Preterm infants are at risk for a condition called necrotizing enterocolitis, which results in inflamed intestines, bacterial infection and damage to the colon and intestines. Without proper care, the infant may die.Pasteurized donor human milk helps offset these potential dangers. Unfortunately, some mothers of preterm infants cannot produce their own milk. That's where Mothers' Milk Bank of Louisiana steps in.As of August 2020, Louisiana provides Medicaid coverage for using pasteurized donor human milk in a hospital. About the speaker: Dr. Mason Rice is the director of the Cyber Resilience and Intelligence Division at Oak Ridge National Laboratory. In this role, he is responsible for an R&D portfolio focused on advanced intelligent systems and resilient cyber-physical systems, including research into (1) AI for national security, (2) cybersecurity for critical systems, (3) machine-augmented intelligence, (4) vulnerability science, and (5) identity science.Following retirement from the Army, Dr. Rice joined ORNL in 2017 as the Cyber-Physical R&D Manager and was soon appointed as the first Group Leader for Resilient Cyber-Physical Systems at ORNL. He ultimately grew the group into four focused research groups, at which point he was selected to be the first Section Head of the new Resilient Cyber-Physical Systems Section.

Over the last few decades, the roles of nurse practitioners (NPs) and physician assistants (PAs), also known as advanced practice providers (APPs), have greatly expanded to touch even more patient-family lives. They are an essential part of the care we provide, and their roles have evolved over time. In this episode, we explore the crucial part they play in our medical system and how that might continue to evolve in the future. For this episode, we are joined by two University of Colorado leaders who continue to help keep our APP programs among the top ranked in the country. Jonathan Bowser is the Child Health Associate and Physician Assistant Program Director at the University of Colorado School of Medicine. He is also the Associate Dean of Physician Assistant Studies, the Section Head of Pediatrics, and an associate professor. Jill Marks, NP, is the Specialty Director of the Acute Care Pediatric Nurse Practitioner Program at the University of Colorado College of Nursing. Dr. Habsha is a Professor at George Brown College of Applied Arts and Technology and is the On-staff Prosthodontist at MedCan clinic in Toronto. She holds an appointment as Staff Prosthodontist at Mount Sinai Hospital where she instructs the dental residents and is involved in various clinical research projects. Dr. Habsha is an Associate Fellow of the Academy of Prosthodontics and Greater New York Academy of Prosthodontics, a Fellow of The Pierre Fauchard Academy and holds memberships in numerous Prosthodontic organizations and societies. Upon graduation, she completed a one-year General Practice Residency at Mount Sinai Hospital in Toronto, ON. Dr. Habsha received her Diploma in Prosthodontics and Master of Science degree, both from the University of Toronto. She is a Fellow of the Royal College of Dentists of Canada (RCDC) and is an examiner and past Section Head for the Oral Examination in Prosthodontics for the RCDC. Dr. Habsha is an Adjunct Assistant Professor at the Department of Dentistry, Eastman Institute for Oral Health at the University of Rochester Medical Center. She has served as an Assistant Professor at the University of Toronto and currently instructs at the graduate level in Prosthodontics at U of T. Dr. Habsha is a Professor at George Brown College of Applied Arts and Technology and is the On-staff Prosthodontist at MedCan clinic in Toronto. She holds an appointment as Staff Prosthodontist at Mount Sinai Hospital where she instructs the dental residents and is involved in various clinical research projects. Dr. Habsha is an Associate Fellow of the Academy of Prosthodontics and Greater New York Academy of Prosthodontics, a Fellow of The Pierre Fauchard Academy and holds memberships in numerous Prosthodontic organizations and societies. Jaw pain, headaches, and facial pain are all common and can have a profound effect on our quality of life. Lucky for us, we have an esteemed guest speaker on the show today to break these topics down and help us understand how we can overcome this pain.Donald R. Tanenbaum, DDS, MPH, graduated from the University of Pennsylvania in 1978 and then received his DDS degree from Columbia University School of Dental Medicine, and his MPH degree from Columbia University School of Public Health in 1982. He has been practicing for over 40 years, providing guidance and care to patients with temporomandibular problems, facial pain, headaches and sleep related breathing disorders. Today he reviews some of the causes and treatments for these issues and explains