A recent #study from Assiut University Hospital in Egypt, published in #Oncotarget, presents a promising strategy for patients with metastatic #colorectalcancer (mCRC). The #research introduces a gentler yet effective maintenance therapy that may extend survival, enhance quality of life, and offer a more accessible treatment option for mCRC patients worldwide. The Challenge of Treating Metastatic Colorectal Cancer Colorectal cancer is one of the most common causes of cancer-related deaths worldwide. When it spreads to other parts of the body—a stage known as mCRC—it becomes much more difficult to treat. At this stage, clinicians often use strong drug combinations like FOLFOX or CAPOX, which mix chemotherapy drugs to stop cancer growth. FOLFOX combines three drugs given intravenously, while CAPOX includes two of the same drugs, with one taken as a pill. While effective, these treatments can cause serious side effects. For example, one of the main drugs, oxaliplatin, can lead to nerve damage, making it painful or difficult to use the hands and feet. Fatigue, diarrhea, and other issues are also common. Over time, these side effects may force clinicians to stop or adjust the treatment, even if it is working. That is where maintenance therapy comes in. After the cancer is controlled, clinicians often switch to a gentler treatment plan to keep it from returning. The challenge is finding a therapy that continues to work without causing too many side effects, especially in places where access to expensive or intensive treatments is limited. BUFFALO, NY - February 18, 2025 – A new #researchpaper was #published in Oncotarget, Volume 16, on February 12, 2025, titled “Could Panitumumab with very low dose Capecitabine be an option as a maintenance regimen." In this study, researchers Doaa A. Gamal, Aiat Morsy, and Mervat Omar from Assiut University Hospital, evaluated a new maintenance treatment for metastatic colorectal cancer (mCRC). Their findings suggest that a combination of two drugs—Panitumumab, a targeted therapy that blocks a protein called epidermal growth factor receptor to slow cancer growth, and low-dose Capecitabine, a chemotherapy drug that converts into 5-fluorouracil (5-FU) inside the body to stop cancer cells from growing and dividing—could help extend survival in patients with mCRC. This regimen appears to be both effective and well-tolerated, especially for patients with wild-type KRAS mCRC who had previously responded to treatment. Colorectal cancer is one of the leading causes of cancer-related deaths worldwide. Standard treatment often involves a combination of chemotherapy and targeted therapies, but many patients face challenges related to treatment toxicity and resistance, which can lead to treatment interruptions. This study tested whether a lower-intensity maintenance treatment could help keep the cancer under control after initial treatment. The study involved 25 mCRC patients with wild-type KRAS and BRAF, who first received six rounds of standard 5-FU-based chemotherapy with Panitumumab. Patients who responded well then switched to a maintenance treatment of Panitumumab every two weeks and a low, continuous dose of Capecitabine. The results showed that patients had a median progression-free survival of 18 months and a median overall survival of 45 months, indicating a strong potential benefit. Patients with metastases detected at the same time as the primary tumor showed a longer progression-free survival than those with metastases appearing later. The treatment was also well tolerated, with only 8% of patients experiencing severe side effects such as skin rash or diarrhea, which were managed with standard treatments. "In our research, the toxicity profile was very acceptable, and no patients needed to stop treatment or had a dose modification due to toxicity." Finding a way to keep cancer under control while reducing side effects is a major goal in cancer treatment. While other maintenance therapies like Bevacizumab and Cetuximab have been studied, this research suggests that Panitumumab with low-dose Capecitabine could be a promising new option. Panitumumab is already an FDA-approved drug, but its role in maintenance therapy had not been extensively explored. The results of this study suggest that this combination may help delay disease progression while keeping side effects manageable, ultimately improving patients' quality of life. Although larger studies are needed, these findings open the door for further clinical trials to confirm the benefits of this regimen. If validated, this approach could change the standard of care for mCRC patients, particularly those who cannot tolerate more intensive chemotherapy. While that's true, she emphasises that hard work is essential; one must give their 100% to whatever they do and never underestimate yourself. "You don't know what will happen to you, you never know who is watching you...you never know what opportunities will come to you," is what Islam teaches her and she transfers the same lesson to her juniors. She also talks about how Islam guides her to treat everyone equally and share her learnings and experiences with her juniors, peers and community, so that when she grows, her community grows with her. Sara's leads with positivity and believes that talking to people nicely will always make them respect her even if they fail to like her for whatsoever reason. À Derna, en Libye, des inondations sans précédent faisaient plusieurs milliers de morts, il y a un an de cela, dont 145 ressortissants égyptiens, de jeunes hommes venus travailler sur la côte libyenne. Plus de la moitié de ces victimes égyptiennes étaient originaires d'un seul village, Nazlat Al Sharif, niché dans la vallée du Nil, à quelques heures de route au sud du Caire, un village qui a perdu à lui seul 75 jeunes hommes, des pères, des fils, des frères. Un an après, la douleur des familles est toujours vive et la situation financière de toute la communauté est en jeu. Gamal tremble de colère au souvenir de la catastrophe qui a brisé sa famille, au cours de laquelle il a perdu son fils unique, emporté par le torrent meurtrier à Derna, en Libye. « Mon fils est mort en Libye, mon fils Ayman ! se lamente-t-il. Il laisse derrière lui trois enfants et sa femme, une de ses filles est née après sa mort. » Adossés au mur d'une des petites maisons de terre du village, Gamal et ses voisins sont toujours sous le choc : « C'est un jour qui a marqué notre histoire, un jour affreux, affreux pour le village… », explique Gamal, adossé au mur d'une des petites maisons de terre de ce petit village de la vallée du Nil, en Égypte, dans lequel tous les habitants sont encore sous le choc. « Il y a trois morts dans certaines maisons, trois morts, quinze pour la seule rue là-bas », pointe du doigt Youssef, une jeune homme du quartier.Au total, Nazlat Al Sharif, qui compte à peine un millier d'habitants, pleure la mort de 75 jeunes hommes. « C'est difficile pour les mères et les épouses, et les enfants sont encore jeunes, c'est vraiment une situation difficile et un sujet permanent, quelque chose qu'on ne peut évidemment pas oublier, pas au bout d'un an, ni même de deux ou je ne sais combien d'années. C'est dans le cœur et dans l'esprit des gens, c'est dur et c'est impossible à oublier », raconte Hind, qui a perdu son mari Mohamed.