Dr. Melissa Kalt, M.D. specializes in removing the trauma of narcissistic abuse from the body. She brings so much insight to the topic, and I thoroughly enjoyed interviewing her for our audience. This interview is extremely informative and helpful. Dr. Melissa is a graduate of the Medical College of Wisconsin – triple-board-certified in Internal Medicine, Pediatrics, and Lipidology. She was voted to the US Best Doctors™ list by her peers year over year 2009-2018. While single parenting five children, Dr Melissa was promoted from staff physician to Assistant Clinical Professor to Medical Site Director of three clinics at Froedtert and Medical College of Wisconsin, an academic medical institution. By 2012, Dr Melissa had achieved every goal on her over-achiever to-do list yet was miserable. She began an extraordinary journey that solved her most unsolvable problem. It all started with the question - What if life doesn't have to be this way? The answer - Connect to the truth of who you are. Dr Melissa got really clear about who she was, who she needed to be, and what needed to change. She identified, then extracted herself from several narcissistic relationships and committed to healing her family's trauma. By extracting the illusion and getting clear about the truth of who she was – everything changed. In 2018, Dr Melissa made the leap to expand beyond the limits of traditional medicine and left her 20+ year medical career to follow her Why. That same year, she found her forever partner in love and business, whom she married in 2019.

Centralized distribution models can help health systems streamline day-to-day operations, including mitigating drug shortages, and maximizing contract compliance and efficiency. Jake Olson, 340B pharmacy manager at Froedtert Memorial Lutheran Hospital in Milwaukee, Wisconsin, joins us to discuss how centralized distribution models have operationalized Froedtert's 340B program. Mitigating Drug ShortagesInventory optimizing helps ensure organizations meet their resource demands. Jake discusses minimizing repetitions within an organization's entities by centralizing supply ordering to a single location. For Froedtert, this means purchasing bulk drugs to one large warehouse and then redistributing among hospitals. Contract ComplianceFroedtert's 60,000 square foot drug distribution warehouse is centrally located in Milwaukee. There, Jake works alongside Froedtert's contract manager, a set up that simplifies in-person communication regarding the complex factors of compliance, negotiation, and management. The process boosts speed, efficiency, and clarity among the team. Patient CareJake and Froedtert consider centralized distribution models essential for improving patient care. While Jake's model may not suit every health system, he believes organizations can adapt distribution methods to their unique needs, simplifying vendor, client, and inventory management for better efficiency and improved patient care. This is the last of 3 interviews from listeners of the podcast. Lynn from Minnesota shares her journey. Here is some of what she had to share."It's that whole moderation aspect or the balance aspect. I used to joke that having fibromyalgia was like living between two bookends on a narrow shelf. It sounds limiting, and I don't mean to make it sound that way, but I have found that I must be so intentional. I pay attention to everything. Fortunately, I have a disciplined personality, which works for me.You also commented about the brains of fibro people sometimes coming alive at nine o'clock or at the end of the night. And I went, yes. I was yelling out while listening to that. So again, that was just another example of something new that I had learned. When I heard your podcast, it was more like, Wow.Here's a doctor that pays attention, that cares. And here are the stories of these other people. I'm truly not alone. And then hearing what other people have done for success. So often mimicking what I've already learned, but it was confirmation that I had approached things in a good way, and I knew that."But just to hear and have that, I guess connection with other people is something that is lacking when you are just living in an environment where all your peers or family members are totally healthy. It's that sense of hope."Froedtert & the Medical College of WisconsinTosa Health Center1155 N. Mayfair RdWauwatosa, WI 532261-414-777-7700 Pelvic Sense. We explore how Andres Gonzales, vice president of community engagement and chief diversity officer at Froedtert Health, is making a lasting impact on the health care system through his passion for creating equity. He discusses his journey from Puerto Rico to Cleveland and how he has used his lived experiences to form initiatives that empower all communities. Gonzalez also shares about how Froedtert's transformation into an anti-racist health care system is being driven by conversations with community members and stakeholders. Don't miss this engaging episode that reminds us that we all play a part in equity and engagement.Episode Highlights11:56 - If you look at historically how things have transpired in the U.S., at the core of all these issues, or the ills … is really systemic racism and discrimination. And we know that health care is not exempt. We're not obviously immune to that. And so for us, it was about acknowledging that individually, organizationally, but then admitting that openly to our community. And that vulnerability became extremely important.15:36 - One of the things that we heard loud and clear right away from the community was, "You guys cannot be the ones kind of leading the way. It would be great if you can actually empower us, give us some speaking points, and allow some of our trusted members of the community to be the ones that are carrying that message, because they obviously have earned that right."21:46 - I think that that's what fills me every morning, that I know that we can actually effect change, and even if it's in one life that we touch and we can make a difference, that's a win for us. That's actually someone who we impacted positively.Connect with Becky Dubin JenkinsLinkedInConnect with Andres GonzalesLinkedInGuest Bio:Andres Gonzalez, vice president of community engagement and chief diversity officer for Froedtert Health, is responsible for overseeing the integration of diversity initiatives throughout the organization, including expanding workforce diversity and strengthening community relationships. In our latest episode, Andres Gonzalez, the vice president of community engagement and chief diversity officer at Froedtert and the Medical College of Wisconsin, discusses how to be a catalyst for change and lifts Milwaukee as a place where organizations are intentionally investing in the community. Gonzalez also says that, too often, people in positions of power try to prescribe solutions to communities without first understanding their lived experiences and that this undermines trust. Finally, Gonzalez shares personal stories about what it was like being uprooted from his beloved home country of Puerto Rico to move with his family to Cleveland and how he supported his mom as she attempted to access health care with limited English skills.Episode Highlights11:31 - It's about building trust and understanding what's the lived experience in the community. So, again, we understand what is happening, what people are living through, and then we have a better insight and understanding of what we might be able to actually offer. 21:09 - Let's unpack the pledge. Let's understand what we're signing on to here and then let's make sure that we stay the course and that we are accountable, and also transparent about our journey and how we're either delivering or not on those efforts.33:31 - I oftentimes remind a lot of the folks that I'm mentoring because I'm actually right now in the face of my career where I can pay it forward, but I'm trying to build the next cadre or leaders that will probably pick up the mantle and run with it, and we'll continue this work forward.Connect with Tammy Belton-DavisLinkedInConnect with Andres GonzalezLinkedIn