their biopsychosocial nature. He also discusses the importance of identifying habits that can perpetuate pain and stop or modify them. He touches on bite plates, injections, PT, posture and ergonomics as all ways to alleviate pain.Dr. Tanenbaum has co-authored a book for the public entitled, ‘Dr, Why Does My Face Still Ache?” and continues to give lectures to dental students and residents, medical grand rounds and to numerous dental societies and study clubs. Research has shown that homicide is the second-leading cause of pregnancy related-deaths. This episode sheds light on a troubling rate of homicides involving the pregnant and postpartum population in the United States, with a particular spike in 2020. Dr. Veronica Gillispie-Bell, a board-certified OBGYN, joins the discussion to unpack the findings of her research. Some of those key findings showcase the disproportionate impact on Black women and girls, the prevalence of firearms in these homicides, and the alarming fact that pregnant and postpartum women face a 35% greater risk of homicide than their non-pregnant counterparts. The evaluation and treatment of children with obesity has evolved, and how we handle these diagnoses today is different than in years past. What's important to emphasize is that obesity is a chronic disease, and the medical community needs to treat it as such. To learn more about this important topic and highlight an impressive woman in medicine, we are joined by Ihuoma Eneli, MD. Weeds are a constant fact of a gardener's life. As our guest, Toni DiTommaso, says we can count on three things in life: death, taxes, and weeds. But if it weren't for weeds, the world would have less topsoil than it has now, and humanity might have suffered mass starvation by now. Why? Because the plants we call weeds do a vital job in ecosystems: they quickly establish in, protect, and restore soil that has been left exposed by natural and human-caused disturbances. That said, weeds, in addition to being a nuisance, cause more yield loss worldwide and add more to farms' production costs than insect pests, crop pathogens, root-feeding nematodes, or warm-blooded pests (rodents, birds, deer, etc.). So perhaps, we should start this discussion with a definition. What exactly is a ‘weed'? The Oxford Dictionary defines a weed as a wild plant growing where it is not wanted and in competition with cultivated plants. Similarly, the Miriam-Webster dictionary describes it as a plant that is not valued where it is growing and is usually of vigorous growth. Penn State Extension tells us that weeds compete with other plants for nutrients, water, and light, as well as potentially harbor diseases and pests. Simply put, a weed is a plant ‘out of place'. Plants that are characterized as weeds can reproduce via seeds, rhizomes, cuttings or runners. Their seeds are typically plentiful and tiny which can disperse easily and/or remain dormant for many years. They also can grow in less-than-ideal environments, soils, and conditions. As any gardener knows, they tend to grow fast and can outcompete other plants. Weed management can also be frustrating as some of these plants can break off and re-sprout or self-pollinate if pulled out. The best place to begin is to learn how to identify weeds. In this episode, we are fortunate to hear from Antonio (Toni) DiTommaso, Professor and Section Head, School of Integrative Plant Science Soil and Crop Sciences Section at Cornell University's College of Agriculture and Life Sciences (CALS). The focus of his research is to gain a more in depth understanding of the basic biological and ecological principles governing weeds to find safe, effective, sustainable and economically viable weed management strategies. In addition to his teaching and research duties, he is also a co-author of the book, Weeds of the Northeast.The fully updated second edition provides detailed illustrations for easy identification of more than 500 common and economically important weeds in the Northeast including New York.A practical guide, it includes a dichotomous key as well as descriptions and photos of floral and vegetative characteristics, giving anyone who works with plants the ability to identify weeds before they flower. Comparison tables make it easy to differentiate between many closely related and similar species. The following question refers to Section 7.2 of the 2022 AHA/ACC/HFSA Guideline for the Management of Heart Failure. The question is asked by Cleveland Clinic internal medicine resident and CardioNerds Intern Akiva Rosenzveig, answered first by UPMC Harrisburg cardiology fellow and CardioNerds Academy House Faculty Leader Dr. Ahmed Ghoneem, and then by expert faculty Dr. Randall Starling. Dr. Starling is Professor of Medicine and an advanced heart failure and transplant cardiologist at the Cleveland Clinic where he was formerly the Section Head of Heart Failure, Vice Chairman of Cardiovascular Medicine, and member of the Cleveland Clinic Board of Governors. Dr. Starling is also Past President of the Heart Failure Society of America in 2018-2019. Dr. Staring was among the earliest CardioNerds faculty guests and has since been a valuable source of mentorship and inspiration. Dr. Starling's sponsorship and support was instrumental in the origins of the CardioNerds Clinical Trials Program.  