À écouter aussiReportage international - Inondations en Libye: un village égyptien endeuilléOutre la perte de leurs proches, les habitants du village comptaient sur l'argent qu'envoyaient ces jeunes travailleurs depuis la Libye, dont la disparition est tragiquement synonyme d'absence de revenus, pourtant essentiels pour tout le village et la communauté. « L'un d'eux avait 18 ans, l'autre 23 ans, décrit Fawzya en parlant de ses deux fils disparus. De très jeunes hommes, l'un s'appelait Emad, l'autre Mohamed… Le corps d'Emad est resté là-bas, il est enterré là-bas. Quant à celui de Mohamed, il nous est revenu. Nous n'avons plus rien, nous n'avons plus de revenus, nous sommes désormais des gens pauvres. Nous n'avons reçu aucun revenu depuis que nos fils sont morts. »Dans ce village de paysans, le travail dans les champs environnants paie peu, à peine 2 000 livres égyptiennes par mois, soit presque 40 euros. Un travail dont la tâche physique est rude, trop rude pour que des personnes âgées ou de jeunes mères ne puissent y prétendre. « Les gens sont épuisés, épuisés… Le gouvernement nous a aidé une fois à hauteur de 100 000 livres égyptiennes, mais on a besoin de revenus mensuels », explique Gamal.Si ces 100 000 livres, pour chaque famille de défunt, constituent une somme importante, ils ne pallient pas à l'absence de revenu régulier pour les foyers Nazlat Al Sharif et n'estompent guère la tristesse des familles. « Nous voulons nos enfants, pas de l'argent, se désole Fawzya. Chaque jeudi, nous nous rendons ensemble sur leurs tombes, je ne vais même plus sur la tombe de mon père ou ma mère, je vais sur la tombe de mon enfant, de mon fils, c'est tout. »Une nouvelle génération de jeunes hommes quitte peu à peu Nazlat Al Sharif pour la Libye, en quête de revenus pour leurs proches endeuillés. Appearing under a pseudonym created to keep the author and his family safe from harm, Adam Gamal served in the most elite units in the US Army, deployed more than a dozen times, and finally retired in 2016. Currently an international consultant for a security organization, in the first half, he shared his account of how America fights its battles abroad in the modern age of terrorism, as well as offered commentary on various conflict zones. He described how he emigrated from Egypt to the US as a young man in order to experience greater freedoms, and then after 9-11 enlisted in a secretive military group referred to as "the Unit," which he said has been responsible for preventing many terrorist attacks throughout the Western world. Gamal rose through the ranks and became one of the Unit's most skilled operators.He offered an analysis of the Israel-Gaza conflict and suggested that Hamas staged their attack as a way to derail the Abraham peace accords between Israel and various Gulf states because they believed if that happened, the Palestinian cause would be forgotten about. Gamal argued that a two-state solution is the only way to resolve the problem in that region. He also gave his take on other areas of geopolitical concern, including China, Russia, and Iran, and emphasized the importance of having diversity in the US armed forces.-----------Author Mike Ricksecker has been researching unexplained phenomena worldwide for over 30 years and has appeared on several popular television shows. In the latter half, he discussed his ongoing work on the nature of time, dimensions, and the possibilities of time travel, along with stargates, magnetic anomalies, and ancient Egyptian mysteries. He detailed his Stacked Time Theory, which is the idea that all time, past, present, and future are here right now. "Each of those moments has its own frequency to it," he explained, and if we can tap into those different frequencies, "we can move up and down that stack at will." Such occurrences can play out in our everyday lives, he added. For instance, two moments can bleed into each other in what has been called a "time slip" that is sometimes mistaken for a ghost or haunting.Precognitive dreams and premonitions demonstrate an aspect of time travel, he remarked, and they may be fostered by a change in our brainwave states while asleep. Ricksecker delved into the concept of stargates or portals and how people may travel or disappear into them and related this to the Einstein-Rosen bridge, the scientific theory that wormholes exist out in space. The symbolism of stargates is seen at temples in ancient Egypt with their massive stairways, he added. Describing what he called "Egypt's Area 51," he said it's a military installation with unfinished or remnants of pyramids that were dug out, and there are rumors that the site contains an ancient circular stargate. Adam Gamal (not his real name) came to America from Egypt at age 20. He didn't know English, had no family, and had to be led out of the subway by his hand to find his way to the YMCA for his first night in New York City. He worked three jobs, learned English, and joined the US Army. His dedication and skills led to an invitation to join a group whose name was classified. ‘The Unit' isn't just the name of the book. From his first deployment in Bosnia, to his search for Saddam Hussein in Iraq, to his work in Africa fighting against the very same Muslim Brotherhood that terrorized his community in Egypt when he was a child, he offers a gripping first-hand account of our nation's most secret military unit. His tales of a cat-and-mouse game tracking terrorist Aden Hashi Ayro - the head of the Hizb al-Shabab, or youth wing of the Islamic Courts Union, who it was believed led militias within Somalia - are filled with tension.Gamal also goes into great detail about the diversification of our nation's defenses.