Midweek special episode!! Dr. Melissa Kalt, M.D. specializes in removing the trauma of narcissistic abuse from the body. She brings so much insight to the topic, and I thoroughly enjoyed interviewing her for our audience. This interview is extremely informative and helpful. To further pursue work with her, visit here. Check out her program here!! --- Support this podcast: https://anchor.fm/covertnarcissism/support

Tom Robertson, Executive Director of the Vizient Research Institute and Cathy Jacobson, President and CEO of Froedtert Health in Milwaukee return to discuss macroeconomic challenges facing the U.S. health care system rooted in its Byzantine financing system involving counter-productive cross-subsidies between payers and a historic imbalance in payments across clinical services. They consider the merits of systemic changes reducing payment rate disparities, and the potential advantages of a future in which providers were payer agnostic and more empowered to invest in traditionally under-funded areas such as mental health and primary care. Cathy shares Froedtert's initiatives to identify adverse social determinants of health early and a community-wide partnership that uses a single tool to curate social services resources for medical providers. Guest speaker: Catherine A. Jacobson, FHFMA, CPA President and CEO Froedtert Health Moderator: Tom Robertson Executive Director Vizient Research Institute Show Notes: [1:00] Health care finance is like a Jenga Game – the reimbursement system is Byzantine and doesn't make sense [2:20] Health systems make their entire margins on very few things- they lose money on other things that they must do but are paid for by the former. [4:13] Health care needs to move away from the current reimbursement system [9:17] It would be far better if pricing was the same for all payers, government and private, and we competed on access, service and quality rather than price [12:15] We get what we pay for – if we don't pay for it, we don't have it. (Example: behavioral health, family medicine) [14:42] Price leveling would better enable providers to address social determinants of health [16:52] Froedtert approach to mitigating social determinants of health: 1. Identify affected patients much earlier and more consistently than we did before, 2. Created a community-wide partnership to curate all available social services resources in the market by using one common software tool [19:09] Health care organizations are economic anchors in the community [20:00] Froedtert creating educational grants targeted at effective high schools and especially vulnerable populations [21:40] If revenue was more fungible, would invest heavily in behavioral health Links | Resources: Cathy Jacobson's Biographical Summary Click here Subscribe Today! Tom Robertson, Executive Director of the Vizient Research Institute sits down with Cathy Jacobson, President and CEO of Froedtert Health In Milwaukee, Wisconsin to discuss everything from agile decision-making during the COVID pandemic to deliberate system rationalization of clinical resources, including program consolidation and efficient utilization of both tertiary and lower acuity facilities. As a long-time chief financial officer, Cathy shares insights around economic incentives and the importance of avoiding inadvertent structural impediments to doing the right things. Guest speaker: Catherine A. Jacobson, FHFMA, CPA President and CEO Froedtert Health Moderator: Tom Robertson Executive Director Vizient Research Institute Show Notes: [1:13] America has the best health care in the world, but not the best health care system. (Cost, quality, and access for all citizens of the United States) [1:13] We do best in science, innovation, treatment and education [04:08] Hospital systems must have local scale to accomplish rationalization of services; provider consolidation across distant markets does not serve that purpose [10:05] The pandemic led to command centers that made decision-making more agile; Froedtert will keep the “recovery command” infrastructure in place moving forward because decisions are “deliberate but at speed” [12:43] Unexpected consequence of the pandemic: Froedtert advanced the pace of their long-term cost reduction goals by one year [14:16] During the pandemic shutdown, Froedtert management intentionally thought about what new structures to keep after the pandemic (i.e. organizational change and enhanced communication) [17:05] Video visits are down from mid-pandemic peaks; Froedtert targeting areas to re-establish more virtual [18:24] When patients are seen in video visits, not as many lab tests and x-rays are done [19:21] When volume comes back, it's going to come back sicker and worse – we're going to see double-up rebound on Medicare payer mix. [23:05] For systems to work, it's essential not to inadvertently create financial incentives that impede good people doing the right things; at Froedtert, individual hospitals are not evaluated on their own performance, everyone rides on the system's bottom line.