The Decipher the Guidelines: 2022 AHA / ACC / HFSA Guideline for The Management of Heart Failure series was developed by the CardioNerds and created in collaboration with the American Heart Association and the Heart Failure Society of America. It was created by 30 trainees spanning college through advanced fellowship under the leadership of CardioNerds Cofounders Dr. Amit Goyal and Dr. Dan Ambinder, with mentorship from Dr. Anu Lala, Dr. Robert Mentz, and Dr. Nancy Sweitzer. Question #27 Which of the following sentences regarding diuretics in the management of heart failure is correct? A In HF patients with minimal congestive symptoms, medical management with diuretics alone is sufficient to improve outcomes. B Prescribing a loop diuretic on discharge after a HF hospitalization may improve short term mortality and HF rehospitalization rates. C The combination of thiazide (or thiazide-like) diuretics with loop diuretics is preferred to higher doses of loop diuretics in patients with HF and congestive symptoms. D The maximum daily dose of furosemide is 300 mg. Answer #27 Explanation Choice B in correct. The guidelines give a Class 1 recommendation for diuretics in HF patients who have fluid retention to relieve congestion, improve symptoms, and prevent worsening heart failure. Recent data from the non-randomized OPTIMIZE-HF (Organized Program to Initiate Lifesaving Treatment in Hospitalized Patients with Heart Failure) registry revealed reduced 30-day all-cause mortality and hospitalizations for HF with diuretic use compared with no diuretic use after hospital discharge for HF. Choice A is incorrect. With the exception of mineralocorticoid receptor antagonists (MRAs), the effects of diuretics on morbidity and mortality are uncertain. As such, diuretics should not be used in isolation, but always combined with other GDMT for HF that reduce hospitalizations and prolong survival. Choice C is incorrect. The use of a thiazide or thiazide-like diuretic (e.g., metolazone) in combination with a loop diuretic inhibits compensatory distal tubular sodium reabsorption, leading to enhanced natriuresis. In a propensity-score matched analysis in patients with hospitalized HF, the addition of metolazone to loop diuretics was found to increase the risk for hypokalemia, hyponatremia, worsening renal function, and mortality, whereas use of higher doses of loop diuretics was not found to adversely affect survival. The guidelines recommend that the addition of a thiazide (e.g., metolazone) to treatment with a loop diuretic should be reserved for patients who do not respond to moderate- or high-dose loop diuretics to minimize electrolyte abnormalities (Class...

The Osteosarcoma Institute (OSI) is currently supporting two clinical trials and seven laboratory research projects for osteosarcoma. In this OsteoBites episode, you will meet Lee J. Helman, MD, Director of the OSI and Chand Khanna, DVM, PhD, Chair of the OSI's Strategic Advisory Board. Dr. Helman and Dr. Khanna will provide an overview of the OSI's osteosarcoma research portfolio and explain how the OSI identifies promising osteosarcoma research projects. Lee J. Helman, MD has been studying the biology and caring for pediatric patients with sarcomas for over thirty years. Dr. Helman completed his post-doctoral training at the National Cancer Institute (NCI). He then became Head of the Molecular Oncology Section, Pediatric Oncology Branch, NCI, in 1993. He served as Chief of the Pediatric Oncology Branch from 1997-2007 and served as Scientific Director for Clinical Research in the Center for Cancer Research, National Cancer Institute from 2007 to 2016. He joined Children's Hospital Los Angeles (CHLA) and the University of Southern California (USC) in 2017 as the Section Head of Basic and Translational Research within the Cancer and Blood Disease Institute (CBDI) and the Division of Hematology, Oncology and Blood and Marrow Transplantation. He remains an adjunct professor at CHLA. He has also trained many investigators in the field of pediatric sarcomas over the course of his career. He is currently focusing on improving outcomes in osteosarcoma as the Director of the Osteosarcoma Institute. Chand Khanna, DVM, PhD is Chief Science Officer with Ethos Veterinary Health, and President of Ethos Discovery(501c3), its incubator of scientific innovation. His responsibility at Ethos Discovery is