizes and convenes stakeholders and develops community partnerships with potential health related grantees in East Texas, consisting of 22 rural counties and one primarily urban county, Harris County and Miller County, Arkansas (Texarkana area). In addition, he monitors federal and state legislation related to health with intentions for future gaps in funding health resources in rural East Texas. Kevin's 30-plus-year career in the Air Force culminated in his service as the Senior Advisor to the Air Force Surgeon General. In this role he defined, shaped, and executed health care policy in support of 45,000 personnel at 75 medical treatment facilities around the globe, ensuring a cost effective, patient-centered, and prevention-based healthcare continuum for 2.6M beneficiaries. He earned an Associate of Applied Science degree from the Community College of the Air Force, Bachelor of Science degree from Wayland Baptist University, a Master's of Business Administration degree from Webster University and is a 2023 National Rural Health Association Fellow. He is married to his wife Sheliea and they have 2 children, Noah and Lydia. Lisa Rantz is the Executive Director of Hilo Medical Center Foundation which serves as the Hawaii Island Area Health Education Center, focusing on health career recruitment from K-12 to graduate medical education. She is the President of the Hawaii State Rural Health Association and in 2023 joined the National Rural Health Association Board of Trustees. Lisa is a 2023 NRHA Fellow and has over fifteen years of experience in facilitation and focus groups in education and health care, Lisa has developed impactful programs to address rural health challenges and advocate for community needs. Her accolades include the 2020 Small Business Advocate of the Year, the 2021 Peacemaker Award, the 2022Athena Leadership Award, the 2023 Community Star recognition for Hawaii, and the 2024 NRHA Volunteer of the Year award. Dr. Shaver is an Assistant Professor of Health Management and Leadership at Texas Tech University Health Sciences Center and a founding member of the New Mexico Rural Health Association. Dr. Shaver has a strong background in the social service sector and has experience administering disease prevention, treatment, and health and wellness programs. She endeavors to bridge gaps between traditional silos in healthcare and foster collaboration among healthcare professionals through Systems Thinking. By embracing this holistic framework, she aims to contribute to the development of sustainable, inclusive, and equitable healthcare systems. Dr. Shaver's research centers around understanding and addressing the unique healthcare challenges faced by rural and frontier communities along the United States-Mexico border and countries within Latin America and the Caribbean.

Joe Paduda is the principal of Health Strategy Associates and the author of the thought-provoking Managed Care Matters blog. He is also the president of CompPharma, LLC, a research and consulting firm dedicated to improving pharmacy programs in workers' compensation. In 2012, Joe received IAIABC's President's Award for his efforts to address the opioid problem in workers' compensation.
