Understanding the physiological regulation of iron is crucial for interpreting ferritin results in context.Causes of HyperferritinaemiaRaised ferritin can result from: Hereditary haemochromatosis (genetic iron overload) Chronic inflammatory states (e.g., infection, autoimmune disease) Liver disease (e.g., alcoholic liver disease, hepatitis) Malignancy Metabolic syndrome It is important to consider these differential diagnoses when encountering an elevated ferritin[2].Hereditary Haemochromatosis: When to SuspectPrimary care clinicians should suspect hereditary haemochromatosis in patients with persistently raised ferritin and transferrin saturation, especially with suggestive symptoms (e.g., fatigue, arthralgia, diabetes, liver dysfunction) or family history[2]. Early recognition is key, as treatment can prevent complications.How to TestInitial investigations should include: Repeat ferritin measurement to confirm persistence Transferrin saturation (TSAT): TSAT >45% is suggestive of iron overload Liver function tests and assessment for other causes of raised ferritin[2] If hereditary haemochromatosis is suspected, genetic testing (HFE gene) should be considered.Who to Refer (and to Whom)Refer patients with confirmed iron overload (high ferritin and TSAT) or positive HFE mutation to hepatology or a relevant specialist for further assessment and management[2]. Referral is also warranted if there are signs of organ involvement (e.g., abnormal LFTs, diabetes, cardiac symptoms).Management PrinciplesFor hereditary haemochromatosis, mainstay of treatment is venesection (therapeutic phlebotomy) to reduce iron stores. Primary care plays a role in monitoring, supporting adherence, and managing comorbidities. For other causes, management is directed at the underlying condition.Practical Tips for Primary CareDo not ignore isolated raised ferritin-always interpret in clinical context. Exclude common secondary causes (infection, inflammation, liver disease) before pursuing rare diagnoses. Family screening may be appropriate in hereditary haemochromatosis cases[2]. Released April 10th, ‘Heresy' is the new single from Sydney metalcore quartet Harroway. The track takes a deep dive into the mind of vocalist Matt Banks. It sees Harroway explore a desire to escape from emotional pain and suffering. Banks pleads for relief, projecting undertones of existential questioning and a longing for peace or oblivion. Pulling from his experience living with Hemochromatosis, a disorder where the body creates excessive amounts of iron and becomes toxic, the single comes with a swirling sense of self resignation as Banks grapples with existential crisis, mental anguish, and the search for a meaning. The band have been on a national tour supporting metalcore legends Sienna Skies and No Home throughout April and May. Christy Sutton, DC's interests in asking why health problems occur, what the root causes are, and what are the safest and most effective solutions to health problems are the foundation of her writing, teaching, and clinical practice. I am starting to travel a lot for my job, and I am wondering if I should be concerned about going through the XRay machines at airport security several times a year? I am also curious if you think the check in using Clear (where they scan your retinas) are safe. I was considering signing up, but I wanted to get your thoughts first. Thanks! Ashley: Hello Dr. Cabral! I have been listening to your podcast and following your protocols since I was diagnosed with an autoimmune disease called undifferentiated connective tissue disease in 2018. I am currently doing the CBO protocol after completing a 7 day detox. I feel so much better and down about 7 pounds! I am about a week and a half in to the CBO protocol. My only issue is that I am experiencing constipation, loose stools, and afternoon fatigue. Also having so muscle pain at night that feels like “growing pains”. I was recently diagnosed with periampullary diverticulum of the gallbladder, and was told by my GI specialist that this might be why I have a hard time digesting seeds and nuts and probiotics would help. From personal experience I have struggled to find a decent probiotic that worked for me. I don't know much about this diagnosis. In February 2022 I did a candida test which was negative. I also did an igG 184 food panel and had lots of sensitivities which indicated leaky gut. In May 2022 I completed a SIBO test which was also negative. I'm just wondering if anyone had any experience with periampullary diverticulum of the gallbladder and if I should continue to follow the sensitive food guide as is when I know I don't have SIBO or candida overgrowths? Thank you! Amanda: Hi Dr. Cabral, I've listened to your methylation podcasts & know I have issues bc of genetic testing. The problem is, the SNPs I have showing red aren't the ones you mentioned in your podcast. I read that I should avoid methyl Bs because of having COMT/TXNRD2 A4251G TT risk alleles combined with COMT V158M AA risk alleles & COMT H62H TT risk alleles (all three are red for me) because this means I have trouble breaking down dopamine, epinephrine, norepinephrine and estrogen: "It has been clinically observed that the body may have trouble tolerating methyl donors (folate, B12, methionine, betaine, choline) with a COMT V158M + and a VDR Taq + status.” Also said with lower COMT activity to avoid supplements w/ catechol structure - quercetin, EGC/G, and luteolin (which you said help methylation). My B6 is low (0.96) but B12 looks okay (1.2) and I was advised to take your Activated B complex, but I'm nervous about the methyl Bs after reading above. My brother also has low B6 and vit C and has seizures if he ever misses doses of Keppra. My question is whether you know anything about these other SNPs, if you advise supplementing with B6 only, or if I can somehow get more genetics guidance through your practice?