to develop innovations in the form of novel diagnostics and therapeutics for both human and animal patients afflicted with similar complex disease conditions (i.e., osteosarcoma; so called Comparative Oncology). Dr. Khanna is a veterinary oncologist and both osteosarcoma and metastasis biologist, who has worked in this scientific field for over 20 years, most notably as a senior investigator of the Tumor and Metastasis Biology section of the Pediatric Oncology Branch at the National Cancer Institute and founding Director of its Comparative Oncology Program. Dr. Khanna's research career has focused on improving the understanding of the biology of metastasis in osteosarcoma and the use of a cross species (mouse, dog, human), Comparative and translational approach to develop novel therapeutics for osteosarcoma metastasis. Episode 154: Orthopedic surgeon Brian Cole discusses advances in the treatment of knee, elbow and shoulder injuries

Today we have Dr. Brian Cole, an orthopedic surgeon who specializes in cartilage restoration, orthobiologics, and advanced surgical techniques for the treatment of knee, elbow, and shoulder injuries. He is the team physician for the NBA's Chicago Bulls and the co-team physician for the Chicago White Sox. He also is the host of the Sports Medicine Weekly Podcast. Brian practices orthopedic sports medicine at Midwest Orthopaedics. He also is a professor of Orthopaedics, Anatomy and Cell Biology at Rush University Medical Center in Chicago. He is Managing Partner of Midwest Orthopaedics and is the department's Associate Chairman and the Section Head of the Cartilage Research and Restoration Center. In addition to this work, he also serves as the Chairman of Surgery at Rush Oak Park Hospital. In today's interview, we talk to Brian about his cutting-edge research into ways to treat knee, shoulder, and elbow injuries.  Brian shares his novel approach to dealing with ACL tears, one of the most common sports injuries, and his investigations of methods to enhance the healing and recovery time following ACL reconstructions. He also talks about new advances in minimally invasive surgical techniques for many common injuries.  We have a particularly interesting conversation with Brian about exciting developments in the use of stem-cell treatments as well as the use of bone marrow aspirate to treat injuries. Show notes: [00:03:53] Marcas opens the interview mentioning that Brian was in the eighth grade when he fell in love with a popular sit-com from the 1970s,  “The Bob Newhart Show.” Marcas asks Brain what he loved about the show and what impact it had on him. [00:05:07] Brian enrolled in the University of Illinois after graduating from high school. Marcas asks Brian if knew he wanted to major in biology and psychology when he arrived on campus. [00:05:58] Ken mentions that after Brian's undergrad, he travelled upstate to the University of Chicago, where he earned an MD and an MBA. Ken asks what led Brian to pursue both an MD and MBA. [00:09:52] Ken explains that after the University of Chicago, Brian moved to New York City for an orthopaedic research fellowship in metabolic bone disease at the Hospital for Special Surgery. Brian also decided to do his residency there as well. Ken asks how that came about. [00:11:31] Marcas mentions that after Brian finished his fellowship and residency, he went to the University of Pittsburgh for a sports medicine fellowship. Marcas asks what led Brian there and what drove his interest in sports medicine. [00:13:10] Marcas asks Brian about a fortuitous phone call he received when he was a fourth-year resident. [00:14:34] Ken explains that Midwest Orthopaedics is one of the nation's most respected private orthopaedic practices.  Ken notes that through a partnership with Rush University Medical Center, Midwest has developed a national reputation as a leader in sports medicine; hip, knee, spine, and cartilage restoration; as well as shoulder care and pain management. Rush also is an academic medical center that includes a 671-bed hospital and is a center for basic and clinical research. Ken asks Brian to describe the scope of the work that goes on at Midwest and Rush. [00:17:20] Marcas comments that Brian is also the head team physician for the Chicago Bulls and the co-team physician for the Chicago White Sox, and asks Brian to describe some of the work that he does in that capacity. [00:20:09] Marcas explains that Brian treats a wide range of patients with injuries and pain, from athletes to non-athletes, and from children to senior citizens, and that he has performed more than 20,000 surgeries over the course of his career. Marcas asks Brian to give a sense of the patients he sees and what his average day at the office is like. [00:24:00] Ken points out that Brian is known for focusing on treating the patient and not the x-ray or MRI.

Helman, MD has been studying the biology and caring for pediatric patients with sarcomas for over thirty years. Dr. Helman completed his post-doctoral training at the National Cancer Institute (NCI). He then became Head of the Molecular Oncology Section, Pediatric Oncology Branch, NCI, in 1993. He served as Chief of the Pediatric Oncology Branch from 1997-2007 and served as Scientific Director for Clinical Research in the Center for Cancer Research, National Cancer Institute from 2007 to 2016. He joined Children's Hospital Los Angeles (CHLA) and the University of Southern California (USC) in 2017 as the Section Head of Basic and Translational Research within the Cancer and Blood Disease Institute (CBDI) and the Division of Hematology, Oncology and Blood and Marrow Transplantation. He remains an adjunct professor at CHLA. He has also trained many investigators in the field of pediatric sarcomas over the course of his career. He is currently focusing on improving outcomes in osteosarcoma as the Director of the Osteosarcoma Institute. As the Osteosarcoma Institute Operations Manager, Chelsey Blanks schedules meetings for the board, oversees various operational aspects of the organization (including HR, IT, and office management), facilitates the grant cycle, and manages OSI Connect (formerly OSI Hotline) that offers free expert advice for osteosarcoma patients and families no matter where they are in their journey. The following question refers to Section 7.4 of the 2022 AHA/ACC/HFSA Guideline for the Management of Heart Failure. The question is asked by New York Medical College medical student and CardioNerds Intern Akiva Rosenzveig, answered first by Cornell cardiology fellow and CardioNerds Ambassador Dr. Jaya Kanduri, and then by expert faculty Dr. Randall Starling.Dr. Starling is Professor of Medicine and an advanced heart failure and transplant cardiologist at the Cleveland Clinic where he was formerly the Section Head of Heart Failure, Vice Chairman of Cardiovascular Medicine, and member of the Cleveland Clinic Board of Governors. Dr. Starling is also Past President of the Heart Failure Society of America in 2018-2019. Dr. Staring was among the earliest CardioNerds faculty guests and has since been a valuable source of mentorship and inspiration. Question #6 Mr. D is a 50-year-old man who presented two months ago with palpations and new onset bilateral lower extremity swelling. Review of systems was negative for prior syncope. On transthoracic echocardiogram, he had an LVEF of 40% with moderate RV dilation and dysfunction. EKG showed inverted T-waves and low-amplitude signals just after the QRS in leads V1-V3. Ambulatory monitor revealed several episodes non-sustained ventricular tachycardia with a LBBB morphology. He was initiated on GDMT and underwent genetic testing that revealed 2 desmosomal gene variants associated with arrhythmogenic right ventricular cardiomyopathy (ARVC). Is the following statement true or false? “ICD implantation is inappropriate at this time because his LVEF is >35%” True   False   Answer #6 Explanation This statement is False. ICD implantation is reasonable to decrease sudden death in patients with genetic arrhythmogenic cardiomyopathy with high-risk features of sudden death who have an LVEF ≤45% (Class 2a, LOE B-NR). While the HF guidelines do not define high-risk features of sudden death, the 2019 HRS expert consensus statement on evaluation, risk stratification, and management of arrhythmogenic cardiomyopathy identify major and minor risk factors for ventricular arrhythmias as follows: Major criteria: NSVT, inducibility of VT during EPS, LVEF ≤ 49%. Minor criteria: male sex, >1000 premature ventricular contractions (PVCs)/24 hours, RV dysfunction, proband status, 2 or more desmosomal variants. According to the HRS statement, high risk is defined as having either three major, two major and two minor, or one major and four minor risk factors for a class 2a recommendation for primary prevention ICD in this population (LOE B-NR). Based on these criteria, our patient has 2 major risk factors (NSVT & LVEF ≤ 49%), and 3 minor risk factors (male sex, RV dysfunction, and 2 desmosomal variants) for ventricular arrhythmias. Therefore, ICD implantation for primary prevention of sudden cardiac death is reasonable. Decisions around ICD implantation for primary prevention remain challenging and depend on estimated risk for SCD, co-morbidities, and patient preferences, and so should be guided by shared decision making weighing the possible benefits against the risks,

Marta has an interesting background: advanced degree in physics, Master in international relations at the Fletcher School, management studies at the INSEAD Business School and recently completed the Visionary Leadership Programme at Ashoka. She started her career in an engineering company, worked for many years at the International Atomic Energy Agency in a number of technical and management positions. She and Dr. Hoffman then encourage listeners to acknowledge the role of optimizing factors such as table height, body posture, and more in order to maximize the longevity of a surgeon's career and thus patient care. The co-hosts then cover pitfalls commonly identified in normal, open surgery (e.g. excessive neck flexion, truncal deviation, arm abduction) and laparoscopic surgery (e.g. body posture, monitor height). They transition to highlight possible resolutions to these pitfalls, including anti-fatigue mats, appropriate footwear, and proper posturing; ultimately addressing a need for further research in this aspect of medicine. Lastly, they cover the role of robotic surgery and the potential to either mitigate or worsen the risk for work-related MSK injuries.
